1.Effects of family-centered care in children with Helicobacter pylori infection-associated gastritis

Objective:To explore the effect of family-centered care (FCC) on compliance behavior, emotional behavior and quality of life in children with Helicobacter pylori (Hp) infection-associated gastritis.Methods:From March 2019 to March 2021, convenience sampling was used to select 102 children with Hp infection-associated gastritis who treated in the Affiliated Hospital of Jining Medical University and their 102 primary caregivers as the research object. The subjects were divided into the observation group and the control group by random number table method, with 51 children and 51 primary caregivers in each group. The control group received routine nursing, while the observation group implemented FCC on the basis of routine nursing. The compliance behaviors of the two groups of children were compared. Before and after nursing, the Mishel Uncertainty in Illness Scale-Family Member form (MUIS-FM) was used to evaluate the illness uncertainty of the primary caregivers of the two groups of children. The State-Trait Anxiety Inventory-Form Y (STAI-Y) and the Pediatric Quality of Life Inventory TM4.0 (PedsQL TM4.0) were used to assess the anxiety and quality of life of the two groups of children. Results:After nursing, the compliance behavior rate of the observation group was higher than that of the control group, the scores of MUIS-FM and STAI-Y were lower than those of the control group, and the score of PedsQL TM4.0 was higher than that of the control group, with statistical differences ( P<0.05) . Conclusions:The FCC for children with Hp infection-associated gastritis can effectively enhance their compliance behavior, relieve their anxiety and depression, and improve their quality of life.
2.Epidemiological characteristics and risk factors of rotavirus infection in children aged 3-5 years who seek treatment for diarrhea in Jining area

Objective:To analyze the epidemiological characteristics and risk factors of rotavirus infected in children aged 3-5 years seeking treatment for diarrhea in Jining area.Methods:The epidemiological data of 698 children aged 3-5 years seeking treatment for diarrhea in Jining area hospitals from September 2022 to September 2023 were collected, and fecal samples were collected for laboratory examination. The epidemiological characteristics of the rotavirus infection were analyzed, and logistic regression analysis was applied to analyze the risk factors of rotavirus infection in children aged 3-5 years seeking treatment for diarrhea in Jining area. A predictive model of rotavirus infection in children aged 3-5 years seeking treatment for diarrhea in Jining area was established based on decision tree, and the predictive performance of the model was verified by receiver operating characteristic (ROC) curve.Results:The detection of rotavirus antigen showed 302 positive cases, with a positive rate of 43.27% (302/698). Gender distribution: There was no statistically significant difference in the positive rate of rotavirus infection in these diseased children etween different genders ( χ2=1.862, P=0.172). Age distribution: The positive rate of rotavirus infection in these diseased children showed a decreasing trend with age ( χ2=28.893, P<0.001). Time distribution: There was a statistically significant difference in the positive rate of rotavirus infection in these diseased children among different months ( χ2=241.607, P<0.001), showing obvious seasonal characteristics, the high incidence months were from October to the following March, the highest positive rate was in December, and the lowest was in July. Rotavirus genotype: The G genotype result showed that G9 was the most common, P genotype result showed that P[8] was the most common, and the G/P combination genotype result showed that G9P[8] was the most common. Multivariate Logistic regression analysis showed that having finger sucking habits ( OR=4.193, P=0.018), lack of vaccination against rotavirus ( OR=1.947, P=0.002), whether to clean hands before feeding ( OR=4.719, P=0.007), and a history of contact with rotavirus infected children ( OR=4.976, P=0.001) were independent risk factors for rotavirus infection in children aged 3-5 years old who seek treatment for diarrhea in Jining area. The sensitivity of the decision tree model was 58.94%, the specificity was 87.12%, and the AUC was 0.814, indicating that the four selected risk factors could predict rotavirus infection well. Conclusions:Children aged 3-5 years old who seek treatment for diarrhea in Jining area have a relatively high positive rate of rotavirus infection, showing obvious age and time characteristics. The genotype distribution was mainly G9, P[8], and G9P[8]. Rotavirus infection is related to finger sucking habits, lack of vaccination against rotavirus, whether to clean hands before feeding, and a history of contact with rotavirus infected children.
3.Epidemiological characteristics of respiratory syncytial virus infection in preschool children and risk factors for severe pneumonia

Objective:To describe the epidemiological characteristics of respiratory syncytial virus (RSV) infection in preschool children and explore the risk factors for severe pneumonia.Methods:Epidemiological data of 279 preschool children with RSV infection were investigated. The children were screened for severe pneumonia and separated into ordinary and severe types. General data and laboratory test data from both groups were compared, and binary logistic regression model analysis was applied to determine the risk factors for severe pneumonia.Results:Preschool children with RSV infection were mostly male (63.08%), <6 months old (65.95%) and had poor living environment (53.05%), with main symptoms of cough (91.04%) and wheezing (69.18%), the lung auscultation was mainly characterized by wheezing (86.74%), and imaging findings were mainly patchy shadows (76.34%), the onset season was concentrated in autumn (31.18%) and winter (43.37%). The detection rate of severe pneumonia in 279 pediatric patients was 20.27% (56/279). The proportions of onset season being autumn or winter, low birth weight infants, history of respiratory infections within 3 months, delayed treatment, neutrophils count <10×10 9/L, C-reactive protein≥10 mg/L, procalcitonin≥1.5 ng/mL, albumin<30 g/L, CD4 + /CD8 + <1.2 in the severe types were higher than those in the normal types ( P<0.05). Logistic regression analysis showed that the onset season was autumn or winter ( OR=2.316, 95% CI: 1.235-4.345), low birth weight infants ( OR=2.679, 95% CI: 1.442-4.977), history of respiratory infections within 3 months ( OR=2.815, 95% CI: 1.539-5.148), delayed treatment ( OR=2.869, 95% CI: 1.581-5.206), low albumin<30 g/L ( OR=2.756, 95% CI: 1.495-5.080), and low CD4 + /CD8 + <1.2 ( OR=3.016, 95% CI: 1.695-5.366) were risk factors for severe RSV pneumonia in preschool children ( P<0.05). Conclusions:Autumn and winter, low birth weight infants, history of respiratory infections within 3 months, delayed treatment, low albumin, and low CD4 + /CD8 + are related to the occurrence of severe RSV pneumonia in preschool children. Therefore, it is necessary to strengthen the attention to the condition of preschool RSV infected children with the above risk factors, and actively intervene in controllable factors to reduce the risk of severe pneumonia.
4.Effect of emotional imagery desensitization therapy on post-traumatic stress disorder among school-age children with fracture

Objective:To explore the effect of emotional imagery desensitization therapy on posttraumatic stress disorder among school- age children with fracture, to alleviate the adverse psychological state of the children.Methods:By a randomized controlled trials, a total of 78 school- age children with fracture at the Affiliated Hospital of Jining Medical University from July 2021 to July 2023 were divided into experimental group and control group according to the admission time, with 39 cases in each group. Both groups carried out routine care, the control group was given routine psychological intervention, the experimental group implemented emotional imagery desensitization therapy. Before and after 3 months of intervention, the effects was assessed by Post-Traumatic Stress Disorder Self-rating Scale (PTSD-SS) and Medical Coping Modes Questionnaire (MCMQ), respectively.Results:During the research process, there were 2 cases of dropout in the control group due to disconnection, and 4 cases in the experimental group due to disconnection and voluntary withdrawal. 37 children in the control group [20 males and 17 females, age (9.27 ± 1.62) years], and 35 children in the experimental group [21 males and 14 females, age (9.57 ± 1.63) years] completed the intervention. Before intervention, the difference of PTSD-SS and MCMQ scores between two groups was no statistically significant (all P>0.05). After intervention, the subjective assessment, re-experiencing, avoidance symptoms, hypervigilance, functional impairment factor scores and total PTSD-SS scores were (1.51 ± 1.06), (10.74 ± 3.30), (10.86 ± 2.93), (11.94 ± 3.18), (2.31 ± 1.30), (37.37 ± 6.37) in the experimental group, lower than in the control group (2.08 ± 0.80), (12.92 ± 3.63), (12.73 ± 2.99), (14.65 ± 4.23), (2.97 ± 0.87), (45.35 ± 5.86), the differences were statistically significant ( t=2.53-5.54, all P<0.05). After intervention, the confrontation subscale scores were (23.49 ± 2.48), higher than in the control group (21.89 ± 2.38), the avoidance, acceptance- resignation subscale were (10.31 ± 1.89) and (6.83 ± 1.98), lower than in the control group (11.57 ± 1.79) and (7.86 ± 2.12), the differences were statistically significant ( t=2.79, 2.89, 2.14, all P<0.05). Conclusions:Emotional imagery desensitization therapy can effectively alleviate post- traumatic stress disorder and promote positive coping mode in school-age children with fracture.
5.Treatment of Palpitations Based on the Theory of "Wind and Fire Inciting Each Other"

By combing the physiological characteristics of wind and fire, the article makes it clear that wind and fire are evils, and external wind is easy to attack and even worse when it encounters fire; burnt and moving fire evils set off internal winds; and the incitement of each other can lead to the occurrence of palpitations. From the perspective of the theory of "wind and fire inciting each other", the clinical diagnosis and treatment of palpitation is discussed, which clarifies that there is an internal and external distinction between wind and fire, and there is an internal and external distinction between the real and the imaginary, which should be carefully distinguished in the diagnosis and treatment of palpitation. In the treatment, wind medicine can be skillfully applied to drive away the wind evil to calm the internal and external, and it is also necessary to clearly identify the source of the fire evil and properly choose the corresponding treatment for the fire evil in order to extinguish the burnt fire. As the positive qi exists and the evil will not dry up, we should not forget to take care of the heart in the treatment. An example of a medical case is attached in order to provide new ideas for the clinical treatment of palpitation.
