1.Comparison of long-term effects between procedure for prolapse and hemorrhoids and Milligan Morgan hemorrhoidectomy in the treatment of Ⅲ and Ⅳ degree of hemorrhoids

Objective To compare the long-term effects between procedurefor prolapse and hemorrhoids(PPH) and Milligan-Morgan hemorrhoidectomy( MMH ) in the treatment of Ⅲ and Ⅳ degree of hemorrhoids. Method The clinical data of 43 cases with Ⅲ and Ⅳ degree hemorrhoids received PPH (PPH group)and 71 cases received MMH (MMH group) were analyzed retrospectively from October 2001 to July 2003. Results Late bleeding in 5 cases of PPH group and no one of MMH group (P <0.05).Hemorrhoidal recurrence prolapsed in 9 cases of PPH group and 5 cases of MMH group (P < 0.05). The rectal neck pressure preoperative,after 6 months, 1 year and 3 years were (122.0 ± 11.3), (90.0 ± 10.4),(103.0 ± 13.8) and (113.0 ± 11.2) cm H2O (1 cm H2O =0.098 kPa) of PPH group,after 6 months was decreased obviously than preoperative (P<0.05),and they were ( 126.0 ± 13.5), (91.0 ± 12.4), (78.0 ±9.8) and (81.0 ± 7.5 ) cm H2O of MMH group,after 6 months, 1 year and 3 years were decreased obviously than preoperative (P < 0.05). Conclusion PPH is safety and effect at short time,but it brings more late bleeding and anal pain,and its rate of prospective hemorrhoidal recurrence is more higher than MMH.
2.Studies on the protection mechanism of glucocorticoids against the inflammatory injury of vascular endothelial cells

Objective To explore the protection mechanism of glucocorticoids (GC) against the inflammatory injury of vascular endothelial cells (VEC). Methods An inflammatory injury model of VEC was reproduced by treating the human umbilical vein endothelial cells (HUVEC) with lipopolysaccharides (LPS) in vitro. The effect of dexamethasone (Dex) on apoptosis of HUVEC and the release of IL-6 and sICAM-1 of HUVEC was observed, and then the effect of RU486 (an antagonist for glucocorticoid receptor) on reversing the effect of Dex was also observed. Results LPS could induce apoptosis of HUVEC, and the apoptosis rate reached 36.7%?3.9%. On the other hand, glucocorticoid (Dex) could obviously inhibit the apoptosis of HUVEC in LPS, and the apoptosis rate was reduced to 13.2%?0.9%. Objective To investigate the effect of dexamethasone (Dex) on the transcription of 11 ?-hydroxysteroid dehydrogenase type 1 (11?-HSD1) gene in vascular endothelial cells. The roles of p38 mitogen-activated protein kinase (p38MAPK) signaling pathway and glucocorticoid receptor (GR) were also observed. Methods Vascular endothelial cells were co-cultured with different concentrations of Dex (10-9-10-3mol/L) for 24h. Reverse transcription polymerase chain reaction (RT-PCR) was employed to detect 11?-HSD1 mRNA in each group. Normally cultured cells, without contact with Dex, served as normal control group. The cells were co-cultured with p38MAPK specific inhibitor SB20358 (10-2mol/L) and GR specific inhibitor RU486 (10-6mol/L) for 24h, then RT-PCR was employed to detect 11?-HSD1 mRNA in each group. Among the groups, Dex-treated cells and non-intervened cells were grouped as negative control and normal control, respectively. 4.Protocols designing of treating varicose veins of the lower extremity by combining surgery and sclerotherapy, report of 31 cases

Objective To evaluate combination of traditional surgery and sclerotherapy in treating varicose veins of the lower extremity.Method This protocol includes high ligation of the great saphenous vein (GSV),stripping or laser closing of GSV above knee,injection of sclerosing agent into GSV below knee;ligating the perforator veins;sclerosing agent injection for varicose vein in 31 cases of varicose vein of the lower extremitis,among them seven were recurrent cases from April 2015 to June 2015.The clinical classification was as follows:nine cases of C2,four cases of C3,seventeen cases of C4,and one case of C6.All the perforator veins were located before the operation by color ultrasound.Results The number of perforator veins varied significantly (P <0.01) among the inner and back side of calfs (4.16 ± 1.86),the lateral calfs (1.29 ± 1.10),and thighs (0.16 ± 0.45).Twenty-four patients received high ligation of GSV combined with stripping of above-knees.Two patients received high ligation of GSV combined with the laser closing of above-knees.Five cases were treated conservatively (in which four have stripped or closed and one was on normal condition).Below-knee GSV were treated by sclerosing foam in 27 cases,and four cases were untreated (in which three have closed and one was normal).All the thirty-one cases underwent mini-incision for ligating the perforators and having the varicose veins closed using the sclerosing foam.All patients' GSV with sclerotherapy have got sclerosed.There were no complications of deep vein thrombosis,pulmonary embolism,or visual disturbance.Conclusion This operation design ensures the effect and prevents the recurrence of varicose vein,as well as reduces surgical trauma.
5.Mechanism of 11?-HSD2 gene expression up-regulated by endotoxin in vascular endothelial cells

Objective To investigate effects of endotoxin on 11?-HSD2 gene transcription in vascular endothelial cells to observe the role of p38 mitogen-activated protein kinase (p38 MAPK) signaling pathway. Methods The effects of endotoxin in the presence or absence of p38 MAPK specific inhibitor SB203580 on the transcription of 11?-HSD2 in vascular endothelial cells was evaluated by reverse transcription DNA polymerase chain reaction. Results Treatments of endotoxin (1.0, 10, 20, 50, 100 ?g/ L) for 24 h increased the ratios of 11?-HSD2mRNA/?-actin mRNA in vascular endothelial cells. The induction of 11?-HSD2 mRNA by endotoxin could be inhibited partially by 10 mmol/ L SB203580. Conclusion Endotoxin stimulated the transcription of 11?-HSD2 gene in vascular endothelial cells. The activation of p38 MAPK might be an important mechanism of 11?-HSD2 gene induced by endotoxin.

Objective To study the preventive effect of volatile oil extracted from Foeniculum vulgare Mill. Seeds on the formation of postoperative intra-abdominal adhesion. Methods Thirty-eihgt SD rats were randomly divided into sham operation group (10), operation group (14) and volatile oil treated group (14):sham operation group was only operated by abdominal incision, the rest two groups were established animal model of abdominal adhesion by rubbing the procussus vermiformis of cecum with dry sterile gauze, clamping and scuffing abdominal wall. Half of rats were separately killed on day 7 and day 14 after surgery, respectively.The degree of adhesion was evaluated according to Phillips 5-scale grade and the feature of this model. Results The scores of intra-abdominal adhesion were significantly lower in the carbachol group than those in operation group both on 7 d and 14 d(P<0.01 ). Conclusion Volatile oil extracted from Foeniculum vulgare Mill. Seeds may take a significant role in the prevention of postoperative abdominal adhesion in rats.

Hyperpolarization-activated cyclic nucleotide-gated (HCN) channels are confirmed to be expressed in bladder interstitial Cajal-like cells (ICC-LCs), but little is known about their possible role in cystitis-associated bladder dysfunction. The present study aimed to determine the functional role of HCN channels in regulating bladder function under inflammatory conditions. Sixty female wild-type C57BL/6J mice and sixty female HCN1-knockout mice were randomly assigned to experimental and control groups, respectively. Cyclophosphamide (CYP)-induced cystitis models were successfully established in these mice. CYP treatment significantly enhanced HCN channel protein expression and I(h) density and significantly altered bladder HCN1 channel regulatory proteins. Carbachol (CCH) and forskolin (FSK) exerted significant effects on bladder ICC-LC [Ca²⁺]i in CYP-treated wild-type (WT) mice, and HCN1 channel ablation significantly decreased the effects of CCH and FSK on bladder ICC-LC [Ca²⁺]i in both naive and CYP-treated mice. CYP treatment significantly potentiated the spontaneous contractions and CCH (0.001-10 µM)-induced phasic contractions of detrusor strips, and HCN1 channel deletion significantly abated such effects. Finally, we demonstrated that the development of CYP-induced bladder overactivity was reversed in HCN1 -/- mice. Taken together, our results suggest that CYP-induced enhancements of HCN1 channel expression and function in bladder ICC-LCs are essential for cystitis-associated bladder hyperactivity development, indicating that the HCN1 channel may be a novel therapeutic target for managing bladder hyperactivity.
