1.The analysis of Montreal classification for 102 patients with Crohn's disease

Objective To analysis the features of Montreal classification,serum markers,treatment and prognosis of Chinese patients with Crohn's disease(CD),and to estimate the relationships between clinical classification and anti-sarccharomyces cerevisiae antibody(ASCA),treatment and prognosis.Methods A retrospective study of 102 consecutive definite CD eases were performed and all subjects were classified into subtypes according to Montreal classification.The results of ASCA,extra-intestinal manifestation,treatment and disease behavior at follow-up were recorded and compared among different subtypes.Results The A2 subtype(52.9%)was dominated in Chinese CD patients.Ileocolon location(40.2%)and stricture lesion (53.9%)were common.The complication rate was dependent on disease course(P<0.05),and intestinal fistula was associated with disease location(P=0.074).B1 subtype had higher progressive rate than B2 subtype(P=0.018).ASCA was not associated with disease loeation,disease behavior,treatment and disease progression(P>0.05).Conclusions Crohn's disease mainly attack young people with male predominance.Early-onset CD patients have higher ASCA positive rate and disease progressive rate.The disease behavior progresses associated with disease course,and the rate of complication and the increase of surgery.The penetrating behavior is the main cause for surgery.Montreal classification is useful to predict the disease course,the need for surgery as well as the prognosis.
2.Clinical efficacy and security evaluation of prostaglandin E1 on liver cirrhosis

Objective To observe the clinical effects of prostaglandin E1 on liver cirrhosis patients and evaluate its security. Methods 68 cases with liver cirrhosis admitted in Luqiao hospital were randomly divided into experimental group(34 cases)and control group(34 cases)equally. Patients in control group were received normal therapy,while in experimental group were added prostaglandin E1 on basis of normal therapy. The efficacy and adverse reactions were observed and compared. Results Compared with before treatment,the average value of albumin in two groups were increased obviously,which in experimental group from (27.9 ±4.1)g/L to (36.5 ±4.3)g/L,control group from (27.8 ±4.0)g/L to (31.7 ±4.2)g/L,the differences were significant(P<0.05 ),and the difference between two groups was significant(P<0.05 ),too. The content of alanine aminotransferase and aspartase aminotransferase in two groups were decreased significantly(P<0.05 ),and experimental group was more lower than control group(P<0.05 ). The number of effcacy in experimental group was 28 and account for 82.35%,while in control group was 1 1 and account for 32.35%,the difference was significant(P<0.05). Conclusion Prostaglandin E1 can improve liver function,and has good clinical effects and high security in treatment with liver cirrhosis patients.
3.A study on the association between the inflammatory factors and hallucination in patients with Parkinson disease

Objective To investigate the incidence and risk factors of hallucination in patients with Parkinson’s disease (PD) by using a questionnaire. Methods Ninety-one patients with PD from outpatient and inpatient departments of Neurology and 38 healthy subjects were recruited. Mini-mental state examination (MMSE), unified Parkinson's disease rating scale-Ⅲ(UPDRS-Ⅲ), Hoehn-Yahr, interleukin 6 (IL-6) and C-reactive protein (CRP) were examined in all PD. According to the PPQ-B scores, the PD patients were divided into two groups:the hallucination group (PPQ-B≥1) and non-hallucination group (PPQ-B=0). Logistic regression model was used to explore the risk factors. Results Twen?ty-nine of 91 patients had hallucination and the incidence was 32%. Age and the levels of IL-6 and CRP were signifi?cantly higher in hallucination group than in non-hallucination group. Meanwhile, the duration of illness and MMSE were significantly higher in hallucination group than in non-hallucination group. Logistic regression revealed that the age and duration of illness were independent risk factors of hallucination and the Odds Ratios (OR) were 1.09 (95% CI:1.04~1.32, P<0.05) and 1.43 (95%CI:1.21~1.87, P<0.05), respectively. The risk of hallucination increased by 51%and 59%for each doubling of IL-6 and CRP levels in PD patients and the ORs were 1.51 (95%CI:1.13~2.11, P<0.05) and 1.59 (95%CI:1.07~2.66, P<0.05), respectively. Conclusions Controlling the levels of serum IL-6 and CRP plays an impor?tant role in delay or prevention of the hallucination of PD patients.
4.The experiment in the mechanisms of microbubble-mediated GFP gene enhancement in skeletal muscle in C57B10/mdx mice

Objective To investigate the role of sonoporation and the deblic of microbubbles with perfluoropropane gas and albumin in the mechanisms of microbubble-mediated gene enhancement by experimenting in skeletal muscle in C57B10/mdx mice. Methods Plasmid DNA (10 μg) encoding green fluorescent protein (GFP) was mixed with Optison or SonoVue dissolved in saline and injected into the tibialis anterior (TA) muscle of /C57B10/mdx mice with and without adjunct ultrasound. The efficiencies of GFP transgene expression were determined under different experimental conditions. C57B10 mice as normal control:①C57B10 mice + saline (4 left TAs);②C57B10 mice + saline + ultrasound (4 right TAs) ;③C57B10 mice + Optison(4 left TAs);④C57B10 mice+ Optison + ultrasound(4 right TAs);⑤ C57B10 mice + SonoVue(4 left TAs) ;⑥C57B10 mice + SonoVue + ultrasound(4 right TAs). Mdx mice groups:① mdx mice + saline(4 left TAs) ;② mdx mice + saline + ultrasound(4 right TAs);③ mdx mice + Optison (4 left TAs) ; ④ mdx mice + Optison + ultrasound (4 right TAs); ⑤mdx mice + SonoVue(4 left TAs) ;⑥mdx mice + SonoVue + ultrasound(4 right TAs). Mice were sacrificed 1 week after plasmid DNA injection. Fibres with fluorescence green signals were determined as GFP-positive fibres by fluorescence microscopy. Readout was performed on the section with the maximum number of transfected fibers. Results C57B10 mice: ?Optison without ultrasound had significantly increased gene expression compared with negative control ( P <0. 01). SonoVue without ultrasound did not enhance gene expression. ?Optison with ultrasound had significantly increased gene expression compared with negative control (P < 0.01). ?SonoVue with ultrasound had significantly increased gene expression compared with negative control ( P<0. 01).Mdx mice:? Compared with C57B10 mice, GFP alone demonstrated significant GFP expression in mdx mice ( P <0. 01) , Optison demonstrated significant GFP expression in mdx mice ( P <0.01), and SonoVue demonstrated significant GFP expression in mdx mice ( P <0. 01). ?Microbubble groups (Optison and SonoVue) had significantly increased gene expression compared with negative control (P <0. 01). 5.Clinical characteristic and treatment of maxillofacial trauma in the elderly

Objective To analyze the clinical characteristic and provide therapeutic techniques of maxillofacial trauma in the elderly. Methods Geriatric maxillaofacial trauma patients admitted to our hospital from January 1992 to December 2002 was retrospectively reviewed. Data were collected according to etiology, location, comorbidities, associated injuries, therapeutic methods and the outcome. The patients were compared with a control group consisting of 200 young and adult patients admitted at the same period. Results For the 126 geriatric trauma patients, the major causes were traffic accidents(61.1%) and violence assault(25.4%). A large proportion of facial injuries occurred on the lower part of the face. 69.1% of the patients presented with preexisting diseases. Most of the fractures were not treated(35.7%). The length of hospitalization and ICU stay were relatively longer when compared with the control group(P
6.The clinical effect and cost analysis of patients of oral and maxillofacial space infection treated with improved closed negative pressure drainage

Objective To explore the clinical effect of patients of oral and maxillofacial space infection treated with improved closed negative pressure drainage combined with hyperbaric oxygen, and the possibility of using this method to decrease the hospital expenses. Methods 156 patients with oral and maxillofacial space infection were divided into control group, closed negative pressure drainage group and improved closed negative pressure drainage combined with hyperbaric oxygen group by random number table. 52 patients in each group. The granulation tissue growth time, the healing of wound, clinical effect of the 8thday, hospitalization days, wound healing time, antibiotic cost, average cost in hospital were recorded after treatment. Results The average wound healing time of patients in improved closed negative pressure drainage combined with hyperbaric oxygen group was(6.06 ± 0.23)days, while the granulation tissue growth time was(16.13 ± 2.89)days, both of them were much shorter than the control group[(15.46 ± 4.68)days and(28.60 ± 3.50)days respectively],as well as the closed negative pressure drainage group [(7.43 ± 0.75) days and (22.67 ± 4.34) days respectively], and the differences were statistically significant which the F values equaled to 6.213 and 8.451, P values all less than 0.05. The total healing rate of patients in closed negative pressure drainage group and improved closed negative pressure drainage combined with hyperbaric oxygen group were both 100.00%(52/52), the difference was statistically significant compared with control group (χ2=6.89, P<0.05). The average hospitalization days of the patients in the closed negative pressure drainage group and improved closed negative pressure drainage combined with hyperbaric oxygen group were (8.20 ± 1.49) days and (7.45 ± 0.42)days, the antibiotic cost were(5 068.34 ± 1 074.68)RMB and (5 001.00 ± 456.00) RMB, the average total cost were (9 457.43 ± 647.23)RMB and (9 249.00 ± 367.00)RMB, all these indexes were much lower that which in control group[(18.40 ± 5.89)days,(21 000.43 ± 2 036.48) RMB and (31 000.66 ± 2 711.36) RMB], and the differences were statistically significant, F values equaled to 15.221, 29.434 and 81.220 each, P values were all less than 0.01. Conclusions Improved closed negative pressure drainage combined with hyperbaric oxygen method could improve the clinical effect of patients with oral and maxillofacial space infection, speed up the granulation tissue, shorten the in-hospital time as well as reduce the medical cost.

Purpose:
The purpose of the current study was to explore the functions and potential mechanism of miR-451a in breast cancer (BC).
Methods:
Quantitative reverse transcription real-time polymerase chain reaction was used to analyze the expression of miR-451a in human normal mammary cells (MCF-10A) and BC cells. Colony formation assay, terminal-deoxynucleoitidyl transferase mediated nick end labeling assay and transwell assays were conducted to validate the effect of miR-451a on proliferation, apoptosis, migration and invasion of BC cells, respectively. RNA pull-down, RNA immunoprecipitation and luciferase reporter assays were applied to investigate the upstream and downstream mechanisms of miR-451a in BC cells.
Results:
MiR-451a was expressed at a low level in BC cells. Overexpression of miR-451a repressed BC cells proliferation, migration and invasion. Moreover, long non-coding RNA AC092127.1 acted as a sponge of miR-451a to enhance the expression level of AE binding protein 2 (AEBP2) that was demonstrated to be the target gene of miR-451a in BC cells. Finally, rescue experiments validated that miR-451a and AEBP2 involved in AC092127.1-mediated BC cell growth, migration and invasion.
Conclusion
In a word, AC092127.1/miR-451a/AEBP2 axis contributes to BC cell growth, migration and invasion. Our results may help to find novel potential targets for BC treatment.

9.Clinical diagnosis and surgical management of ureteral obstruction after renal transplantation

Objective To investigate the diagnosis and surgical management of ureteral obstruction after renal transplantation.Method A respective study was performed on 15 cases of ureteral obstruction who received renal allografts.All of the cases were confirmed,including 5 cases of stenosis of ureterovesical junction,4 cases of stenosis of distal ureter,2 cases of blood clot blockage,2 cases of edema due to perforation of distal ureter,1 case of ureteral calculi,and 1 case of extrinsic compression of hematoma.All 14 patients underwent open surgical management to reestablish ureter with D-J catheter as stent for 12-16 weeks.One patient was subjected to percutaneous nephrostomy followed by percutaneous ureteroscopy,the obstructed site was identified at ureterovesical junction and antegrade balloon dilation and D-J stent in ureter were performed subsequently.Result Fifteen patients were diagnosed as ureteral obstruction by colour Doppler ultrasound and MRU.All of the patients were resolved the obstructions after surgical managements and the renal function got improved.No reobstruction occured during one year follow-up period.Conclusion Colour Doppler ultrasound and MRU can definitely be used to diagnose ureteral obstruction after renal transplantation.Early diagnosis and surgical management are very important.Open surgical management of the obstructed ureter is an effective method.Antigrade balloon dilation is a therapeutic approach to treat ureteral obstruction.
10.Relationship between single nucleotide polymorphism-56 of calpain-10 gene and glucose and lipid metabolism in polycystic ovary syndrome patients

Objective To study the effect of desogestrel and ethinyl estradiol(DEE)pre-treatment combined with gonadotropin releasing hormone agonist(GnRH-a)stimulation in in vitro fertilization-embryo transplantation(IVF-ET).MethodsA retrospective analysis was performed in 101 infertile women who received a short protocol of GnRH-a for IVF-ET treatment from June 2004 to June 2007 in the Reproductive Medicine Center of First Affiliated Hospital of Wenzhou Medical College.Patients had been pre-treated with oral contraceptive pill(OCP)for two months before GnRH-a combined with recombinant follicle stimulation hormone(r-FSH)treatment(study group,n=42)or had not been pretreated with OCP(control group,n=59).A statistical analysis of two groups was carried out for the assessment of ovulation stimulating effect of OCP and its influence on the IVF.Results Serum FSH was significantly decreased after OCP in the study group.Twelve pregnancies were obtained including 1 case of spontaneous abortion at 7 weeks in the study group,and 11 pregnancies were obtained including 2 cases of spontaneous abortion during 7-9 weeks in control group.The clinical pregnancy rates in the study group(23%,12/53)was higher than that in the control group(17%,11/63),but the differences were not significant(P>0.05).The miscarriage rate in the study group(8%,1/12)was lower than that in the control group(18%,2/11),however no significant differences were found between them(P>0.05).The cycle cancellation rate in patients of the study group(5%,3/56)was significantly lower than that in patients of the control group(17%,13/76,P<0.05).The differences between patients of the two groups with respect to age,basal level of FSH and luteinizing hormone (LH),antral follicle counts,the mean number of oocyte retrieval,the days of stimulation,total dose of rFSH used,fertilization rate and embryo cleavage rate,however were insignificant.ConclusionOCP pretreatment combined with short protocol of GnRH-a stimulation in IVF could significantly decrease the cycle cancellation rate,with a declining miscarriage rate and increasing pregnancy rate.
