1.Effects of rotating-knee system versus hinge-knee system replacement in knee salvage of tumor

BACKGROUND: With the development of n0eo-adjuvant chemotherapy, 5-year survival rate of bone tumors has significantly increased. Limb salvage has replaced amputation to become a major means of treating knee tumors and has achieved good results. OBJECTIVE: To investigate and compare the effect of knee joint reconstruction using hinge-knee system and rotating knee system. DESIGN, TIME AND SETTING: Comparative observation. The patients were selected from Department of Spine and Orthopedic Surgery, First Affiliated Hospital of Gaungxi Medical University between June 1999 and March 2006. PARTICIPANTS: A total of 72 patients with tumor round the knee were selected from the First Affiliated Hospital of Gaungxi Medical University, and 9 cases were lost in follow-up. Of the remaining 63 cases, there were 35 males and 28 females, aged 31 years (range, 15-58 years); 40 cases underwent knee joint reconstruction using hinge-knee system and 23 using rotating knee system. METHODS: The tumor excision of superior tibia was performed according to Ennecking principle. After joint reduction of hinge-knee prosthesis, the bolt was adjusted and locked; the high polymer polyethylene pad was placed in rotating knee system, and the joint rotation axis was assembled. The tumor excision of inferior tibia was performed from the medial thigh along medial vastus muscle posterior margin till medial patellar ligament of superior tibia. After joint reduction of hinge-knee prosthesis, the bolt was adjusted and locked; the high polymer polyethylene pad was placed in rotating knee system, and the joint rotation axis was assembled. MAIN OUTCOME MEASURES: The treatment effect was evaluated according to MSTS functional scores, ambulation scores, flexion-extension-angle, knee rotation and infection, relapse and operative time. RESULTS: The 63 patients were followed up for 9 months to 8 years. In hinge-knee system group, operative time was (2.663±0.336) hours; there were 5 cases of relapse, 3 cases of metastasis/death and 2 case of infection. In rotating knee system group, operative time was (2.572±0.288) hours; there were 3 cases of relapse, 2 cases of metastasis/death, and 2 case of infection. One case in rotating knee system group developed fracture and dislocation of the polyethylene bush of rotating axis and there was no syndrome after changing fittings. No prosthesis loosening was found in two groups. With exception of patients with relapse and metastasis/death, 50 cases were evaluated, and no significant difference was found in MSTS scores between two groups (P > 0.05); the total score and single item score of ambulation scores in rotating knee system group were significantly greater than hinge-knee system group (P < 0.05). The extension angle in two groups reached or was close to 0°, and the flexion-extension angle of hinge-knee system group (119.375±17.490)° was similar to rotating knee system group (125.000±15.340)° (P=0.260). CONCLUSION: The rotating knee system displays superior function than hinge-knee system in knee salvage after surgery. Ambulation scores revealed that the knee joint function of the rotating knee system is superior over hinge-knee system, while the MSTS scores did not. Therefore, ambulation scores is more sensitive and appropriate to evaluate the function after knee salvage. There were no significant differences in operative time and flexion-extension ability and syndrome between two methods.

: Objective To explore the effect of ultraviolet with specialized electro-magnetic wave on pressure sore.Methods105 sores in 80 patients were divided into ultraviolet with specialized electro-magnetic wave group and the routine group.Healing time for the sores was observed.ResultsHealing rate of pressure sore within 5 d was 94.1% and 40.5% in the ultraviolet with specialized electro-magnetic wave group and the routine group,respectively (P<0.001).ConclusionThe efficacy of ultraviolet with specialized electro-magnetic wave is superior to routine therapy on pressure sore.

ObjectiveTo research the apoptosis of breast cancer cells by application of small direct-current electric field(EF) and its related possible mechanism.MethodsSmall direct-current EFs were applied in human breast cancer cell line MCF7 in vitro to observe the effects on apoptosis of cancer cells by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling(TUNEL)and to detect the mRNA expression of p53, Rb and E2F1 genes by reverse transcriptase-polymerase chain reaction (RT-PCR).ResultsA lot of cancer cells were observed slaughed from culture dish by 200mV/mm of the field strength and the number of the apoptotic cancer cells was gradually significantly increased with the application of EFs(P<0.01). mRNA expression level for p53 and Rb genes was elevated but that for E2F1 gene was decreased overtly in cancer cells exposed to EFs.ConclusionApoptosis of breast cancer cells can be induced by small direct-current EFs and the change for expression of p53, Rb and E2F1 genes may be involved in the mechanism.

ObjectiveTo observe the apoptosis of breast cancer cells induced by small direct-current electric field (EF) and its related possible mechanism in expression of p21 and p16 genes.MethodsSmall direct-current EF was applied in human breast cancer cell line MCF7 in vitro and the effect of EF on apoptosis and expression of p21 and p16 important genes of breast cancer cells was observed.ResultsThe small direct-current EF had an obvious apoptosis inducing effect on breast cancer cells. A lot of cancer cells were sloughed from culture dish by 200 mV/mm of the EF strength. The number of the apoptotic cancer cells was gradually significantly increased and mRNA expressions of p21 and p16 genes were raised following the EF strength being up (P<0.01).ConclusionThe small direct-current EF has an obvious apoptosis inducing effect on breast cancer cells. It may be related with the expression of p21 and p16 genes.

OBJECTIVE To study the distribution and drug resistance of coagulase negative Staphylococcus(CNS)that leads to nosocomial infection.METHODS Nosocomial CNS was identified and then drug resistance test was performed by K-B method.Nitrocefin method and the Congo red method were utilized to detect ?-lactamase and the slime,respectively.RESULTS Of all 162 CNS strains isolated,there were 102 strains of MRCNS and 60 strains of MSCNS including 83 S.epidermidis strains,accounting for 51.2%.Among all the MRCNS and MSCNS strains above,the positive rates of the ?-lactamase were 100.0% and 5.0%,respectively,and the positive rates of the slime were 17.6% and 1.7%,respectively.The resistance rates of MRCNS to 12 types of antibiotics were higher than those of MSCNS(P
7.Improvement of Growth and Periplocin Yield of Periploca sepium Adventitious Root Cultures by Altering Nitrogen Source Supply

Objective To increase the ultimate yield of periplocin in Periploca sepium adventitious root cultures by a two-stage culture based on nitrogen source.Methods Firstly,the effects of nitrogen source(NH<'+><,4>-NO<'-><,3>-)at different ratios and different total initial nitrogen amounts on the accumulation of biomass and secondary metabolites in adventitious root cultures of P sepium were investigated,and growth and production media for the two-stage culture based on the above results were established.Results The highest biomass and periplocin content were obtained in the culture medium of 15 mmol/L total nitrogen amount with NH<'+><,4>-NO<,3>(1:2)and 30 mmol/L total nitrogen amount with nitrate as the sole nitrogen source.By adopting a fed-batch cultivation strategy,the dry weight adventitious root,periplocin content and yield were increased by 136%,108%,and 389%,respectively when compared with those of the control,reaching up to 8.13 g/L,157.15 μg/g,and 1277.63 μg/L,respectively.Furthermore,it was found that in the process of two-stage culture,the adventitious roots grew thicker significantly after they were transferred into production medium directly.Conclusion The ultimate yield of periplocin in P.sepium adventitious root cultures could be significantly increased by a two-stage culture based on nitrogen source.
8.Screening and identification of an ssDNA aptamer targeting the extracellular domain of Mucin16 by CE-SELEX

Objective CA125 has been proved to be closely related to peritoneal metastasis of gastric cancer.This study aimed to screen and identify a novel ssDNA aptamer targeting the extracellular domain of Mucin16. Methods Using capillary elec-trophoresis, we screened the aptamers targeting the synthetic peptide of the extracellular domain of Mucin16 and quantitatively deter-mined the Kd value of each cycle and the affinity of the aptamers for the synthetic peptide by CE-SELEX.Then we evaluated the ability of the obtained aptamers to target cancer cells using confocal laser imaging. Results After five cycles of screening, sequencing, af-finity determination, we obtained an ssDNA aptamer targeting the extracellular domain of Mucin16, with a Kd value of 122.7 nm and visible green fluorescent signals on the cell membrane of the human ovarian cancer cell line expressing Mucin16, but not on that of the normal hepatocytes not expressing Mucin16.This confirmed the binding ability of the aptamer to the extracellular domain of Mucin16. Conclusion A novel aptamer targeting the synthetic peptide of the extracellular domain of Mucin16 was successfully obtained by capil-lary electrophoresis, which could be used as a new agent in the diagnosis and treatment of peritoneal metastasis of gastric cancer.

Objective Degenerative lumbar scoliosis is frequent clinic spine malformation ,and it is complicated by lumbar inter-vertebral disc ,joint of lumbar vertebra cataplasia and lumbar spinal stenosis .The aim of this paper is to study surgical therapeutic regimen for analyzing clinic feature of degenerative lumbar scoliosis .Methods We comprehended symptom and analyzed imageology feature for lumbar spinal stenosis through reviewing 48 cases of operation from August 2003 to August 2010 ,and then approached its therapeutic principle and regimen .Results There were good therapeutic effect on the basis of comprehending degenerative lum-bar scoliosis and designing different treatment plan by different case feature .Conclusion Ii is need to know degenerative lumbar scoliosis again ,in order to work out individual therapeutic regimen based on clinic feather ,process segment ,state of spinal stenosis , angle of lumbar scoliosis ,degree of vertebra rotation and lumbar destabilizing .
10.Imaging measurement of brain function when ankle dorsiflexion and planter flexion

ObjectiveTo measure cortical areas involved ankle dorsiflexion and planter flexion.MethodsSix-health subjects performed ankle dorsiflexion and planter flexion. While, functional MRI at 1.5 T was made.ResultsWhen ankle dorsiflexion, the excited cortical areas included bilateral primary motor cortex (MⅠ) and primary somatosensory cortex (SⅠ), but area on contralateral side bigger than that on ipsilateral side, as well as chief visual cortexes and vermis of cerebellum. Inhibited areas included area 19, area 20 and area 39. When ankle planter flexion, the excited areas included contralateral secondary motor area (MⅡ) and striate body, ipsilateral supplementary motor area (SMA) and limbic system. Inhibited areas included bilateral upper middle precentral gyrus and postcentral gyrus, superior parietal lobule, inferior parietal lobule and prefrontal cortex.ConclusionThere is a great difference in the brain function images respectively in ankle dorsiflexion and planter flexion.
