1.Molecular epidemiology of TEM and SHV extended-spectrum ?-lactamases in Klebsiella pneumoniae and Escherichia coli clinical isolates from Chengdu,China

Objective To investigate TEM and SHV ESBLs in Klebsiella pneumoniae and Escherichia coli clinical isolates from West China Hospital of Sichuan University and detect resistance of ESBLs-producing isolates. Methods The TEM and SHV ESBLs-encoding gene was amplified by PCR and was sequenced. And the MIC of eight antibiotics against the ESBLs-producing strains were detected by agar dilution. Results All strains were resistant to cefotaxime; eleven strains were resistant to aztreonam; two were resistant to ceftizidime; eleven, five and three were resistant to ciprofloxacin, amikacin and cefoxidine respectively; All strains were susceptible to imipenem. Ten strains of twelve ESBLs-producing strains carried bla SHV-2, two carried bla TEM-19. Conclusions ESBLs producers were mainly resistant to cefotaxime and aztreonam and most of them were multi-drug resistance; Cefotaxime resistance is partially due to production of SHV-2 and TEM-19 in this study.
2.Analysis of Drug-Resistance of Acinetobacter Baumannii Producing AmpC Enzyme

Objective To investigate the drug-resistance and distribution of 142 strains acinetobacter baumannii.Methods Drug-resistance test of acinetobacter baumannii strains was observed in 13 kinds of antibiotics.The drug sensitivity tests was performed by the method of Kirby-Bauer paper-diffusion with the standard of NCCLS.AmpC enzyme was examined by cefoxitin three dimension test and PCR amplification of ampC structure gene were studied.Results The main sources of specimen were sputum,wound secretion,urine and blood.The respiratory tract was a major site to the development of acinetobacter baumannii.Acinetobacter baumannii strain emerged mostly in the intensive care unites.The drug resistance to cefotaxime,ceftriaxome and aztreonam were high.PCR amplification showed that of 142 acinetobacter baumannii strains,23 strains had ampC structure gene which accounted for 16 2% total strains.The drug resistance of acinetobacter baumannii producing AmpC enzyme were significantly higher than those of non-producing AmpC.The best choice of treatment was imipenem.Conclusions Acinetobacter baumannii have higher multiple-antibiotic resistance,the finding prompting us to project prospective control strategies.

Hepatitis B-related acute-on-chronic liver failure (ACLF) is a severe clinical syndrome characterized by jaundice, coagulopathy, ascites and hepatic encephalopathy and with a high mortality rate of 65% to 93%. It involves significant ethical issues when a randomized, double-blinded, placebo-controlled clinical study is conducted to such a serious disease. Therefore, a prospective cohort study design was utilized to explore a new treatment modality of applying integrated traditional Chinese and Western medicine.

This study took geo-authentic medicinal material Rhizoma Coptidis as an example and analyzed soil characteristics of the growing environment of Rhizoma Coptidis . The methods of ISSR, HPLC, 1H-NMR and NIR were respectively used for exploring its genotype and investigating on its chemitype . In the respects of an-ti-endotoxin, anti-bacterial activities and insulin resistance that was related to the treatment of diabetes, the pharmacodynamic type was studied and the relationship was comprehensively analyzed . The quality evaluation of geo-authentic medicinal materials was preliminarily established based on ecotype , genotype chemitype and phar-macodynamic type of Rhizoma Coptidis . Simultaneously, quality standards of Rhizoma Coptidis were set up in combination with genuine character , which provided the scientific basis for establishing Chinese medicine char-acteristic methods of quality evaluation of Rhizoma Coptidis .
5.High Frequency Convex Ultrasound Probe in the Diagnosis of Ovarian Tumor in Infants and Young Children

Purpose To explore the value of high frequency convex ultrasound probe in the diagnosis of ovarian tumor in infants and young children. Materials and Methods Sixty-nine infants and young children with ovarian tumor were examined with high frequency convex ultrasound probe, and the ultrasound findings were analyzed. Results Out of 69 cases, 45 had tumors on the right side of ovary and the rest 24 on the left side;27 cases of simple cyst, 24 cases of benign teratoma, 9 cases of borderline teratoma, 3 cases of serous cystadenoma, 3 cases of mucous cystadenoma, 3 cases of yolk sac tumor;24 accompanied with torsion or necrosis. The ultrasound of ovarian tumor presented the following features:polycystic structures were common;the strong echo posterior acoustic shadow was not obvious; no specific manifestation was shown to distinguish benign from malignant tumors. Doppler ultrasound of tissues around the tumor was significant in detecting ovarian tumor torsion and necrosis. Conclusion High frequency convex ultrasound probe can provide clear images and distinct dissection information, which is beneficial in diagnosis and differential diagnosis of ovarian tumor in infants and young children.
6.A case-control study on risk factors for abnormal liver function in middle-aged and elderly adults in Shanghai

Objective To investigate certain risk factors for and their impact on abnormal liver function in middle-aged and elderly adults.Methods A case-control study was constructed based on the SAGE cohort of 8642 registered residents aged 50 years or over in Shanghai.Of them,137 individuals with abnormal liver function,defined as aspartate transaminase (AST)> 40 U/L or alanine aminotransferase (ALT)> 40 U/L,were randomly selected as the observation group,while 411 healthy controls were 3 ∶ 1 matched with the cases in the observation group by gender and age (1 year).Face-to-face administered questionnaires and physical examinations were conducted and serum samples were tested for ALT,AST,glucose (GLU),total cholesterol (TC),triglycreide (TG),hepatitis B surface antigen (HbsAg) and anti-hepatitis C virus antibody (anti-HCV Ab).Chi square test and rank sum test were used for single factor analysis,and logistic regression analysis was used for multiple factors.Results The prevalence of HBsAg positive patients was 12.4 % (68/548) Univariate analysis showed that hepatitis virus infection and body mass index (BMI) were associated with abnormal liver function (both P<0.000).Multivariate logistic regression analysis showed that hepatitis virus infection (OR=1.85,95% CI:1.04 3.29,P-0.036) and obesity (OR=3.60,95%CI:1.92-6.73,P<0.001) increased the risk of abnormal liver function,whereas chronic medication (OR=0.51,95% CI:0.32-0.80,P =0.004) decreased the risk of abnormal liver function.Conclusions Among the study population,hepatitis virus infection and obesity are risk factors for abnormal liver function in middle-aged and elderly people.After adjustment for potential confounders,chronic medication is negatively correlated with abnormal liver function and may be a protective factor for liver function.
7.Recent research on programmed cell death in bacteria.

Similar to multicellular animals, single-cell organisms, such as bacteria show the phenomenon of programmed cell death (PCD). The PCD not only can play an important role in various physiological procedures, but also can eliminate bacteria with irreversible injuries. The PCD of single cell in a colony is for the benefits of other bacteria in the same colony to achieve the development and reproduction of the whole colony. Disturbing or destroying such PCD may provide a new way for antibiotic drug research and development.

Objective To investigate the expression of osteopontin (OPN)and matrix metalloproteinase-2(MMP-2)in hepatic alveolar echinococcosis(HAE) ,and explore the role of OPN and MMP-2 in the invasion and metastasis of HAE infection .Methods Expression of OPN and MMP-2 in HAE tissues from 69 patients within12 nomral liver tissues .HAE were detected using SP im-munohistochemical technique ,the correlation between OPN and MMP-2 expression and clinieopahtologic features was analyzed .Re-sults OPN and MMP-2 mainly distributed in the cyst wall about granuloma inflammatory cells of HAE and and the peripheral por-tion of stromal cell and liver cells ,the positive expression rate in HAE focus and normal liver tissue at the juncti-on of the invasive margin the most obvious .The results showed that the positive rates of OPN and MMP-2 in HAE tissue were 72 .5% (50/69)and 59 .4% (41/69) ,respectively ,and in normal liver tissue were 16 .7% (2/12) and 8 .3% (1/12) ,respectively .The positive rates of OPN and MMP-2 in HAE were significantly higher than those in normal liver tissue(P<0 .01) .The positive rates of OPN and P21 in HAE tissues with metastasis were 86 .8% (33/38)and 76 .3% (29/38) ,respectively ,which were significantly higher than those without metastasis of OPN and MMP-2 positive expression rate of 54 .8% (17/31) and 38 .7% (12/31) ,(P<0 .01) .The positive expression of OPN and MMP-2 were not related to the size of tumor bulk ,HBsAg ,gender ,age and nation of tumor .Rank correlated analysis showed that OPN and MMP-2 were positive correlated(r=0 .36 ,P<0 .01) .Conclusion OPN and MMP-2 mainly distrib-utes in the cyst wall about granuloma inflammatory cells of HAE and and the peripheral portion of liver cells ,the positive expres-sion rate in HAE and normal liver tissue at the junction of the invasive margin the most obvious ,which might be invasion and me-tastasis of HAE .
9.Effectiveness and safety of submaximal angioplasty and stenting for patients with severe carotid artery stenosis before CABG

Objective:To investigate the feasibility and safety of submaximal balloon dilation and to perform small-diameter stent for symptomatic carotid artery severely stenosis before coronary artery bypass grafting(CABG).Methods:From January 2016 to December 2019, 30 patients of the Department of Neurointervention in Beijing Anzhen Hospital with symptomatic carotid artery stenosis(≥70%) and the left main trunk or triple-vessel of coronary artery disease were analyzed retrospectively. General information, clinical characteristics, and imaging data of all cases were collected. All patients underwent submaximal balloon dilation and small-diameter stent implantation. Preoperative comorbidities or risk factors included hypertension 23 cases(76.7%), diabetes 10 cases(33.3%), hyperglycemia 14 cases(46.7%), moking 13 cases(43.3%). Left main trunk disease 6 cases(20.0%), three-vessels disease 24 cases(80.0%), mitral regurgitation 1 case(3.3%), stable angina 25 cases(83.3%), myocardial infarction 8 cases(26.7%), cerebral infarction 24 cases(80.0%) and transient ischemia attack(TIA) 6 cases(20.0%) caused by ipsilateral carotid artery stenosis. The median National Institutes of Health Stroke Scale(NIHSS) score was 2(0-3), and the median modified Rankin Scale(mRS) score was 1(0-1) before the operation. The mean interval between carotid artery intervention and CABG was(23.4±8.2)days.Results:29 cases(96.7%, 29/30) underwent CAS-CABG operation successfully. In one case of carotid artery extreme tortuosity, the emboli protective device could not place the distal carotid artery. In the operative procedure, 27 cases(90.0%, 27/30) underwent with 3mm diameter balloon, only 3 cases(10.0%) with 3 mm balloon after pre-dilatation with 2 mm diameter balloon because of severely high-grade stenosis(99%). 25 cases(83.3%) with 7mm diameter stents and 5 cases(16.7%) with 6 mm diameter stents, including 22 cases(73.3%) with a closed-cell stent and 8 cases(26.7%) with an open-cell stent. In the perioperative period, the heart rate of two patients was lower than 50 BPM during operation and returned to normal after using atropine immediately. Another patient presented with chest tightness during interventional therapy. TNI elevation was examined urgently. After oxygen inhalation and intravenous infusion of Nitroglycerin, the patient's symptoms improved rapidly. No cardiac and cerebrovascular complications occurred during the perioperative period of CABG, no cardiac-related complications occurred within 30 days of follow-up, one case of TIA and 1 case of cerebral infarction. After intensive anti-platelet aggregation and lipid-lowering treatment, two patient's symptoms improved. There were no death cases in all patients during carotid artery interventional therapy, perioperative CABG and 30-day follow-up. Thirty days later, we performed a clinical follow-up of 23 cases, median 4.5(3.0-7.9) months, mRS Score Median 1(0-1). One patient presented with TIA, any patient had no symptoms of the cardiac or nervous system. Image follow-up of 17 cases, median 3.5(2.8-4.5) months, carotid artery ultrasound showed in-stent restenosis(stenosis rate>50%) in 1 case, the patient was asymptomatic restenosis, continue treatment of aggressive anti-platelet and lipid-lowering drugs.Conclusion:Submaximal balloon dilation and performing small-diameter stent for symptomatic carotid artery severely stenosis before CABG is safe and feasible, which could not only reduce the incidence of vagus reflex resulted in acute coronary syndrome during carotid artery stenosis intervention but also morbidity of acute ischemic stroke events during CABG.
10.Effects of leaf removal on growth and physiological characteristics of Rehmannia glutinosa.

Effects of the leaf-clipped treatment on growth and physiological properties of Rehmannia glutinosa were studied. Result showed that with the increase of the cutting leaves degree, growth rates of shoots and roots were decreased, sugar contents declined, chlorophyll contents decreased and root activities also inhibited. Compared with the normal plant (CK), the root inhibit rates in T1, T2, T3 were 17.53%, 33.41%, 59.47%, respectively. Physiological indexes including chlorophyll contents, root activities and sugar contents also were impacted by the leaf-clipped treatment. The results indicate that to balance source-sink relationship is a very essential method for improving production of R. glutinosa.
