1.Efficacy and immune of gamma globulin in treatment of dilated cardiomyopathy complicating heart failure *

Objective To observe the anti-cardiac myosin heavy chain IgG changes and efficacy of the antibody titration during the treatment of dilated cardiomyopathy (DCM ) complicating heart failure by a large number of gamma globulin .Methods 28 inpa-tients with dilated cardiomyopathy complicating heart failure were taken as the experimental group ,28 cases of healthy examination as the control group .The anti-cardiac myosin heavy chain IgG antibodies were detected by ELISA ;moreover ,among the patients with heart failure treated by the conventional therapy ,14 cases were selected and added large dose of gamma globulin for observing the curative effect .The remaining 14 cases were taken as the standard control group of this experiment .The anti-cardiac myosin heavy chain IgG antibodies were detected by ELISA .Results The anti-cardiac myosin heavy chain IgG antibodies before gamma globulin therapy in the DCM complicating heart failure group had statistical difference compared with the healthy examination goup (P<0 .05);in the 14 cases of heart failure treated by large dose of gamma globulin ,11 cases (78 .57% ) in the treatment group were improved ,while 5 cases(35 .71% ) in the standard control group were improved ,the anti-cardiac myosin heavy chain IgG antibody had no significant differences between the gamma globulin treatment group and the healthy examination group .The left ventricular ejection fraction(LVEF) in the gamma globulin treatment group was increased from (36 .12 ± 9 .98)% to (46 .15 ± 12 .18)% ,the left ventricular end-diastolic diameter was decreased from (59 .68 ± 9 .60) mm to (50 .05 ± 10 .20) mm ,the effects after gamma glob-ulin treatment were more significant than the conventional traditional control group .Conclusion Large dose of gamma globulin for treating dilated cardiomyopathy complicating heart failure has more significant effect than the traditional method ,reduces the titra-tion of in vivo anti-cardiac myosin heavy chain IgG antibody and contributes to explain the drug action mechanism from immune mechanism .
2.Effects of myocardial PPAR-gamma expression change on reperfusion arrhythmias in ischemia/reperfusion rats

Objective To investigate the effects of PPAR-gamma expression on reperfusion arrhythmias in different precon ditioning myocardial ischemia/reperfusion(I/R) animal model.Methods Thirty-two SD rats were randomly divided into 4 groups(n =8):rosiglitazone+ I/R group (ROS group),GW9662 + I/R group(GW group),I/R group and sham group (Sham group).The I/ R animal model was constructed by ligation of the left anterior descending coronary artery,with ischemia for 30 min and reperfusion for 120 min.The dynamic limb Ⅱ lead electrocardiogram monitoring was performed;PPAR-gamma mRNA was detected by fluorescent quantitative PCR and the change of PPAR-gamma protein expression was detected by Western blot.Results The increasing range of QRS wave width detected before operation,at 30 min of ischemia,at 1,2 h of reperfusion from large to small were in turn the ROS group,I/R group,GW group and Sham group;the reperfusion arrhythmia score in the ROS group was significantly higher than that in the other groups(P<0.05),while the GW group was relatively reduced.The expression level of PPAR-gamma mRNA in the ROS group detected by fluorescent quantitative PCR was significantly up-regulated(P<0.05),while which in the GW group was down-regulated compared with the I/R group and ROS group(P<0.05).The expression of PPAR-gamma protein was similar to that of PPAR-gamma mRNA.Conclusion Up-regulation of myocardial PPAR-gamma expression may increase the occurrence of reperfusion arrhythmias in myocardial I/R animal model.
3.Relationship of serum levels of homocysteine,cystain-C and serum uric acid with carotid atherosclerosis in H-type hypertension patients

Objective To investigate the relationship of serum levels of homocysteine, cystain C and uric acid with ca?rotid atherosclerosis in H-type hypertension patients. Methods A total of 132 H-type hypertension patients were collected to be studied. Their carotid intima-media thickness (IMT) was measured by Doppler ultrasonography. And according to the result of carotid artery atherosclerosis, all patients were divided into normal cIMT group (n=40),thickened cIMT group (n=43) and plague formation group (n=49). Their serum Hcy, Cyst-C, UA, blood glucose, blood lipid, blood urea nitrogen and se?rum creatinine were compared among three group and their relationship with cIMT were analyzed. Logistic regression was used to screen risk factors for carotid atherosclerosis. Results There was no significant difference in serum levels of blood glucose,blood urea nitrogen, creatinine, serum creatinine (Scr), triglyceride (TG), total cholesterol (TC), low density lipppro?tein cholesterol (LDL-c) among these three groups (P>0.05). The level of high-density lipoprotein (HDL-c) in thickened cIMT group was higher than that in plague formation group, and lower than normal cIMT group, while both serum levels of Cyst-c and UA were lower in thickened cIMT group than those in plague formation group but higher than those in normal cIMT group (P<0.05). In addition, serum level of Hcy in normal cIMT group was higher than that in thickened cIMT group and plague formation group. The cIMT grade was positively correlated with serum levels of Hcy, Cyst-C and UA (r=0.26, 0.30, 0.23, P<0.05), but was negative correlated with HDL-c(r=-0.38, P<0.05). Further more, Logistic regression analy?sis showed that Hcy,Cyst-C and UA were independent risk factors for cIMT. Conclusion Serum levels of Hcy, Cyst-C and UA are closely related to the cIMT,which indicates that they are independent risk factors of cIMT and may be used as mark?ers in judging the developments and preventions of arteriosclerosis.
4.Effects of PPARγon malignant arrhythmia in myocardial ischemia/reperfusion model rats

Objective To investigate the effects of peroxisome proliferator activated receptor gamma (PPARγ) on malignant arrhythmia in myocardial of ischemia/reperfusion(I/R) rats. Methods Twenty-four SD rats were randomly divided into four groups:Sham group, I/R group, rosiglitazone (ROS) group and PPARγ inhibitor GW9662 (GW) group. The myocardial I/R injury was induced by ligation of the left anterior descending coronary artery, with ischemia for 30 min and reperfusion for 2 h. The whole process limb Ⅱ lead electrocardiogram was applied to observe the frequency of malignant arrhythmia and record the corrected changes of QT(QTc) interval. RT-PCR was used to detect the expression of PPARγ mRNA. Results There were 5 cases of malignant arrhythmia in ROS group, 2 in I/R and 1 in GW group, 0 was found in Sham group. There was a prolongation of the QTc interval in ROS group than the other groups after ischemic stage (P<0.05). Compared with I/R group and ROS group, the QTc intervals were shorten in ischemia 30 min and reperfusion process in GW group (P<0.05). Compared with sham group, the expression of PPARγ mRNA was significantly increased in other three groups (P<0.05). The expression level of PPARγ mRNA was the highest in ROS group. The expression level of PPARγ mRNA was reduced in GW group compared with that of I/R group and ROS group (P<0.05). Conclusion Over expression of PPARγ may lead to the occurrence of malignant arrhythmia in myocardial ischemia/reperfusion rats.
5.Expression of hepatitis B virus genes in early embryonic cells originated from hamster ova and human spermatozoa transfected with the complete viral genome.

AIMTo detect the expression of hepatitis B virus (HBV) genes (HB S and C genes) in early embryonic cells after introducing motile human sperm carrying HBV DNA into zona-free hamster oocytes via the in vitro fertilization (IVF) technique.
METHODSHuman sperm-mediated HBV genes were delivered into zona-free hamster oocytes by the IVF method. Polymerase chain reaction (PCR) was used to detect HB S and pre-Core/Core (pre-C/C) coding genes both in one- and two-cell embryos. Reverse transcription-PCR (RT-PCR) analysis was used to study the expression of the two genes. Fluorescence in situ hybridization (FISH) analysis using the full-length HBV DNA as the hybridization probe was performed to confirm the integration of viral DNA in the host embryonic genome.
RESULTSBoth HB S and pre-C/C coding genes are present and transcribed in one- and two-cell embryos originated from hamster ova IVF with human spermatozoa carrying HBV DNA sequences.
CONCLUSIONSperm-mediated HBV genes are able to replicate and express themselves in early embryonic cells. These results provide direct evidence that HBV DNA could transmit vertically to the next generation via the male germ line.

6.QUADAS-C—A tool for assessing risk of bias regarding Quality Assessment of Diagnostic Accuracy Studies-Comparative

This paper introduced the Quality Assessment of Diagnostic Accuracy Studies-Comparative (QUADAS-C), illustrated the comparison with the QUADAS-2, and using QUADAS-C together with QUADAS-2 to present QUADAS-C results through systematic reviews. Like the domain for QUADAS-2, QUADAS-C retained four domains, including patient selection, index test, reference standard, flow, and timing, and comprised additional questions for each QUADAS-2 part. Unlike the QUADAS-2 tool, the starting question of each domain for QUADAS-C was designed to summarize the risk of biased information captured by QUADAS-2. QUADAS-C only dealt with the risk of bias but did not include the part of concerns regarding applicability. The answers to signaling questions for each domain of QUADAS-C would lead to a 'low''high' or 'unclear' risk of biased judgment for the original study.
7.Epidemics study of acute kidney injury on patients in dermatology department

Objective To assess the epidemiology and clinical correlates of acute kidney injury(AKI)on patients in department of dermatology. Methods A total of 4710 patients hospitalized in dermatology department with at least two creatinine tests within 7 days were selected as research objects.AKI was defined and staged accord-ing to Kidney Disease Improving Global Outcomes criteria. Results The incidence of AKI was 6.3%. AKI inci-dences of psoriasis with systemic reaction(14.8%),erythroderma(12.8%),drug eruption(12.6%)and systemic lupus erythematosus (12.5%) were significant higher than the total incidence. In multivariate logistic regression model,these 4 skin diseases were independent risk factor of AKI incidence. The OR(95% confidencel interval) was 2.970(2.047~4.310),1.932(1.005~3.717),2.446(1.752~3.415)and 2.254(1.287~3.946). The in-hospital mortality was much higher in patients with AKI than those without(3.1% vs 0.1%,P < 0.001). And the AKI was related to in-hospital death after adjusted by age and comorbidities(Odds ratio 24.630,95%confidencel interval 7.385~82.149 ). Conclusion AKI is common in patients hospitalized in department of dermatology and is associated with patiensts′medical burden ,which shoule be noticed.
8.Incidence and risk factors of hyperkalaemia after acute kidney injury in cardiology department

Objective To investigate the incidence of hyperkalaemia and analyze the risk factors in pa-tients with acute kidney injury(AKI)in cardiology department.Methods We conducted a retrospective case-con-trol study,using the electronic medical information of the patients in Guangdong General Hospital. Results A to-tal of 37 837 patients were included in this study and 1 571(4.3%)patients with AKI were detected.Hyperkalae-mia occurred in 517 patients(1.4%).The incidence of hyperkalaemia in AKI patients was higher than that in non-AKI patients(10.1% vs. 1.0%,P < 0.001)and the incidences of hyperkalaemia at AKI stage 1~3 were 2.6 %, 13.9% and 20.6 %,respectively. Multiple logistic regression analysis demonstrated that AKI stages,chronic kid-ney disease and heart failure were risk factors for hyperkalaemia.AKI and hyperkalaemia were related to increased hospitalization expenses,delayed hospital stay,renal replacement therapy and in-hospital mortality. Based on AKI,the combination of hyperkalemia could significantly increase clinical burden and adverse outcomes. Conclu-sion In cardiology department,the monitoring of serum creatinine and potassium level should be emphasized.
9.Compound ANBP promotes diabetic wound healing by regulating TCA circulation-related enzymes

AIM:To investigate the effects of the compound ANBP on wound healing in diabetic rats and ex-plore its mechanism of action.METHODS:Ninety male SD rats were randomly divided into blank,model,compound ANBP,Beifuxin,and nicotinamide mononucleotide(NMN)groups,with 16 rats in each group.Wound healing in each group was observed and samples were taken on days 3,7 and 14 to analyze the wound healing rate.Local histopathological changes were observed using HE and Masson staining.The expressions of pyruvate dehydrogenase E1 subunit alpha 1(PDHA1),citrate synthase(CS),isocitrate dehydrogenase(IDH1)and oxoglutarate dehydrogenase(OGDH)were de-tected through immunofluorescence and Western blot.The number and morphology of mitochondria in the wound tissue were observed using transmission electron microscopy.RESULTS:Histomorphological changes revealed significant im-provement in diabetic wound healing in the blank and compound ANBP groups compared to that of the model group.The wound healing rates of the blank,compound ANBP,Beifuxin,and NMN groups were significantly increased on days 3,7,and 14(P<0.01).Compared to the model group,granulation tissue generation was higher in the other groups,cover-ing the wound defect and producing abundant collagen fibers.At 3,7,and 14 days after intervention,the blank,com-pound ANBP,Beifuxin,and NMN groups showed significantly enhanced fluorescence intensities of TCA cycling-related enzymes PDHA1,CS,IDH1,and OGDH indicating increased expression of these enzymes.The levels of the TCA cy-cling-related enzymes were significantly increased(P<0.01)in the compound ANBP,Beifuxin and NMN groups but were significantly decreased(P<0.01)in the model group.An increase in the number and density of mitochondria and a de-crease in the cavitation rate of mitochondria with improved morphology(P<0.05)was observed in the group treated with compound ANBP.CONCLUSION:Compound ANBP may increase the number of mitochondria,improve mitochondrial morphology and function,upregulate the expression levels of PDHA1,CS,IDH1,and OGDH proteins,and accelerate the regeneration of wound granulation tissue,thus promoting the healing of diabetic wounds in rats.

Chimeric antigen receptor T (CAR-T) cell has achieved excellent efficacy in hematological tumors, especially for lymphoma. Many products have been approved to market all over the world, and 2 products targeting CD19 have been approved to treat relapsed and refractory large B-cell lymphoma in China. The current experiences of using CAR-T cells come from previous clinical studies. How to use CAR-T cells in a standardized and rationalized way is still a challenge faced by our clinicians. Based on the CAR-T cell treatment experiences from Peking University Cancer Hospital and the latest research progresses in CAR-T in China and abroad, this article will elaborate on patient screening, peripheral blood mononuclear cell collection, bridging treatment, lymphocyte depletion chemotherapy, CAR-T cell infusion, the monitoring and treatment of adverse events after infusion, and long-term follow-up after infusion, in order to guide clinicians to better use CAR-T cell and to bring maximum benefits to patients.
