1. Influence of cranial base angle and posterior occlusal plane inclination on sagittal dentoskeletal types

BACKGROUND: Cranial base angle is related to the formation of sagittal dentoskeletal type, and the effect of occlusal plane or posterior occlusal plane on the formation of sagittal dentoskeletal type remains controversial. OBJECTIVE: To compare the importance of occlusal plane inclination and posterior occlusal plane inclination in determining the formation of sagittal dentoskeletal type, and to explore the combined influence of the cranial base angle and posterior occlusal plane inclination on the formation of sagittal dentoskeletal type, and its correlation. METHODS: The study was implemented in accordance with the ethical requirements of Hospital of Stomatology, Southwest Medical University. Patients and their guardians signed the informed consents. Totally 207 lateral cephalograms from patients (100 males and 107 females, aged 12-20 years) were selected. Dentoskeletal landmarks and subsequent measurements were performed and analyzed using Uceph software and SPSS 17.0 software, respectively. The samples were classified as three sagittal dentoskeletal types according to the anteroposterior dysplasia indicator (APDI) value: Class I, class II, and class III. The SNK and LSD methods were used for comparison among the three groups. The Pearson correlation coefficient analysis was used to analyze the correlation between skull base angle (NSBa angle), occlusion plane (FH-OP), posterior occlusion plane (FH-POP) and other related measurement indexes. Descriptive statistical methods were used to group occlusal plane inclination, posterior occlusal plane inclination and skull base angle, and the two-sample f-test was used to compare relevant bone and tooth indexes. Four dentoskeletal groups were established according to the posterior occlusion plane inclination and skull base angle: Small skull base angle-flat posterior occlusion plane group (S-F), small skull base angle-steep posterior occlusion plane group (S-S), large skull base angle-flat posterior occlusion plane group (L-F), large skull base angle-steep posterior occlusion plane group (L-S). Intergroup comparison was completed using crossover one-way analysis of variance. RESULTS AND CONCLUSION: The skull base angle was significantly smaller in the class III group than the class I and II group. Significantly higher posterior occlusion plane inclination was observed in the class II group than the class I and III groups. No differences were found in the APDI and ANB between occlusal plane groups. Significant differences in the ANB angle and APDI value were shown on the small skull base angle-flat posterior occlusal plane group and the large skull base angle-steep posterior occlusal plane group. These results indicate that among the factors affecting the formation of sagittal dentoskeletal type, the posterior occlusal plane is more representative than the occlusal plane. The curvature of the skull base and the inclination of the posterior occlusal plane affect the formation of sagittal dentoskeletal type. During the formation of sagittal dentoskeletal type, there may be some compensatory mechanisms between the curvature of the skull base and the posterior occlusal plane.

Objective Previons studies showed that,the gene of P16 is a kind of gene suppressing the cancer.its function once lose,the cell may change to cancer.Methods The expression of P16 has a closely relation with the malignant latent and evolve of the oesophagus cancer.ResulIs This research explored the relation between the P16 expression and the prognosis of oesophagus cancer,the results showed the positive rate of the expression of P16 is more low the rate ofexistence is more low,it in dicates the expression rate of P16 can reflect the prognosis of oesophagus cancer cell.Conclusion The expression of P16 missing or descending is closely rehted with the aggravate and evolve of oesophagus cancer,therefore the expression of P16 had a relation with the prognosis of oesophagus cancer.
3.Comparison study of time course of DKI and DTI in stroke

Objective To evaluate the changes of diffusion kurtosis imaging (DKI) parameters with time in cerebral in farction patients,and contrast with diffusion tensor imaging (DWI).Methods DWI and DKI scans were performed in 95 patients of cerebral infarction.The patients were divided into five groups according to the time of cerebral infarction:Hyperacute phase (n=10),acute phase (n=12),early subacute phase (n =33),late subacute phase (n =20) and chronic phase (n =20).Parameters of DKI were obtained,and the parameters and percentage change of diffusion metrics from normal to ischemic tissue were compared.The evolution rule of parameter with time was analyzed.Results Mean kurtosis (MK),axial kurtosis (K//),radial kurtosis (K⊥) of DKI parameters increased after infarction,and reached the peak at acute phase,and decreased gradually with the prolonging of time.Mean diffusion (MD),axial diffusion (D//),radial diffusion (D⊥) of DTI parameters decreased after infarction,and reached the lowest at the acute phase,and increased gradually with the prolonging of time.The percentage change of MK,K//,K⊥ were higher than those of MD,D//,D⊥,and percent change along the axial direction were significantly larger than that along the radial direction.Conclusion DKI is superior to DTI in evaluating cerebral infarction,and can analyze the changes of microstructure of cerebral infarction comprehensively.
4.Chinese herbal medicine for the treatment of endocrine therapy-related osteoporosis among patients with breast cancer: A systematic review and meta-analysis

Objective:
To assess the efficacy and safety of combining traditional Chinese medicine (TCM), specifically Chinese herbal medicine (CHM), with Western medicine (WM), compared to WM alone to treat breast cancer endocrine therapy-related osteoporosis (BCET-OP) by meta-analysis.
Methods:
Thirty-eight randomized controlled trials involving 2170 participants were analyzed. Eight databases were searched for articles published between inception and December 2023. Quality assessment was performed using the Risk of Bias 2 tool.
Results:
Significant increases were observed in the TCM-WM group in lumbar vertebrae bone mineral density (BMD) (P < .001, mean difference (MD) = 0.07, 95% confidence interval (CI): 0.06 to 0.08), lumbar vertebrae T-score (P = .0005, MD = 0.21, 95%CI: 0.09 to 0.33) and collum femoris BMD (P = .01, MD = 0.10, 95%CI: 0.02 to 0.19). No significant difference was observed between the groups in the collum femoris T-score and estradiol levels. Bone gla-protein levels were significantly increased in the TCM-WM group (P = .0002, MD = 0.52, 95%CI: 0.25 to 0.79). Beta-CrossLaps decreased significantly in the TCM-WM group (P = .0008, MD = −0.10, 95%CI: −0.16 to −0.04). No significant difference was observed between the TCM-WM and WM groups in alkaline phosphatase, in procollagen type I N-terminal propeptide, and in the Kupperman index. The visual analog score (VAS) was decreased in the TCM-WM group compared to the WM group (P < .001, MD = −1.40, 95%CI: −1.94 to −0.87). No significant difference in adverse events was observed between the two groups.
Conclusion
Combining CHM with WM in patients with BCET-OP significantly improved BMD, T-score, and certain bone turnover markers and reduced the VAS score, indicating potential benefits for bone health and related pain. Adverse event analysis revealed no differences between the groups, supporting the feasibility of the combination therapy. However, further research, particularly in diverse populations, is required.
5.Determination of the protein-binding rates of gardenioside, paeoniflorin and naringin in Weiyanling granules in different species of plasma

Objective To study the protein binding rates ofgardenioside,paeoniflorin and naringin in Weiyanling granules in bovine serum albumin,rat plasma and human plasma.Methods The equilibrium dialysis method was carried to determine the binding-rates of three components in Weiyanling granules in bovine serum albumin,rat plasma and human plasma.The HPLC method was used to determine the mass concentration of three components in weiyanling granules in inner and outer dialysis and to calculate the plasma protein-binding rates.Results The linear relationship,precision,extraction recovery and stability of gardenoside,paeoniflorin and naringin all conformed to the methodological requirements.Within the low,middle and high concentration (5.084,10.04 and 20.08 g/L) of Weiyanling granules,the protein-binding rates of gardenioside and bovine serum albumin were 23.77% ± 13.39%,10.75% ± 1.34%,5.82% ± 6.53%,respectively;the protein-binding rates of paeoniflorin were 51.81% ± 6.53%,30.12% ± 6.61%,21.39% ± 9.45%,respectively;the protein-binding rates of naringin were 70.21% ± 3.31%,67.61% ± 1.31%,56.00% ± 3.46%,respectively;the protein-binding rates of gardenioside and rat plasma were 71.56% ± 3.00 %,43.56% ± 12.22%,40.63% ± 4.73%,respectively;the protein-binding rates ofpaeoniflorin and rat plasma were 55.72% ± 1.60%,47.59% ± 6.33%,40.07% ± 0.72%,respectively;the protein-binding rates of naringin and rat plasma were 85.85% ± 3.53%,85.38% ± 0.99%,79.99% ± 2.57%,respectively;the protein-binding rates of gardenioside and human plasma were 13.56% ± 2.40%,3.02% ± 2.41%,5.82% ± 2.08%,respectively;the protein-binding rates of paeoniflorin and human plasma were 9.49% ± 5.23%,5.01% ± 1.10%,3.98% ± 1.54%,respectively;the protein-binding rates of naringin and human plasma were 25.04% ± 2.61%,26.09% ± 13.82%,20.20% ± 2.24%,respectively.Conclusions Within 5.084,10.04 and 20.08 g/L,the protein binding-rate of gardenioside,paeoniflorin and naringin were shown asfollow,rat plasma >bovine serum albumin > human plasma.There were significant differences in species.
6.Three-dimensional finite element analysis of extrusion of the maxillary canine during orthodontic treatment with invisible aligner without brackets

BACKGROUND: With the advancement of science and technology, invisible aligner without brackets have emerged. However, this new type of digital orthodontic technology is rarely reported on its biomechanics. OBJECTIVE: By the use of rectangular attachments of different sizes and different bonding positions, to obtain different force of canines and the stress supporting of periodontal tissue during the extrusion movement, providing reference for the clinical use of rectangular attachment in the orthodontic treatment of low canine tooth. METHODS: A three-dimensional finite element model of appliance-attachments-canine-periodontium-cancellous bone-cortical bone was established based on the scan data of the isolated canine. The attachments were set in different directions (vertical, horizontal), sizes (3, 4, 5 mm in height) and positions (in the occlusal side of the crown center, in the gingival side of the crown center). The 0.25-mm forced occlusal displacement was loaded to observe the deformation and equivalent stress of the tooth and periodontal membrane. RESULTS AND CONCLUSION: (1) If the size of rectangular attachment increases, the force of the tooth will grow in a certain range and the tooth will have better effect of extrusion with a larger level of tilt motion in other directions. (2) The extrusion effect of the horizontal attachment is better than that of the vertical attachment, which is more difficult to have distal incline, but is prone to lingual incline.
7.Construction of evaluation index system for employment quality for medical graduates

Objective:To establish an index system for evaluating the employment quality of medical students in universities.Methods:Literature review and expert consultation methods were used to formulate preliminary evaluation indicators. Nine experts were selected to conduct three rounds of correspondence through Delphi method, and the indicators and the weights of the first, second and third levels were determined. Excel 2016 and SPSS 22.0 were used to make data entry and analysis.Results:The effective recovery rate of the questionnaire was 100.0%, the average expert authority coefficient was 0.749(>0.700), and the expert coordination coefficients were 0.228 and 0.212 ( P<0.001). Finally, an evaluation index system for the employment quality of medical students in universities was constructed, which was composed of 5 first-level indicators, 31 second-level indicators and 66 third-level indicators. Among them, the weights of the first-level indicators were 0.283, 0.250, 0.200, 0.108 and 0.158, respectively. Conclusion:The employment quality evaluation index system for medical students constructed in this study is systematic, comprehensive and multi-dimensional, which can provide a reference for the evaluation on medical graduate employment quality, and its scientificity and practicability need to be further explored in empirical research.
8.Advances in molecular mechanism of berberine in treatment of glioma

Glioma is one of the most common primary intracranial tumors, accounting for 80% of malignant brain tumors. The conventional treatment of glioma is surgical resection followed by temozolomide chemotherapy, but the drug resistance will gradually appear that results in a poor prognosis of the patient. Berberine is an alkaloid extracted from Coptis Rhizoma, which has a wide range of pharmacological activities. It exerts its pharmacological effects on glioma such as inhibiting tumor growth through controlling different molecular and cellular pathways. In this article, the application of berberine in the treatment of glioma and the research progress of specific molecular mechanism are reviewed.
9.Analysis of " Internet plus healthcare" related policies in the eastern, central and western regions of China before and after the COVID-19 pandemic

Objective:To analyze the commonalities and differences of " Internet plus healthcare" related policies in the eastern, central and western regions of China before and after the COVID-19 pandemic and to provide references for further improving China′s " Internet plus healthcare" policy.Methods:Collecting the policies of " Internet plus healthcare" promulgated by 31 provinces(autonomous regions, municipalities)in China from April 2018 to December 2020 on the websites of various government departments, extracting keywords based on content analysis methods, and calculating degree centralities using UCINET 6 software. A comparison of the policies of the eastern, central and western regions between the pre epidemic stage(April 2018 to December 2019)and the COVID-19 pandemic stage(January 2020 to December 2020)was carried out.Results:In the pre epidemic stage, Internet diagnosis and treatment, " Internet plus medical service price" and quality supervision were the focus of attention in all regions. But the eastern region paid more attention to the policy planning of Internet hospital operation and Internet medical application, the central region focused on the transformation and efficiency improvement of medical service process by Internet technology, and the western region paid more attention to encourage the application of telemedicine to solve the difficulty of seeing a doctor. In the pandemic stage, the " Internet plus service price" was the policy focus in all regions, mostly in medical insurance payment, and the policy difference was not obvious among the eastern, central and western regions.Conclusions:Before and after the COVID-19 pandemic, the policy of " Internet plus healthcare " promulgated by the local governments in eastern, central and western regions had different emphasis and different characteristics. Our suggestion was to improve the policy system, standardize industry supervision, strengthen information security, orderly interconnect medical insurance, and actively promote the exchange of policy experiences among the provinces.
10.Correlation between serum lipoprotein (a) and the severity of white matter hyperintensities in middle-aged and elderly people in the community

Objective:To investigate the correlation between serum lipoprotein (a) [LP(a)] and the severity of white matter hyperintensities (WMHs) in the middle-aged and elderly people in the community.Methods:Consecutive middle-aged and elderly residents residing in the community underwent physical examinations in the Affiliated Jiangning Hospital of Nanjing Medical University from June 2016 to August 2021 were retrospectively collected. Fasting venous blood was collected on the next day of admission to detect the level of Lp(a). During hospitalization, cranial MRI examination was performed and the severity of WMHs was graded using the Fazekas visual scoring method. Ordinal multivariate logistic regression analysis was used to determine independent related factors for the severity of WMHs.Results:A total of 1 752 patients were included in the analysis. There were 969 males (55.31%) and 783 females (44.69%). Their age was 66.18±10.32 years old. There were 1 167 patients (66.61%) in the mild WMHs group, 407 (23.23%) in the moderate WMHs group, and 178 (10.16%) in the severe WMHs group. Ordinal multivariable logistic regression analysis showed that after adjusting for confounding factors, a higher serum Lp(a) level was independently related to the severity of WMHs (with the first quartile as a reference, the third quartile: odds ratio 1.441, 95% confidence interval 1.050-1.976, P=0.023; the fourth quartile: odds ratio 1.717, 95% confidence interval 1.252-2.354, P=0.001). Conclusion:Serum Lp(a) is independently correlated with the severity of WMHs.
