1.Investigation of Viral Hepatitis Seroepidemiology by Using Protein Microarray

Objective The anti-HCV positive results obtained from ELISA assay was confirmed by using protein microarray, and then the difference of detection rate of different antibody function domains was studied through observing low risk population. Methods The HCV mixing antigen, core area antigen, NS3 domain antigen, NS4 domain antigen, NS5 domain antigen were spotted on the slide. The 48 serum samples, which was positive HCV detected by using ELISA assay, were chosen from the specimens applying to investigation of viral hepatitis seroepidemiology in 2001, and furthermore the antibodies containing different function domain were detected by using protein microarray. Results The negative results were obtained from different domains of 15 samples. Then 1-4 antibodies from 33 samples were positive. The total detection rate of anti-C, anti-NS3, anti-NS4, anti-NS5 was 51.5%, 51.5%, 48.5% and 57.6% respectively. In these above statistics, the detection rate of anti-C from Zhoushan, Quzhou, Wenzhou, Shaoxing was 53.3%, 33.3%, 44.4%, 66.7% respectively; the detection rate of anti-NS3 was 33.3%, 100%, 55.6%, 66.7% respetively; the detection rate of anti-NS4 was 60.0%, 0%, 33.3%, 66.7% respectively; 抗the detection rate of anti-NS5 was 66.7%, 0%, 44.4%, 83.3% respectively. Conclusion The difference of distribution of different antibody function domains existed through studying low risk population. Moreover, the protein microarray assay was highly specific for detecting and identifying HCV antibody in low risk population.
2.Design of mobile ECG monitoring microsystem terminal

This paper introduces the design method of mobile ECG monitoring system terminal for healthcare at home. The adoption of active electrode and digital trap contributes to the elimination of the external interference. Low-power-consumption MSP430 micro-controller applied, the realtime acquisition, procession and transmission to PDA of ECG are performed.
3.Application of Table Tennis for Mentally Retarded Children

4.Factors related to the therapeutic efficacy of 131 Ⅰ in Graves' disease

ObjectiveTo explore the efficacy of 131 Ⅰ treatment in Graves' disease,and to analyze the related factors.MethodIn 87 patients with Graves' disease,thyroid uptake ratio( TUR ) and its effective half-life(EHL) were compared before and after 131 Ⅰ treatment.The weight of thyroid gland was evaluated with radio-imaging and type B ultrasonography.ResultThe dose of 131 Ⅰ was ( 185.2 ± 148.0 ) MBq.The TUR of tracer dose and therapeutic 131 Ⅰ dose were 76.5 % ±8.2% and 73.3 % ±9.0% ( t =2.451,P =0.008 ).The EHL were ( 5.2±0.7 ) and ( 5.0 ±0.8 )days,respectively ( t =1.998,P =0.023 ).After followe-up of ( 57.0 ±26.3 ) months,49 patients ( 56.3 % ) became euthyroid,14 ( 16.1% )manifested delayed hypothyroidism,and 24 (27.6%)remained in hyperthyroidism.Thyroid autoantibodies were found in 34.5% patients,of whom,the incidence of hypothyroidism was higher in patients with positive autoantibodies than those with negative ones (30.0% vs 8.8%,x2 =6.560,P =0.009 ).ConclusionBoth TUR and EHL of therapeutic doses of 131 Ⅰ are lower than the tracer doses.Positive thyroid autoantibodies may affect the outcome of the 131 Ⅰ treatment.
5.Detection of K-ras gene mutation in pancreatic juice for diagnasis of pancreatic cancer:a meta-analysis

Objective To systematically assess the detection of K-ras gene mutation at codon 12 in pancreatic juice for diagnosis of pancreatic cancer.Methods A comprehensive electronic searching was performed to retrieve relevant studies on K-ras gene mutation in pancreatic juice for diagnosis of pancreatic cancer.Data on accuracy of included studies were extracted,Meta-DiSc1.4,Stata10.0,was applied for further heterogeneity exploring,meta-analysis and publication bias testing.Results Fifteen studies met the inclusion criteria.Heterogeneity was not found among these studies,including threshold effect,different test methods,randomization and blind.Pooled accuracy indicators like sensitivity,specificity and diagnostic odds ratio(DOR) were 0.61(95%CI 0.56-0.66),0.82(95%CI 0.77-0.86) and 6.28(95%CI 4.42-8.91),respectively.Area under curve(AUC) of SROC(summary receiver operating characteristics) was 0.8207 and Q index was 0.7542.Publication bias had little influence on meta-analysis with the fail-safe number of 694.0108.Conclusions This meta-analysis demonstrates that the detection of K-ras gene mutation in pancreatic juice has moderate effect for diagnosis of pancreatic cancer.It is not enough to be an early diagnosing or screening indicator for pancreatic cancer,but it can be used clinically as an important adjunct to cytology,imageology and enzymology in the diagnosis of pancreatic cancer.
6.Method for Extracting Vascular Perfusion Region Based on Ultrasound Contrast Agent.

Vascular perfusion distribution in fibroids contrast-enhanced ultrasound images provides useful pathological and physiological information, because the extraction of the vascular perfusion area can be helpful to quantitative evaluation of uterine fibroids blood supply. The pixel gray scale in vascular perfusion area of fibroids contrast-enhanced ultrasound image sequences is different from that in other regions, and, based on this, we proposed a method of extracting vascular perfusion area of fibroids. Firstly, we denoised the image sequence, and then we used Brox optical flow method to estimate motion of two adjacent frames, based on the results of the displacement field for motion correction. Finally, we extracted vascular perfusion region from the surrounding background based on the differences in gray scale for the magnitude of the rich blood supply area and lack of blood supply area in ultrasound images sequence. The experimental results showed that the algorithm could accurately extract the vascular perfusion area, reach the precision of identification of clinical perfusion area, and only small amount of calculation was needed and the process was fairly simple.

7.Experimental study of suture of core long-distance buried for repair of flexor tendon rupture

ObjectiveTo adopt a new suture technique the core long-distance buried for repair of flexor tendon rupture which was designed by author and could lessen adhesion.Methods60 human cadaver flexor digitorum tendons were divided into 3 groups and sewed by core long-distance buried suture, Kessler's technique and Bunnell's technique respectively. Instron-testing machine was used to test gap formation force, 2 mm gap formation force and maximum load. 30 hen second and third toe flexor profundus tendons were divided into 2 groups, each group of tendons were transected and repaired by core long-distance buired suture and Kessler's technique respectively. Information of tendon healing and tendon adhesion was observed in the 3rd, 4th and 6th weeks after operation.ResultsIn the group of using core long-distance buried suture technique, the gap formation, 2 mm gap formation and maximum load in were higher than that using Kessler's technique and Bunnell's technique, the effects of lessening adhesion and promoting tendon healing were better than that in the Kessler's technique group.ConclusionCore long-distance buried suture can offer great tensile strength needed by clinic, and allow early motion and lessen adhesion after operation.
8.Algorithm of real-time QRS complex detection for the GPRS mobile ECG telemonitoring system

Objective To develop a real-time QRS complex detection algorithm of dynamic ECG signals for the GPRS mobile telemonitoring system. Methods Before the first and second derivatives of ambulatory ECG signals were processed by moving average method, the signals sampled from CM5 monitoring lead were filtered with the average of continuous four ECG sample signal points. The R waves could be detected precisely by local minima of second derivatives and Q & S waves were located correctly by cross-zero points of first derivatives of ambulatory ECG signals in a short-time searching windows. The QRS recognition thresholds, which could revise themselves according to the detected values and vary with the analyzing signals, were designed in this paper. Results With a polynomial computation complexity, the novel algorithm insensitive to baseline draft and noise caused by mobile communication filtered power-line interference and most of muscle noise and reduced the search time below 0.02 s during detecting each Q wave, R wave and S wave. For the normal and clinical patients, this algorithm correctly detected up to 99.8% of the QRS complex of ambulatory ECG signals. Conclusion The algorithm can meet the need of real-time QRS complex detection and analysis for the GRRS mobile ECG telemonitoring system.
9.A case-control study on risk factors for abnormal liver function in middle-aged and elderly adults in Shanghai

Objective To investigate certain risk factors for and their impact on abnormal liver function in middle-aged and elderly adults.Methods A case-control study was constructed based on the SAGE cohort of 8642 registered residents aged 50 years or over in Shanghai.Of them,137 individuals with abnormal liver function,defined as aspartate transaminase (AST)> 40 U/L or alanine aminotransferase (ALT)> 40 U/L,were randomly selected as the observation group,while 411 healthy controls were 3 ∶ 1 matched with the cases in the observation group by gender and age (1 year).Face-to-face administered questionnaires and physical examinations were conducted and serum samples were tested for ALT,AST,glucose (GLU),total cholesterol (TC),triglycreide (TG),hepatitis B surface antigen (HbsAg) and anti-hepatitis C virus antibody (anti-HCV Ab).Chi square test and rank sum test were used for single factor analysis,and logistic regression analysis was used for multiple factors.Results The prevalence of HBsAg positive patients was 12.4 % (68/548) Univariate analysis showed that hepatitis virus infection and body mass index (BMI) were associated with abnormal liver function (both P<0.000).Multivariate logistic regression analysis showed that hepatitis virus infection (OR=1.85,95% CI:1.04 3.29,P-0.036) and obesity (OR=3.60,95%CI:1.92-6.73,P<0.001) increased the risk of abnormal liver function,whereas chronic medication (OR=0.51,95% CI:0.32-0.80,P =0.004) decreased the risk of abnormal liver function.Conclusions Among the study population,hepatitis virus infection and obesity are risk factors for abnormal liver function in middle-aged and elderly people.After adjustment for potential confounders,chronic medication is negatively correlated with abnormal liver function and may be a protective factor for liver function.

OBJECTIVE To realize the timeliness of the mobile phone and network during the application of the infectious disease reporting and supervision,and the preciseness of the infectious disease forecast,thus build the automatic reporting and supervision system of the infectious disease in hospital with the military and local features.METHODS Using the existing iatrical data and computer resources with telecommunication wide band network and the special mobile phone resources to construct the reporting and supervision system of the epidemic situation.Adopting B/S framework,the downstage supporter adopted the comprehensive technology with Net,J2EE,and Java,the backstage supporter adopted the large scale database of Oracle,using software and NTFS with code technology to make epidemic situation reporting and supervise the network.RESULTS This system might realize the most important function of the infectious disease with timely reporting,precise filling,automatic alarming,dynamic supervision and automatic statistical calculation.CONCLUSIONS This system may provide the important practical value for the epidemic situation reporting and supervision and the abrupt public medical accidents forecast.
