1.Relationship between cholecystolithiasis and long-term complications induced by endoscopic sphincte-rotomy for choledocholithiasis

Objective To explore the long-term efficacy of endoscopic sphincterotomy (EST) for choledocholithiasis and to evaluate the necessity of cholecystectomy after EST. Methods Two hundred and fifty seven patients who underwent EST for choledocholithiasis in 2006 were followed up for an average period of 34. 8 months (26-48 months). According to the existence of cholecystolithiasis, the patients were divided into group A (combined with cholecystolithiasis, n = 151) and group B (without cholecystolithiasis, n = 106) , and group A was further divided into group A1 as undergoing cholecystectomy after EST (n =56) and group A2 as not having cholecystectomy after EST ( n = 95). Results Of the 257 patients, late complications occurred in 31 patients (12. 1% ) , including recurrent choledocholithiasis in 25 (9.7% ), cholangitis in 27 (10. 1% ) , acute pancreatitis in 2 (0. 8% ) and cholangiocarcinoma in 1 (0.4% ). The rates of late complications and recurrent choledocholithiasis were significantly higher in group A2 than those in group A1 (P<0.05). Conclusion EST is safe and effective for choledocholithiasis. Cholecystectomy after EST is necessary in patients with cholecystolithiasis.
3.SMN1 gene deletion analysis using mid-trimester amniotic fluid cells by real-time PCR

Objectives To investigate the prenatal diagnosis method of spinal muscular atrophy with amniotic fluid sample.Methods Totally 1 064 amniotic fluid samples from mid-trimester pregnant women were enrolled during January 2015 and January 2016 in 4 hospitals.Genetic analysis was performed for detecting potential contamination of maternal tissue by a genetic technique based on short tandem repeat ( STR) markers.Deletion of SMN1 gene was detected in 1 062 uncontaminated amniotic fluid samples by real-time PCR and multiplex ligation-dependent probe amplification ( MLPA) respectively.Results Two contaminated amniotic fluid samples were detected within 1 064 mid-trimester pregnant women by STR genotyping.The other 1 062 uncontaminated amniotic fluid samples were tested by real-time PCR.There were 37 samples with heterozygous deletion of Exon 7 of SMN1 gene ( 3.67%) , 34 samples with heterozygous deletion of Exon 8 of SMN1 gene (3.2%) and two samples with homozygous deletion of Exon 7 and Exon8 of SMN1 gene ( 0.19%) respectively , while other samples observed with no deletion of Exon 7 and Exon8 in SMN1 gene.Totally 41 samples with heterozygous or homozygous deletion of SMN 1 gene and 55 samples with undetected deletion of SMN 1 gene were confirmed by MLPA and the results showed 100%consistence with that of real-time PCR.Conclusions Both real-time PCR and MLPA are suitable for detecting the deletion of SMN 1 gene with amniotic fluid sample . Real-time PCR exhibits less sample requirement and time compared with MLPA .
4.Study on the correlation between IL-1β ,Hcy ,Cys-C and HbA1c levels and senile type 2 diabetic peripheral neuropathy

Objective To investigate the correlation of interleukin-1β (IL-1β) ,homocysteine (HCY) ,cysta-tin (Cys-C) and glycosylated hemoglobin (HbA1c) levels and senile type 2 diabetic peripheral neuropathy. Methods 104 patients with type 2 diabetes mellitus received in the hospital from June 2014 to December 2016 were selected as study group ,52 patients with peripheral neuropathy were served as study group A (n=52) and 52 patients with type 2 diabetes were served as study group B (n=52) ,and 52 healthy persons who under-went health assessement were taken as the control group.The Cys-C level was measured by immunoturbidime-try ,the levels of HCY and IL-1β were measured by enzyme-linked immunosorbent assay.The HbA1c level was measured by high performance liquid chromatography ,the IL-1β ,HCY ,Cys-C and HbA1c levels were com-pared in the three groups ,and the relationship between IL-1β ,HCY ,Cys-C and HbA1c levels and senile type 2 diabetic peripheral neuropathy were analyzed.Results Single factor variance analysis showed that the levels of IL-1β ,HCY ,Cys-C and HbA1c in the three groups were statistically significant (P<0.05) ;LSD-t test showed that the IL-1β ,HCY ,Cys-C and HbA1c levels in the study group B were higher than those in control group , and IL-1β ,HCY ,Cys-C and HbA1c levels in the study group A were higher than those in the study group B , and the difference was statistically significant (P< 0.05) ;logisticis regression analysis showed that IL-1β , HCY ,Cys-C ,HbA1c levels were associated with type 2 diabetic peripheral neuropathy (P<0.05) ;Spearman test showed that IL-1β ,HCY ,Cys-C ,HbA1c levels were positively correlated with type 2 diabetic peripheral neuropathy (P<0.05).Conclusion IL-1β ,HCY ,Cys-C and HbA1c levels are positively correlated with senile type 2 diabetic peripheral neuropathy in the elderly ,and the IL-1β ,HCY ,Cys-C and HbA1c levels can be used to determine type 2 diabetic peripheral neuropathy in clinical work.
5.Role of K(Ca)3.1 channel in proliferation and migration of rat vascular smooth muscle cells of the proliferative phenotype.

OBJECTIVETo investigate the role of K(Ca)3.1 channel in the proliferation and migration of rat vascular smooth muscle cells of the proliferative phenotype.
METHODSRat vascular smooth muscle cells (VSMCs) were cultured with tissue adhesion method. The morphological characteristics of the fist and ninth passages of VSMCs were observed with light and electron microscopy and immunocytochemistry. The expressions of K(Ca)3.1 channel mRNA and protein in the cells were detected using RT-PCR and immunocytochemistry, respectively. MTT and transwell assay were employed to assess the effect of the K(Ca)3.1 channel blocker TRAM-34 on the proliferation and migration of VSMCs.
RESULTSThe first and ninth passages of VSMCs showed morphological characteristics of contractile and proliferative phenotypes, respectively. Compared with the first- passage cells, the ninth-passage VSMCs exhibited significantly increased K(Ca)3.1 channel mRNA and protein expressions with enhanced cell proliferation and migration (P<0.01), which was inhibited by the application of TRAM-34 (P<0.01). TRAM-34 produced no obvious effect on the first-passage VSMCs.
CONCLUSIONUpregulated expression of K(Ca)3.1 channel can promote the proliferation and migration of rat VSMCs of the proliferative phenotype.

6.Ca(2+)-activated K(+) channel switching in smooth muscle participates in atherosclerosis development in diabetic rats.

OBJECTIVETo investigate the changes in aorta morphology and Ca(2+)-activated K(+) (KCa) channel expression in the diabetic rats.
METHODSA diabetic rat model was established by a single intraperitoneal injection of streptozotocin (30 mg/kg) after a modified high fat and glucose diet for 8 weeks. Pathological changes in the aorta were observed with HE staining, elastic fiber staining, Masson's trichrome staining and immunohistochemistry. Both the mRNA and protein levels of KCa channels in the aorta were measured by RT-PCR and Western blotting.
RESULTSEarly atherosclerotic changes were observed in the aorta wall of the diabetic rats. The mRNA and protein levels of KCa1.1 channel α- and β-subunits were significantly decreased, while the expression of KCa3.1 channels was obviously enhanced in the middle layer of the aorta in the diabetic rats.
CONCLUSIONKCa channel switching in smooth muscles may play a role in the development of atherosclerosis in diabetic rats.

7.Risk perception, emergency preparedness and emergency literacy among medical students under COVID-19 epidemic

Objective:
To investigate risk perception, emergency preparedness and emergency literacy among medical students under COVID-19 epidemic.
Methods:
An online survey was conducted by using the self designed COVID-19 questionnaire on risk perception, emergency preparedness and emergency literacy from March 27 to March 30, 2020. The convenience sampling method was used to investigate 4 864 medical students from 2 colleges and technical secondary schools in Sichuan Province.
Results:
The scores of risk perception, emergency preparedness and emergency literacy associated with COVID-19 were(35.18±4.33)(28.30±4.16) and(25.23±2.97) respectively. Risk perception of medical students was positively correlated with emergency preparedness and emergency literacy(r=0.35, 0.40), emergency preparedness was positively correlated with emergency literacy(r=0.51)(P<0.05).
Conclusion
Considering interrelationships among risk perception, emergency preparedness and emergency literacy associated with COVID-19, medical students should be given targeted prevention and control training.
8.A qualitative study on the psychological resilience of people living with HIV

Objective:To explore the psychological resilience of people living with human immunodeficiency virus (PLHIV) , so as to provide a reference for improving the psychological resilience of PLHIV and promoting them to return to normal life as soon as possible.Methods:From April to July 2019, purpose sampling was used to select 14 cases of PLHIV treated with antiretroviral treatment at a designated acquired immune deficiency syndrome (AIDS) clinic and a center for disease control and prevention in Luzhou City, Sichuan Province. The in-depth interview was conducted with PLHIV and data analysis was carried out using the method of Colaizzi phenomenology.Results:PLHIV psychological resilience was refined into three themes, including acceptance, coping and reconstruction.Conclusions:Based on the psychological resilience of PLHIV, medical staff can help the infected people to accept the facts of infection as soon as possible and master effective response strategies, and promote the ideal reconstruction of the infected people, and join the work of AIDS prevention and control, and work together to achieve "Zero AIDS".

以PD-1/PD-L1抑制剂为代表的免疫检查点抑制剂(immune checkpoint inhibitors,ICIs)疗法改变了以往手术、放疗或 化疗的常规癌症治疗方法,甚至已成为部分癌症的一线治疗方式。然而,PD-1/PD-L1抑制剂并不能使大部分肿瘤患者获益,甚至 部分患者接受治疗后出现了肿瘤暴发性进展现象。在PD-1/PD-L1抑制剂治疗前,对相关疗效标志物进行检测,可以筛选出可能 获益的人群,从而有效避免延误治疗无效患者的病情及不必要的经济损失。为进一步指导临床,本文就PD-1/PD-L1抑制剂的疗 效标志物作一系统综述。
