1.Treating Gonarthritis with 3 Therapeutic Regimens: Cost-effectiveness Analysis

OBJECTIVE: To evaluate the therapeutic effects and cost-effectiveness of 3 therapeutic regimens in the treatment of gonarthritis. METHODS: The cost-effectiveness analyses were conducted among 3 therapeutic regimens: regimen A(sodium hyaluronate), regimen B(sodium hyaluronate plus compound betamethasone) and regimen C(sodium hyaluron_ate plus triamcinolone acetonide acetate). RESULTS: The total costs for Group A, B and C were 956.5 yuan, 1 206 yuan and 998.5 yuan, respectively. The effective rates were 79.2%, 91.3% and 96.4%, respectively. The cost-effectiveness ratios were 12.08, 13.21 and 10.36 respectively. As compared with regimen A, the incremental cost-effectiveness ratios for regimen B and C were 20.62 and 2.44 respectively. CONCLUSION: Regimen C is more preferable among the three therapeutic regimens.
2.The clinical significance of the detection of hepatitis B virus large surface protein for the diagnosis and treatment

Objective To explore the correlation between hepatitis B virus large surface protein(HBV-LP)and hepatitis B virus deoxyribonucleic acid(HBV-DNA),analyze the clinical significance of detecting the two index dynamically for diagnosis and treatment of hepatitis B.Methods Enzyme linked immunosorbent assay(ELISA)and fluorescent quantitation polymerase chain reaction(FQ-PCR)were used to detect the levels of HBV-LP and HBV-DNA in serum specimen of 230 hepatitis B patients.Results There was a positive correlation between the content of HBV-LP and copy numbers of HBV-DNA in serum of hepatitis B positive cases(r=0.84,P<0.01).The positive rate was 84.78% in HBV-LP and 84.35% in HBV-DNA of 230 HBV positive cases.As a result there was no significant difference(P>0.05)in this study.Therefore,the positive rate was 63.63 %(35/55)in HBV-LP and 58.18%(32/55)in HBV-DNA of 55 HBV negative cases.There was also no significant difference(P>0.05)in this study.However,the positive rate of HBV-LP(82.47%)was higher than HBE(52.06%)in the 194 HBV-DNA positive cases.There was significantly different(P<0.01).Conclusion HBV-LP and HBV-DNA are the significant index of the degree of hepatitis B virus replication in hepatitis B positive cases,especially in determining the virus replication and prognosis of treatment in HBe Ag negative,providing the reliable laboratory data for the clinical diagnosis and treatment.
3.Determination of three aconitum alkaloids in Zhentong Huoluo Tincture by RP-HPLC

AIM:To establish a method for detecting three aconitum alkaloids(aconitine,mesaconitine,hy-paconitine) in Zhentong Huoluo Tincture(Radix Aconiti,Radix Aconiti kusnezoffii,Radix et Rhizoma Rhei,Rhizoma Pinelliae,Rhizoma Arisaematis ect.).METHODS:A ZOBAX Extend-C_ 18(250 mm?4.6 mm,5 ?m) column was used to determine the aconitum alkaloids in Zhentong Huoluo Tincture.The mobile was gradient elution,0.1% die-thylamine(adjust pH 9.0 with phosphoric acid) as mobile A;0-20 min A∶B(63∶37),20-45 min A∶B(63∶55∶37→45),at the rate of 1 mL/min.The temperature of column was 30 ℃,and the wavelength was at 232 nm.RESULTS:The linearity of the aconitine in 9.74 ng-974 ng was 0.999 9;that of the mesaconitine in 23.86-2 386 ng was 0.999 9 and that of the hypaconitine in 29.94 ng-2 994 ng was 0.999 9.The RSD of precision for these three were 1.50%、0.68% and 0.49%,respectively.The average recoveries and its RSD were 100.2%(RSD 3.2 %)、102.9%(RSD 2.2%) and 98.2(RSD 2.0%),respectively for aconitin,mesaconitine and hypaconitine.The reproducibilities were all less than 2.0%,and the sample solution was stable in 24 h.CONCLUSION:The method is simple,accurate and has good stability,can be used for the quantity control in the product of Zhentong Huoluo Tincture.
4.A Fast Determination Method for Phenolphthalein Illegally Added into Diet Health Products by Raman Spectroscopy

Objective:To determine phenolphthalein illegally added into diet health products by Raman spectroscopy. Methods:Raman spectroscopy was used to determine phenolphthalein added into diet health products. The diet health products were extracted by suitable solvents, and then the extracting solution was measured by Raman spectroscopy. Results: A calibration curve was built and the analysis was performed on the samples with phenolphthalein at different concentrations. The results were accordance with the real added amount. Conclusion:With simple sample preprocessing method, Raman spectroscopy can be used in the fast detection of phe-nolphthalein illegally added into diet health products. The method is fast and simple with low cost, and can provide both qualitative and quantitative results. The detection limit is 1% for phenolphthalein in diet health products.
5.Analysis of Correlated Factors Affecting Tumor Volume Doubling Time of Breast Cancer

Objective To measure volume of breast cancer , calculate tumor volume doubling time (TVDT), and analyze the correlated factors affecting TVDT using three-dimensional ultrasound (3D-US). Methods We applied 3D-US to measure the volume of breast cancer of BI-RADS-US 4A classified by conventional ultrasound. The breast cancer case scanned by 3D-US at least twice (the interval is 3 months at least) without any medical intervention were included in the study. We calculated TVDT according to the formula, and analyzed the affecting factors of TVDT using multiple linear regression. Results Sixty-nine cases were enrolled in the study. The TVDT of breast cancer were from 66 to 521 days , in an average of 185 ± 126 days and the median time of 164 days. We found that: ① there were no statistics significances in TVDT between different breast cancer pattern , smoothing border lines , speculated sign , hyperechoic halo , microcalcification and different rear echo (P > 0.05). ② TVDT of different age groups, lymph node metastasis, pathological grade and NPI score were significantly different (P < 0.05), while menstrual cycle, family history of breast cancer and molecular typing showed no difference (P > 0.05). ③ TVDT of patients with different expression of ER, PR and Ki-67, molecular typing showed statistically difference (P < 0.05), but there was no significant difference between different level of HER2 (P >0.05). ④ multi-factor analysis showed that the NPI score, lymph node metastasis, Ki-67 and molecular typing of breast cancer were relative factors in TVDT (P < 0.05). Conclusions The NPI score , lymph node metastasis , Ki-67 and molecular typing significantly correlate with TVDT of breast cancer. Triple-negative breast cancer in molecular typing has the fastest growth rate.
6.Analysis of 3D power Doppler ultrasound parameters in the diagnosis and efficacy of BI-RADS Ⅳ breast masses

Objective To investigate diagnostic efficiency of vascular parameters in the diagnosis of BI-RADS Ⅳ breast masses using three-dimensional power Doppler ultrasound (3D-PDUS).Methods After adjusting the ultrasonic instrument under property conditions,3D-PDUS were applied to scan BI-RADS Ⅳ breast masses before surgery with QLab software,the following parameters of breast masses were obtained:total tumor volume,vascularization index (VI),flow index (FI),and vascularization flow index (VFI).According to the pathological results,the diagnostic value of vascular parameters from 3D-PDUS were assessed with receiver operating characteristic (ROC) curve,and the diagnostic efficiency were analyzed by best cut-off limits of various vascular parameters.Results The volume and vascular parameters of benign group were remarkably lower than those of malignant group (all P <0.001).The area under the ROC curve (AUC) of VI (0.873) >VFI (0.866) >FI (0.778).AUC of FI was significantly smaller than that of VI (P =0.016) and of VFI (P =0.039),while no statistical difference between that of VI and of VFI (P =0.294).The diagnostic accuracies of VI (cut-point:1.2,sensitivity:84.1%,specificity:83.3 %,accuracy:83.8 %) and of VFI (cut-point:0.5,sensitivity:82.5 %,specificity:85.2%,accuracy:82.9%) were higher than that of FI (cut-point:35.3,sensitivity:74.6%,specificity:70.4 %,accuracy:72.6 %).AUC of VI (P =0.001) and of VFI (P =0.033) in small-volume group (<2cm3) were both larger than those of large-volume group (≥2 cm3),no significant difference in AUC of FI between these two groups (P =0.09).Conclusions Vascular parameters obtained using 3D-PDUS have affirmative value in the diagnosis of BI-RADS Ⅳ breast masses,especially in masses less than 2 cm3 in volume.
7.Determination of sulfur dioxide in traditional Chinese medicine by alkaline solution extraction and ion chromatography.

OBJECTIVEAn ion chromatographic method was developed to analyze sulfur dioxide in traditional Chinese medicine (TCM).
METHODThe samples were extracted with alkaline solution and reacted with formaldehyde to form hydroxymethyl sulfonic acid. The extracts were cleaned by C18 column and SO3(2-) was determined with ion chromatography equipped with a conductance detector. In the method, AS11-HC chromatographic column was used and the mobile phase was 15 mmol x L(-1) KOH.
RESULTThe calibration curve was in good linearity in the range of 1 to 100 microg (r =0.9992). The detection limit was 0.53 mg x kg(-1) and the average recovery was 103.7%.
CONCLUSIONThe method is simple, sensitive, precise, short-time-consuming, and the random error is smaller, which is applicable to the testing of large quantity batches of TCM.

8.Relationship among thyroid stimulating hormone level and sex hormones, blood lipids and insulin resistance in patients with polycystic ovary syndrome

Objective To explore the relationship among thyroid stimulating hormone level and sex hormone level,blood lipid level and insulin resistance in patients with polycystic ovary syndrome. Methods From February 2014 to February 2017,eighty cases with polycystic ovary treated in the People's Hospital of Longhua,Shenzhen were selected as the research object. The clinical data of all patients were collected to check the indexes of thyroid function,lipid metabolism,glucose metabolism,and sex hormone related indexes,and the insulin resistance index (HOMA-IR) was calculated according to the related indexes of glucose metabolism. The patients were divided into group according to the level of thyrotropin,group A (thyrotropin<2. 5mIU / L),group B (2. 5mIU / L<thyrotropin<4. 0mIU / L) and group C (thyrotropin>4. 0mIU / L). According to the patient's insulin resistance index,the patients were divided into IR group and non IR group. According to the blood lipid metabolism index,the patients were divided into abnormal blood lipid group and non lipid abnormality group. The levels of thyrotropin and sex hormone related indexes were compared in different groups, and the correlation between thyrotropin level and various indexes was analyzed. The TSH level in patients with polycystic ovary syndrome was positively correlated with body mass index, three acylglycerol,low density lipoprotein cholesterol and insulin resistance index( r=0. 272,0. 317,0. 364,0. 227) ( P=0. 015,0. 004,0. 003,0. 043),and there was a negative correlation with luteinizing hormone and high-density lipoprotein cholesterol(r=-0.616,-0.254) (P=0.000,0.034).Conclusion Hyperthyroidism in patients with polycystic ovary syndrome is associated with abnormal blood lipid,abnormal sex hormone levels,and insulin resistance.
9.Analysis of Serum Furin,TGF-β,VEGF and netrin-1 Levels Expression and Polymorphism of r2071410 C/T Locus in the Furin Gene P1 Promoter Region in Patients with Different r-AFS Stages of Endometriosis

Objective To investigate Furin,tumor growth factor-β(TGF-β),vascular endothelial growth factor(VEGF),neuron towards axon guidance factor-1(netrin-1)level expressions and polymorphism of Furin gene P1 promoter r2071410 locus C/T in endometriosis(EMT)patients with different r-AFS stages,and explore the correlation with the incidence of EMT in Shenzhen.Methods A total of 102 patients diagnosed with EMT from May 2021 to January 2023 in the People's Hospital of Longhua,Shenzhen were selected as EMT group,and the EMT group was divided into stage Ⅰ to Ⅱ group and stageⅢ to Ⅳ group according to r-AFS staging method.At the same time,78 non-EMT patients were collected as control group.The levels of serum Furin,TGF-β,VEGF and netrin-1 were detected by enzyme-linked immunosorbent assay(ELISA),and the polymorphism of the r2071410 C/T locus in the P1 promoter region of Furin gene was analyzed by reverse transcription-real time quantitative polymerase chain reaction(RT-qPCR).Results The levels of Furin(140.84±47.02pg/ml),TGF-β(376.46±82.36ng/L)and VEGF(167.67±53.02ng/L)in EMT group were higher than those in control group(55.49±13.67pg/ml,216.37±15.04ng/L,102.27±8.45ng/L),while the level of netrin-1(48.37±15.20pg/ml)was lower than that of the control group(165.85±15.63pg/ml),with significant differences(t=28.409,20.347,16.915,36.653,all P<0.05).The levels of Furin(192.41±20.62pg/ml),TGF-β(452.61±72.03ng/L)and VEGF(201.84±28.01ng/L)in patients with stage Ⅲ to Ⅳ were higher than those in patients with stage Ⅰ to Ⅱ(78.05±16.54pg/ml,283.75±56.92ng/L,126.07±19.35ng/L),while the level of netrin-1(37.95±11.34pg/ml)was lower than that of stage Ⅰ to Ⅱ(61.05±9.52pg/ml),with significant differences(t=31.071,18.054,19.183,21.625,all P<0.05).Pearson/Spearman correlation analysis showed that Furin was positively correlated with levels of TGF-β and VEGF and clinical stage(r=0.614 9,0.752 6,0.790 5,均 P<0.05),but negatively correlated with netrin-1 level(r=-0.670 1,all P<0.05).The TT genotype and T allele frequencies at r2071410 C/T locus of Furin gene P1 promoter in EMT group(42.16%,55.39%)were higher than those in control group(7.69%,19.87%),while the TT genotype and T allele frequencies at stages Ⅲ to Ⅳ(51.79%,65.18%)were higher than those at stage Ⅰ to Ⅱ(30.43%,43.48%),and the differences were statistically significant(x2=26.500,46.472,4.721,9.626,all P<0.05).The Furin level of patients with different genotypes in EMT group was different(F=51.286,P<0.001),in which the Furin level of TT genotype(216.29±68.53pg/ml)was higher than that of CC(83.04±21.37pg/ml)and CT genotype,(89.18±20.95pg/ml),and the differences were statistically significant(t=27.146,25.719,all P<0.01),but there was no significant difference between CC and CT genotypes(t=1.326,P>0.05).Conclusion The level of Furin in EMT patients was increased,and it was certainly correlated with the levels of TGF-β,VEGF,netrin-1 and clinical stage.Meanwhile,the r2071410 C/T locus in the promoter region of Furin gene P1 showed a polymorphic distribution,in which the Furin level increased more significantly in patients with TT genotype,which may be related to the incidence of EMT in Shenzhen area.
10.Investigation of the Soochow University model of comfortable medical procedures during peri-anesthesia period for patients undergoing radical resection of lung cancer through thoracoscope

Objective:To construct a Soochow University model of comfortable medical procedures during peri-anesthesia period for patients undergoing radical resection of lung cancer through thoracoscope.Methods:In this prospective study, eight hundreds and sixty patients undergoing radical resection of lung cancer through thoracoscope were enrolled in our hospital in 2018.During peri-anesthesia period, the 860 patients were randomly(by the random number table method)divided into the control group(the group C receiving routine medical procedures, n=430)and the comfort group(the group S receiving the comfortable medical procedures of Soochow University model, n=430). The scores of Visual Analogue Scale(VAS), Richmond Agitation-Sedation Scale(RASS), and Thirsty Analogue Scale(TAS)were recorded at 5 min after extubation(T1), at out of the post-anesthesia care unit(T2), at 1 h(T3), 6 h(T4), 12 h(T5), 24 h(T6), 2 d(T7)and 3 d after surgery(T8)in two groups.The incidences of postoperative sore throat(POST), nausea and vomiting(PONV), catheter-related bladder discomfort(CRBD), thirst and hypothermia were recorded at T2, T6 and T8, respectively.Meanwhile, the peri-anesthesia comfort questionnaire(PCQ)and peri-anesthesia satisfaction questionnaire(PSO)were completed at T6 and T8.Results:The incidences of POST(48.6% vs.16.2%), PONV(24.9% vs.13.0%), CRBD(78.8% vs.20.9%)and thirst(74.9% vs.20.0%)were higher in the group C than in the group S at T2( P<0.05). The comfort score and satisfaction score were lower in the group C than in the group S at T6(3.14±1.04 vs.4.92±1.42, 2.67±0.89 vs.3.30±1.01)and at T8(3.84±1.83 vs.5.05±1.77, 2.74±0.84 vs.3.26±1.06)( P<0.05). Conclusions:The Soochow University model of comfortable medical procedures during peri-anesthesia period for patients undergoing radical resection of lung cancer through thoracoscope has been successfully constructed, and this model can reduce the incidence of complications, and improve the comfort and satisfaction during peri-anesthesia period.
