1.Effect of pidotimod,zinc gluconate,bifico on immune function in the treatment of infants with persistent

Objective To observe the effect of pidotimod,zinc gluconate,bifico on immune function in the treatment of infants with persistent chronic diarrhea,and to provide new options for clinical treatment.Methods The infants with persistent chronic diarrhea were selected as study objects,all patients were randomly divided into 40 patients of the observation group and 40 patients of control group,both group were given conventional treatment, which including ORS oral rehydration,vitamin supplements,electrolytes,correct acid -base balance,smecta.The con-trol group were given zinc gluconate and bifico(Bifico 20mg/kg,tid;oral zinc gluconate,10 -20mg/d (zinc compu-ting),tid),the observation group were given pidotimod(400mg/times,qd)on the basis of the control group.Both group had been treated for three weeks and been followed for 12month.The symptoms improvement was observed be-fore and after treatment,the time of stool frequency normalization and stool Characters normalization were recorded,the blood were extracted for detecting immunoglobulin (IgA,IgG)and T lymphocyte subsets (CD +3 ,CD +4 ,CD +8 ),the effect and Diarrhea relapses of follow -up of 6 months were evaluated.Results The time of stool frequency normali-zation and stool characters normalization in the observation group and the control group were (4.5 ±1.5 )d and (7.8 ±2.2)d,(4.3 ±1.6)d and(7.5 ±2.0)d,the observation group were significantly shorter than the control group,t =7.146,8.108,all P <0.01.The number of diarrhea relapses of follow -up of 6 months in observation group and the control group were (2.05 ±1.62)times and (3.73 ±1.65 )times,(3.38 ±2.12)times and (5.15 ± 2.32)times,the observaiton group were significantly less than the control group,t =3.845,3.636,all P <0.05.The IgA,IgG,CD +3 ,CD +4 ,of observation group and control group after treatment were (1.75 ±0.10)g/L vs.(1.50 ± 0.11)g/L,(8.95 ±1.42)g/L vs.(8.02 ±1.25)g/L,(57.5 ±6.6)% vs.(50.5 ±6.0)%,(39.5 ±3.5)% vs. (35.6 ±3.6)%,the observation group were significantly higher than before treatment (t =3.633,5.835,9.534, 6.427 and 3.274,3.643,4.732,3.658,all P <0.05).The IgA,IgG,CD +3 ,CD +4 of observation group after treatment was significantly higher than the control group (t =3.553,3.434,3.568,3.336,all P <0.05).The total effective rate of observation group was 90.0%,the control group was 72.5%,the difference was statistically significant(χ2 =6.664,P <0.05).Conclusion The method containing pidotimod,zinc gluconate and bifico can adjust the immune dysfunction,improve immune and humoral immunity,prevent secondary infection,improve nutrition,promote mucosal tissue repair,improve treatment efficacy and reduce the long -term recurrence,worthy of clinical use in the treatment of infants with persistent chronic diarrhea.
2.Treatment of thoracolumbar vertebral fractures with monosegment pedicle instrumentation through posterior approach

Objective To investigate the method and effect of monosegment pedicle instrumentafinn on thoraeolumbar vertebral fracture.Methods Sixteen patients with thoracolumbar vertebral fractures treated by open reduction and monosegment pedicle instrumentation through posterior approach.Posterolateral bone grafting was done in all cases,9 cases were quite decompressed using laminectomy.The follow-up time Was 8-24 months.The spine radiographs and CT scan were taken pre-and post-operation.The parameters used for clinical evaluation were the Denis pain scale,neurological evaluation by the Frankel scale and bone graft healing evaluation by the Suk scale.Results All of the fractures were reduced satisfactorily during the follow-up.The height of the injured vertebral improved rate was 30.8%,kyphosis of the injured vertebral segment rectified obviously.There was no implant break,and no signs of losing were observed in any patient.Nine patients which had neurological deficiency had average improvement more than one degree by the Frankel scale.After operation the pain score was P1 in 14 patients and P2 in 2 patients.Condnsion For the appropriate patients with thoracolumbar vertebral fractures,open reduction and internal fixation of monosegment pedicle instrumentation through posterior approach is a safe and effective method.
3.The efficacy of antithymocyte globulin combined with cyclosporine immunosuppressive therapy in the treat-ment of severe aplastic anemia

Objective To investigate the effect of anti thymic globulin (ATG)combined with cyclosporine (CSA)immunosuppressive therapy in patients with severe aplastic anemia (SAA).Methods 20 patients with SAA were treated with CSA combined with immunosuppressive therapy,and the changes of blood indexes after treatment were observed.Results After treatment,4 cases (13.33%)were cured,11 cases (36.67%)were relieved,8 cases (26.67%)were improved,7 cases (23.33%)were ineffective,the total effective rate was 76.67%.After the treatment of blood routine,examination of Hb,WBC and PLT treatment 6 months compared with before treatment,the difference were statistically significant (t =5.678,3.675,7.685,all P <0.05).Patients after treatment,1 case had allergic reaction,which accounted for 3.33%,and was finally given the glucocorticoid,then improved.1 case had extremely low WBC,accounting for 3.33%,appeared serious infection and death after testing.Conclusion The effect of ATG combined with CSA immunosuppression in the treatment of SAA is effective obviously,and it has a certain safety,vwhich is worthy of clinical recommendation.
4.Effect of combined with small doses of thalidomide and low -dose dexamethasone VAD scheme in treatment of elderly multiple myeloma

Objective To observe the curative effect of combined with small dose of thalidomide and low -dose dexamethasone VAD regimen for the elderly multiple myeloma (MM).Methods 58 MM cases were randomly divided into control group(28 cases)and treatment group (30 cases).The control group received VAD regimen.The treatment group added with small doses of thalidomide treatment (TVAD scheme).The,hemoglobin (HGB),creati-nine (Cr),bone marrow plasma cell counting,M protein level,Carr's score (KPS)and side effects of chemotherapy were observed in the two groups.Results The effective rates of treatment group and control group were 76.67% and 50.00%,the curative effect of treatment group was better (χ2 =4.459,P <0.05).The HGB,Cr,plasma cells,Mpro-tein and carr's score were improved,but the curative effect of the treatment group was better(P <0.05).Adverse reac-tions such as leucopenia,neuropathy,constipation,gastrointestinal reaction and pulmonary infection were observed in two groups,but there was no significant difference.Conclusion TVAD regimen for MMhas good curative effect and less side effects,it is worthy of promotion.
5.Content Determination of Glabridin in Clycyrrhizae Radix et Rhizoma from Different Regions by HPLC

OBJECTIVE:To establish a method for the content determination of glabridin in Clycyrrhizae Radix et Rhizoma from different regions. METHODS:HPLC was performed on the column of Inertsil ODS-SP with mobile phase of acetoni-trile-0.05%phosphoric acid(gradient elution)at a flow rate of 0.8 ml/min,detection wavelength was 280 nm,the column tempera-ture was 35 ℃,and the injection volume was 20 μl. RESULTS:The linear range of glabridin was 0.906-18.12 μg/ml(r=0.999 7), RSDs of precision,stability and reproducibility tests were lower than 3%;recovery was 98.73%-101.90%(RSD=1.25%,n=6). There were obvious differences among the glabridin contents in G. uralensis from different regions. CONCLUSIONS:The method is simple and accurate with high precision,and can be used for the content determination of glabridin in Clycyrrhizae Radix et Rhi-zoma.

7.Effect of Early Intervention on Expression of NB-3 in Neonatal Rats with Cerebral Injury Induced with Intrauterine Infection

Objective To investigate the effect of early intervention on adhesion molecule NB-3 expressing in cerebral injured neonatal rat induced with intrauterine infection. Methods Pregnant rats were consecutively injected with lipopolysaccharide (LPS) 380 μg/kg or saline 17 d after gestation. The offspring from the LPS group accepted early intervention (I group, n=40) or not (NI group, n=40), and those for saline group were as control group (n=40). They were assessed with suspension test 14 d and 28 d after born, and the expression of were NB-3 were detected with immunohistochemistry 1 d, 7 d, 14 d, 21 d, 28 d after born. Results The expression of NB-3 was different among these 3 groups, as well as the scores of suspension test (P<0.001). Conclusion Intrauterine infection increases the expression of NB-3 of neonatal rats with brain damage. The early intervention may increase persistently the expression of NB-3, and improve the motor function.

9.Retrospective analysis of perioperative complications and risk factors of posterior lumbar interbody fusion for recurrent lumbar disc herniation

Objective To retrospectively analyze the perioperative complications of posterior lumbar interbody fusion (PLIF) for recurrent lumbar disc herniation and identify potential risk factors that correlate with those complications.Methods All of 71 patients with recurrent lumbar disc herniation were treated surgically with PLIF,discharged from our department between January 2008 and December 2014.Demographic and operation data were collected and perioperative complications were recorded.We analyzed whether the clinical factors (age,gender,BMI,co-morbidity,smoking,time of recurrence,blood loss,operation segment,operation time) were in correlation with perioperative complication by univariate analysis.Then we integrated the statistically significant indicators into Logistic regression equation to determine the related risk factors for complication.Results The study group consisted of 71 cases,including 42 males and 29 females.The age was 19 to 64 years old with an average of 50.6 years old,and the average BMI was 23.6 kg/m2.26 cases had perioperative complications,while there were two or more complications in 5 patients,and no mortalities.Neurologic deterioration or neuropathic pain (10 cases,14.1%) and dural tears (6 cases,8.5%) were the most common intraoperaitive complications.The other complications included nerve root or cauda equine injury,deep or superficial wound infection,urinary tract infection,respiratory system complication,cardio-vascular complication and delirium.Univariate analysis suggested that age,gender,co-morbidity,smoking,time of recurrence,operation segment,operation time were not associated with perioperative complication.The multivariate Logistic regression analysis showed BMI and blood loss were closely related to perioperative complication.Conclusion Complications of posterior spinal fusion surgery for recurrent lumbar disc herniation are affected by many factors.The most common complications are transient neurologic deterioration or neuropathic pain and dural tears.BMI and blood loss are independent risk factors.
10.Comparison of the diagnostic performances between strain elastography and shear-wave elastography in differentiation of benign and malignant solid breast lesions

Objective To compare the diagnostic performances of strain elastography (SE) and shear-wave elastography (SWE) in differentiating benign and malignant solid breast lesions. Methods From January 2015 to December 2015, 150 patients with 155 lesions were examined with SE and SWE respectively in the First Affiliated Hospital with Nanjing Medical University. Elasticity scores on a improved 5-point scale,strain ratio (SR), maximum elasticity (Emax), mean elasticity (Emean), standard deviation of the elasticity (Esd) and the lesion-to-fat elasticity ratio (Eratio) were assessed. The receiver operating characteristic (ROC) curve of SE and SWE parameters were plotted to assess the value in distinguishing benign and malignant breast lesions. The results were compared using the area under the ROC curve (AUC). The accuracy of the parameters with the largest AUC were compared between SE and SWE by McNemar test in different breast imaging reporting and data system (BI-RADS) categories. Results The AUC of elasticity scores, SR, Emax,Emean, Esd and Eratio were 0.823, 0.810, 0.877, 0.835, 0.881 and 0.853, respectively. The SE and SWE parameter with the largest AUC were elasticity scores and Esd respectively. The accuracy of Esd was higher than that of elasticity scores(86.3% vs 64.7%)in 4A lesions (2=4.639, P < 0.05). There were no significant differences in accuracy in other BI-RADS categories. Conclusions Both SE and SWE were helpful for the differentiation of benign and malignant solid breast lesions, and the diagnostic performance of SE and SWE was similar. The optimization of elastography can be achieved by combination with BI-RADS.
