1.Management of the recalcitrant bronchopleural fistula using the transsternal transpericardial approach

Objective To review the experience of closure of recalcitrant bronchopleural fistula using the transsternal transpericardial approach. Methods 3 patients with recalcitrant bronchopleural fistula underwent transsternal transpericardial closure of a bronchopleural fistula and drainage by open thoracostomy. Results All patients were survived and there was no recurrence of bronchopleural fistula. Conclusion Transsternal transpericardial approach seems to be a safe and effective method in cases of bronchopleural fistula complicated with empyema. It has the added advantage of less recurrent fistula formation.
2.Individual operation design for unilateral cleft lip repair

Objective:To evaluate the effectiveness of individual operative design for unilateral cleft lip repair.Methods:Individual operation design was applied for the repair of unilateral cleft lip in 40 cases.The operations were conducted according the designs and the secondary nasal deformities were corrected at the same time.One week after operation,dermal sutures were removed,the effects were evaluated by 3 professional doctors.7 labial morphometric lines were measured.The ratio of the line length on healthy side to the correspondence on the cleft side(RLL)were calculated for objective evaluation.Results:First intention was found in all cases,the labial bow of both sides was symmetried after operation.In dynamic state and static state,the effects of the operations were satisfactory.RLL of 0.91-1.10 of the 7 lines was observed in 67.5%-95.0% of the patients.Conclusion:The individual operation design is feasible for unilateral cleft lip repair.

OBJECTIVE To discuss the effect of regular emptiness of humidification pot on ventilator associated pneumonia(VAP) in the patients with mechanical ventilation.METHODS Totally 240 mechanical ventilation patients were randomed to 2 groups,in an observation group the humidification pots were emptied per 24 hours and in a control group the fluid was added only when its water was lower limit and compared their incidence rate of VAP.RESULTS Ninety-six of 240 patients with mechanical ventilation had developed VAP and 238 pathogen strains were cultured.Altogether,the incidence of VAP in observation group was lower than those in control group significantly,?2=4.4,P

The “vertical tongue”method was used in speech training for 12 patients with functional speech disorder of consonant /L/.Af-ter treatment,the average vocal intilligibility of the 12 patients increased from 86.3% to 98.9%(P <0.05)./L/consonant average intelligi-bility increased from 42.9% to 85.2%(P <0.05).
5.Imaging characteristics of hepatocellular adenoma compared with pathologic findings

Objective To retrospectively compare CT and MR features of hepatocellular adenoma with pathologic findings.Methods Twelve patients with histopathologically proved hepatocellular adenoma were classified on the basis of pathologic and genotype phenotype findings into four groups:steatotic type,cytological abnormality type,telangiectatic adenoma with inflammatory infiltrates type and atypical adenoma type.The CT and MR features of each type were reviewed retrospectively compared with the pathological results.Results In this retrospective study,12 patients were examined with CT (8 patients) and MR (8 patients).Among 12 patients,4 patients showed a steatotic type.One patient showed hypo-density on the non-enhanced CT and 3 patients demonstrated hypo-density on all phases of the post-contrast scans.Two lesions showed iso-intense signal on the in-phase T1 WI with signal dropout on the out-of-phase T1WI,and hypo-intense signal on the T2 WI with fat suppression sequences.One lesion demonstrated moderate hypointense signal on all phases of the post-contrast MRI scans.Two patients with the telangiectatic adenoma irflammatory infiltrates type were found.One patient showed hypo-density on the non-enhanced CT scans and hyper-density on all phases of the post-contrast CT scans.One patient demonstrated iso-intense signal and the other hypo-intense signal on the T1 WI,and both displayed moderate hyper-intense signal on the T2WI with fat suppression sequences and hyper-intense signal with gradual enhancement on all phases of post-contrast MR scans.There were 3 patients with a cytological abnormality type.One patient appeared hypodensity and 1 patient showed uniform iso-density on non-enhanced CT scans.All patients who had undergone contrast-enhanced CT scans were found to have hyper-density on the hepatic arterial-dominant phase and became slightly lower on the portal venous phase.On the delay phase the density reduced further.One mass showed iso-intense signal on the T1WI and hyper-intense signal on the T2WI with fat suppression sequences.There were 3 patients with an atypical adenoma type.One patient appeared uniform hypo-density on the nonenhanced CT and hyper-density on the hepatic arterial-dominant phase and became iso-dense on the portal venous phase.On the delay phase,it was slightly hyper-dense.Two out of the three lesions showed isointense and one hypo-intense signal on the in-phase T1 WI,and hypo-intense,hyper-intense,and iso-intense signal on the T2WI with fat suppression sequences,respectively.Two patients examined on all phases of post-contrast MRI scans.The result was similar to the CT findings.Conclusion The imaging features of hepatocellular adenoma are closely associated with pathological characteristics.
6.Value of MRI in qualitative and quantitative diagnosis of bone marrow infiltration in lymphoma

Objective To evaluate the value of MRI in qualitative and quantitative diagnosis of bone marrow involvement in patients with lymphoma. Methods 28 diagnosed lymphoma with bone marrow infiltration and 31 healthy spines as controls were included.MRI performance and the signal intensity ratio on T1WI (SIR1) of the spine bone marrow and cerebrospinal fluid were analyzed and statistical analyses were performed.Results Qualitative diagnosis among these patients,was shown than MRI results indicated that 27 cases had abnormal signal. The sensitivity of MRI was 96.4 %. There were four main patterns of marrow infiltration in MRI,containing nodular pattern 21.4 % (6/28),scattered pattern 53.6 % (15/28),mottled pattern 14.3 % (4/28) and uniform pattern 7.1% (2/28) respectively.By quantitative diagnosis the SIR1 of study group (1.251±0.253) was apparently lower than that in the control group (2.625±0.434) with statistical significance (t =15.022,P < 0.001).The results of multiple comparisons showed that the SIR1 of mild degree (1.390±0.172),moderate degree (0.982±0.790) and severe degree (0.908±0.122) patients with lymphoma compared with the normal controls had significant differences (all P =0.000),and difference between mild and moderate degree had statistical significance (LSD,P =0.012),so did the difference between mild and severe degree (LSD,P =0.025).However,no significant difference could be seen between moderate and severe degree (LSD, P =0.757). Positive linear correlation was existed between the persentages of SIR1 and tumor cells in bone marrow (r =-0.765, P < 0.001). Conclusion As a noninvasive and direct-viewing technique, MRI presents a global view of bone marrow with high sensitivity in detecting bone marrow involvement in lymphoma. To a certain extent, it could be possible to estimate the infiltration degree and evaluation of the tumor burden in bone marrow by quantitative measurement of MRI.
7.Influential factors for G-band chromosome exhibition in spermatogonial stem cells of mice

BACKGROUND:Rapid development of spermatogonial stem cells is the new hope for assisting reproductive technologies,and the stability of the number and structure of the chromosome cultured in vitro is one of the important factors in usage.OBJECTIVE: To study the influential factors on G-band exhibition in mouse spermatogonial stem cells, so as to offer a technology to identify karyotype of stem cells in culture.DESIGN: Observational experiment.SETTING: Luzhou Medical College.MATERIALS: The experiment was conducted in the Laboratory of Medical Molecular Biology, Luzhou Medical College from March 2004 to April 2005. 10 days old male and female Kunming mice were provided by Department of Animal of Luzhou Medical College (number of license No. 17 of experimental animal quality administration in Sichuan province).There were low-sugar DMEM medium, 10 mg/L Mitomycin-C, IMDM medium, 1 ×10-5 mol/L colchicines (PBS allocation),7.5 mmol/L KCL, fixative (mixture glacial acetic acid with methanol in 1:3), Giemsa staining solution and 0.25% zymine.METHODS: Bone marrow was aspirated from the thigh bone of mouse for feeder layer cells preparation. Cells from the male mice testis of 7-8 days after birth were prepared and made into cell suspension. After adjusting the cell density to 3×105 L-1, they were inoculated into the feeder layer of bone marrow stromal cells. Cells were cultured at 37 ℃ in CO2 incubator containing CO2 of 0.05 volume fraction and 70% humidity. The proliferation groups of stem cells cultured 15-20 days were selected, stirred and spread, and then treated with colchicine for 4-6 hours. Cell suspension was collected,and then stained after hypotonic treatment. The cells whose chromosomes were dispersed well and in metaphase were selected, and number of chromosomes was counted, and then the morphology of chromosomes was observed.MAIN OUTCOME MEASURES: Culture of bone marrow stromal cells and spermatogonial stem cells and coloration and count of chromosomes.RESULTS: The karyotype of spermatogonial stem cells was the same as the body cells of normal mouse, which showed granule or rod-shape. Karyotype was 20 pairs, 40 bars. Three kinds of chromosome morphology could be observed under oil immersion lens. The first type was condensed, which could be counted in total, but the band could not be seen.The second type was chromatosome that spread completely in the center of equatorial plate and were in metaphase. In this phase, total numbers and band were seen clearly. Last type-chromosomes had already folded and moved towards two poles and concentrated gradually, the total number of chromosomes could be counted, but bending could not be seen clearly.CONCLUSION:Many factors can affect the karyotype of spermatogonial stem cells, including the phase of cell division,effect of hypotonic solution, diffusion of the cell when dropping slides, concentration of trypsin and digestion time, etc.
8.Effect of miR-5590-3p on the invasion and proliferation of gastric cancer cell line HS-746T by regulating TGFBR2 expression

Objective:To explore the inhibition of microRNA (miRNA, miR)-5590-3p on the expression of transforming growth factor beta typeⅡreceptor (TGFBR2) gene and its effect on the invasion and proliferation of gastric cancer cell line HS-746T.Methods:The gastric cancer cell line was cultured in vitro. Real-time quantitative polymerase chain reaction (qRT-PCR) was used to analyze the expression of miR-5590-3p in gastric cancer tissues and cell lines. The miR-NC and miR-5590-3p mimic were transfected into gastric cancer cell line HS-746T by lipofection, and named as miR-NC group and miR-5590-3p group, respectively. qRT-PCR was used to measure transfection effects. Transwell assay and cell counting kit-8 (CCK-8) assay were used to detect cell invasion and proliferation after transfection. The bioinformatics software predicts and the dual luciferase reporter gene system validates the target gene of miR-5590-3p. qRT-PCR and western blot were used to measure the expression of TGFBR2 and its downstream proteins in the transfected cells. Results:The expression of miR-5590-3p in gastric cancer tissues was significantly lower than that in adjacent tissues ( P<0.01). The expression of miR-5590-3p in gastric cancer cell lines was significantly lower than that in normal gastric mucosal epithelial cells ( P<0.05), and lowest in HS-746T cells ( P<0.01). After transfection, the expression of miR-5590-3p in miR-5590-3p group (11.76±0.21) was significantly higher than that in miR-NC group (1.06±0.21), with statistically significant difference ( P<0.01). The number of invasive cells in miR-NC group and miR-5590-3p group were (101.20±15.47) and (26.53±6.53), respectively, and the invasion ability of miR-5590-3p group was significantly decreased ( P<0.01). Compared with the miR-NC group, the cell proliferation ability of the miR-5590-3p group was significantly decreased ( P<0.05). Bioinformatics software showed that the target gene for miR-5590-3p is TGFBR2. The dual luciferase reporter gene system confirmed that miR-5590-3p can target the TGFBR2 gene ( P<0.01). Western blot results showed that compared with miR-NC group, the expression of TGFBR2 in HS-746T cells in miR-5590-3p group was significantly decreased ( P<0.01), the expression of ZEB-1 and ZEB-2, and the expression of CDK1 and cyclin B proteins were decreased as well. Conclusions:miR-5590-3p can inhibit the invasion and proliferation of gastric cancer cell HS-746T by targeting and regulating the expression of TGFBR2 gene.
9.Relationship between serum levels of PCT and neutrophil CD64 with effect of hormone therapy and complications in patients with nephrotic syndrome

Objective To investigate the relationship between serum levels of PCT and neutrophil CD64 contents with the effect of hormone therapy and complications in the patients with primary nephrotic syndrome.Methods Sixty-five patients with primary nephrotic syndrome in our hospital from September 2015 to September 2016 were selected as the research subjects,all cases were treated with hormonal therapy,the serum levels of PCT and neutrophil CD64 were detected and their relationship with the curative effect and complications of nephrotic syndrome was analyzed.Results According to the PCT and neutrophil CD64 median levels,the cases were divided into the high level group and low level group,the results found that serum creatinine,serum protein,urine protein and pathological types had no statistical difference between the high level group and low level group.The hormone sensitivity had 15 cases in the patients with high PCT level,which was significantly lower than 21 cases in the patients with low PCT level;the hormone sensitivity had 14 cases in the patients with high neutrophil CD64 level,acute renal failure,infection and thrombus in the patients with high PCT level had 8,10,6 cases,which were significantly lower than those in the patients with low PCT level;acute renal failure,infection and thrombus in the patients with high neutrophil CD64 level had 7,11,6 cases,which were significantly higher than those in the patients with low neutrophil CD64 level (P<0.05).Conclusion The levels of serum PCT and neutrophil CD64 are significantly correlated with the therapeutic effect and clinical prognosis in the patients with nephrotic syndrome.
10.A study on the relationship between vascular endothelial growth factor-D, endostatin and the progress of colorectal cancer

Objective To investigate if vascular endothelial growth factor-D(VEGF-D) and endostatin(ES) promote angiogenesis in colorectal cancer(CRC). Methods One hundred and five patients undergoing CRC surgery were enrolled in this study. CRC and normal tissue microarray were produced and measured for VEGF-D and ES by immunohistochemmical staining.Results VEGF-D and ES expression were stronger in CRC tissues than in control normal mucosa. MVD was higher in cancer tissues than in normal ones. ES down-regulated expression was associated with less invasiveness, positive lymph node metastasis, distant metastasis and in cases of early Dukes′ stage, while the expression of VEGF-D in corresponding tissues was on the opposite to ES. MVD increased along with tumor invasion, lymph involvement, distant metastasis and late Dukes′ stage. Conclusions VEGF-D in accord with ES promotes the angiogenesis and progression of CRC.
