1.Clinical effect of quadruple scheme combined with bismuth in the treatment of patients with helicobacter pylori

Objective To discuss the eradication effect and application value of quad solution containing bismuth agent in the treatment of helicobacter pylori.Methods 210 patients with helicobacter pylori infection were selected.The patients were randomly divided into three groups A,B,C,respectively,and the three group were given three different bismuth agent quadruple therapy.Group A received furazolidone, tetracycline and rabeprazole joint colloidal bismuth pectin capsules;Group B received amoxicillin and levofloxacin, rabeprazole joint colloidal bismuth pectin capsules;Group C received furazolidone, amoxicillin and rabeprazole joint colloidal bismuth pectin capsules. The treatment effect was recorded.Results There were 2,4,3 cases in A,B,C groups lost to follow-up,including 12 cases in group A positive review,positive review of 11 cases of group B,group C positive review of 13 cases,the therapeutic effect among three groups had no statistically significant difference (P>0.05).The total cost,eradication rate and C/E of group A were 198.38 yuan,82.35%,240.90,which of group B were 479.92 yuan,83.33%, 575.93,which of group C were 280.98 yuan,80.60%,348.61.C/E of group A had the most economic advantage. Conclusion Tetracycline or amoxicillin combined furazolidone, levofloxacin combined amoxicillin quad solution containing bismuth agent can be used as an effective method to the eradication of helicobacter pylori,but the tetracy-cline,furazolidone quadruple scheme through more continue to advantage,more suitable for popularization and applica-tion in clinic.

Objective To study the distribution of hepatitis B virus genotype and subgenotype in Handan and clarify the genotype-related difference in clinic.Methods PCR-RFLP was used to detect the HBV genotype and subgenotype in 108 HBV-infected patients from Handan Infectious Disease Hospital from January,2006 to April,2006.Analyze the correlation of HBV genotype and clinical characteristicly of patients.Results Among 108 HBV-infected patients,the proportion of genotype B and C is 6.5% and 93.5% respectively.All genotype B patients were subgenotype Ba.In genotype C group,88.1% was subgenotype C2;genotype distribution showed no significant difference between male and female.ALT and AST level were higher in genotype C group than those in genotype B group.Conclusion HBV genotype C is predominant in chronic hepatitis B patients in Handan city.HBV subgenotype C2 is the major subgenotype in Handan city.The positive rate of HBeAg and HBV DNA levels are higher in genotype C than those in genotype B group.Genotype C and genotype B have no difference in the severity of liver disease and HBV-DNA.associated with.Clinical feature of subgenotype C1 and subgenotype C2 is similar.

Objective To explore the effect of preventive antibacterial treatment on spontaneous bacterial peritonitis (SBP) in the patients with severe hepatitis. Aim To investigate the effect of small inter-ference RNA-mediated silcencing of the Bmi-1 gene on cell invasion and metastasis in human nasopharyngeal carcinoma cell line CNE-1 . Methods Chemically syn-thesized siRNA targeting the Bmi-1 gene was transfect-ed into CNE-1 cells, which had high invasive and me-tastatic potential. The expression of Bmi-1 mRNA and protein were detected by quantative Real-time PCR and Western blot, respectively. The effects of Bmi-1 knockdown on CNE-1 cells migration and invasion were analysied by Transwell migration assay and Matrigel in-vasion assay. Results Transfected with Bmi-1 siRNA significantly down-regulated the expression of Bmi-1 mRNA and protein as compared with the control group. CNE-1 cells transfected with Bmi-1 siRNA had lower levels of invasion and migration capacity than cells in the control group. Conclusion SiRNA-media-ted silencing of the Bmi-1 gene could significantly in-hibit cell migration and invasion in human nasopharyn-geal carcinoma cell line CNE-1 .
6.Qualitative research on the factors affecting compliance of patients with maintenance hemodialysis

Objective To explore the factors affecting compliance of patients with maintenance hemodialysis and to provide reference for the improvement of patients' compliance.Methods Ten patients with maintenance hemodialysis were individually interviewed in this study.The data were analyzed based on phenomenological method.Results Three factors were found,including feasibility of doctor's advice,characteristics of patients such as awareness of disease related knowledge,recognition todisease progression and usual behavioral pattern,as well as social factors such as economic status,life pressure,family support,professional supervision and guidance.Conclusion In order to enhance patients' compliance,it is suggested to improve thefeasibility of doctors' advice and health education,as well as pay close attention to the influence of patients' recognition,psychological status and social factors on the compliance.
7.Preventive Effect of Heparin Combined with Low Molecular Heparin on Primary Thrombosis in Arteriove-nous Fistula Patients

To evaluate the efficacy of heparin combined with low molecular heparin against early thrombosis in chronic patients after native arteriovenous fistula ( AVF) . Methods: A total of 299 patients were enrolled in the study from January 2012 to December 2014 and randomly divided into two groups. The control group received 5000IU low molecular heparin after the operation for 7 days and the trial group received 3 000u heparin additionally through free arterial end and venous terminal during the operation. The thrombosis formation rate and adverse drug reactions ( ADR ) of one week and 4 weeks after the operation were observed in the two groups. Results:One week after the operation, the thrombosis formation rate in the control group and the trial group was 3. 4% and 0, respectively, and four week after the operation, that was 4. 8% and 0. 67%, respectively. The incidence of ADR showed no statistical-ly difference between the two groups (P>0. 05), and no severe ADR appeared in both groups. Conclusion:Heparin combined with low molecular heparin may be more effective and safer in the prevention of primary thrombosis in the patients after AVF.
8.Effect of ropivacaine administered intrathecally on pulmonary function after caesarean section

ObjectiveTo investigate the effect of ropivacaine administered intrathecally on the pulmonary function after caesarean section.MethodsThirty-six ASA Ⅰ or Ⅱ parturients,aged 22-34 yr,with body mass index ≤35 kg/m2,undergoing elective cesarean section,were randomly divided into 2 groups (n=18 each):bupivacaine group (group B) and ropivacaine group (group R).Spinal anesthesia was performed at L3,4 interspace and 0.5% bupivacaine 1.8 ml (in cerebrospinal fluid 3 ml) and 1.0% ropivacaine 1.4 ml (in cerebrospinal fluid 3 ml) were injected into the subarachnoid space over 10 s in groups B and R respectively.Forced vital capacity (FVC),forced expiratory volume first second (FEV1) and peak expiratory flow (PEF) were measured by spirometry before surgery (T0),and at 1,1.5 and 2 h after intrathecal injection (T1-3).The motor block score was recorded at T1-3.It is regarded as the criteria for respiratory dysfunction that the measured values of FVC and FEV1 are lower than their 80 % and 70 % predicted levels respectively.ResultsFVC and FEV1 were significantly decreased at T1 compared with the baseline value at T0 in both groups (P<0.05).Compared with that at T1,PEF was significantly increased at T2,3 in group R (P <0.05).Compared with group B,PEF at T1 and motor block score atT3 were significantly decreased in group R (P<0.05).The respiratory dysfunction was not found in both groups.ConclusionSpinal anesthesia with ropivacaine and bupivacaine can inhibit the parameter level of carly pulmonary function afrer caesarean section in parturient without respiratory complications,but the pulmonary function still remains normal.
9.Quality evaluation of artificial musk based on its inhibitory effect on cyclooxygenase-2.

The inhibitory ratio (1%) of artificial musk on cyclooxygenase-2 (COX-2) was determined by enzyme immunoassay (EIA). The dose-effect relationship between concentrations of artificial musk and 1% was established. It was found that artificial musk had obvious inhibitory action on COX-2. The concentration for 50% of maximum inhibitory effect (IC50) was about 2.26 mg x mL(-1). There was a good relationship between the logarithm concentrations of artificial musk and 1% when the concentrations of artificial musk ranged from 0.31-20.0 mg x mL(-1). The results indicated that this EIA method could be applied to evaluate the anti-inflammatory activity of artificial musk quickly, conveniently, sensitively and exactly. This paper provided a novel method and foundational research for the bioassay of artificial musk.
10.Effect of transcatheter hepatic arterial chemoembolization on liver cancer patients with portal vein tumor thrombus

Objective To explore the effect of transcatheter hepatic artery chemoembolization(TACE)between different types of liver cancer patients with portal vein tumor thrombus(PVTT).Methods 413 liver cancer patients with PVTT were divided into two groups,TACE(332 cases)group and control group(81 cases);To compare the survival rate and the curative effect of two group.Results Survival rate in the TACE group of 3 months,6 months,12 months,24 months were 93.4%,71.7%,44.9%,24.4%,in the control group 3 months,6 months,12 months,24 months survival rate were 24.7%,2.5%,0,0.The difference has statistical significance(P<0.01).In the TACE group,type Ⅰ,Ⅱ,Ⅲ,Ⅳ of PVTT patients treatmented with TACE also has different survival rate and different curative effect(P<0.01).Conclusion TACE treatment is particularly effective for type Ⅰ,type Ⅱ and type Ⅲ patients of PVTT.
