1.Effectiveness of comprehensive intervention on rural community patients with diabetes

Objective To evaluate the impacts of comprehensive diabetes intervention to provide evidence for effective diabetic control in rural communities.Methods Diabetic patients from four communities at Wujin District of Changzhou City were selected.One hundred and seventy-three patients from two communities received comprehensive intervention,including medication therapy,diabetes education,and diet.exercise,and behavior instruction(intervention group);142 patients from the other two communities got medication therapy only (control group).All the participants were interviewed with structured questionnaire and phyrsical examination.The second epidemic investigation was conducted at 4 years to evaluate the effects of the intervention.Results At the end of the intervention,the awareness rates of normal fasting plasma glucose,major diabetes risk factors,healthy diet,and therapeutic strategies in the intervention group were 90.8%.74.6%,89.6%,and 59.5%,respectively,which were significantly higher than those in the control group (all P<0.05).For the intervention group,the rates of diet behavior or physical exercise change were 68.8% and 58.4%.respectively.which were much higher than those of the control group(all P<0.05).The average levels of systolic pressure,diastolic pressure,fasting plasma glucose,and total cholesterol were (130.6±10.4)mm Hg(1 mm Hg=0.133 kPa),(78.4±7.5)mm Hg,(6.2±3.1)mmol/L,and (4.5±0.9)mmol/L,respectively,in the intervention group,which were lower than those in the control group(all P<0.05).Conclusion Comprehensive intervention shows significant effects on rural community patients with diabetes.
2.Analysis on the prevalence and influencing factor of metabolic syndrome in rural elderly population in Changzhou of Jiangsu province

Objective To investigate the prevalence and influencing factor of metabolic syndrome(MS)in rural elderly population. Methods With randomized cluster sampling,investigations including questionnaires,physical examination,fasting plasma glucose,blood lipids were performed in rural population aged over 60 years in Changzhou city in 2004-2005. Results A total of 4976 elderly people were enrolled in the study.The prevalence of MS was 24.1%,and 10.3%in men,34.3%in women by international diabetes federation(IDF)2005 definition.86.2%individuals had one or more of the metabolic disorders.Logistic regression analysis showed that sex,smoking,sweetmeat,family history of essential hypertension(EH)and tea drinking were influencing factors of MS. Conclusions The prevalence of MS is high in rural elderly population,especially in females.

Objective To study the EnCor vacuum negative pressure rotary system in ultrasonic BI-RADS 2~3 scale mammary gland disease application value.Methods Under the guidance of ultrasound, the application EnCor vacuum negative pressure rotary cutting system for 142 patients, 366 cases of breast lesion resection, specimens sent to pathological examination.Results A total of 366 breast lesions, 359 complete resection, the success rate was 98.1%, 2 transit operation, and transfer rate was 0.5%, 5 partial nephrectomy, residual rate was 1.4%.359 complete resection of the pathological results for breast malignant lesions in 4, the infiltrating ductal carcinoma 1, 3 ductal carcinoma in situ;breast benign lesions in 355, of which 132 breast fibroadenoma, adenosis with adenoma formation of 60, 99 breast adenosis, breast adenosis with duct expansion 43, 11 breast intraductal papilloma, one with four individual catheter atypical increase the living.Two transit operation pathological results were fibroadenoma, 5 disease of partial nephrectomy for adenosis with duct expansion.Conclusions The excision of breast neoplasm EnCor vacuum negative pres-sure rotary cutting system, not only small trauma, good cosmetic effect, and reduce misdiagnosis and im-prove the patients'satisfaction.
4.Modulation of non-ion channel proteins by membrane potential.

The different concentration of specific ion species and the electrodiffusion of the ions down their electrochemical gradient generate transmembrane potential. The regulation of membrane potential for the function of numerous membrane proteins such as ion channels, transporters, pumps and enzymes plays primary role in the conversion of extracellular electric stimulation into a sequence of intracellular biochemical signals. Some ion channels regulated by membrane potential are well known, and the membrane non-ion channels protein is also modulated physiologically by membrane potential.

5.Preparation of gastric retenting and chronopharmacologic drug delivery tablets of sinomenine hydrochloride.

OBJECTIVETo prepare the gastric retenting and chronopharmacologic drug delivery tablets containing sinomenine hydrochloride as a model drug, evaluate the effects of the coating layers formulation and technics on drug release behavior, and to elucidate the mechanism of drug release based on obtained results.
METHODThe gastric retenting and chronopharmacologic drug delivery tablets were prepared by press-coated technics. The types of disintegrants were chosen according to the expanding rate and the lag-time. The effects of formulation and technics of coating layer on the release characteristic of the drug were investigated by dissolution testing. The mechanism of drug release was proved by erosion test.
RESULTThe tablets had typical chronopharmacologic drug delivery properties with a lag time followed by a rapid release phase. CMS-Na was selected as the disintegrant. The lag-time was prolonged with the increase of the ratio of HPMC/carrrageenan and the amount of matrix material in coating layer. The compressing pressure and preparation method of coat material had minor influence on the lag-time of the tablets. Coating layer erosion and tablet core swelling were involved in the mechanism of drug release.
CONCLUSIONThe tablets had typical chronopharmacologic drug delivery properties. A suitable lag-time can be achieved by adjusting formulation of coating layer to meet the requirement of chronotherapy.

6.Effects of vinorelbine combined with adriamycin on breast cancer MCF-7 cells and related mechanisms

Objective:To investigate the in vitro anti-cancer effect of Vinorelbine (NVB) combined with adriamycin (PLD) on human breast cancer MCF-7 cells and related mechanisms.Methods:The effects of NVB and PLD alone or in combination on the proliferation of breast cancer cells were detected by CCK-8 experiment. Flow cytometry was used to detect cell apoptosis and changes in reactive oxygen species (ROS) levels. Western blot experiment was carried out to detect protein expression.Results:The results of CCK-8 showed that compared with the blank control group, the inhibition rates of the vinorelbine treatment group, the adriamycin treatment group and the combined treatment group were 27.6%, 31.2% and 65.4%, compared with the NVB group and PLD group, the difference between the combined treatment group was statistically significant ( P=0.005 vs 0.001) . The results of flow cytometry showed that the proportion of apoptotic cells in each group was 3.54%, 16.95%, 15.01% and 32.24%, compared with the NVB group and PLD group, the difference between the combined treatment group was statistically significant ( P=0.006 vs 0.005) . The levels of reactive oxygen species in each group were 1, 1.03, 1.06 and 1.57, compared with the NVB group and PLD group, the difference between the combined treatment group was statistically significant ( P=0.008 vs 0.007) . Western blot results showed that the expression of p-ERK and p-STAT3 decreased after the combination of NVB and PLD, which inhibited the ERK/STAT3 signaling pathway. Conclusions:The combination of NVB and PLD can promote the apoptosis of breast cancer cells and inhibit the proliferation of breast cancer cells with high efficiency and low toxicity. Its mechanism of action may be related to the up-regulation of ROS levels in cells, thereby inhibiting the activation of the ERK/STAT3 pathway.
7.Effect of emotional imagery desensitization therapy on post-traumatic stress disorder among school-age children with fracture

Objective:To explore the effect of emotional imagery desensitization therapy on posttraumatic stress disorder among school- age children with fracture, to alleviate the adverse psychological state of the children.Methods:By a randomized controlled trials, a total of 78 school- age children with fracture at the Affiliated Hospital of Jining Medical University from July 2021 to July 2023 were divided into experimental group and control group according to the admission time, with 39 cases in each group. Both groups carried out routine care, the control group was given routine psychological intervention, the experimental group implemented emotional imagery desensitization therapy. Before and after 3 months of intervention, the effects was assessed by Post-Traumatic Stress Disorder Self-rating Scale (PTSD-SS) and Medical Coping Modes Questionnaire (MCMQ), respectively.Results:During the research process, there were 2 cases of dropout in the control group due to disconnection, and 4 cases in the experimental group due to disconnection and voluntary withdrawal. 37 children in the control group [20 males and 17 females, age (9.27 ± 1.62) years], and 35 children in the experimental group [21 males and 14 females, age (9.57 ± 1.63) years] completed the intervention. Before intervention, the difference of PTSD-SS and MCMQ scores between two groups was no statistically significant (all P>0.05). After intervention, the subjective assessment, re-experiencing, avoidance symptoms, hypervigilance, functional impairment factor scores and total PTSD-SS scores were (1.51 ± 1.06), (10.74 ± 3.30), (10.86 ± 2.93), (11.94 ± 3.18), (2.31 ± 1.30), (37.37 ± 6.37) in the experimental group, lower than in the control group (2.08 ± 0.80), (12.92 ± 3.63), (12.73 ± 2.99), (14.65 ± 4.23), (2.97 ± 0.87), (45.35 ± 5.86), the differences were statistically significant ( t=2.53-5.54, all P<0.05). After intervention, the confrontation subscale scores were (23.49 ± 2.48), higher than in the control group (21.89 ± 2.38), the avoidance, acceptance- resignation subscale were (10.31 ± 1.89) and (6.83 ± 1.98), lower than in the control group (11.57 ± 1.79) and (7.86 ± 2.12), the differences were statistically significant ( t=2.79, 2.89, 2.14, all P<0.05). Conclusions:Emotional imagery desensitization therapy can effectively alleviate post- traumatic stress disorder and promote positive coping mode in school-age children with fracture.
8.Prokaryotic expression,purification and functional analysis of human RPL22 gene

Objective To understand the bioinformatics of human ribosomal protein L22(RPL22)gene,prokary-otic expression and purification of human RPL22 recombinant protein,synthesis of human RPL22(labeled as agent M)in vitro,study the effects of agent M on proliferation,cycle and apoptosis of NSCLC A549 cells,and to analyze the effect of combination chemotherapy with M and cisplatin(DDP).Methods RPL22 bioinformatics was ana-lyzed.RPL22 gene was cloned by PCR,the single chain oligonucleotide was designed and synthesized to obtain the target gene,and connect the pET-28α vector.The pET-28α-RPL22 recombinant plasmid was constructed,and the receptive cell DH5α of Escherichia coli was transformed.The expression of recombinant protein RPL22 was analyzed by SDS-PAGE and Western blot.The semi-inhibitory rate(IC50)of M and DDP in A549 cells and the effect of M on the proliferation of A549 cells were detected by CCK-8.The effect of agent M on apoptosis and cycle of A549 cells was detected by flow cytometry.qPCR and Western blot assays were used to detect M and DDP on phosphati-dylinositol-3-kinase/protein kinase B(PI3K/AKT)and adenosine 5'-monophosphate activation of protein kinase/mammalian target of rapamycin(AMPK/mTOR)signaling pathway,respectively.Results RPL22 protein was an unstable and hydrophilic protein.The recombinant protein was dissolved and purified to obtain high concentration of recombinant protein.In A549 cells,the IC50 of reagent M was 400 μg/L,and the IC50 of DDP was 10 μmol/L,and the concentration of reagent M was inversely proportional to the cell activity.The inhibitory effect of reagent M(200 μg/L)combined with DDP(2.5 μmol/L)was similar to that of DDP(10 μmol/L)alone.Reagent M could induce G2 cell cycle arrest and promote apoptosis,and might be involved in the regulation of PI3 K/AKT and AMPK/mTOR pathways.Conclusion The recombinant RPL22 protein is cloned and expressed successfully.
9.A randomized controlled study of executive function differences in patients with Parkinson’s disease and multi-system atrophy

Objective To investigate the differences and similarities of cognitive impairment,especially executive function,between patients with Parkinson’s disease(PD) and multi-system atrophy(MSA) by neuropsychological tests. Methods 26 patients with Parkinson’s disease,18 with multi-system atrophy and 20 with normal control were selected continuously. Age,education degree and course of disease were collected in detail. Montreal Cognitive Assessment (MoCA),Trail Making Test (TMT),Stroop Color-Word Test,Clock Drawing Test (CDT) and Rey-Osterriche complex figure test were given to evaluate cognitive and executive function of 26 patients with PD,18 patients with MSA and 20 normal controls randomly selected. Results Parkinson’s disease group and multi-system atrophy group had significantly lower scores in each test than the control group (P<0.05). In the MOCA test,the scores of Parkinson’s patients were significantly lower than those of the multi-system atrophy group,[(22.34±3.18) vs(24.31±1.68),P<0.05]. The number of errors in the Stroop chromatogram test in Parkinson’s group was higher than that in the multi-system atrophy group[(10.74±6.31) vs(8.26±4.62),P<0.05]. In the remaining tests,there was no significant difference between PD and MSA patients (P>0.05). Conclusion Parkinson’s disease patients and multi-system atrophy patients have different degrees of cognitive impairment and executive function decline than the normal population,Multisystem atrophy in terms of overall cognitive impairment is worse in patients with Parkinson’s disease than in patients with Parkinson’s disease,Parkinson’s disease patients have more attention deficit and more severe system atrophy,while patients with multiple system atrophy are more likely to have decreased self-examination ability and error correction ability.
10.Genetic studies of primary biliary cholangitis in the post-GWAS era

With the constant increase in the awareness of primary biliary cholangitis (PBC) and the continuous improvement in related diagnostic methods in the past two decades, the incidence and prevalence rates of PBC tend to increase and PBC is now the most common autoimmune liver disease worldwide. A series of family-based studies in the early stage have shown that PBC has strong genetic tendency, and subsequent genomic analyses have been performed for PBC in different populations and have obtained a large amount of genetic data. Future genetic studies of PBC will focus on translating these results into clinical practice.
