1.Prevalence of perimenopausal syndrome among 1594 healthy women aged 40-60 years

Objective To investigate the occurrence and influencing factors of perimenopausal syndrome (MPS) and natural menopause women age 40-60 years in Guiyang city, with the purpose of providing information for planning care for menopausal women. Methods A convenient sampling was conducted among 40-60 years old healthy women visiting the three Medical Examination Centers of affiliated hospital of Guizhou Medical University, 1594 subjects were interviewed using a self-designed questionnaire. Improved Kupperman Scale was used to evaluate the MPS. Results The percentage of MPS in the 40-60 years old women is 61.60%, of them, the proportion of mild, moderate and severe cases were 38.96%, 21.52% and 1.13% respectively. Mild and moderate account for the most cases. The top 5 symptoms of MPS were:mood agitation (60.85%), wakefulness (58.22%), muscle or joint ache (57.40%) , fatigue (57.09%) and dizziness (48.68%). The incidence of MPS was related to the following factors: age, marital status, degree of education, family monthly income, current menstrual conditions, age of first sexual life and the frequency of pregnancies and deliveries. The occurrence of MPS were associated with age, educational status,family income,current menstruation condition, age of first sexual intercourse and number of pregnancies. As age increases, the severity of MPS arise. With menstrual changes from normal to irregular and menopause, the rate and severity of MPS increased. With the frequency of pregnancies increases, the possibility of moderate and severe degree of PMS increase. The natural menopause age of healthy women in Guiyang was 49.17 ± 3.12. Conclusion The occurrence of perimenopausal syndrome was related to age,current menstruation condition and frequency of pregnancies. Care for perimenopausal period should start as early as possible.
2.Mediation effect of cognitive emotion regulation on relationship between life events and suicide idea-tion among college students

Objective To investigate the mediating effect of the positive and the negative strategy of cognitive emotion regulation on the relationship between life events and suicide ideation among college students,as well as the moderating effect of social support on the mediation model.Methods Participants were 2 471 college students in Hunan province,and they completed the Adolescent Self-rating Life Events Check List,the Cognitive Emotion Regulation Questionnaire, the Social Support Scale for university students and the Suicide Ideation Questionnaire for university students .Results Negative strategy of cognitive emo-tion regulation played a mediating role between life events and suicide ideation among college students (β=0.17, t=8.43, P<0.01).Mediating effect quoted 0.28×(0.18-0.05U).Social Support (U) played a moder-ating role on the second path of mediation (β=-0.05, t=-2.47, P<0.05) .When U changed one unit,medi-ating effect changed 0.05 unit.Conclusion Life events influence suicide ideation through negative strategy, and with social support increasing,the effect of negative strategy on suicidal ideation weakened.
3.Effects of self-differentiation, social support and social adaption on suicidal ideation in college students

Objective To explore the effects of self-differentiation,social support,social adaptation on suicidal ideation.Methods A simplified cluster sampling method,involving the random selection of 3097 college students in 23 departments,was used to estimate the scores of Chinese version of Self Differentiation Inventory for university student(SDI),Social Support Rating Scale(SSRC),College Student Adaptability Scale (CSAS) and Adult Suicidal Ideation Questionnaire(ASIQ).The independent-samples t-test,Pearson correlation coefficients and stepwise regression analyses were used as statistical analysis techniques.Results The correlation coefficients were negatively significant between suicidal ideation and self-differentiation,social support and social adaptation (r=-0.24,-0.17,-0.29,P<0.001).Social adaptation showed the strongest predictability for suicidal ideation (β=-0.19,P<0.001),followed by self-differentiation (β3=-0.13,P<0.001),social support (β3=-0.08,P<0.001).Independent samples t-test showed that college students with suicidal ideation showed a significant reduction in interpersonal adaptation,social support,self-differentiation compared to those without suicidal ideation.Conclusion Self-differentiation level,social support and social adaptability may be effective predictors for suicidal ideation among college students.Future intervention programs may focus on enhancing the interpersonal skills of college students to reduce prevalence of suicidal behavior.
4.Anatomy and imaging study of a new upper-agger nasi pathway of frontal sinus surgery.

OBJECTIVE:
To investigate the new surgical pathway of endoscopic frontal sinus surgery for frontal sinus lesions through the upper-agger nasi approach.
METHOD:
The computed tomography (CT) scans from 32 patients were collected and subjected to three-dimensional reconstruction by Mimics. The distance in sagittal planes from anterior ethmoid artery to midpoint of axilla and to skull base attachment at anterior middle turbinate was measured. The distance in coronal planes between the perpendicular plate of middle turbinate and the orbital lamina was also detected as well as the height of agger nasi. Three-dimensional structures of the frontal sinus and its surrounding cells was reconstructed by Sinuses Trachea I software. We integrated the CT scans and the above data for simulating surgical operation on cadaveric heads.
RESULT:
(1) Skull base attachment at anterior middle turbinate located at the anterior or posterior of aperture of frontal sinus. (2) The mean distance between anterior ethmoid artery and midpoint of axilla was (22.23 ± 2.78) mm on the left side and (22.30 ± 2.80) mm on right. The mean distance between anterior ethmoid artery and skull base attachment at anterior middle turbinate was (15.31 ± 2.82) mm on left and (15.39 ± 3.53) mm on right. The distance between perpendicular plate of middle turbinate and orbital lamina was (7.61 ± 1.34) mm on left and (7.80 ± 1.40) mm on right side. The height of the agger nasi was (8.33 ± 2.14) mm on left and (8.00 ± 2.57) mm on right. There was no statistical difference in the above data between left and right side (P > 0.05). (3) The visible three-dimensional structure showed that skull base attachment at the anterior middle turbinate was closely adjoined the aperture of frontal sinus, the space between sub-outer side of the attachment and orbital lamina, above the agger nasi cell or the upper area of the agger nasi cell was solely cell structures.
CONCLUSION
Endoscopic frontal sinus surgery for frontal sinus lesions through the upper-agger nasi approach was practicable to solitary frontal sinus lesions and to solve the complex frontal sinus or frontal recess lesions by flexible operation according to the feature of the lesions.

5.The anatomy study of the frontal beak approach of Draf II b frontal sinusotomy.

OBJECTIVE:
To investigate the possibility and anatomy landmark of the frontal beak approach of endoscopic frontal sinusotomy to the frontal sinus lesions.
METHOD:
(1)Twenty cases of frozen cadaveric head underwent spiral computed tomography scans. Then data were transferred into the Mimics image workstation to reorganize CT images in the coronal, sagittal, and axial planes. The anatomic parameters related to surgical approach points were measured, such as the distance between vertical plate of the middle turbinate and lamina papyracea and the thickness of the frontal beak. (2) 3D visual model of the frontal cell and the drainage way of the frontal sinus was produced with the application of Sinuses Trachea I software. (3)The endoscopic frontal sinus surgery were performed on 20 cases of subjects (objects)to find out the anatomy landmarks of the frontal beak approach, measure the parameters such as the distance between middle turbinate and lamina papyracea, and evaluate the potential surgical complications during operation.
RESULT:
(1)The frontal beak is a white bony arcs located at the attachment point of middle turbinate front inserted to the skull base. Its position was relatively constant, before frontal sinus above. (2)The distance between the middle turbinate vertical plate and lamina papyracea was (7. 61 ± 1. 34) mm. The thickness of the frontal beak in surgical approach was (3. 27 ± 0. 91) mm. (3) 3D visual structure of the frontal sinus and its ventilation pathway: the shape of unilateral frontal sinus looked like the cone, which was transited by the drainage pathway of the frontal sinus. The front part of the frontal sinus ostium is surrounded by the frontal beak. The upper part the frontal beak connected to the floor of the frontal sinus. (4) Frontal beak can be used as an landmark of frontal beak approach in the endoscopic frontal sinus surgery. But the lateral view of frontal sinus still was limited in the operation.
CONCLUSION
The endoscopic frontal sinus surgery with the approach of the frontal beak is easy to operate and learn. In this area between the double "L", the operation is safe.

6.Research on application of advanced 3D printing navigation templates in assisting placement of atlantoaxial pedicle screw

Objective To explore the accuracy and clinical efficacy of advanced 3D printing navigation templates in assisting placement of atlantoaxial pedicle screw.Methods A retrospective analysis was carried out on 49 cases of patients with atlanto-axial vertebral fractures and dislocations between June 2013 and June 2016,and all of them were given posterior incision,reduction and internal fixation of atlantoaxial pedicle screw.The patients were divided into advanced 3D printing navigation template group (14 cases),early 3D printing navigation template group (16 cases),and routine pedicle screw placement group (19 cases).Atlantoaxial CT data of patients in advanced 3D printing navigation template group and early 3D printing navigation template group were input into Mimics 17.0,then advanced 3D printing navigation template group and early 3D printing navigation which were used in clinic surgery were designed and printed.The relationship between positions of pedicle screw with the pedicle and bone cortex in plain CT image was observed after operation.The quality of the screw position was assessed and the accuracy of three kinds of screwing methods was compared.The accuracy of the screwing angle was assessed by comparing with the differences between the preoperative designed channel inclination angle and postoperative actual screwing angle.Three groups were compared for differences between operation time,intraoperative blood loss,and scores of cervical nerve scale and visual analogue scale (VAS) of neck and shoulder pain by Japanese Orthopaedic Association (JOA).Results All 49 cases of patients successfully completed the surgery.Patients of the routine pedicle screw placement group,early 3D group and advanced 3D group correspond operation time for 141.2±20.7 min,112.5±12.1 min and 103.1±10.4 min,intraoperative blood loss for 314.0±81.4 ml,243.6±71.2 ml and 181.0+59.1 ml;total accuracy of screwing for 75.0% (54/72),93.75 % (60/64) and 96.43 % (54/56).There were statistically significant differences among the routine group,3D group and advanced 3D group in the mentioned programs.There were no statistical differences between advanced 3D group and 3D group in the inclination angle and head tilt angle with the pre-designed values,while there was statistically significant difference between the routine group and the pre-designed value.The accuracy of the inclination angle and bead tilt angle screwing angle were obviously superior in the advanced 3D group and early 3D group to that of the routine group.There were statistically significant differences between preoperative with postoperative VAS scores and JOA scores in the same group,while there were no statistically significant differences among groups in JOA.But there was statistically significant difference between the routine group and the advanced 3D in VAS,and there was no statistically significant difference between the routine pedicle screw placement group and the early 3D in VAS.All three groups of patients had bony fusion of atlantoaxial vertebral body,without loosening,dislocation and fracture of the internal fixators.Conelusion Advanced 3D printing templates in assisting the surgical treatment for atlantoaxial fracture and dislocation can improve the accuracy of pedicle screwing and safety of the surgery,reduce the surgery risk,and obtain satisfied clinical curative effects.
7.Asymmetric osteotomy via posterior adjacent vertebrae in treating traumatic thoracolumbar kyphotic scoliotic deformity

Objective To investigate the clinical efficacy of asymmetric osteotomy via posterior adjacent vertebrae in the treatment of traumatic thoracolumbar kyphotic scoliotic deformity.Methods A retrospective case series study was conducted on the clinical data of 16 patients with traumatic thoracolumbar kyphotic scoliotic deformity admitted to our department from January 2012 to January 2017.There were 10 males and six females,aged (42.5 ±7.6) years (range,20-62 years).According to the location of injured vertebrae,there were two patients with T11,five with T12,six with L1,and three with L2,all of which had scoliosis deformity and obvious low back pain.All patients underwent asymmetric osteotomy via posterior adjacent vertebrae.The operation time and intraoperative bleeding were recorded.The imaging parameters such as kyphosis Cobb angle,scoliosis Cobb angle,distance between C7 plumbline and central sacral vertebral line (C7-CSVL),and distance between C7 plumbline and sagittal vertical axis (SVA) were measured before and after operation.At the same time,the visual analogue scale (VAS),Oswestry dysfunction index (ODI),and SRS-22 scale were used to evaluate the clinical efficacy.The AISA score was used to evaluate the neurological function recovery before and after operation.Results All patients were followed up for (26.3 ± 16.8) months (range,15-65 months).The operation time was (6.0 ± 1.4) hours (range,4.5-9.0 hours),and the intraoperative bleeding was (900.5 ± 360.8)ml (range,800-1600 ml).The kyphosis Cobb angle was improved from (70.3 ± 9.8) °before operation to (12.2 ± 2.9) ° after operation (P < 0.01).The scoliosis Cobb angle was improved from (47.6 ± 11.6) ° before operation to (4.0 ± 0.9) ° after operation (P < 0.01).The C7-CSVL decreased from preoperative (3.1 ±0.8)cm to postoperative (1.2 ±0.4)cm (P <0.01),and the SVA decreased from preoperative (5.0 ± 0.9) cm to postoperative (2.9 ± 0.5) cm (P < 0.01).No severe complications such as spinal cord and nerve injury occurred.The VAS decreased from preoperative (6.8 ± 1.0) to (1.9 ± 0.9) points at the last follow up.The ODI decreased from (54.6 ± 4.2) % before operation to (8.1 ± 2.5) % at the last follow up.The SRS-22 score was increased from (64.6 ±7.5) points before operation to (87.4 ± 3.2) points at the last follow-up.In terms of the ASIA classification,two patients were improved from grade C to grade D after operation,and six patients were improved from grade D to grade E.Conclusion Asymmetric osteotomy via posterior adjacent vertebrae is safe and effective in the treatment of traumatic thoracolumbar scoliosis,with high correction rate of scoliosis and kyphosis at the same time.
8.Clinical effect of external-internal four paper splints fixation at a fixed point in the treatment of the unstable distal radius fracture

Objective:To compare the clinical effects of external-internal four paper splints fixation at a fixed point with the routine paper splint fixation in the treatment of the unstable distal radius fracture.Methods:A total of 66 patients with type C1 or C2 distal radial fractures, from November 2016 to May 2019, were analyzed retrospectively. According to different external fixation methods, patients were divided into 2 groups with 33 patients in each group. Modified group treated with external-internal four paper splint fixation at a fixed point and normal group treated with routine paper splint fixation. The 4th and 6th weeks after the operation, the reduction of the distal radius fracture was evaluated based on anteroposterior and lateral wrist radiographs. After 3 months follow-up, the radial height, radial inclination and palm dip angle were measured and the healing time were recorded. The Gartland-Werley functional scale was used to evaluate the wrist function from four aspects of residual malformation, subjective evaluation, objective evaluation and complications, and to evaluate the clinical efficacy.Results:After 3 months follow-up, all patients got clinical healing. The loss of palm dip angle [(2.9 ± 2.4)° vs. (4.6 ± 3.2)°, t=-2.524], radial inclination [(3.4 ± 2.2)° vs. (5.0 ± 3.0)°, t=-2.506] and radial height [(2.4 ± 1.9) mm vs. (3.6 ± 2.1) mm, t=-2.302] in modified group were significantly less than those in normal group ( P<0.05). There was a lower Gartland-Werley score in modified group compared with normal group [(1.52 ± 2.75) vs. (5.76 ± 8.68); t=-2.677, P<0.05]. The excellent and good rate was 97.0% (32/33) in the modified group and 78.8% (26/33) in the nomal group. There was significant difference between two groups ( Z=-2.315, P=0.021). Conclusions:The external-internal four paper splint fixation at a fixed point can control the redisplacement of unstable distal radius fractures effectively, and maintain the reduction. The effect of modified paper splint fixation at a fixed point is better than routine paper splint fixation.
9.Application of Mimics three-dimensional imaging in percutaneous left atrial appendage closure with Watchman system.

OBJECTIVEThe three-dimensional (3D) structure of left atrial appendage (LAA) in atrial fibrillation patients were reconstructed by Mimics 3D imaging system, aiming at guiding for selection of both the size and location of the closure devices and making preliminary risk assessment of LAA closure with Watchman system.
METHODSInclusion criteria were: ten voluntary patients with both atrial fibrillation and indication for LAA closure aging from 40 to 85 years old with contraindication for oral anticoagulants or unwillingness to take long-term oral anticoagulation therapy from May to December 2014. 3D reconstruction of LAA was preoperatively made by Mimics 3D imaging system. With the Mimics 3D reconstruction model and the results of both transesophageal echocardiography (TEE) and LAA radiography, the size and location for the closure device were chosen. The devices were planted at the ostium of the LAA.
RESULTSTen atrial fibrillation patients were enrolled (average age: (66.3±11.9) years old) and all successfully implanted with the Watchman LAA closure devices. Nine of them were with non-valvular atrial fibrillation with average CHADS2-VAS score (3.2±1.7) and HAS-BLED score (2.7±1.6). The rest one was a valvular atrial fibrillation patient with the history of the percutaneous balloon mitral valvuloplasty (PBMV) without surgical indications of mitral valve replacement (MVR). There was no blood leakage around the device by regular postoperative TEE and LAA radiography examinations. There were no complications of bleeding, embolism, or stroke through both at peri-operative period and at 1 month follow-up post procedure.
CONCLUSIONPreoperative Mimics 3D reconstruction of LAA by Mimics 3D imaging system among atrial fibrillation patients provides essential information guiding the successful LAA closures.

10.Xenogenic bone with chitosan / norvancomycin sustained-release system for treatment of infectious bone defects in rabbits

Objective To investigate the effects of xenogenic bone with chitosan/norvancomycin sustained-release biomaterials in treating infectious bone defects in rabbits.Methods Xenogenic bone with chitosan/norvancomycin sustained-release biomaterials was made by electrospinning technique.Rabbit infectious bone defect models were made by Methicillin-resistant Staphylococcus aureus.A successful model was evaluated with the standard of more than three points in Norden score assessment.All models were divided into two groups by random number table method,with eight models in each.The control group was treated with surgical debridement,and the experimental group was implanted with bone particles of xenogenic bone with chitosan/norvancomycin sustained-release system after debridement.Postoperatively,general conditions,X-ray,histological results of HE staining,and bacteriological examination results of the rabbits were observed.Results X-ray showed significant bone defects,sequestration,periosteal reaction,and soft tissue swelling after one month of modeling,with Norden score of (3.84 ± 0.52) points.The general conditions were good and the sinus tracts were healed in experimental group after two months of treatment.The control group demonstrated generally poor conditions with swollen sinus and purulent discharge.Two rabbits were died of sepsis.The pathological scores of tibial were (0.41 ± 0.08) points in experimental group,and (3.27 ± 0.26) points in control group by gross observation.The pathological score of experimental group was significantly lower than control group(P < 0.05).The bone defects were basically repaired in experimental group.The longest diameter of bone defect in experimental group was (0.11 ± 0.02)cm,significantly smaller than (0.48 ± 0.06) cm in control group (P < 0.05).There were no obvious signs of osteomyelitis and the bone defects were well repaired in experimental group.Periosteal reaction,soft tissue swelling,a substantial number of bone destruction,and sequestration were observed in control group.The Norden score was (1.32 ± 0.23) points in experimental group,lower than (5.21 ± 0.48) points in control group(P < 0.05).HE staining showed a large amount of trabecular bone formation,bone cell formation,and fibrous hyperplasia in experimental group,with no obvious signs of infection.On the other hand,infiltration of inflammatory cells,necrotic tissue,and sequestration were observed in control group.The histological score was(0.61 ± 0.10) points in experimental group,lower than (4.21 ± 0.41) points in control group (P <0.05).The negative rate of bacterial culture in experimental group was 33%,lower than 100% in control group (P < 0.05).Conclusion Xenogenic bone with ehitosan/norvancomycin sustained-release biomaterials has excellent effect in infection clearance and bone defect reparation in treatment of infectious bone defects in rabbits.
