1.Application of Dexmedetomidine-assisted Controlled Anti-hypertension Combined with High Capacity Hemo-dilution in Spinal Surgeries

OBJECTIVE:To explore the safety,superiority and reliability of the application of dexmedetomidine-assisted con-trolled anti-hypertension combined with high capacity hemodilution in spinal surgeries. METHODS:40 patients with spinal surger-ies under general anesthesia were randomly divided into dexmedetomidine group (group D) and normal saline (group S) with 20 cases in each group. Both groups were given controlled anti-hypertension combined with high capacity hemodilution to carry out blood conservation and intraoperative wakeup experiment. Group D was given 1 μg/kg dexmedetomidine within 10 min by micro pump 10 min before anesthesia induction;maintained with 0.4-0.8 μg/(kg·h)during controlled anti-hypertension;injected with dex-medetomidine 0.2 μg/(kg·h) during wakeup experiment and anesthesia maintenance period until tracheal catheter was extracted. Group S was given normal saline instead,and the usage was same as group D. Heart rate (HR),mean arterial pressure (MAP) and central venous pressure(CVP)of patients were observed and recorded before pump injection of dexmedetomidine(T0),1 min after tracheal intubation(T1),10 min after anesthesia induction(T2),30 min after anesthesia induction(T3),wakeup(T4),at the end of controlled anti-hypertension(T5),at the end of operation(T6),during tracheal extubation(T7). Meanwhile,the wakeup ex-periment time,blood loss when wakeup,total amount of nitroglycerin,mean density of inhaled isoflurane,total amount of nitro-glycerin,total blood loss,and post-operative wakeup time of both groups were recorded. The case number of post-operative de-layed wakeup chill and dysphoria were calculated in both groups. RESULTS:Compared with at T0,HR of both groups at T1 and T7 were faster,and MAP and CVP increased,and that of group S was more significant than that of group D(P<0.05). HR and MAP of both groups at T3 were significantly lower than T0(P<0.05). Compared to group D at T4,MAP and CVP of group S at T4 were increased and HR were faster (P<0.05);the wakeup experiment time and post-operative wakeup time of group D were shorter than those of group S (P<0.05);the blood loss when awaken and total blood loss were less than group S (P<0.05);the total amount of nitroglycerin,mean density of inhaled isoflurane and total amount of remifentanil were all lower than those of group S (P<0.05);the incidence of post-operative delayed wakeup,chill and dysphoria in group D were significantly lower than those of group S (P<0.05). CONCLUSIONS:The application of dexmedetomidine-assisted controlled anti-hypertension combined with high capacity hemodilution in spinal surgeries can bring more stable and effective controlled anti-hypertension. It can also reduce the amount of nitroglycerin,remifentanil,propofol and isoflurane significantly. It is a reliable and safe drug to be applied in assist-ed controlled anti-hypertension combined with high capacity hemodilution in spinal surgeries.
2.PCD-hIL-2 and Chinese medicine accelerate the immunofunction reconstitution in bone marrow transplantation mice

Objective: In order to study how to accelerate the reconstitution immunofunction in BMT mice, first of all, we established a immunodeficiency model of BMT in BALB/C mice. Then BMT mice were injected with PCD-hIL-2 directly into skeletal muscle, and treated with traditional Chinese medicine. Methods: The experiment groups are designed as(A)Chinese medicine + PCD-hIL-2;(B)PCD-hIL-2;(C)Chinese medicine +hIL-2;(D)Chinese medicine;(E)hIL-2;(F)BMT;(G)normal control;(H)radiation control. Results: We compared groups A B C D to E or F groups, found(1)The splenocytes/thymocytes count increase obviously.(2)Killing activity of NK cell rises obviously in vivo.(3)The response of splenocytes、thymocytes、BM cells to mitogen goes up.(4)The reactivity of splenocytes to foreign IL-2 goes up. (5)CFU-GM count is increased. Conclusion: The expression of hIL-2 is very low by nude DNA injection ,but it is enough to have biological function and therapeutic effect .If only Chinese medicine was applied, the immunological condition was obviously recovered.
3.Clinical characteristics of 113 brucellosis with liver injury

Objective To summarize the characteristics of liver injury due to brucellosis to provide reference for clinical diagnosis and treatment of brucellosis.Methods A total of 359 patients with brucellosis at the First affiliated Hospital of Xinjiang Medical University during 2010-2016 were enrolled, among them 113 (31.5%) developed liver injury.Alanine aminotransferase (ALT), aspartate transaminase (AST), γ-glutamyl transferase (γ-GT), serum albumin (Alb), alkaline phosphatase (ALP) and total bilirubin (TBil) were obtained before and after treatment.Ultrasound of upper abdomen was performed to observe the morphological changes of liver and spleen at admission.The routine blood test, erythrocyte sedimentation rate, C-reactive protein and other laboratory examinations were also performed.The data of patients before and after therapy were analyzed by t test.Results A total of 113 patients were included in this study.Eighty-eight (77.9%) were male.The mean age was (43.2±15.8) years old, among whom 81(71.7%) cases were between 31 and 60 years old.Seventy-one cases (62.8%) was in acute phase, and 42(37.2%) in chronic phase.The Han, Uighurs and Kazak nationalities accounted for 41.6%(47 patients),31.0%(35 patients) and 18.6%(21 cases), respectively.At admission, ALT level was (98.54±59.32) U/L before treatment, and decreased to (38.18±17.13) U/L after treatment, with statistically significant difference (t=6.627, P<0.05).AST levels before and after treatment were (93.17±59.19) U/L and (30.67±12.56) U/L, respectively, with significant difference (t=8.042, P<0.05).γ-GT levels before and after treatment were (162.27±48.19) U/L and (69.53±32.17) U/L, respectively, with statistically significant difference (t=8.271, P<0.05).Alb was (32.31±5.29) g/L before therapy, and increased to (38.00±4.27) g/L after therapy, with statistically significant difference (t=4.429, P<0.05).Conclusions Liver injury is common in patients with brucellosis.Elevation of transaminase is usually mild to moderate, accompanied by reduced ALB.For brucellosis patients with liver injury, antimicrobial treatment combined with liver protecting drugs could improve liver function effectively, and even within the normal range.
4.Stability and time of miniscrew used as orthodontic anchorage in dogs

Objective To evaluate the effects of different loading occasions of orthodontic force on the stability of miniscrew as an anchorage.Methods The titanium miniscrews were respectively implanted into canine and posterior region of both upper and lower jaws of 3 crossbred dogs.The implants were immediately loaded with 200 g orthodontic force or loaded after 2 weeks of implantation.The dogs were sacrificed after 8 weeks.The samples including the microscrew implant and its around bone tissue were extracted,fixed and X-ray photographed.Then the specimens were decalcified,embedded,sectioned and stained with HE and GOMORI.The sections were examined under light microscope.Results The stability of miniscrew in posterior region of jaws was higher than that in canine region in X-ray photograph.There was more bone trabecular formation between miniscrew and jaw bone in non-immediately loaded group than those in immediately loaded group with HE and GOMORI.Conclusion The non-immediately loaded implant as an anchorage can afford more stability than the immediately loaded implant.
6.Methylation status of TES gene promoter region in 25 patients with leukemia

Objective To study the methylation status of Testin protein (TES) promoter region in bone marrow of leukemia pa-tients ,and to explore the relationship between TES gene and leukemia .Methods The bone marrow specimens of 13 patients with primary acute myeloid leukemia and 12 patients with acute myeloid leukemia were studied .Bone marrow samples from 15 healthy subjects were used as controls ,the methylation level of TES gene promoter in bone marrow was detected by BSP ,the methylation status between healthy subjects and leukemia patients ,and whether there was any difference in methylation status between different types of leukemia were analyzed .Results In 25 cases of leukemia ,there were 21 cases of bone marrow samples ,and the hypermeth-ylation of TES gene promoter region was positive ,with a positive rate of 84 .0% .The methylation of the gene was not detected in bone marrow samples from 15 healthy subjects .The methylation rate of TES gene in bone marrow samples of 12 patients with acute lymphoblastic leukemia was 83 .3% ,the methylation rate of TES gene in bone marrow samples of 13 patients with primary acute myeloid leukemia was 84 .6% ,and there was no significant difference between the two groups (P>0 .05) .Conclusion Methylation of TES gene might be one of the biomarkers in the diagnosis of leukemia .
7.Prognostic significance of preoperative serum gamma glutamyl transpeptidase level in patients with hepatocellular carcinoma after liver resection

Objective To evaluate the prognostic significance of preoperative serum gamma glutamyl transpeptidase (GGT) level in patients with hepatocellular carcinoma (HCC) after liver resection.Methods A total of 432 patients undergoing hepatectomy for HCC were divided into normal GGT group (175 patients with GGT ≤ 50 U/L) and high GGT group (257 patients with GGT > 50 U/L).After balancing baseline characteristics by propensity score analysis,disease-free survival (DFS) and overall survival (OS) were compared between the two groups.Independent risk factors influencing DFS and OS were identified by Cox multivariate analyses.Results Propensity score analysis identified 124 matched pairs of patients from each group.In the propensity-matched cohort,DFS at 1,3,and 5 years in normal GGT group (69.3%,36.1%,12.8%) was significantly higher than that in high GGT group (60.6%,18.7%,7.5%;P=0.039).OSat1,3,and5 years innommlGGTgroup (90.7%,73.7%,66.1%) was also significantly higher than that in high GGT group (89.2%,63.6%,43.3%;P =0.024).COX multivariate analyses revealed that alpha-fetoprotein ≥400 ng/ml,GGT > 50 U/L,macrovascular invasion,tumor size ≥ 10 cm,and tumor number ≥3 were independent risk factors for DFS in patients with HCC after liver resection.Albumin < 35 g/L,GGT > 50 U/L,macrovascular invasion,tumor size ≥ 10 cm,and tumor number ≥ 3 were identified as independent risk factors for OS.Conclusions Preoperative serum GGT level is an independent factor predicting tumor recurrence and long-term survival in HCC patients after liver resection.
8.Diagnostic value of linked color imaging for Helicobacter Pylori infection compared with white light imag-ing

Objective To investigate the ability of linked color imaging(LCI)for diagnosing Helicobacter Pylori(Hp)infection compared with conventional white light imaging(WLI). Methods We prospectively collected subjects who underwent gastroscopy. Images under both WLI and LCI were recorded and analyzed. Software was used to measure pixel brightness for red(R),green(G),blue(B)of endoscopic images from suspected lesion sites. Biopsies were taken from these sites and Warthin-Starry silver staining was used to detect if Hp was present. R/(G+B)value was used to construct receiver operating characteristic curve(ROC)to predict Hp infec-tion and the area under curve(AUC),cut-off point,sensitivity,as well as specificity were calculated with the patho-logy as the gold standard. Results Forty-seven subjects(23 men,24 women;23 Uygur subjects,24 Han subjects) were included in a mean age of 49 years old. Ninety-one biopsies were obtained. Forty-four biopsies (48.4%)were Hp positive according to the pathology. The AUC of LCI was 0.616,with the cut-off point at 0.967 and sensitivity at 0.955 and specificity at 0.298. The AUC of WLI was 0.529,with cut-off point at 2.638 and sensi-tivity at 0.455 and specificity at 0.766. The AUC of Han and Uygur subjects were 0.650 and 0.549 by LCI. The AUC of atrophy gastritis subjects and non-atrophy gastritis subjects were 0.628 and 0.603. Conclusion LCI was superior to WLI in predicting Hp infection. LCI may act as an objective and quantified endoscopic diagnostic meth-od in Hp infection.
9.Circulating tumor cells in the diagnosis and treatment of early and advanced prostate cancer.

Circulating tumor cells (CTC) are tumor cells that escape from the primary or metastatic tumor into the circulatory system, and closely related to cancer metastasis. Since the samples can be obtained through simple and minimally invasive blood sampling operations, CTCs have a great clinical potential. PCa is one of the most common malignant tumors in men. In recent years, many scholars have conducted studies as to whether CTC technology can be used for the diagnosis and treatment of PCa, as well as for more accurate prediction of the risk of progression. This article summarizes the advances in researches relating CTC technology and the diagnosis and treatment of PCa. CTC detection has been developed from simple counting to phenotypic classification, and even to its combination with the determination of the expressions of specific genes (such as AR, AR-V7, etc.) and single-cell sequencing. Some reports showed that CTC technology has a certain significance in the early diagnosis of PCa, but its main value is demonstrated in drug sensitivity and prognosis evaluation in the late stage of the malignancy. The standardized detection methods and reference values of CTCs in PCa will be important research orientations in the near future.

10.Organic erectile dysfunction and metabolic syndrome in young and middle-aged men: analysis of 154 cases.

OBJECTIVETo investigate the association of erectile dysfunction (ED) with metabolic syndrome ( MS) and the total testosterone ( tT) level in young and middle-aged men.
METHODSThis study included 154 organic ED outpatients aged 20 -59 years and 103 age-matched men with normal sexual life. We obtained their waist circumference ( WC) , blood pressure ( BP) , fasting blood glucose ( FBG) , triglyceride ( TG) , high density lipoprotein ( HDL) , tT, IIEF -5 score, erectile function indexes and other relevant variables, and compared them between the ED and non-ED groups as well as between the ED patients with MS and those without MS.
RESULTSThe prevalence of MS was significantly higher in the ED patients than in the non-ED males ( 40. 9 vs 17.4%, P < 0. 01). Statistically significant differences were found between the ED and non-ED groups in WC ( [90. 80 ±4. 90] vs [87. 70 ±4. 60] em, P <0.01), SBP ([134.40±14.40] vs [129.90±12.40] mmHg, P CONCLUSIONThe prevalence of MS is significantly higher in young and middle-aged ED patients than in normal males, and MS is associated with a lower testosterone level and poorer erectile performance. Central obesity is closely correlated with ED in young and middle-aged men.
