1.Systematic review of efficacy and safety of rosuvastatin in treatment of patients with primary hyperlipidemia in China

Objective:To evaluate the efficacy and safety of rosuvastatin and atorvastatin in the treatment of the patients with primary hyperlipidemia in China.Methods:The related literatures in CNKI,VIP,Wanfang medicine network,PubMed/MEDLINE,CBM and Chinese dissertations full text database were retrievaled by computer from the establishment time of database to December 31,2015. Two researchers according to the inclusion and exclusion criteria independently selected the studies and extracted the data and assessed the quality of the literatures.The Revman 5.0 software was used to perform Meta analysis of all effect indicators in various groups.Results:A total of 7 randomized controlled trial (RCT)were included,and there was no significant abnormality in bias evaluation. 8 weeks after treatment, the total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C)and high density lipoprotein cholesterol (HDL-C)levels of the patients in 5 mg rosuvastain group and 10 mg atorvastatin group had no significant differences between before and after treatment (P >0.05);the HDL-C levels of the patients in 10 mg rosuvastatin group and 10 mg atorvastatin group had significant differences between before and after treatment (P < 0.05),but the TG,TC,and LDL-C levels had no significant differences (P >0.05);the TG,TC,LDL-C and HDL-C levels of the patients in 5 mg and 10 mg rosuvastatin groups had no significant differences between before and after treatment (P > 0.05).12 weeks after treatment,there were no significant differences in the TC and LDL-C levels between 10 mg rosuvastatin group and 10 mg atorvastatin group (P >0.05),but there were significant differences in the TG and HDL-C levels (P <0.01).The incidence of adverse reactions of the patients in three groups had no significant differences (P >0.05).Conclusion:5 mg rosuvastatin and 10 mg atorvastatin in the treatment of the patients with primary hypercholesterolemia have similar lipid-lowering effect;with the the increase of the treatment time and the dose,10 mg rosuvastatin can obviously reduce the TG level and increase the HDL-C level of the patients,and the incidence of adverse reactions of two kinds of doses of rosuvastatin has no obvious difference.
2.Characteristics of three dimensional reconstruction of the portal vein systems of patients with portal hypertension

With the development of modern imaging technologies,three-dimensional (3D) reconstruction techniques based on the computed tomography (CT) or magnetic resonance imaging (MRI) data has been gradually applied in clinical diagnosis and treatment of hepatobiliary diseases.In order to study the characteristics of the 3 D model of portal branches and collateral circulation in various types of portal hypertension,the data of 64-slice spiral CT scan of5 patients with portal hypertension were collected and imported to the abdominal medical image three-dimensional visualization system (MI-3DVS) for sequence segmenting and 3D reconstruction,and then the results of 3D models were compared with operation findings.The 3D models were vividly,visually and clearly displayed the portal system and the collateral circulation,which is helpful in the diagnosis and treatment of the portal hypertension.
3.Changes in myocardial proteomics in late phase of limb ischemic preconditioning in rats

Objective To investigate the changes in myocardial proteomics in the late phase of limb ischemic preconditioning (LIP) in rats.Methods Twelve pathogen-free adult male Sprague-Dawley rats,aged 8-9 weeks,weighing 260-280 g,were randomly assigned into LIP group (n=6) and control group (group C,n=6) using a random number table.Limb ischemia was preceded by 3 cycles of 5-min ischemia which was induced by ligation of the root of the right hindlimb with a rubber band followed by 5-min reperfusion in group LIP.At 24 h after LIP,the tissues were obtained from the left ventricle,and the isobaric tags for relative and absolute quantification technique and liquid chromatography-mass spectrometry were applied to detect the differences in protein expression profiles between the two groups (the difference in expression between the two groups> 1.2 times and P<0.05).The identified differentially expressed proteins were analyzed using the bioinformatics,and some were further verified by Western blot.Results A total of 55 proteins were identified to be differentially expressed,and among the 55 proteins,the expression of 35 proteins was up-regulated,and the expression of 20 proteins was down-regulated.Bioinformatics analysis showed that most of the 55 proteins were organelles,cell membrane or macromolecular compounds,were involved in the process such as metabolism,biological regulation,stress response and signal transduction,and showed functions such as the binding affinity to molecules,catalytic activity,anti-oxidant activity,and modulation of the activity of enzyme.The results verified by Western blot were consistent with those shown by using the isobaric tags for relative and absolute quantification analysis.Conclusion The late phase of LIP can induce changes in the expression of the 55 proteins involving regulation of energy metabolism,anti-oxidant action,regulation of gene expression,and protein folding and degradation in the myocardium,which may be the mechanism of myocardial protection in rats.
4.Evidence-based Pharmacoecomic Evaluation of Venlafaxine and Escitalopram in the Treatment of Depres-sive Disorder

OBJECTIVE:To systematically evaluate the pharnacoeconomic effect of venlafaxine and escitalopram in the treat-ment of depressive disorders. METHODS:Usingvenlafaxineescitalopramdepressiondepressive episodeantidepressantas key words,retrieved from CNKI,VIP and Wanfang database during base creation date to Dec. 2015,related literatures about venlafaxine and escitalopram in the treatment of depressive disorders were selected. The cure rate of two therapy plans for depres-sive disorders and HAMD score were analyzed statistically by cost-effectiveness analysis of pharnacoeconomics. RESULTS:There was no statistical significance in cure rate and HAMD score between 2 groups(P>0.05),so minimum-cost analysis was adopted. The costs of venlafaxine and escitalopram in the treatment of depressive disorders were 772.03 yuan and 935.77 yuan. CONCLU-SIONS:Therapeutic efficacy of venlafaxine is similar to that of escitalopram in the treatment of depressive disorders,but the for-mer scheme is more economical.
5.Systematic review of tandospirone and buspirone in treatment of patients with generalized anxiety disorder

Objective To evaluate the therapeutic effect and safety of tandospirone and buspirone in the treatment of generalized anxiety disorder.Methods The clinical control study oftandospirone and buspirone in the treatment of generalized anxiety disorder was earched by PubMed,CBM,CNKI,VIP,and Wanfang Data,with deadline from January 2000 to June 2016.Meta-analysis was carried out using RevMan5.0 software to each effect index.Results A total of seven RCTs were included involving 615 patients were identified.Meta-analysis showed that the significant efficiency rate[P=0.34,OR=1.19,95%CI (0.83~1.69)] and HAMA[P=0.80,MD=-0.08,95%CI (-0.72~0.56)] of tandospirone and buspirone had no significant difference.Two groups of adverse drug reactions,dizzy,dry,constipine,insomnia,anorexia,and nausea had no significant difference (P>0.05).Conclusion The efficacy and adverse reaction oftandospirone and buspirone in the treatment of generalized anxiety disorder are fairly.
6.Study on protective mechanism of kushenin injection on colonic mucosa of experimental colitis rats.

OBJECTIVETo study the effect of NOD2 on colitis pathogenesis in experimental rats, and discuss therapeutical effect and mechanism of kushenin injection (OMT) on colitis in experimental rats.
METHODFourty Sprague-Dawley (SD) rats were randomly divided into four groups: the normal control group, the model group, the SASP group, and the OMT group, with 10 rats in each group. Except the normal control group, models were established in the remaining three groups with TNBS. The OMT group was injected with kushenin injection, the SASP group was orally administered with mesalazine suspension, the model group and the normal group were orally administered with distilled water for 15 days. Colon lesion score and histological score of experimental rats were observed. Expression of NOD2, NF-kappaB p65 protein in rats colonic mucous was detected by immunohistochemistry. Expression of IL-6 in rat colon mucous was detected by ELISA.
RESULTCompared with normal control group, the expression of NOD2, NF-kappaB p65 and IL-6 in colonic mucosa of the model group were significantly increased (P < 0.01). The SASP group and the OMT group showed lower expressions of NOD2, NF-kappaB p65 and IL-6 in colonic mucosa than the model group (P < 0.01, P < 0.05).
CONCLUSIONThe over expression of colonic mucosa proteins NOD2 and NF-kappaB p65 and increasing secretion of IL-6 take part in the appearance and development of ulcerative colitis. OMT can attenuate ulcerative colitis and protect colonic mucosa by inhibiting expression of NOD2, NF-kappaB p65 and decreasing IL-6.

7.Safety of three-dimensional technique in patients undergoing complicated hepatectomy.

OBJECTIVETo assess the value of abdominal three-dimensional medical image visualization system (MI-3DVS) in assisting complicated hepatectomy.
METHODSTwenty-four patients undergoing complicated hepatectomy for hepatic carcinoma or hepatic focal nodular hyperplasia were enrolled in this study. Three-dimensional models of the organs, vessels and tumors were reconstructed with MI-3DVS, and virtual operations were carried out to assess the feasibility of hepatectomy. The diameter of the liver tumors, intraoperative blood loss and transfusion, complications, in-hospital mortality rate, and one-year survival rate were analyzed in these cases.
RESULTSThe operations were safely completed in all the cases without perioperative deaths. The mean diameter of liver tumor was 9.8∓4.3 cm, and the median volumes of intraoperative blood loss and transfusion were 800 ml and 600 ml, respectively, with a blood transfusion rate of 91.7% (22/24). The incidence of complications was 29.2% (7/24), and the one-year survival rate was 37.5%.
CONCLUSIONThree-dimensional techniques such as volumetric analysis and risk evaluation of residual liver blood supply and drainage can increase the accuracy of surgical planning and improve the safety of complicated hepatectomy.

8.Role of silent mating type information regulation 2 homolog 1 on lipopolysacchride-induced mitochondrial injury in insulinoma cell lines-1

Objective To investigate the role of silent mating type information regulation 2 homolog 1(SIRT-1) in lipopolysaccharide (LPS)-induced mitochondrial injury on INS-1(insulinoma cell lines-1)cells. Methods INS-1 cells were divided into 5 groups: control group, 1 mg/L LPS group, LPS group with 10 μmol/L RSV (resveratrol) pretreatment for 1 h, LPS group with 20 μmol/L EX527 pretreatment for 1 h, LPS group together with EX527 pretreatment for 1 h, then incubated these INS-1 cells with RSV and LPS for 24 h. The cell viability and ATP generation were detected, then total, cytoplasmic and mitochondrial protein were isolated from INS-1 cells. The protein expression of SIRT1, TLR4, acetylated FoxO1, and cytochrome C (CytC), Mfn1 (mitofusion1), Mfn2 (mitofusion2), and Fis1 (fission1) were tested by Western-blot. Mfn1, Mfn2, and Fis1 genes expression were detected by real-time PCR. The comparison of multiple groups was performed by one-way ANOVA. LSD-t method was used to compare between every two groups. Results After 1 mg/L LPS stimulation for 24 h, there was decreased cell viability (n=4, F=13.98, P<0.01) and ATP generation (n=4, F=27.92, P<0.05) in INS-1 cells. RSV pretreatment could maintain ATP production, but it could not reverse the EX527-induced ATP decrease (P>0.05). Furthermore, RSV upregulated gene and protein expression of SIRT1, Mfn1 and Mfn2, whereas decreased TLR4 and Fis1 expression. LPS-induced CytC released to cytoplasm was alleviated by RSV (P<0.01), but there was no significant change about FoxO1 protein expression (P>0.05). Conclusions RSV may regulate FoxO1 acetylation followed by its effects on SIRT1 activity, which may be partly the mechanism of mitochondrial damage induced by LPS on INS-1 cells.
9.Digestive tract reconstruction after laparoscopic proximal gastrectomy and the application prospect of esophagogastric anastomosis with double flap technique

Laparoscopic proximal gastrectomy (LPG) can be selected for the treatment of early upper gastric carcinoma, but gastroesophageal reflux after operation would seriously affect the quality of life of patients. Esophagogastric anastomosis with double flap technique is a digestive tract reconstruction method using the anastomosis between the esophagus and the anterior wall of the stomach. Compared with other digestive tract reconstruction methods, esophagogastric anastomosis with double flap technique can maintain the postoperative body mass of patients in good condition, improve the nutritional status and the long-term quality of life of patients. Esophagogastric anasto-mosis with double flap technique has good anti reflux effects and retain the possibility of endoscopic examination and treatment. By reviewing literatures at home and abroad, and combined with clinical experiences, the authors discuss current status and digestive tract reconstruction methods of LPG, and deeply investigate the application prospect of esophagogastric anastomosis with double flap technique.
10.Systematic review of efficacy and safety of fondaparinux in the treatment of non-ST-elevation acute coronary syndrome in China

OBJECTIVE To systematically evaluate the efficacy and safety of fondaparinux versus low molecular weight heparin(nadroparin,enoxaparin)in the treatment of non-ST-elevation acute coronary syndrome (NSTE-ACS). METHODS The computer searched PubMed ,Medline,Embase,EBSCO,CNKI,Wanfang medical network ,VIP Chinese Journal Full-text Database and relevant clinical trial registration network for the clinical retrospective cohort study (RCS)of fondaparinux (as trial group)and low molecular weight heparin (natroparin,enoxaparin)(as control group )in the treatment of NSTE-ACS. The retrieval time limit was from the establishment of the database to August 2021. Newcastle Ottawa scale (NOS)was used to evaluate the quality of literature. Outcome indicators included primary efficacy indicators (incidence of acute myocardial infarction and recurrent angina pectoris during hospitalization ),secondary efficacy indicators [revascularization of target vessels during 话:0835-2862024。E-mail:xiexingxing07@163.com hospitalization, prothrombin time (PT), activated partial thromboplastin time (APTT)],safety indicators (incidence of serious cardiovascular events ,severe bleeding ,slight bleeding and severe puncture site co mplications during hospitalization ),combined endpoint indicators (30 and 180 days combined endpoint). RevMan 5.3 software was used for Meta-analysis of each effect index. RESULTS Finally,17 RCS articles were included,involving 4 946 patients with NSTE-ACS ,including 2 507 in the trial group and 2 439 in the control group.The results of NOS literature quality evaluation showed that there were 8 high-quality studies ,accounting for 47.06% . The results of Meta-analysis showed that there was no significant difference in the incidence of acute myocardial infarction ,recurrent angina pectoris,revascularization of target vessels ,PT and serious cardiovascular events between 2 groups (P>0.05);there was significant difference in the APTT (MD=1.34,95%CI of 0.22-2.45,P<0.05),the incidence of severe bleeding (RR=0.47, 95%CI of 0.30-0.74,P<0.05),the incidence of slight bleeding (RR=0.48,95%CI of 0.32-0.71,P<0.05),the incidence of severe puncture site complications (RR=0.48,95%CI of 0.25-0.95,P<0.05),30 day combined endpoint (RR=0.57,95%CI of 0.46-0.72,P<0.05),180 days combined endpoint (RR=0.73,95%CI of 0.54-0.98,P<0.05). CONCLUSIONS Fondaparinux in the treatment of NSTE-ACS in China has the same efficacy as low molecular weight heparin (nadroparin,enoxaparin),and has more obvious advantages in drug safety such as bleeding ,severe puncture site complications.
