1.Investigation and analysis of sub-health status in young and middle-aged intellectuals in Guangxi

Objective To investigate the status of sub-health in young and middle-aged intellectuals in Guangxi,and to provide a reference basis for improving the health status of young and middle-aged intellectuals. Method Convenient random sampling was used to select 3862 young and middle-aged intellectuals in Guangxi and the sub-health measurement scale version 1.0 (SHMS V1.0) was used to analyze the mental sub-health status. Results The total scores was (59.53±10.48) points and the score of SHMS V1.0 in the physical subscale, psychological subscale, social subscale were (59.27±13.39),(56.26±10.39),(63.49±11.92) points.The difference of the young and middle-aged intellectuals of age, occupation, gender, marriage,dwelling,work day time, breakfast, sleeping time, exercise, smoking and drinking were statistically significant (P<0.05). Conclusions The physical and psychological sub-health status of the young and middle-aged intellectuals in Guangxi is not unsatisfactory.Puting forward the nursing strategy of installing information exchange platform , forming a good living habits , creating a good atmosphere to improve the psychological sub-health status of the young and middle-aged intellectuals are of great importance.
2.Injuries by Sharp Instrument and Preventive Measure:An Investigation among 154 Interns

OBJECTIVE To investigate the occurrence of injuries by sharp instrument among new medical staff and their preventive consciousness and measures.METHODS Through questionaires,one hundred and fifty four interns who graduated from 2004 to 2006 were surveyed retrospectively.RESULTS Of the new interns,126(81.8%)persons were injured 585 times by sharp instrument,87.8% of whom were injured twice.During the internship,83 persons had passed sharp instrument injury prevention training,and earned priorities in its prevention.In the group of 83 persons trained,59 persons(71.1%) were injured;in another group of 71 persons untrained,67 persons(94.4%) were injured,there was significant difference at the rate of injuries between two groups.Opening ampules,and manipulating needles of syringes or infusion devices were the three major causes due which sharp injuries happened,were accounted for 35%,27% and 22%.CONCLUSIONS New medical staff have high rate of the sharp instrument injuries during internship,the occurrences of injuries are mostly due to the inadequacy in consciousness,the inpreficiency in operating,the high rate of with sharp instrument.Therefore,to reduce the sharp instrument injuries,developing good operating habit;performing operating procedure conscientiously;using medical equipment which can prevent injuries are the effective measures.
3.Visualized Precise Electro- physiological Diagnosis and Treatment of Peyronie's Disease in Plateau Areas

ObjectiveTo investigate the efficacy and treatment options of precise electrophysiological diagnosis combined with percutaneous low-frequency electrical stimulation for penile scleroderma in high-altitude hypoxic areas. MethodsA total of 54 patients with electrophysiological diagnosis and treatment of corpora cavernosa were included in the retrospective analysis. Their age ranged from 20 to 70 years with a mean of 45.5±11.6 years and disease duration from 2 to 16 months. PDE5i, levocarnitine oral solution, and traditional Chinese medicine for invigorating blood circulation and eliminating stasis were administered orally before treatment. The visual precision electrophysiological diagnosis was performed after discontinuation of all oral drugs for 1 week before treatment. The electrophysiological diagnostic parameters were recorded and contrasted before and after the set-up treatment, and the corresponding parameters for precision electrophysiology were adjusted for percutaneous low-frequency electrical stimulation treatment, 2-3 hours each time, once daily, and 10 times per course. At the same time was administered assisted nasal cannula oxygen inhalation for 2 to 3 hours (4L/min). Before and after electrophysiological diagnosis and treatment were evaluated by using the numeric assessment scale of pain extent, the international index of erectile function score (IIEF-5), penile curvature measurement, color Doppler measurement of plaque areas and comparison of whole body and disease local infrared thermogram temperature changes. ResultsAll patients successfully completed 2-4 sessions of percutaneous low-frequency electrical stimulation (20 sessions to 40 sessions). There was a significant statistical difference (P < 0.01) between the mean values before and after treatment for the degree of weakness / erection pain (7.1±1.6 vs 2.0±1.0);(8.2±3.2 vs 16.0±5.1) before and after IIEF-5 treatment, respectively (P < 0.01),Pre- and post-treatment of PCDDU measured mean plaque size was 0.23(0.09~0.54)cm2 vs 0.23(0.09~0.54)cm2,with no statistically different change in improvement value (P = 0.189). Four cases had no penile camptodactyly, Before electrical stimulation, 50 (92.6%) patients had different degrees of penile curvature deformity, and there was no change in the improvement of the contrast bending angle before and after treatment (P >0.05). The comparison of TMT images before and after treatment with visual precision electrophysiology diagnosis suggested that temperature change >1.5℃ in the whole body and penile and bilateral inguinal regions, etc. could be considered effective electrical stimulation treatment. There were no recurrences of induration, increased pain and penile curvature at 3~10 months follow-up, and no patient required subsequent surgery. ConclusionThe results of this study show that visualizing the electrophysiological diagnosis situation to set local and holistic combined treatment parameters, and treating PD by means of percutaneous low-frequency electrical stimulation at meridian and neuromuscular acupoints of traditional Chinese medicine could achieve good safety and efficacy, especially in the early stage,which has a clear effect on pain treatment, thus opening new avenues for PD treatment.
4. Application of endocrine therapy combined with paclitaxel (albumin bound) in patients with intermediate and advanced prostate cancer

Objective: To observe the efficacy of icalutamide and goserelincombined with paclitaxel (albumin bound) and prednisone in patientswith intermediate and advanced prostate cancer.Methods: One hundred patients with intermediate and advancedprostate cancer were randomly divided into control group (n = 50)and observation group (n = 50). The patients in the control groupwere treated with oral bicalutamide and goserelin acetate sustainedrelease.The patients in the observation group were treated with oral bicalutamide, goserelin acetate sustained-release, paclitaxel (albumin bound) andprednisone. After 3 cycles of treatment, the effects and adverse reactions were compared.Results: The complete remission rate and the partial remission rate of the observationgroup were higher than those in the control group (22.0% vs 8%, 60% vs 40%, both P <0.05). The median survival time and the median progression-free survival time of theobservation group were longer than those of the control group (51 months vs 33 months,54 months vs 30 months, both P < 0.05). The serum prostate specific antigen, prostatevolume and maximum urine flow rate of the patients in the observation group wereimproved. The adverse reactions between the two groups had no significant differences (allP > 0.05).Conclusion: The endocrine therapy combined with paclitaxel (albumin bound) and prednisonein patients with intermediate and advanced prostate cancer can achieve good efficacy.
5.Effect of Jingui Shenqiwan Combined with Fracture Three-stage Syndrome Differentiation on Recovery of Patients with Osteoporotic Vertebral Compression Fractures After Operation

Objective:To evaluate the short-term and long-term efficacy of Jingui Shenqiwan combined with three-stage syndrome differentiation on osteoporotic vertebral compression fractures (OVCFs) after operation, and its regulatory effect on biomarkers of bone metabolism. Method:One hundred and thirty-five patients were randomly divided into control group (67 cases) and observation group (68 cases) by random number table. A total of 58 patients in control group completed the treatment (4 patients were exfoliated, 2 patients were lost to follow-up, 3 patients were eliminated); and 60 patients in observation group completed the treatment (3 patients were exfoliated, 2 patients were lost to follow-up, 3 patients were eliminated). Both groups patients were given calcitonin injection through intramuscular injection, 20 u/time, 1 time/week, for 12 weeks, calcium carbonate D3 chewable tablets (Ⅱ), 1 tablet/time, 2 times/day, and alendronate sodium tablets, 70 mg/time, 1 time/week. Patients in control group got Bushen Jiangu capsule, 4 grains/time, 3 times/day. And patients in observation group got modified Jingui Shenqiwan combined with fracture three-stage symptom differentiation, 1 dose/day. The courses of treatment in the two groups were 24 weeks, and a 24 week follow-up was provided. Before the operation and at the 12th and 24th week after operation, the short-term efficacy indexes, such as back pain, lumbar function, traditional Chinese medicine (TCM) syndromes and complications, were recorded. And the long-term efficacy indexes, such as recovery of responsible vertebral body, lumbar function, bone density and quality of life and incidence of 48 week re-fracture, were also recorded. 6.The effects of hypoxia on silencing HLA⁃G expression and biological behaviors of JEG⁃3 cells and EPAS1

Objective:
To investigate the effect of silencing human leukocyte⁃associated antigen⁃G ( HLA⁃G) expression in the chorionic trophoblast cell line JEG⁃3 cells under hypoxic conditions on the biological function of JEG⁃3 cells and through the hypoxia response pathway of endothelial PAS1 region protein 1 (EPAS1)is involved in the molecular mechanism of preeclampsia under high altitude hypoxia.
Methods:
The expression of HLA⁃G in JEG⁃3 cells was inhibited by transfection of small interfering RNA (siRNA) . The JEG⁃3 cells after transfection were divided into four groups : normoxic control group , hypoxic control group , normoxic inhibition group , and hypoxic inhibition group. CCK⁃8 test and Transwell test were used to detect the proliferation and invasion ability of the cells in four groups ; The effects of four groups of apoptosis and cell cycle were detected by flow cytometry; HLA⁃G and EPAS1 mRNA and protein expression levels were detected by real⁃time fluorescence quantitative PCR ( qPCR) and Western blot.
Results :
① Compared with the normoxic control group , hypoxic control group , normoxic inhibition group , and hypoxic inhibition group could reduce the proliferation activity and invasion ability of JEG⁃3 cells , and significantly increase the apoptosis rate. The hypoxic control group and hypoxic inhibition group also produced anobvious necrotic cell population ; Under the condition of hypoxia , after reducing the expression of HLA⁃G , the cell necrosis rate was further aggravated ; Whether under normoxia or hypoxia , inhibition of HLA⁃G expression caused the cells to be blocked in the G1 phase. ② Compared with the normoxic control group , hypoxia control group , normoxic inhibition group , and hypoxia inhibition group decreased the expression of HLA⁃G protein , and hypoxia and inhibition of HLA⁃G had a synergistic effect; Hypoxia⁃inducible factors⁃2α ( HIF⁃2α ) , vascular endothelial growth factor(VEGF) and endothelin⁃1(ET⁃1) protein expression could be added , inhibition of HLA⁃G decreased the expression of inducible nitric oxide synthase( NOS2) .
Conclusion
In the hypoxic environment , silencing HLA⁃G may affect the biological behavior of trophoblast through the EPAS1 hypoxic response pathway and participate in the
occurrence and development of preeclampsia.

The plateau pika (Ochotona curzoniae) has a strong adaptability to hypoxic plateau environment. We found that the sperm-specific lactate dehydrogenase (LDH-C4) gene Ldh-c expressed in plateau pika cardiac muscle. In order to shed light on the effect of LDH-C4 on the anaerobic glycolysis in plateau pika cardiac muscle, 20 pikas were randomly divided into the inhibitor group and the control group, and the sample size of each group was 10. The pikas of inhibitor group were injected with 1 mL 1 mol/L N-isopropyl oxamate, a specific LDH-C4 inhibitor, in biceps femoris muscle of hind legs, each leg with 500 μL. The pikas of control group were injected with the same volume of normal saline (0.9% NaCl). The mRNA and protein expression levels of Ldh-c gene in plateau pika cardiac muscle were determined by real-time PCR and Western blot. The activities of LDH, and the contents of lactate (LD) and ATP in cardiac muscle were compared between the inhibitor group and the control group. The results showed that 1) the expression levels of Ldh-c mRNA and protein were 0.47 ± 0.06 and 0.68 ± 0.08, respectively; 2) 30 min after injection of 1 mL 1 mol/L N-isopropyl oxamate in biceps femoris muscle, the concentration of N-isopropyl oxamate in blood was 0.08 mmol/L; 3) in cardiac muscle of the inhibitor group and the control group, the LDH activities were (6.18 ± 0.48) U/mg and (9.08 ± 0.58) U/mg, the contents of LD were (0.21 ± 0.03) mmol/g and (0.26 ± 0.04) mmol/g, and the contents of ATP were (4.40 ± 0.69) nmol/mg and (6.18 ± 0.73) nmol/mg (P < 0.01); 5) the inhibition rates of N-isopropyl oxamate to LDH, LD and ATP were 31.98%, 20.90% and 28.70%, respectively. The results suggest that Ldh-c expresses in cardiac muscle of plateau pika, and the pika cardiac muscle may get at least 28% ATP for its activities by LDH-C4 catalyzed anaerobic glycolysis, which reduces the dependence on oxygen and enhances the adaptation to the hypoxic environments.

8.Changes in the intestinal flora of rats under high altitude hypoxia

The structure and diversity of the intestinal flora in rats exposed to high altitude hypoxia was investigated. Animal experiments strictly follow the regulations of Medical Laboratory Animal Ethics Committee of Qinghai University, School of Medicine. SD rats were randomly divided into a control group, a moderate altitude hypoxia group, and a high altitude hypoxia group. The pH value of the feces was measured and histopathological changes in the small intestine were determined by HE staining, and the intestinal flora were characterized by 16S rDNA high throughput sequencing technology on the 3rd, 7th, 15th, and 30th day of hypoxia exposure. Compared with the control group, the fecal pH value of rats in the moderate altitude hypoxia group and the high altitude hypoxia group was decreased significantly. The lamina propria and submucosa capillaries were slightly dilated and congested on the 3rd day in the moderate altitude hypoxia group. In the high altitude hypoxia group the submembrane capillaries were dilated and congested, the lamina propria of the mucosa showed mild edema, and the lymphatic vessels were dilated on the 7th day. The composition and diversity of intestinal flora in these rats changed significantly with prolonged exposure to the high altitude hypoxic environment. A total of 35 phyla, 87 classes, 205 orders, 337 families, 638 genera, and 256 species were annotated in the three groups of rats, including Firmicutes, Clostridia, Clostridiales, Ruminococcaceae, Akkermansia, and Lactobacillus_murinus. Compared with the control group, the intestinal flora of the hypoxic groups showed the most significant changes by the 15th day. There were 9 microbiota of gut microorganisms with relative abundance in the moderate altitude hypoxia group, of which Rikenellaceae_RC9_gut_group bacteria was the most common, there were 19 different microbiota of gut microorganisms with higher relative abundance in the high altitude hypoxia group, of which Ruminococcaceae bacteria was the most common. The results of this study indicate significant changes in the intestinal flora with high altitude hypoxia, and establish a foundation for further research on the initiation and development of diseases and drug metabolism in high altitude hypoxia.
9.The role of activation of the ferroptosis pathway in the inhibition of esophageal cancer proliferation and metastasis by realgar

This study was designed to investigate the role of activation of the ferroptosis pathway in the inhibition of esophageal cancer proliferation and metastasis by realgar, using esophageal cancer Eca109 and KYSE150 cells as the target cells. The rate of inhibition and half-inhibitory concentration (IC50) were measured by the CCK-8 method; clone formation ability was measured by clone formation assay; the changes in reactive oxygen species (ROS) were detected by flow cytometry; the ultrastructure of the cells was observed by transmission electron microscopy; the distribution of intracellular iron particles was observed by Prussian blue staining; the expression of glutathione peroxidase 4 (GPX4) was detected by immunofluorescence staining; the scratch assay was used to detect the cell migration ability; the Transwell assay was used to detect the cell invasion ability; and western blotting was used to detect E-cadherin, Slug and N-cadherin protein expression in the cells. The results show that realgar inhibited the proliferation of Eca109 and KYSE150 cells in a time- and concentration-dependent manner, and the IC50 of Eca109 and KYSE150 cells was 64.297 and 51.337 μmol‧L-1, respectively. Compared with the control group, many mitochondria in the cytoplasm of Eca109 and KYSE150 cells in the realgar 2IC50 group were swollen and blue-stained particles of different sizes and amounts were found, and ROS fluorescence intensity was significantly increased while GPX4 protein expression was significantly reduced (

Objective: To explore the drug resistance of Isolated From Blood Culture Escherichia coli (E. coli) in a hospital in Qinghai over the past seven years, to evaluate the ability of matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) to analyze the homologous origin of E. coli, and to establish a protein fingerprint library to match with it, adjuvant clinical experience medication so as to provide the basis for the prevention and control of hospital-acquired infections. Methods: Retrospective analysis of blood cultures sent to hospitals from January 2016 to December 2022. Drug resistance and resistance changes in E. coli.A total of 1 841 E. coli strains were isolated from Qinghai Provincial People's Hospital from January 2016 to December 2022; all strains were identified by MALDI-TOF MS, and the VITEK2.0 drug sensitivity analyzer was applied for drug sensitivity analysis of the strains, and the mass spectrometry homology analysis and self-constructed protein fingerprint library were carried out by MALDI-Biotyper software; the protein fingerprint library was built by using WHONET5.6 software was used to statistically analyze the drug sensitivity results, SPSS23.0 software was used to analyze the relationship between fingerprint typing and drug sensitivity, and the χ2 test was used for intergroup comparisons. Results: A total of 1 841 strains of E. coli were detected in 4 582 positive blood culture specimens from January 2016 to December 2022, with a detection rate of 40.17%; the resistance rate of E. coli from blood sources to piperacillin/tazobactam and ceftriaxone was on the rise, and it was slightly decreased to cefepime, amikacin, levofloxacin, and sulfamethoxazole, and there was not much change to the rest of the drugs; After MALDI-Biotyper clustering analysis, the 1841 E. coli strains from Isolated From Blood Culture were classified into two major clusters and five subtypes, of which type Ⅰa1 accounted for about 40%, type Ⅰa2 accounted for about 2.7%, type Ⅰb accounted for about 3.8, type Ⅱa accounted for about 46%, and type Ⅱb accounted for about 7.5%. The detection rate of type Ⅰa1 E. coli was higher in general surgery (50.45%) and emergency surgery (50.92%), and the detection rate of type Ⅰb E. coli was higher in emergency medicine(10.05%)than in other departments. The drug sensitivity results of different subtypes were compared with each other, the resistance rate of type Ⅰa1 E. coli to cefepime was 21.3% higher than that of the remaining four types, and the difference was statistically significant (χ2=37.74,P=0.000); the resistance rate of type Ⅱ E. coli(>60%) to sulfamethoxazole was higher than that of type Ⅰ (<60%) as a whole, and the difference was statistically significant (χ2=15.248,P=0.004); and a preliminary database of homologous protein fingerprints of E. coli has been established E. coli homologous protein fingerprint library and validated. The drug susceptibility results of 1 288 E. coli strains in the validation set were statistically analyzed and compared with those in the training set. There was no significant difference(P>0.05). Conclusion: In recent years, the resistance rate of E. coli isolated from a hospital in Qinghai province to piperacillin/Tazobactam, cefepime, amicacin and other antibiotics has changed greatly. A fingerprint database of E. coli homologous protein was established, and it was found that the drug sensitivity data of E. coli were different among different fingerprint types. According to drug sensitivity, drug use could assist clinical experience and provide evidence for prevention and control of hospital illness.
