ObjectiveTo investigate the efficacy of along-meridian distal point selection in treating post-cerebral infarction shoulder-hand syndrome.MethodSixty patients with post-cerebral infarction shoulder-hand syndrome were randomly allocated to treatment and control groups, 30 cases each. The control group was treated by routine neurological medical treatment, conventional acupuncture for stroke-attack on meridians and rehabilitation training and the treatment group, by along-meridian distal point selection in addition. The therapeutic effects were evaluated in both groups after four courses of treatment. The VAS scores, affected-side hand swelling scores and upper limb FMA scores were compared between the two groups before and after treatment. ResultThe VAS score, affected-side hand swelling score and upper limb FMA score were significantly better in both groups after treatment than before (P<0.01). Post-treatment VAS score, affected-side hand swelling score and upper limb FMA score were significantly better in the treatment group than in the control group; there were statistically significant differences (P<0.01,P<0.05). ConclusionAlong-meridian distal point selection has clinically a better improving effect on the pain, affected-side hand swelling and upper limb function in patients with post-cerebral infarction shoulder-hand syndrome.

Objective To evaluate the value of clinical data and various methods of diagnostic imaging in the patients with pancreatic cancer. Methods Two hundreds and fifty-six patients with pancreatic cancer admitted into Peking Union Medical College Hospital between 1996 and 2004 were included. The clinical symptom and the accuracy of various imaging methods (B-US, CT, ERCP, EUS, DSA and MRI) in assessing pancreatic carcinoma were analyzed. Results Weight lost was the most common symptom in 79% of patients with pancreatic carcinoma. Jaundice was mostly seen in patients with pancreatic head carcinoma. Upper abdominal pain was in 86% of patients with carcinoma in pancreatic body and tail. The diagnostic accuracies of B-US, CT, ERCP, EUS, DSA and MRI were 83% , 89% , 90% , 91% , 82% and 79% respectively. Conclusion B-US and CT scanning should be undergone in patients with abdominal pain, jaundice and weight lost who were suspected to have pancreatic carcinoma. Further imaging techniques such as EUS, ERCP and MRI should be advised in patients highly suspected to have pancreatic carcinoma, in spite of the results of B-US and CT were negative.

Objective To investigate the diagnostic value of capsule endoscopy (CE), CT enterography (CTE), ileocolonoscopy and small bowel follow through (SBFT) for small bowel Crohn's disease (CD). Methods Fifty-seven consecutive patients with CD underwent ileocolonoscopy, CTE, CE, and SBFT. It included the presence of the following symptoms and signs: abdominal pain, weight loss, diarrhea, fever and positive fecal occult blood test. The location and the characteristics of intestinal and extra-intestinal lesions detected by four techniquks were compared. The proportions of patients with positive findings using each examination were compared. Results Of the 57 patients, 50 underwent ileocolonoscopy, terminal ileum lesion was found in 33 patients (66. 00% ), the remaining 17 (34.0%)were normal; among 34 patients who had CTE, 29 of small bowel lesion were found (85. 29% ); CE were performed in 27 patients, due to prolonged gastric transit one time, the capsule did not reach the cecum in one patient during battery lifetime. CE showed small bowel lesion in 26 patients (96.30% ); SBF was performed in 39 patients and 26 of small bowel lesion were detected (66. 67% ). CE had the highest diagnostic yield for CD and ileocolonoscopy had the lowest, and there were statistically significant difference among the 4 examinations (P = 0. 006 ). The combinded positive rates of two methods were: CE + CTE 92. 86% (13/14), SBFT + CTE 90. 91% (20/22), CE + ileocolonoscopy 95. 65% (22/23), CE + SBFT100% (17/17), ileocolonoscopy + CTE 89. 66% ( 26/29 ), ileocolonoscopy + SBFT 77.78% ( 28/36 ), but there were no significant differences between each two examinations. Conclusion CE, CTE have a higher yield in depicting mild to moderate finding of CD than SBFT. CE is better for assessing early mucosal disease, whereas CTE is better for detecting transmural and extraluminal abnormalities. Most important, CE plus CTE may depict nonobstructive CD of the small bowel when conventional techniques such as ileocolonoscopy or SBFF have negative or inconclusive finding. CE provides us explanations for the symptoms of patients, decision to follow up or therapy.

Objective To observe the value of contrast-enhanced ultrasonography (CEUS) in quantitatively evaluating the evolution of the blood supply of rabbit liver VX2 tumor models in early stage. Methods Eighteen liver VX2 tumor models of rabbits were divided into 3 groups randomly (each n=6). CEUS was performed on the 10th, 15th and 20th day group after establishemnt, resepectively. Time intensity curve (TIC) was plotted, the rabbits' liver tumor and surrounding normal liver tissue were resected for immuno-histochemistry and microvessel density (MVD) count. Results CEUS showed that the rabbit liver VX2 tumor present a typical malignant tumor imaging fast-in and fast-out in early stage. TIC showed that the time to peak (TTP) of hepatic tumor is shorter than that of surrounding normal liver tissue in all three groups (P<0.01), no statistical difference of tumor's TTP was detected among three groups (P>0.05). Absolute enhanced intensity (AEI) and MVD reduced in turn of 10th, 15th and 20th day group. MVD of tumor was higher in 10thday group than surrounding normal liver tissue (P<0.01), while was equal in 15th day group (P>0.05) and lower in 20th day group (P<0.01). AEI of liver tumor had positive correlation with MVD (r=0.83, P<0.05). Conclusion AEI of TIC can be used to quantitatively assess the evolution of the blood supply in rabbit liver VX2 tumor models. The blood supply in early stage of rabbit liver VX2 tumor models shows a gradual descent process from abundant blood supply to medium blood supply and then low blood supply.

Objective To investigate the role of IQ domain GTPase-activating protein 1 (IQGAP1) in angiotensin Ⅱ (Ang Ⅱ)-induced podocyte apoptosis and the underlying mechanism. Methods Differentiated mouse podocytes were exposed to Ang Ⅱ at different concentrations for 6 h or at 10-8 mol/L for variable incubation time.Podocyte apoptosis was assessed by flow cytometry.Expression of IQGAP1 was analyzed by immunofluorescence and Western blotting.IQGAP1 siRNA and MAPK pathway inhibitors(10 μmol/L SB202190,25 μmol/L SP600125,10 μmol/L U0126) were further introduced to investigate the role of IQGAP1 and MAPK signalings in the process. And coimmunoprecipitation was used to evaluate the interaction between ERK1/2 and IQGAP1.Results (1)Ang]] promoted podocyte apoptosis in a dose-and time-dependent manner.(2) IQGAP1 was located in celluar membrane and cytoplasm of cultured podocytes.Exposure to Ang Ⅱ stimulated IQGAP1expression in a dose-and time-dependent manner, and elevated phosphorylation of p38,JNK,and ERK1/2 simultaneously.(3) Pretreatment with SB202190,SP600125,or U0126 dramatically prevented Ang Ⅱ-promoted podocyte apoptosis respectively (P < 0.05).However,the protein level of IQGAP1 was not altered.(4) Knockdown of IQGAP1 with siRNA obviously prevented Ang]Ⅱ-induced apoptosis of podocytes(P < 0.05) and reduced Ang Ⅱ-induced phosphorylation of ERK1/2(P < 0.05),but not that of p38,JNK.This was accompanied by a reduced interaction between ERK1/2 and IQGAP1(P < 0.05).Conclusion IQGAP1 contributes to Ang Ⅱ-induced podocyte apoptosis by interacting with the ERK1/2 signaling protein.
