1.ANALYSIS OF EPIDEMIC CHANGES IN THE EPIDEMIOLOGICAL OBSERVATION POINTS OF SCHISTOSOMIASIS IN HUBEI PROVINCE FROM 1990 TO 1998

Objective To master the trend of schistosomiasis epidemic by means of established observation points and give the scientific reference for the control decision making henceforth. Methods Three villages of schistosomiasis with highly prevalence in Hubei Province were selected as observation points, and their epidemiological status were observed from 1990-1998. Results The schistosomiasis prevalence was considerably decreasing in the 3 villages, from high epidemic areas into medium or light areas. Conclusion It is a right tactic to use the selective population chemotherapy and control snail in high transmission areas of schistosomiasis in high epidemic areas.
2.Clinical, mammographic and sonographic features of pure invasive lobular breast carcinoma

Objective To observe the clinical, mammographic and sonographic features of pure invasive lobular breast carcinoma (ILC), and to explore the value of combined imaging assessment of ILC. Methods Sonographic appearances of 55 patients of ILC were retrospectively reviewed in correlation with mammographic, pathologic and clinical findings, and the causes of false negative were analyzed. Results The main features found in clinical examination were mass (96.15%) and thickening in the size of breast (3.85%). The main mammographic features were spiculated or ill-defined mass (65.38%) and architectural distortion (23.08%). The most common sonographic patterns were mass (98.00%) and distortion (2.00%). The accuracy for detecting ILC of clinical examination, mammography and ultrasonography was 94.55%, 83.87% and 98.04%, respectively, while combining the three modalities, the detection rate increased to 100%. Conclusion Sonography has higher accuracy than mammography in detecting ILC. Combining clinical examination, mammography and ultrasonography can increase the pre-operative diagnostic accuracy of ILC.
3.The characteristics and value of ultrasonography in the diagnosis of thyroid microcarcinoma

Objective To investigate the role of gray-scale sonography in the diagnosis of thyroid microcarcinoma(TMC).Methods The sonographic characteristics of 58 TMC and 61 benign thyroid nodules(≤1 cm)were retrospectively reviewed and compared with each other.The size,echogenicity,internal solid/cystic component,configuration,anteroposterior to transverse dimension ratio(A/T),margin,halo sign and calcification type of the nodules were studied.Statistical analysis was performed using the chi-square test.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of individual sonographic characteristics were calculated respectively.Results In ultrasonography,TMC manifested as marked hypoechoic in 48.3%,solid lesion in 98.3%,irregular-shape in 74.1%,irregular halo sign in 37.0%,with internal microcalcifications in 62.1%.There were significantly statistical differences between the benign and malignant nodules in those findings (P<0.001).However,there was no obviously statistical difference in the obscure margin.The sensitivity,specificity and accuracy were 86.2%,77.0% and 81.5% respectively when using marked internal hypoechoic echo,A/T≥1 and microcalcification as a new combined criterior.Conclusions The gray-scale sonographic findings between the small benign and malignant thyroid nodules(≤1 cm)are different.Combining these sonographic signs can significantly improve diagnostic value of TMC.
4.STUDY ON INCREASING MOLLUSCICIDAL EFFECT BY COMPLEX NICOTINANILIDE WITH NICLOSAMIDE

Objective To explore the increment of molluscicidal effect and reduction of the cost as well as increasing snail control area by complex commercial molluscicides. Methods Nicotinanilide and niclosamide were combined with the proportion of 1∶1 (The complex of short term was complex molluscicide), and then the snail control tests by immersion and spraying methods were performed using the complex molluscicide in laboratory and field. Meanwhile, the snails climbing from water as they contacted with molluscicide was observed. Results Under the temperature 23-25℃ in lab and 8-19℃ in the field, 0.4 mg/L of the complex by immersion for 48 h, the mortalities of the snails were 100% and 97% respectively. Using 0.4 g/m 2 of complex by spraying for 48 h in lab and the field, the mortalities of the snails were 100% and 81.1% respectively. 5.Clinical characteristics of 100 vestibular migraine cases.

OBJECTIVE:
To analyze the characteristics of vestibular migraine (VM), in order to understand the auditory and vestibular function.
METHOD:
One hundred cases of confirmed or suspected VM patients were observed. Routine examination including pure tone test, vestibular dual temperature test and vestibular evoked myogenic potential(VEMP) and video head impulse test(v-HIT) Were conducted.
RESULT:
The incidence of male and female was about 1.00 : 2. 57. The average age of onset was 47 years, of which the youngest was 19 years old, and the oldest was 74 years old. Ninty-three cases of patients presented with vestibular sensitive performance, including photophobia, phonophobia,or motion sensitive. Fourty-two patients had migraine at the onset of the disease. Thirty-four patients had cochiear symptoms, including tinnitus, ear fullness, or epicophosis. In seventeen cases of fatigue, tension or poor sleep can cause vertigo. Nine patients had low blood pressure, 7 patients presented with hypertension. Two cases of vertigo were closely related to the change of position, and 1 case had visual aura. In the experiment, 94 patients had undergone VEMP test,with 44 cases of abnormal VEMP value. Seventy-nine patients had been examined v-HIT,of whom 2 patients were abnormal (both of which were reduced). Pure tone audiometry was abnormal in 35 cases. Five cases of patients showed abnormal vestibular-double temperature test.
CONCLUSION
Vestibular migraine is more prone to female, with young middle age, and can be accompanied by cochlear symptoms. Vestibular function tests (low frequency and high frequency) are normal.

6.POTENTIAL IMPACT OF SOUTH-NORTH WATER TRANSFER PROJECT ON TRANSMISSION AND DISTRIBUTION OF SCHISTOSOM1ASIS JAPONICA

Objective To understand the potential impact of south-north water transfer project on transmission and distribution of Schistosomiasis japonica, and to put forward the countermea-sures of prevention of the disease transferring into other places. Methods The information on the progress of south-north water transfer project and factors related to the distribution of Schistosoma juponicum were collected, and the suggestions on improving the countermeasures were obtained through the group discussions and field visits. Results The potential impact of the project on the disease transferring is existed, mainly the disease transferring will be through the Lixia River basin in Jiangsu Province, and Chaohu areas of Anhui Province in the east route, and Sihu areas of Hubei Province in the middle route. The snail transferring northward will be affected both by the project and global warming, as a result, the endemic areas of Schistosomiasis will probably transfer into the Hongzehu and Chaohu areas in the future. Conclusion In the east route of the project, if the project is not combined with Schistosomiasis control, the endemic areas of Schistosomiasis will extend into other regions, the loss in the society and economy will be very large.
7.Curative effect of arthroscopic treatment for subacromial impingement syndrome

Objective To analyze the clinical curative effect of arthroscopic acromioplasty treatment for subacromial impingement syndrome(SAIS). Methods A total of 52 patients who had SAIS were studied in our hospital by using the Neer score evaluation, the results of the pre-operative and post-operative scores were statistically analyzed. Results All the 52 cases were followed up, the follow-up ranged from 3 to 24 months with an average of 9 months. According to the Neer scoring system, 34 cases less than 70 points in the pre-operation were poor, 14 cases with 71-79 points were fair, 4 cases with 80-89 points were good. And 23 cases with 71-79 points in the post-operation were fair, 24 cases with 80-89 points were good,5 cases with more than 90 points were excellent. The Neer score was(55.25±19.00)before operation, and(81.13±6.28)after operation. There was a significantly statistical difference (t=-9.33, P<0.05). Conclusion The pain and activity of the patients were improved by the arthro scopic acromioplasty. And it has many advantages,such as faster post-operative recovery,low risk of complications and so on.
8.Penicillium marneffei infection after renal transplantation: one case report

Clinical data were retrospectively reviewed for one case of penicillium marneffei infection after renal transplantation (RT) to explore a proper management of peniciliosis marneffei (PSM)transplantation.This case had a history of pulmonary tuberculosis and underwent RT due to uremia.After discharging, postoperative recovery was excellent.Recurrent cough occurred at Month 7 post-operation.Fiberoptic bronchoscopy and pulmonary CT indicated a possibility of pulmonary tuberculosis.However, a definite diagnosis of PSM was confirmed by next generation sequencing (NGS) and pathogenic bacteria culture of alveolar lavage fluid.After adjusting immunosuppressive agents and regular antifungal treatment with voriconazole, respiratory symptoms improved and pulmonary CT hinted at a resorption of lesion.Features of pulmonary CT and bronchoscopic examination were nearly similar to those of tuberculosis.Thus early bacterium culture and NGS may aid an definite diagnosis.Voriconazole is an effective treatment of the disease.
9.Horseshoe kidney transplantation: a report of 2 cases

To explore the feasibility and experience of kidney transplantation with horseshoe kidney as donor kidney. The horseshoe kidney donated by a brain dead donor was obtained by in-situ perfusion and whole piece incision. After methylene blue staining was injected into the inferior pole artery of the kidney and separated from the isthmus along the edge of the stained site, two kidney donors were divided into two recipients with successful implantation. After renal transplantation, the blood supply of the two donor kidneys was good. During the perioperative period, there was no bleeding or urine leakage. Postoperative ureteral obstruction occurred in one recipient. After the double J-tube drainage was ineffective, autologous ureter was anastomoted with the renal pelvis of the transplanted kidney. Up to now, the renal function of two recipients remained stable. According to literature analysis, kidney transplantation with well-functioning horseshoe kidney as donor kidney is safe and feasible, but its postoperative complication rate is higher than that of conventional kidney transplantation.
