1.Assistant therapic effect of the botulinum toxin A on tiptoe after cerebral palsy

ObjectiveTo study the assistant therapic effect of the Botulinum toxin A (BTA) on tiptoe after cerebral palsy(CP).MethodsMulti point injection of BAT into the musculus gastrocnemius combined with the rehabilitate therapy was used on 35 children(39 feet)with tiptoe after CP.The therapeutic effect was evaluated after a month.ResultsThe efficiency rate is 25.6%,the slight efficiency rate is 71.8%,the ineffective rate is 2.6%.There is no toxic or side effect was observed.Conclusions The BTA can relieve spasm of musculature and redress abnormality from convulsing of musculature.
2.The 18F-fluorodeoxyglucose positron emission tomograhy in predicting residual retro-peritoneal tumor after chemotherapy of testis germ cell tumors

Objective:To determine the value of 18F-fluorodeoxyglucose positron emission tomography (18F-FDG PET) scans in identify-ing the residual retroperitoneal tumor after chemotherapy of testis germ cell tumors. Methods:Sixteen testis germ cell tumor patients with metastasis of retroperitoneal lymph nodes who were treated in our hospital from February 2014 to December 2016 were select-ed for the study from February 2014 to December 2016. After 4-6 cycles of chemotherapy, their CT scans showed residual masses with diameters greater than 2 cm. The retroperitoneal lymph nodes were dissected after the 18F-FDG PET exam. The post-surgery pathology results were compared with the results of the 18F-FDG PET exam. Results:Residual tumors were found in 5 of 10 patients with 18F-FDG PET positive. Residual tumor was absent in 4 of 6 patients with 18F-FDG PET negative, while residual mature teratoma tumors were found in two patients. The accuracy rate, sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of the 18F-FDG PET exam were 56.25%(9/16), 71.42%(5/7), 44.44%(4/9), 50.00%(5/10), and 66.67%(4/6), respectively. Conclusion: 18F-FDG PET is highly sensitive. However, many factors influence the result of 18F-FDG PET. Mature teratoma leads to a false negative re-sult, whereas massive tissue inflammation leads to a false positive result. Therefore, more clinical examinations should be made.
3.Influence of overactive bladder on symptoms and quality of life in patients treated with mid-urethral suspension

Objective To study the influence of overactive bladder (OAB) on the symptoms and quality of life (QoL) in patients after Transvaginal Tape-Obturator (TVT-O) treatment for stress urinary incontinence (SUI).Methods Eighty-two SUI women underwent TVT-O were included in this study.The perioperative ICI-Q-SF scores,including frequency of leakage,volume of leakage,and QOL scores,were collected to study the perioperative OAB prevalence and the influence of OAB on the improvement on the symptoms and QOL after TVT-O.Results The total efficacy of TVT-O was 95%.The average symptom and QOL scores in all patients decrease from (8.17±2.29)and (7.95±2.16)to (1.84±2.34)and(1.32±2.24)after TVT-O,P<0.05.The prevalence of OAB decreased from 46% to 24% after TVT-O,P<0.05.The average post-operation symptom and QOL scores of the pre-operation non-OAB group were (1.09±1.60)and(0.82±1.45),while those of the OAB group were (2.71±2.75) and (1.89±2.82),P<0.05.The average post-operation symptom and QOL scores of the post-operation non-OAB group were (1.06±1.68) and (0.56±1.15),while those of the OAB group were (4.25±2.49) and (3.65±3.10),P<0.05.Conclusions TVT-O is effective for female stress urinary incontinence and contributes to the significant improvement of symptoms and QOL in patients with OAB.The treatment will not raise the prevalence of OAB in itself.
4.Bladder outlet and bladder function change after the mid-urethral suspension treatment

Objective To study the changes of the symptoms and quality of life (QOL) in patients after the Transvaginal tape-obturator (TVT-O) treatment and weather would this treatment lead to bladder outlet obstruction (BOO) and bladder function change. Methods A total of 82 stress urinary incontinence (SUI) women who had underwent TVT-O were included, age from 39-82 years, 3-60 months after operation. All patients were divided into 5 groups by the time post-operation. The ICI-Q-SF scores were collected to study the change of the symptoms and QOL of each group perioperation. Qmax, PVR, bladder weight, wall thickness of each group were collected to study whether would TVT-O lead to BOO and bladder function change. Results Forty-five (55%) patients were cured and 33(40 %) improved after TVT-O. Total cure rate was 95 %. There was a significant improvement of average symptom and QOL in all of the 5 groups (3-month, 6-month, 1-year, 2-year, above-2-year) (P<0. 05). Qmax changed from (32. 17±10.74), (25.64±6.05), (25.68±11.36), (25.70±10. 28), (25.29±15. 10)ml/s pre-operation to (28.30±15.21), (25.64±10. 48), (24. 49±7.96),(25.43±11.92), (25. 92± 10. 17)ml/s post-operation in 5 groups(P>0. 05). PVR changed from (0.04±0.20), (0. 00±0.00), (0. 17±0. 39), (2.70±6.01), (1. 96±5.10)ml pre-operation to (1. 79±6.16), (0.00±0.00), (12. 50±29.89), (5.00±15. 54), (33. 70±92.10)ml post-operation in 5 groups(P>0.05). The bladder weight changed from (43. 62±7.81), (41. 57±8. 63), (46.11±8.12), (42. 50±3. 85), (44. 52±4. 38)g pre-operation to (45. 62±5.32), (47.21±5. 47),(44.89±4. 65), (45. 14±4. 13), (46. 70±5.84)g post-operation, which differed significantly in 6-month, 2-year, above-2-year groups (P<0. 05). The bladder wall thickness changed from (2.23±0.51), (2.22±0.47), (2.04±0.38), (2. 19±0. 43), (2. 19±0. 38)mm pre-operation to (2.25±0. 40), (2. 18±0.47), (2. 07±0.47), (2.22±0. 47), (2. 20±0. 44)mm post-operation(P>0. 05).Conclusions TVT-O has a high cure rate for SUI and there was a significant improvement of average symptom and QOL of all patients after the treatment. But TVT-O may lead to BOO and bladder function change since bladder weight increases significantly in some patients.
5.Value of real time shear wave elastography in the antidiastole of biliary atresia in infants within 60 days

Objective To investigate the value of shear wave elastography (SWE) in the differential diagnosis of biliary atresia (BA) in infants within 60 days.Methods SWE was used to detect the hepatic Young's modulus of 85 infants within 60 days who were suspected with cholestatic hepatitis.All the patients were classified into BA group and non biliary atresia (Non-BA) group according to surgical or pathological results.At the same time,45 normal infants within 60 days were selected as the control group.The hepatic Young's modulus were compared among the 3 groups.The sensitivity and specificity of hepatic Young's modulus in suspected BA patients were calculated by ROC.Results In the 85 cases,42 cases were diagnosed as BA,and 43 cases were classified as Non-BA group.The Young's modulus values of the 3 groups were as follows:BA group (15.86 ± 5.89) kPa,Non-BA group (10.89 ± 2.38) kPa,and control group (6.64 ± 0.53)kPa,with a significant difference among the 3 groups(P <0.01).The ROC analysis showed a cut off value of 12.9 kPa for differentiation between BA group and Non-BA group.The area under ROC curve (AUROC) was 0.787,with a sensitivity of 59.5% and a specificity of 88.4%.Conclusions The young's modulus in BA patients can be increased at early time,and the SWE technique can be used to reflect the change of hepatic Young's modulus accurately,which will be helpful in the early differential diagnosis of BA.
6.Study of principles of non-invasive glucose monitoring and instruments with the methods.

Non-invasive blood glucose monitoring will be the development direction for detecting the blood glucose concentration of body in time. In this way, the concentration of the blood glucose can be controlled effectively, then the complicating diseases of diabetes can be reduced, so it is of great significance for diagnosis and treatment of diabetes. The recent developments of non-invasive blood glucose concentration monitoring technologies, including basic principles, results of verification test and instruments, are discussed, especially three methods with instruments facing market. The existing problems of these methods are also discussed. Finally, some difficult points of current non-invasive blood glucose monitoring methods are further discussed and the future trend of the technologies has been pointed out according to the above analysis.

7.Value of shear wave elastography in diagnosing biliary atresia by ultrasonography

Objective To investigate the value of the shear wave elastography ( SWE) in the diagnosis of biliary atresia ( BA ) in infants by ultrasonography . Methods SWE was used to measure the liver stiffness measurement( LSM ) of 138 infants . Following surgical or pathological diagnosis ,the infants were subclassified into BA and non-biliary atresia ( non-BA ) groups . ROC analysis was used to determine the sensitivity and specificity of LSM ,the ultrasonic findings in the differential diagnosis of suspected BA ,and the cut-off value to diagnose BA . Results Of the 138 infants with cholestatic hepatitis ,51 were diagnosed as BA group and 87 cases were assigned to the non-BA group . The ROC analysis showed the cut-off value of LSM for diagnosis of biliary atresia in children with jaundice was 12 .35 kPa ,the AUC was 0 .936 ,and the sensitivity and specificity were 84 .3% and 89 .7% ,respectively .In the parallel test ,the triangular cord ( TC) sign combined with abnormal gallbladder ( AbGB) had the best diagnostic performance in all patients . However ,in patients older than 30 days ,the LSM combined with TC had the best AUC ( 0 .987 ) and diagnostic performance( 98 .6% ) like with TC combined with AbGB . Conclusions Both SWE and gray scale ultrasound have higher diagnostic performance for BA . LSM can be used as a valuable index for diagnosing BA by ultrasound .
8.Value of shear wave elastography for the evaluation of hepatics fibrosis in infants with biliary atresia before Kasai portoenterostomy

Objective:To investigate the application value of shear wave elastography(SWE) in assessing the degree of hepatics fibrosis in children with biliary atresia before Kasai portoenterostomy.Methods:From January 2017 to January 2018, 49 cases of children in Hunan Children′s Hospital with highly suspected biliary atresia in clinical were collected. The stiffness of the lower segment of right anterior lobe of the liver in all cases was assessed using SWE in 3 days before Kasai portoenterostomy. Pathological results of liver tissue specimens from the lower edge of the right liver during the surgery was taken as the gold standard. The correlation between the liver stiffness and the hepatics fibrosis was analyzed using Spearman correlation analysis. The receiver operating characteristic (ROC) curves were created to assess the area under curve (AUC), the best cutoff value, sensitivity, specificity, positive predictive value, negative predictive value and accuracy of the liver stiffness in diagnosing hepatocirrhosis.Results:All of the 49 cases were confirmed by surgery and pathology. According to the stages of hepatics fibrosis, 5 cases were in stage S2, 23 cases in stage S3, 21 cases in stage S4, and no cases of stage S0 and S1. Spearman correlation analysis showed that the correlation coefficient between the liver stiffness and the stages of hepatics fibrosis was 0.779( P<0.001). The area under the curve of the liver stiffness in diagnosing cirrhosis was 0.914. When the cutoff value was 14.45 kPa, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy were 85.7%, 89.3%, 85.7%, 89.3%, 87.8%, respectively. Conclusions:The liver stiffness measured by SWE is positively correlated with the stages of hepatic fibrosis, and the performance of SWE in the diagnosis of cirrhosis is remarkable. SWE can be used to evaluate the stages of hepatics fibrosis in children with biliary atresia before Kasai portoenterostomy.
9.Expression and significance of serum leptin and lung tissue leptin receptor in rats with pulmonary arterial hypertension induced by monocrotaline

Objective To study the significance of leptin and its receptor(OB-R) in the occurrence and development of pulmonary arterial hypertension(PAH) induced by monocrotaline(MCT).Methods Fifteen SD rats were divided into the control group(n=5) and two experimental groups(n=10).The experimental groups were intraperitoneally injected by MCT for constructing the PAH model and the control group was injected by the same dose of solvent groups.The venous blood was extracted at 2,4 weeks after MCT injection in the two experimental groups.The mean pulmonary artery pressure(mPAP) and right ventricular hypertrophy index(RVHI) were measured and then the lung tissue was removed.The pathological change of lung blood vessels was observed.The expression of serum leptin was detected by ELISA.The expression of OB-R in lung tissue was tested by Western blot.Results Compared with the control group,mPAP and RVHI in the experimental groups were significantly increased(P<0.05);the expression levels of serum leptin and lung tissue OB-R were increased significantly(P<0.05),moreover,which were positively correlated with mPAP(r=0.912,P<0.05;r=0.861,P<0.05).Conclusion Leptin and OB-R may play an important role in the occurrence and development of PAH induced by MCT.
10.The effect on postoperative pain and complications in patients with health education for senile fracture characteristics of traditional Chinese medicine

Objective To explore the effect of postoperative pain and complications in patients with health education for senile fracture characteristics of traditional Chinese medicine. Methods A retrospective analysis was performed on the clinical data of 98 cases of senile fracture patients in our hospital from October 2013 to October 2014,and were divided into control group (50 cases) and observation group (50 cases),and were given characteristics of traditional Chinese medicine health education and routine nursing care.The total effective rate,pain score,the incidence of com-plications of the two groups were observed and compared. Results The total effective rate of the observation group was significantly higher than that of control group(P<0.05).The pain score of observe group was significantly lower than that of control group (P<0.05);the incidence rate of complications of observation group was significantly lower than that of control group (P<0.05). Conclusion The implementation of patients before treatment,after treatment of senile fracture characteristics of traditional Chinese medicine and rehabilitation period of the full range of health education, can ef-fectively alleviate the effect of postoperative pain,and reduce the complication rate of reduction.
