1.Role of overexpression of myeloid cell leukemia-1-long in prognosis of malignant pleomorphic adenoma

Objective:To evaluate the clinicopathological significance of the expression of the two myeloid cell leukemia-1 isoforms, myeloid cell leukemia-1-long (Mcl-1L) and myeloid cell leukemia-1-short (Mcl-1S) in malignant pleomorphic adenoma (MPA).Methods:A total of 56 patients with MPA treated with surgery, the expression of Mcl-1L and Mcl-1S were detected by quantitative real-time PCR (qPCR) in mRNA level, and by Western blotting in protein level, respectively. The correlation between clinicopathological parameters (gender, age, site, habits, size, node, metastasis, recurrence) was done using the chi-square test. Kaplan-Merier curve was used to analyze the survival in all patients, Log Rank test was performed to observe the difference between groups. Cox′s proportional hazard test was used to identify the factors that were independent predictors of survival. Results:qPCR showed overexpression of Mcl-1L in 66.1%MPA patients (37/56), with significant difference in terms of tumor site, significantly higher expression of Mcl-1L in MPA (parotid gland: 2.21±1.07, sublingual gland: 2.12±1.08, other sites: 1.83±0.81) than benign pleomorphic adenoma tissue (1.00±0.19) ( P<0.01). Western blotting analysis showed significantly higher expression of Mcl-1L in MPA (parotid gland: 1.02±0.43, sublingual gland: 0.71±0.29, other sites: 0.70±0.30) than benign pleomorphic adenoma tissue (0.39±0.08) ( P<0.01). In malignant pleomorphic adenoma patients, high Mcl-1L expression was significantly associated with node positivity ( P=0.045), advanced tumor size ( P=0.002). Kaplan-Meier survival line analysis indicated a significantly lower survival time in Mcl-1L higher-expression patients (24.79±9.71) than lower-expression patients (30.59±9.01) ( P=0.017). Multivariate analysis indicated Mcl-1L to be an independent prognostic factor for MPA ( P=0.002). Conclusions:The studies suggest that overexpression of Mcl-1L is associated with poor prognosis in MPA. Mcl-1L is an independent prognostic factor in malignant pleomorphic adenoma.
2.Preliminary application of 3D printing individualized plastic guide plate in the treatment of pelvic and acetabular fracture

Objective To introduce the application of 3D printing individualized plastic guide plate in the treatment of pelvic and acetabular fracture and to evaluate its application value.Methods From June 2016 to April 2017,the clinical data of eleven adult patients with pelvic and acetabular fracture was analyzed retrospectively.The pelvic CT scan was performed before operation to obtain the original data.Three-dimensional reconstruction performed by mimics 17.0 software with the data and the fracture reduction performed on the software.3D printing individualized plastic guide plate was designed and made to guide the pre-implantation plate bending and shaping.The pre implantation plate bending was completed according to the plastic guide.Postoperative Matta imaging score was used to evaluate fracture reduction,while the function of the hip joint was assessed by the Harris scores.Results All the operations were successfully completed.There were no adverse complications during or after the operation.Anatomical reduction was performed in 10 cases and functional reduction in 1 case.The excellent and good rate of Matta scores was 90.9%,excellent in 7 cases,good in 3 cases,fair in 1 case and no poor case.The excellent rate of Harris scores was 81.8%,excellent in 6 cases, good in 3 cases,fair in 2 cases and no poor case.Conclusion In the treatment of pelvic and acetabular fracture,the application of 3D printing individualized plastic guide plate may not only make mode simple and feasible and save time and materials,but also simplify the operation,reduce the difficulty of operation and hold satisfactory clinical effect.
3.Evaluating clinical significance of ductular reaction in liver transplantation

Objective:To explore the role of ductular reaction in assessing the efficacy of liver transplantation.Method:From January 2015 to December 2020, he relevant clinical data were retrospectively reviewed for 100 recipients and their corresponding donors at Shulan (Hangzhou) Hospital. They were assigned into two groups of hepatic steatosis (HS group, 65 cases) and non-hepatic steatosis (non-HS group, 35 cases) according to whether or not receiving steatosis donated liver. Furthermore, based upon the occurrence of early allograft dysfunction (EAD), the participants were categorized into two groups of EAD (33 cases) and non-EAD (67 cases). The degree of bile duct reaction ductular reaction was defined by the percentage of staining area occupied by cytokeratin 19 (CK19) -positive bile duct cells in immunohistochemical-stained specimens. Donor of ductular reaction were compared between HS/non-HS and EAD/non-EAD groups. The risk factors for EAD were identified by univariate and multivariate Logistic regression analysis. Subgroup analysis was conducted based upon the level of ductular reaction (DR number) in donors (DR=0.4 as a threshold) and whether or not donors exhibited steatosis. The impact of DR was examined on the incidence of EAD and survival post-liver transplantation in steatosis donors.Result:The level of DR was higher in steatosis donor than that in non-steatosis donor [ (0.59%±0.385%) vs. (0.32%±0.194%), P<0.01]. And it was higher in EAD group than that in non-EAD group [ (0.72%±0.449%) vs. (0.38%±0.226%), P<0.01]. Multivariate logistic regression analysis showed that a high level of ductular reaction was an independent risk factor for EAD post-liver transplantation in donor. Subgroup analysis revealed that receiving a steatosis donor with low ductular reaction (DR<0.4%) had comparable levels of EAD occurrence and overall survival rate to receiving a non-steatosis donor. Conclusion:Steatosis with low ductular reaction donor may be safely applied for liver transplantation. And assessing donor injury based upon ductular reaction can effectively expand the clinical application of steatosis donors.
4.Flexion-lateral curvature-supination reduction for treatment of lower cervical dislocation with unilateral facet interlocking

Objective:To investigate the efficacy of flexion-lateral curvature-supination reduction combined with primary anterior surgery for the treatment of lower cervical dislocation with unilateral facet inter-locking.Methods:A retrospective analysis was performed in the 32 patients who had been admitted to Department of Spine Surgery, Honghui Hospital for lower cervical dislocation with unilateral facet interlocking from November 2015 to October 2018. According to their treatments, they were divided into 2 groups. In the emergency group treated by flexion-lateral curvature-supination reduction combined with primary anterior surgery, there were 13 males and 3 females, aged from 24 to 63 years. In the traction group treated by cranial traction reduction combined with secondary anterior surgery, there were 12 males and 4 females, aged from 20 to 64 years. The operation time, intraoperative blood loss, hospital stay, bone graft fusion, American Spinal Injury Association (ASIA) grade and Japanese Orthopaedic Association (JOA) score were compared between the 2 groups.Results:There was no significant difference in the preoperative general data between the 2 groups, showing they were comparable ( P<0.05). All patients were followed up from 26 to 40 months. The hospital stay for the emergency group [(7.2±1.2) d] was significantly shorter than that for the traction group[(10.9±1.2) d] ( P<0.05). There was no significant difference in the operation time, blood loss, ASIA grade or JOA score between the 2 groups ( P>0.05). All patients achieved osseous fusion of intervertebral space. Conclusion:Compared with traditional methods, flexion-lateral curvature-supination reduction combined with primay anterior surgery shows no significant difference in the recovery of neurological function but leads to a shorter hospital stay.

Objective:
To establish a real-time quantitative PCR detection system for Torque teno virus (TTV) and verify the sensitivity and specificity of the detection system.
Methods:
Primers and FAM-Eclipse probes were designed based on the TTV6 gene sequence registered in GenBank, and were to establish a real-time fluorescent quantitative PCR detecting way based on the FAM-Eclipse probe, the standard curve was constructed and sensitivity and specificity were analyzed.
