1.Impact of paced QRS duration on heart function in patients with right ventricular apical pacing

Objective To investigate the impact of paced QRS duration (pQRSd) on heart function in patients with right ventricular apical pacing. Methods Seventy-six patients with Ⅲ° atrioventricular block received pacemaker treatment were enrolled and randomized into group A (pQRSd < 190 ms, n = 52) and group B(pQRSd≥ 190 ms, n = 24). The concentration of brain natriuretic peptide (BNP),parameters of left atrial diameter (LAD), left ventricular ejection fracetion (LVEF), left ventricular end diastolic diameter (LVEDD), left ventricular end systolic dimension (LVEDD) were measured before operation, at 12 months and 24 months after implanting, respectively. The parameters of echocardiography assay, the concentration of BNP and the incidence of heart failure event after implantation were compared between two groups. Results At 12 months after implanting, LVEF of the patients in the group B decreased significantly compared with that of group A (P < 0.05). However, the echocardiography paramenters and the concentration of BNP were not significantly different between the two groups (P > 0.05). At 24 months after implanting, LAD、LVEDD、LVESD of group B increased significantly compared with those of group A [LAD,( 44.5 ± 6.2) mm vs (41.6 ± 5.1) mm, LVEDD, (52.7 ± 9.3) mm vs (48.2 ± 7.5) mm, LVESD, (37.5 ± 5.6) mm vs (33.8 ± 4.9)mm, each P < 0.05, respectively]. The concentration of BNP of group B increased significantly [(408.2 ± 102.1)ng / L vs (243.7 ± 92.8)ng / L, P < 0.001], and LVEF of the patients in group B decreased significantly compared with those of group A [(46.3 ± 6.8)% vs (51.6 ± 5.2)%, P < 0.001], respectively. No significant difference in the incidence of heart failure event (41.7% vs 26.9%, P > 0.05)between two groups during 24-month follow-up. Conclusion The prolonged paced QRS duration has a detrimental effect on long-term cardiac function during RVA pacing in patients with Ⅲ°atrioventricular block.
2.Comparative study on the application of self-made hemostatic device in minimally invasive surgery

Objective To explore the operation of grasping forceps and nursing staff made telescopic injection device were used for the clinical effect of Hemostasises thoracoscopic lobectomy. Methods 60 cases of thoracoscopic lobectomy for patients according to the date of surgery were divided into operation group and telescopic grasping forceps injection device group 30 patients were given hemostasia operation grasping forceps extraction hemostatic powder and hemostatic powder into the injection device for telescopic wound hemostasis treatment and preventive treatment, were compared between the two groups in the instrument body, completely hemostatic powder cover the wound time, evacuation time of endoscopic instruments, single hemostatic powder wound coverage, wound bleeding rate, the incidence of bleeding again installed. Results The telescopic injection device group and operation time grasping forceps group into cavity instruments, endoscopic evacuation equipment time showed no significant difference (P>0.05). Hemostatic powder completely cover the wounds of time (24.90 ± 5.90) s and (55.60 ± 25.60) s, there was significant difference between two groups (t=6.401, P<0.01). The single ended wound coverage of blood meal was 86.67%(26/30) and 76.67%(23/30), there was significant difference between two groups (χ2=2.016, P<0.05). Wound bleeding rate was 10.00%(3/30) and 33.33%(10/30), there was statistically significant difference between the two groups (χ2=4.812, P<0.05). The incidence of bleeding again to install powder was 10.0%(3/30) and 93.3%(28/30), there was significant difference between two groups (χ2=41.713, P <0.01). Conclusions The nursing staff made telescopic injection device assisted surgery were thoracoscopic lobectomy of the wound to stop bleeding and prevent rebleeding, wound coverage than grasping forceps, completely cover the wound operation less time than grasper, re install a low incidence of hemostatic powder, the whole process to shorten the operation time.
3. Clinical characteristics of 57 patients with polyarteritis nodosa and renal involvement

Objective:
To investigate the clinical characteristics of polyarteritis nodosa (PAN) patients with renal involvement.
Methods:
PAN patients admitted to the department of rheumatology, department of pediatrics, department of nephrology, general internal medicine department and department of vascular surgery at Peking Union Medical College Hospital from June 2012 to August 2018 were enrolled in this study and were divided into two groups according to renal involvement or not. The clinical characteristics were analyzed.
Results:
A total of 94 PAN patients were finally enrolled and 57 (60.64%) presented kidney manifestation. The mean age of onset was (37.76±17.40) years old and the interval from onset to diagnosis was 10 (0 to 240) months. Forty patients were misdiagnosed once or more times. In patients with renal involvement, 9 cases suffered from renal ischemia or infarction, 31 with microscopic haematuria, 26 with proteinuria, renal artery or its branch involved in 17 cases, renal vein thrombosis in 1 case, 4 cases with pyeloureterectasis, one case with renal fascia thickening, 33 cases with impaired renal function (serum creatinine>84 μmol/L) including creatinine>140 μmol/L in 10 patients. Renal artery branch stenosis was the most common presentation [9 cases (52.94%)] of renal vascular involvement, other abnormalities including nodular dilatation [4 cases (23.53%)], occlusion [3 cases (17.65%)].
