1.A comparative evaluation of 3 kinds of cecocolon urinary reservoir

Objective To evaluate and compare 3 kinds of cecocolon urinary reservoir. Methods Penn pouch,indiana pouch or detenial cecocolon reservoir has been performed for 37 patients.The out-come was evaluated by urodynamic and clinically. Results Good continence has been achieved in all patients with an appendiceal continence mechanism.The capacity and intrareservoir pressure of detenial cecocolon was significantly different from penn or indiana pouch 3 months postoperatively (P0.05).The intrareservoir pressure on contraction was lower with penn pouch. Conclusions The penn or modified indiana pouch is better and more ideal as an urinary reservoir whereas the detenial cecocolon pouch is technically simpler.
2.Experience of Minimally Invasive Percutaneous Nephrolithotomy for the Treatment of 216 Cases Upper Urinary Tract Calculus

Objective To investigate effectiveness of treatment of Upper urinary tract calculus by MPCNL application.Methods From May 2000 to May 2007,causing artificial kidney hydronephrosis for the affected kidney,established renal percutaneous channels(F16~18),which established the single channels of 202 cases,dual-channel of 14 cases,by Ureteroscopic Lithotripsy and holmium laser lithotripsy.Results 183 cases of 216 patients were One-time stone;33 cases were second stone.The average clearance rate is 90.32%,the average operation time is 79min,average valume of about bleeding is 80ml,the average hospitalization days are 8 days,kidney ostomy average stay 7.5 days.Of the operation,9 cases of obviously bleeding(4%)before and after operation,1 case uncontrollable bleeding after interventional treatment failure with Nephrectomy,others to stop beeding by symptomatic treatment,2 cases find urinary extravasation postoperative and cured by adjusting location of renal ostomy.Conclusion Minimally invasive percutaneous nephrolithotomy for the treatment of Upper urinary tract calculus is safe and effective for patients with less trauma,quicker recovery and high learance rate,the efficacy is satifactory.
3.Ideas Exploration and Analysis on the Prescription Comment of TCM Injections

OBJECTIVE:To explore the ideas for the prescription comment of TCM injections,and provide reference for clini-cal rational use. METHODS:Totally 9 457 prescriptions in PIVAS and 400 hospitalized medical records in our hospital in 2015 were collected. The prescriptions and hospitalized medical records which contained TCM injections were retrospectively commented in aspects of indications,usage and dosage,drug combination,solvent selection and repeat medication,and analysis of typical case. RESULTS:In 2015,there were 2 544 prescriptions(26.90%)in PIVAS and 195 medical records(48.75%)that used TCM injections;the irrational use was mainly inappropriate solvent selection(50.70%),inappropriate usage and dosage(27.00%),inap-propriate indications (10.33%),inappropriate combination therapy (7.51%) and repeat medication (4.46%). CONCLUSIONS:Prescription of TCM injections should be commented with the medical records and individual analyzed;in terms of unreasonable prescriptions and medical records TCM clinical pharmacists should actively intervene unreasonable prescriptions by the bed.
4.Experimental Study on Yinhuang Solution on Bladder Mucosal Injury in Rat

ObjectiveTo investigate the effects of Yinhuang solution on the bladder mucosal before and/or after mucosal injury.Methods32 SPF female SD rats were randomly divided into 4 groups with 8 rats in each group. Group A was used for the normal control group, group B for the model control group, group C for preventive group, group D for preventive and therapeutic group. Injured rats were created by hydrochloric acid applied on bladder mucosa. After the models were established or before and after the models were established, bladders were perused with Yinhuang solution. Severity of inflammatory infiltration were observed by HE staining, histological score and heat shock protein 70 (HSP70) expressed in the mucosa.ResultsEndoscopic pathological change showed that group B had a large number of inflammatory cell infiltration, significant expansion of capillary, epithelial cell necrosis. Group C had a small amount of expansion of capillary, inflammatory cell infiltration. Group D scattered in expansion of capillary, a small amount of inflammatory cell infiltration. Concerning the results of histological score and HSP70 expression, there was a significant difference between group A and groups B, C or D (P<0.01); there was a significant difference between group B and groups C or D (P<0.01); there was a significant difference between group C and group D (P<0.05).ConclusionYinhuang solution can evidently improve the repair capability of bladder mucosa after injury.

The number of projects and the amount of sponsoring funds have been increasing during the past years.Conventional research branches are respiratory tumor and digestive tumor.Emerging research branches are lung transplantation and lung preservation.Underdeveloped research branches are acute lung injury and acute respiratory distress syndrome and thymic tumor.The position and academic impact of domestic thoracic surgery has been more valuable with the increasing number,quality and achievements of SCI papers.Therefore,international cooperation and megamergers should be encouraged and more attention should be paid to the censorship,implementation and achievements to optimize the value of projects.The aim of this article is providing important information on projects sponsored by National Nature Science Foundation of China(NSFC) in the field of general thoracic surgery,promoting the development of thoracic surgery of China.
6.Experience in the diagnosis and treatment of bronchial anastomosis after lung transplantation

Objective To assess the incidence and prognosis of the bronchial anastomosis complications following lung transplantation.Methods Between January 2003 and July 2016,all 81 cases after lung transplantation at Department of Thoracic Surgery of Shanghai Pulmonary Hospital,Tongji University were retrospectively analyzed.We analyzed the incidence and prognosis of postoperative complications of bronchial anastomosis in lung transplant recipients.Results The overall incidence of bronchial anastomotic complications was 30.9%,including anastomotic infection (18.5 %),anastomotic fistula (4.9 %),stenosis (6.2 %),and bronchomalacia (1.2 %).One case died of infection,and 3 cases died of anastomotic fistula.Conclusion The main bronchial anastomosis complications occurred early,and the prognosis was acceptable.

Objective Evaluation of self-control suspension under the double hook system used to xiphoid thoracoscope fully the effect of thymectomy.Methods There were 17 cases of thymic adenoma diagnosed before surgery in November to December 2016,including 7 males and 10 females,aged(57 ± 10).The total thymomectomy was performed with a double hook method with a double hook procedure.Results All the 17 patients had smooth operation,and no clear hook related complications were observed after the operation.The operation was convenient and the thymus was completely removed.Conclusion It is worth populating that the double pull hook method is convenient and safe to remove the whole thymus gland.

The incidence of lung cancer in China ranks first and second among men and women respectively .Meanwhile, lung cancer is also the leading cause of death in malignant tumors in China .Surgical treatment is of great significance in impro-ving the prognosis of patients with lung cancer, and the number of related clinical trials is increasing year by year.Through a retrospective review of clinical trials in the field of lung cancer and thoracic surgery on the Clinicaltrials .gov platform, this arti-cle finds that relevant clinical trials focus on treatment , perioperative management and basic research.Although the clinical tri-als in China started late, they have developed rapidly and the number of clinical trials ranked second.In addition, there are some shortcomings in the clinical trials of China in terms of trial design and research scope , and there is still a gap between Chinese and foreign trials.
9.A comparative study on the clinical effects of hip arthroplasty through direct anterior approach in lateral decubitus in the treatment of hip osteoarthritis caused by Kaschin-Beck disease and congenital acetabular dysplasia

Objective:To compare the clinical effects of hip arthroplasty through direct anterior approach (DAA) in lateral decubitus in the treatment of hip osteoarthritis caused by Kaschin-Beck disease and congenital acetabular dysplasia.Methods:The prospective study method was used to select the patients who needed hip arthroplasty in the Fourth Department of Orthopedics, the Second Affiliated Hospital of Harbin Medical University from January 2015 to December 2019. All of them were operated with lateral decubitus DAA. According to the inclusion criteria, they were divided into Kacshin-Beck disease hip osteoarthritis group (group A) and congenital acetabular dysplasia hip osteoarthritis group (group B). Hip Harris score, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, visual analogue scale (VAS) score were conducted, and hip abduction angle and flexion angle were measured before surgery, 3, 14 days and 1, 3, and 12 months after surgery.Results:Nineteen and twenty-two patients were included in group A and group B, respectively. All patients successfully completed the surgery. There was no significant difference in Harris score between the two groups before surgery, 3, 14 days, and 1, 12 months after surgery ( P > 0.05). There were no significant differences in WOMAC score, VAS score, hip abduction angle and hip flexion angle between the two groups before surgery and each time point after surgery ( P > 0.05). In the same group, there were significant differences in Harris score, WOMAC score, VAS score, hip abduction angle and hip flexion angle at different time points ( P < 0.001). All postoperative indicators were significantly improved compared with those before surgery. Conclusions:There is no significant difference in the clinical effects of hip arthroplasty through lateral decubitus DAA in the treatment of hip osteoarthritis caused by Kaschin-Beck disease and congenital acetabular dysplasia. This surgical method has good therapeutic effect on both types of hip osteoarthritis.
10.Outcomes of patients awaiting lung transplantation after the implementation of donation after brain death at a single Chinese center.

Voluntary contribution has become the only source of donor lungs in China since 2015. To elaborate the outcomes of patients awaiting lung transplantation (LTx) after the implementation of donation after brain death, we performed a retrospective study that encompassed 205 patients with end-stage lung disease who registered for LTx at Shanghai Pulmonary Hospital from January 1, 2015 to January 1, 2021. A total of 180 patients were enrolled in the study. The median waiting time was 1.25 months. Interstitial lung disease (ILD) (103/180, 57.2%) and chronic obstructive pulmonary disease (COPD) (56/180, 31.1%) were the most common diseases in our study population. The mean pulmonary artery pressure (mPAP) of patients in the died-waiting group was higher than that of the survivors (53.29±21.71 mmHg vs. 42.11±18.58 mmHg, P=0.002). The mortality of patients with ILD (34/103, 33.00%) was nearly twice that of patients with COPD (10/56, 17.86%) while awaiting LTx (P=0.041). In the died-waiting group, patients with ILD had a shorter median waiting time than patients with COPD after being listed (0.865 months vs. 4.720 months, P=0.030). ILD as primary disease and mPAP > 35 mmHg were two significant independent risk factors for waitlist mortality, with hazard ratios (HR) of 3.483 (95% CI 1.311-9.111; P=0.011) and 3.500 (95% CI 1.435-8.536; P=0.006). Hence, LTx is more urgently needed in patients with ILD and pulmonary hypertension.
