1.Precision scientific literature novelty assessment promotes the development of clinical medicine in the era of precision medicine

2.Efficacy of Capparis Spinosa extracts in the treatment of scleroderma:an experimental study in animal models

Objective To observe the effect of ethanol extract and ethyl acetate extract of Capparis Spinosa on the thickness of dermis,synthesis of collagen type Ⅰ,type Ⅲ,and expression of transforming growth factor-β1 in mouse models of scleroderma.Methods Mouse models of scleroderma were established through local injection of bleomycin on the back once a day for 4 weeks.After confirmation of model establishment,72 mouse models were equally and randomly divided into three groups.Two groups received topical treatment with ethanol extract of Capparis Spinosa and ethyl acetate extract of Capparis Spinosa,respectively,no treatment was given to the rest of the control group.After 2-,4-,6-week treatment,8 mice were sacrificed and tissue samples were obtained from the back,and subiected to the measurement of dermal thickness by HE staining,as well as to the analysis of expression of collagen type Ⅰ,collagen type Ⅲ and transforming growth factor-β1 by immunohistochemical staining.Results On week 2,4,6,the thickness of dermis was 23.22,24.94,19.97 μm respectively in mice treated with ethanol extract of Capparis Spinosa,27.66.26.15,22.13 μm respectively in those treated with ethyl acetate extract of Capparis Spinosa.Compared with the mouse models without treatment,the thickness of dermis significantly decreased(F=12.99,P<0.01),the expression of collagen type Ⅰ(F=7.47,P<0.01)and transforming growth factor-β1(F=11.76,P<0.01)were also inhibited in those receiving treatment.However,the expression of collagen type Ⅲ was not affected obviously by the treatment.Conclusion The ethanol extract and ethyl acetate extract of Capparis Spinosa have the effect against skin fibrosis.
3.Comparison of the Effects of 2 Kinds of Administration Routes of Insulin on Related Indexes of Advanced Age Patients with Gestational Diabetes Mellitus

OBJECTIVE:To compare the effects of repeated subcutaneous insulin administration vs. insulin pump continuous subcutaneous administration on related indexes of advanced age patients with gestational diabetes mellitus(GDM). METHODS:Inretrospective study,120 advanced age patients with GDM were randomly divided into group A(60 cases)and group B(60 cases). Group A was given Insulin aspart injection with initial dose of 0.5 U/(kg·d)subcutaneously before meal,adjusted according to fast-ing blood glucose(FPG)and postprandial 2 h blood glucose(2 hPG),and then given Isophane protamine biosynthetic human insu-lin injection with initial dose of 0.5 U/(kg·d)subcutaneously at bedtime,adjusted according to FPG and 2 hPG. Group B was giv-en Insulin aspart injection with initial dose of 0.5 U/(kg·d)added into insulin pump using 40% of total daily insulin as basic pump,increasing to 60% of total daily insulin if blood glucose control was poor,adjusted according to FPG and 2 hPG. Treatment course of 2 groups lasted for 4 weeks. The time of blood glucose reaching target,the amount of insulin were observed in 2 groups, and the levels of FPG,2 hPG,HbA1c,Hcy and Cys-C before and after treatment,the occurrence of patients and neonates compli-cations. RESULTS:The time of blood glucose reaching standard in group B was significantly shorter than group A;the amount of insulin,the incidence of hypoglycemia,premature birth,excessive amniotic fluid and gestational hypertension in group B were sig-nificantly lower than group A,with statistical significance(P<0.05). The incidence of neonatal respiratory distress syndrome in group B was significantly lower than group A,with statistical significance(P<0.05);but there was no statistical significance in the incidence of giant baby and malformation between 2 groups(P>0.05). After treatment,the levels of FPG,2 hPG,HbA1c,Hcy and Cys-C in 2 groups were significantly lower than before treatment,and the group B was significantly lower than the group A, with statistical significance(P<0.05). CONCLUSIONS:The insulin pump continuous subcutaneous administration is significantly better than repeated subcutaneous insulin administration in respects of controlling glucose level,reducing the amount of insulin,the levels of Cys-C and Hcy,maternal and neonatal complications.
4.Impact of obesity on surgical site infection following colorectal cancer surgery

Objective To evaluated the impact of obesity on surgical site infection (SSI)following colorectal cancer surgery.Methods A total of 215 patients undergoing radical surgery for colorectal cancer in a hospital be-tween January 2008 and December 2013 were investigated retrospectively,100 patients were with body mass index (BMI)≥25 kg/m2 (obesity group)and 115 patients with BMI<25 kg/m2 (normal weight group),the intra-opera-tive and postoperative indicators and surgical complications were compared between two groups.Results In obesity group,72(72.00%)patients had intra-operative blood loss of >60 mL,58(58.00%)patients’duration of surgery was >220 min ,20 (20.00%)of whom developed postoperative SSI;in normal weight group,30(26.09%)patients had intra-operative blood loss of >60 mL,20(17.39%)patients’duration of surgery was >220 min,8(6.96%)of whom developed postoperative SSI.Intra-operative blood loss and incidence of SSI in obesity group were both signif-icantly higher than normal weight group,duration of surgery and postoperative length of hospital stay were both longer than normal weight group(both P <0.05).Conclusion Incidence of SSI in colorectal cancer patients is high. Obesity,long duration of surgery,and more bleeding are high risk factors for SSI in colorectal cancer patients.
5.SUMO Expression and Regulation in Oxidative Stress in Cultured Human Lens Epithelial Cells

Objective To observe the expression of small ubiquitin?related modifiers(SUMO)protein in normal cultured human lens epithelial cells(SRA01/04)and discuss regulation effects of SUMO protein on oxidative stress induced by high glucose. Methods The expression and local?ization of SUMO 1,2/3,4 was detected in normal cultured SRA01/04 cells through immunocytochemistry. The mRNA expression levels of SUMO 1?4 were examined by RT?PCR after the SRA01/04 cells treated with high glucose media at different concentrations and time points. Samples were grouped by medium concentrations(glucoses 5.5 mmol/L,12.5 mmol/L,25 mmol/L,50 mmol/L respectively for 24 h)and by treatment time(0 h, 6 h,12 h and 24 h respectively). After highly efficient transfection of GFP?SUMO2 into SRA01/04 cells,the survival and apoptotic rates of transfect?ed and un?transfected cells treated with high glucose was detected by CCK8 method and AV/PI double staining flow cytometry. Results The immu?nocytochemistry results showed that SUMO1,2/3,4 proteins were mainly located in the nucleus of SRA01/04 cells and part of SUMO2/3 was in the cytoplasm. RT?PCR results showed that compared with the low?glucose group,the mRNA expression of SUMO1?4 was increased along the increas?ing glucose concentration in the high?glucose group(P<0.05). Compared with 0 h,the mRNA expression of SUMO1?4 was enhanced at 6 h,12 h and 24 h(P<0.05)in the high?glucose group treated at 50 mmol/L concentration. Compared with the un?transfected cells,the survival rate was in?creased and the apoptotic rate was decreased in GFP?SUMO2 transfected cells in oxidative stress induced by high glucose(P<0.05). Conclusion SUMO protein was positively expressed in SRA01/04 cells and the expression of SUMO mRNA was affected by oxidative stress induced by high glu?cose.
6.Intranasal delivery of bone marrow mesenchymal stem cells for brain injuries:how many questions to be verified?

BACKGROUND:Transplantation of bone marrow mesenchymal stem cells can promote repair of brain injuries in animals. OBJECTIVE:To summarize the research progress in intranasal delivery of bone marrow mesenchymal stem cells to the brain. METHODS:A computer-based online retrieval of PubMed and Wanfang databases was performed to search papers published during January 1999 to January 2014 with the key words of“bone marrow mesenchymal stem cells, brain injury, transplantation”in English and Chinese. Thirty-eight papers were included in the final analysis. RESULTS AND CONCLUSION:Nowadays, many studies have been certified that the transplantation of bone marrow mesenchymal stem cells can significantly ameliorate the function of cranial nerve in animal models of brain injury. Many researchers have searched for the transplantation methods and approaches and have made progresses in many aspects. In this article, we compare the different transplantation ways of bone marrow mesenchymal stem cells to the brain. We focus on the intranasal transplantation route in the fol owing aspects:processing of the nasal mucosa;delivery route to the brain;labeling and intracranial observation of stem cells;animal experiments. We conclude that the intranasal delivery of bone marrow mesenchymal stem cells to the brain has a wide clinical application as a noninvasive transplantation.
7.Changes in distribution and antimicrobial resistance of Acinetobacter baumannii in 5 consecutive years in an intensive care unit

Objective To investigate the change in distribution and antimicrobial resistance of Acinetobacter baumannii (AB) in an intensive care unit(ICU),and provide basis for rational use of antimicrobial agents in the clinical practice.Methods Using retrospective investigation study,data about pathogenic bacteria isolated from patients who were hospitalized in ICU in 2010-2014 were collected,distribution and antimicrobial resistance of AB were statistically analyzed.Results A total of 3 807 bacterial strains were isolated from ICU patients in 2010-2014,488 (12.82%) of which were AB,isolation rate increased from 6.94% in 2010 to 17.33% in 2014 (x2 =45.58,P<0.01).AB was mainly isolated from sputum,accounting for 72.13%,followed by wound secretion,blood,catheter,urine and so on;AB had the lowest resistance rate to amikacin(<30 %),resistance rates to imipenem and meropenem increased significantly year by year (value of trend x2 test were 42.99 and 53.91 respectively,both P<0.001);resistance rates of AB to other antimicrobial agents were all>50%.Conclusion Detection rate and antimicrobial resistance rate of AB increased year by year,clinical surveillance on bacterial resistance should be paid more attention,patients should be isolated by effective measures,so as to control and prevent the prevalence of AB in ICU.
8.A study of relationship between obstructive sleep apnea-hypopnea syndrome in hypertension and insulin

Objective To investigate the association between the obstructive sleep apnea-hypopnea syndrome(OSAHS) in hypertension and insulin.Methods A total of 521 patients were divided into 4 groups according to apnea-hypopnea index and OSAHS degrees.The control group ( group Ⅰ ),mild OSAHS group ( group Ⅱ ),moderate OSAHS group ( group Ⅲ ) and severe OSAHS group ( group Ⅳ ) had 89 patients,194 patients,118 patients and 120 patients respectively.Results The BMI[( 30.4 ± 3.8 )kg/m2],apnea-hypopnea index ( AHI,3.8 ± 0.1 ),Fasting insulin (FIns)[(3.08 ± 0.26 ) mU/L]and insulin resistance ( 2.43 ± 0.27 ) of patients in severe OSAHS group were significantly higher than that of in the control,mild OSAHS group and moderate OSAHS group ( P < 0.01 ).The levels of saturation of minimum oxygen from skin of patients in severe OSAHS group was significantly lower ( MSpO2 ) than in that of the control,mild OSAHS group and moderate OSAHS group.Multiple linear regression analysis showed that fasting plasma insulin and insulin resistance was positive correlation with apnea-hypopnea index,while they also negatively associated with saturation of minimum oxygen.Conclusions FIns and insulin resistance strongly associate with AHI and levels of saturation of minimum oxygen from skin.Hypertensive patients with OSAHS have more chances to suffer with insulin resistance.
9. Advance in evaluation of lumbar function after long fixation of degenerative scoliosis

With the aging of the population, the incidence of degenerative lumbar scoliosis has increased year by year. Long-segment orthopedic fixation surgery is an important method for the treatment of severe degenerative lumbar scoliosis. Currently, the evaluation of postoperative results is mainly based on the degree of relief of postoperative clinical symptoms, as well as the improvement of imaging deformity and balance. The studies show that although surgery has high difficulty and risk, most patients can benefit from surgery. Besides, it is reported that long-segment fixation can alleviate the symptoms of pain and improve the quality of life. However, it also decreases the local activity of the lumbar spine, leading to stiffness of lumbar, which may affect the activities of daily living (ADL) partly. Lumbar Stiffness Disability Index (LSDI) is a scale for evaluating the impact of lumbar spine stiffness on ADL. The scale has certain limitations and needs to be applied to Chinese people on the basis of optimization. In this paper, the researches of lumbar spine function evaluation after degenerative lumbar scoliosis and long segmental orthopedic fixation and the clinical use of LSDI are briefly reviewed.
10.Assessment of left ventricular mvocardial scar with three-dimensional MRI

ObjectiveTo assess the value of free-breathing 3D phase sensitive inversion recovery (PSIR) turbo FLASH for demonstrating the left ventricular myocardial scar in patients with myocardial infarction.MethodsTwenty-three patients with myocardial infarction underwent 2D and 3D phase sensitive inversion recovery (PSIR) Turbo FLASH sequences 10-15 minutes after injection of dimeglumine gadopentetate.The image quality,the area,location and volume of hyperenhanced scar were independently assessed by two experienced radiologists.Student′s t test or rank sum test and Pearson linear correlation were used for the statistics.ResultsThere were no significant differences in the overall image quality (2.57 ±0.59 vs 2.39 ±0.66,Z=-0.93,P =0.35),the area (24.48 ±10.83 vs 29.00 ± 11.56,Z=-1.41,P =0.16) and location ( 16.78 ± 6.51 vs 18.87 ± 6.76,Z =- 1.17,P =0.24) of hyperenhanced scar between 2D and 3D PSIR sequences.The mean volume of hyperenhanced scar was significantly greater in 3D PSIR sequence than that in 2D PSI R sequence [(23.46 ± 10.61 ) vs (31.65 ± 13.86) cm3,t =-2.25,P =0.03].There was a good correlation in the volume of hyperenhanced scar between 2D PSIR and 3D PSIR sequences (r=0.940,P<0.01 ;y =2.851 + 1.228x,R2 =0.883).ConclusionFree-breathing 3D PSIR Turbo FLASH is a promising new imaging technique for accurate assessment of myocardial scar.
