1.Diagnostic Value of Spiral CT Delay Repeated Scanning in IntracerebralSchistosomiasis(An Analysis of 49 Cases)

Objective To explore the specific features and value of spiral CT delay repeated scanning in diagnosis of cerebral schistosomiasis granuloma.Methods The spiral CT dynamil scanning material of 49 cases of intracerebral schistomiasis proved by pathology and clinic were retrospectively analyzed.Results There was no focus enhancement within ninety seconds post contialed;Focus showed a beginning enhancement was begun within the second to the fifith minutes and the peak enhancement was within the fifth to the fifteenth minutes mixed together like a ball gradually ;Focus enhancement decreased with time within the fifteenth to the twentieth minutes and disappeared after the twenty-fifth minutes.Conclusion Spiral CT delay repeated scanning had specific features in diagnosis of cerebral schistosomiasis granuloma,the diagnosis accuracy rates was 100%.
2.Recent advances in the diagnosis and treatment of lysosomal storage diseases

SUMMARY Lysosomal storage diseases are a group of genetic disorders that result from the defect in lysosomal function. Signs and symptoms are variable, it is difficult to diagnose this group of disease merely by the clinical manifestation. The diagnosis usually is made by measuring the activity of the corresponding enzyme. Gene mutational analysis is useful for the diagnosis of some of the lysosome storage diseases. The treatment has focused on the replacement of the defective enzyme responsible for the disease and the hematopoietic stem cell transplantation. Both of them have achieved exciting outcomes in some of the diseases.
3.Psychosocial Factors and Immunity of Patients with Generalized Anxiety Disorder

Objective: To study psychosocial factors and immunity of patients with generalized anxiety disorder Method: 30 patients with generalized anxiety disorder (GAD), 25 patients with neurotic depression and 32 normal controls were collected and assessed with LES (life event scale), SCSQ (simplified coping style questionnaire) and EPQ (Eysenck personality questionnaire) The blood level of IL-2, and rates of NK cell and CD3+ cell of them were also measured Result: The number of negative life events and LEU of negative life events of GAD group were higher that of normal controls, but lower than that of neurotic depression group Their total life events were also more than that of normal controls They had less positive coping than normal controls, more negative coping than both normal controls and patients with neurotic depression Their N score of EPQ was higher than that of normal control and had no significant different to that of patients with neurotic depression They also had higher levels of IL-2 than that of the other two groups There was no significant difference in rates of NK cell and CD3+ cell among all three groups The level of IL-2 was negatively correlated to the number of total life events and L score of EPQ, positively correlated to the rate of NK cell Conclusion: Patients with GAD have psychosocial stress in their life, they incline to negative coping, having unstable mood and impaired immune function Psychosocial stress has influence on immunity
5.Effect of a selective JNK inhibitor on pregnant rats with ethinylestradiol induced intrahepatic cholestasis

Objective To evaluate the influence of SP600125,a selective c-Jun N-terminal kinase (JNK) inhibitor,on the levels of serum total bile salt (TBA) and Bsep,Ntcp expression in the hepatic tissue of rats with ethinylestradiol induced intrahepatic cholestasis.Methods Rats pregnant for 15days were administered the subcutaneous injection of 17- b -estradiol propylene ( EE ) to modulate the ICP animal models,and be SP600125 to intervene.Testing the level of serum TBA and the expression of c-Jun,Bsep,Ntcp in the hepalatic tissue.Results The average gray values of c-Jun in the group of ICP models were significantly lower than the normal control group ( 101.05 ± 5.20 vs 118.99 ± 5.95,P < 0.05 ).After the intervention of SP600125,comparing with the group of ICP models,the expression of Bsep,Ntcp in the group of SP600125 intervention were significantly higher,and this change in the high dose of SP600125 intervention group was more obvious ( low dose intervention group Bsep:0.452 ±0.031 vs 0.291 ±0.043,Ntcp:0.462 ± 0.015 vs 0.285 ± 0.021,P < 0.05 ; high dose intervention group Bsep:0.568 ± 0.038 vs 0.291 ±0.043,Ntcp:0.605±0.020 vs 0.285 ±0.021,P <0.05),while the level of TBA in the serum was significantly lower.Conclusions Treatment with SP600125 can down-regulate the level of c-Jun/AP-1,and it may participate in the lower expression of Bsep、Ntcp in the ICP rats which were induced by 17-bestradiol.
6.The use of Charlson weighted index to evaluate the time of observation in emergency internal observation room

Objective To investigate the role of underlying diseases in predicting the length of stay for observation in emergency department of internal medicine by the Charlson weighted index of comorbidities (WIC).Methods A single-center retrospective analysis of clinical data of 2 836 patients admitted in emergency observation room of the Beijing Chaoyang Hospital from January 1 to June 30 in 2013 was carried out.The patients were divided into two groups according to the length of observation time:Group A (time of observation ≥72 h,n =1908) and Group B (time of observation < 72 h,n =928).The data of the length of observation time were recorded,and the WIC and the APACHE Ⅱ score were calculated.Logistic regression analysis was used to determine the independent predictors for 72-hour observation.Receiver operating characteristics (ROC) curve was used to evaluate the value of WIC in predicting 72-hour observation.Results Of 2836 patients,1176 (41.5%) suffered from respiratory disease,709 (25.0%) had cardiovascular and cerebrovascular diseases,423 (14.9%) were contracted digestive system disease,251 (8.8%) had renal and endocrinology system diseases and 277 (9.8%) had diseases arisen from physicochemical factor and miscellaneous causes.Compared with patients in Group B,the average age,the number of elderly patients residing in apartment exclusively for elderly,the WIC and the APACHE Ⅱ score were higher in patients in Group A.The one-variable and multi-variable Logistic regression analyses showed that age,the WIC score,the APACHE Ⅱ score and residing in apartment for elderly people were related with 72-hour observation in emergency observation room.The area under the ROC curve in predicting 72-hour observation was 0.701 for the WIC score,0.788 for APACHE Ⅱ score and 0.853 for their combination.Conclusions The WIC scoring system can be a good predicting method for 72-hour observation in patients in emergency observation room.
7.Effect of combined with small doses of thalidomide and low -dose dexamethasone VAD scheme in treatment of elderly multiple myeloma

Objective To observe the curative effect of combined with small dose of thalidomide and low -dose dexamethasone VAD regimen for the elderly multiple myeloma (MM).Methods 58 MM cases were randomly divided into control group(28 cases)and treatment group (30 cases).The control group received VAD regimen.The treatment group added with small doses of thalidomide treatment (TVAD scheme).The,hemoglobin (HGB),creati-nine (Cr),bone marrow plasma cell counting,M protein level,Carr's score (KPS)and side effects of chemotherapy were observed in the two groups.Results The effective rates of treatment group and control group were 76.67% and 50.00%,the curative effect of treatment group was better (χ2 =4.459,P <0.05).The HGB,Cr,plasma cells,Mpro-tein and carr's score were improved,but the curative effect of the treatment group was better(P <0.05).Adverse reac-tions such as leucopenia,neuropathy,constipation,gastrointestinal reaction and pulmonary infection were observed in two groups,but there was no significant difference.Conclusion TVAD regimen for MMhas good curative effect and less side effects,it is worthy of promotion.
8.The efficacy of antithymocyte globulin combined with cyclosporine immunosuppressive therapy in the treat-ment of severe aplastic anemia

Objective To investigate the effect of anti thymic globulin (ATG)combined with cyclosporine (CSA)immunosuppressive therapy in patients with severe aplastic anemia (SAA).Methods 20 patients with SAA were treated with CSA combined with immunosuppressive therapy,and the changes of blood indexes after treatment were observed.Results After treatment,4 cases (13.33%)were cured,11 cases (36.67%)were relieved,8 cases (26.67%)were improved,7 cases (23.33%)were ineffective,the total effective rate was 76.67%.After the treatment of blood routine,examination of Hb,WBC and PLT treatment 6 months compared with before treatment,the difference were statistically significant (t =5.678,3.675,7.685,all P <0.05).Patients after treatment,1 case had allergic reaction,which accounted for 3.33%,and was finally given the glucocorticoid,then improved.1 case had extremely low WBC,accounting for 3.33%,appeared serious infection and death after testing.Conclusion The effect of ATG combined with CSA immunosuppression in the treatment of SAA is effective obviously,and it has a certain safety,vwhich is worthy of clinical recommendation.
10.The fluctuation of cytokine mRNA expression level of a novel T-cell-mediated immune hepatic fibrosis model in Balb/c mice

Objective To explore the fluctuation of cytokine mRNA expression level in a novel T-cell-mediated immune hepatic fibrosis model induced by repeatedly injections of Concanavalin A in BALB/c mice. Methods BALB/c mice were divided into different groups. Model group mice were injected weekly up to 20 weeks with Concanavalin A (15mg/kg), via retro-orbital venous plexus under ether anesthesia. Normal control group mice were treated in the same manner weekly with normal saline. Twenty-four hours after Concanavalin A challenge at 1, 5, 12 and 20 week, 8 mice from each time were killed by cervical dislocation, repectively. The livers of different group were excised and fixed in 10% formalin for HE staining and Gomori Ag staining or frozen in optimal cutting temperature (O.C.T.) media in liquid nitrogen for immunohistochemical staining for CD4 +T or CD8 +T cell. After extracting total RNA from liver tissues, IL-2, IL-4, IL-10 and transforming factor ?1 messenger RNA were amplified by reverse transcription polymerase chain reaction. PCR products were electrophoresed on agrose containing ethidium bromide and visualized under ultraviolet light. Densitometric RT-PCR data were standardized with ?-actin signals. Results The histological change of HE staining and Gomori Ag staining indicated the fibrogenesis in model group mice. Immunohistochemical staining for CD4 + or CD8 + T cell indicated that the infiltrating lymphocytes in liver parenchyma were mainly CD4 +T lymphocytes. IL-2 mRNA expression level only increased after the first injection of Concanavalin A. The expression levels of IL-4, IL-10 and transforming growth factor ?1 mRNA significantly increased over the whole experiment period as compared with control group. Conclusions Repeated administration of Concanavalin A can induce T-cell-mediated immune hepatic fibrosis model in BALB/c mice. The expression levels of IL-4, 10 and TGF-?1 increase over the whole experiment period and may play an important role in creating mouse fibrotic model.
