1.Study on the Structure and Function of Repetition in Doctor-patient Outpatient Conversation Based on Corpus

This paper explored the methods of corpus linguistics application based on the corpus of doctor-pa-tient outpatient conversation, which was built by the project team members. Under the framework of the theory of conversation analysis, it discussed the inductive characteristics of subjective and functional repetition, and com-pared the similarities and differences of the repetition strategy use between patients and doctors. This study con-cludes a new OIR mode, enriching the theory of conversation analysis. The corresponding countermeasures of the repetition strategy use that put forward in this article provided a reference to strengthen the doctor-patient commu-nication, enhance the communication effect, and establish the interacted, cooperated and equal doctor-patient re-lationship. Meanwhile, it offered a new viewpoint to the study of doctor-patient communication.
2.Inhibition Function of Dominant-negative Mutant Gene Survivin-D53A to SPC-A1 Lung Adenocarcinoma Xenograft in Nude Mice Models.

Survivin-D53A (SVV-D53A) is a dominant-negative mutant survivin, which represents a potential promising target for cancer gene therapy. The present study was designed to determine whether SVV-D53A plasmid encapsuled by DOTAP: Chol liposome would have the anti-tumor activity against SPC-A1 lung adenocarcinoma, and to detect the possible mechanisms. In our experiment, SPC-A1 cells were transfected in vitro with SVV-D53A plasmid and examined for protein expression by Western blot, then flow cytometric analysis was used to detect apoptosis. SPC-A1 lung adenocarcinoma xenografts were established in vivo in the nude mice, which received the i. v. administrations of SVV-D53A plasmid/liposome complexes. After mice were sacrificed, the paraffin-embedded tumor tissue sections were used for proliferating cell nuclear antigen (PCNA) expression and terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) assay. Compared with the control group, the mice treated with SVV-D53A plasmid had an obviously reduced tumor volume, with high level of apoptosis and decreased cell proliferation in tumor tissue. The research results proved that the administration of SVV-D53A plasmid resulted in significant inhibition of SPC-A1 cells both in vitro and in vivo. The functional mechanism is that the anti-tumor response causes and induces tumor cell apoptosis.

3.The impact of initial recurrence site on the prognosis for pancreatic cancer after radical operation

Objective:To investigate the impact of initial recurrence site on the prognosis of patients with pancreatic cancer after radical operation.Methods:Clinical data of 172 patients who underwent radical resection of pancreatic cancer and were pathologically confirmed as pancreatic ductal adenocarcinoma in Ningbo University Affiliated Lihuili Hospital from January 2015 to June 2021 were analyzed retrospectively. According to the classification of the initial recurrence or metastasis after operation: no recurrence, local recurrence (residual pancreas, mesenteric vein, pancreaticoenterostomy surrounding tissue), liver metastasis, abdominal or retroperitoneum metastasis, multiple site recurrence and other pattern recurrence. The effect on prognosis was analyzed by COX risk ratio model, the overall survival and recurrence-free survival were calculated by Kaplan-Meier, and the survival curve was drawn. Log-rank test was used to compare the survival rate of different recurrence sites and different treatments after recurrence.Results:12 patients had local recurrence, 69 had liver metastasis, 25 had abdominal or retroperitoneal metastasis, 17 had multiple site recurrence, and 5 had other site recurrence and 44 had no recurrence. The median follow-up time was 15.5 months (3-69 months). The median overall survival was 19 months (95% CI 16.273-21.727). The 1- , 3- and 5-year postoperative cumulative survival rate was 71.0%, 27.8% and 20.2%, respectively. Univariate analysis showed that CA125, tumor size, lymph node metastasis, microvascular invasion, tumor differentiation degree, adjuvant chemotherapy, initial recurrence site were significantly correlated with overall survival (All P value <0.05). Multivariate analysis showed that CA125 ≥30 IU/ml ( OR=2.669, P=0.001), microvascular invasion ( OR=1.736, P=0.028), poor tumor differentiation ( OR=1.604, P=0.027), adjuvant chemotherapy ( OR=0.439, P<0.001), initial recurrence site (All P value <0.05) were the independent risk factors for overall survival. The median recurrence-free survival of 172 patients was 9 months (95% CI 7.075-10.925). Univariate analysis showed that CA125, tumor size, lymph node metastasis and microvascular invasion were significantly correlated with recurrence-free survival (All P value <0.05). Multivariate analysis showed that CA125 ( OR=1.640, P=0.026), tumor size ( OR=1.774, P=0.011) and microvascular invasion ( OR=1.563, P=0.034) were the independent risk factors for recurrence-free survival. After surgery, the median survival time of patients with local recurrence, other pattern recurrence, abdominal or retroperitoneal metastasis, multi-site recurrence and liver metastasis was 28, 22, 21, 15 and 14 months, respectively. Among them, the overall survival of patients with postoperative local recurrence was longest, which was significantly longer than that of patients with multi-site recurrence ( P=0.035) and liver metastasis ( P=0.007); the survival of patients with abdominal or retroperitoneal metastasis was also longer than that with liver metastasis ( P=0.005); and all the differences were statistically significant. In 128 patients with recurrence, the median overall survival of 26 patients without adjuvant therapy was 10 months (95% CI 6.877-13.123); the median overall survival of 68 patients with adjuvant chemotherapy was 15 months (95% CI 13.013-16.987); the median overall survival of 34 patients with comprehensive treatment of surgery and radiotherapy was 19 months (95% CI 15.100-22.900), which was significantly higher than the other two groups, and there were significant statistical differences among the three groups ( P<0.001). Conclusions:The initial recurrence site of pancreatic cancer after radical operation is an independent risk factor for overall survival. Compared with local recurrence and abdominal or retroperitoneal metastasis, patients with multi-site recurrence and liver metastasis have a poor prognosis. Comprehensive treatment after recurrence can significantly prolong the overall survival.
4.Risk factors for early recurrence of patients with single large hepatocellular carcinoma after hepatectomy

Objective:To investigate the risk factors for postoperative early recurrence of patients with single large hepatocellular carcinoma (HCC) (tumor diameter≥5cm).Methods:Clinical data of 135 single large HCC patients who underwent radical resection from Jan 2015 to Sep 2020 in Ningbo Medical Centre Lihuili Hospital were analyzed.Results:Seventy-five HCC patients suffered recurrence,among those 42 patients had early recurrence(within 12 months). Multivariate analysis showed that alpha-fetoprotein (AFP)≥400 ng/ml ( OR=3.510,95% CI: 1.528-8.064; P=0.003) and tumor microvascular invasion (MVI) ( OR=2.769,95% CI: 1.143-6.706; P=0.024) were independent risk factors for early recurrence of single large hepatocellular carcinoma. Survival analysis showed that early recurrence risk factors significantly reduced recurrence free survival (RFS)(AFP≥400 ng/ml, χ 2=23.038, P<0.001; MVI positive , χ 2=10.554, P=0.001) and overall survival (OS) (AFP≥400 ng/ml, χ 2=14.336, P<0.001; MVI positive, χ 2=10.481, P=0.001) in single large hepatocellular carcinoma patients. Conclusion:AFP≥400 ng/ml and MVI positive are independent risk factors for postoperative early recurrence in single large hepatocellular carcinoma patients.
5.Cognitive impairment in patients with lacunar stroke

Lacunar stroke (LS) is one of the subtypes of small cerebral vascular disease. Recent studies have shown that LS has a high risk of cognitive impairment, which imposes a heavy burden on patients, their families and society. However, at present, there is a lack of comprehensive discussion on the related factors, pathogenic mechanism, clinical features, prevention and treatment of cognitive impairment in LS patients. Therefore, this article reviews the relationship between LS and cognitive impairment, in order to provide clinical basis for early prevention and rehabilitation plan, and improve long-term follow-up awareness and comprehensive management ability of cognitive function in LS patients.
6.Slicing Angle Recognition of Fritillariae Thunbergii Bulbus Based on Improved YOLOv7-tiny Algorithm

ObjectiveTo realize the automatic recognition of the slicing angles of Fritillariae Thunbergii Bulbus (FTB) based on the improved YOLOv7-tiny algorithm. MethodFirstly, a diverse dataset of FTB images, totaling 16 000 pictures, with various angles was constructed. Furthermore, improvements were made to YOLOv7-tiny by replacing standard convolutions with ghost convolution (GhostConv), incorporating the coordinate attention (CA) mechanism as a preferred addition, substituting some activation functions with HardSwish function for decreasing the floating point operations. Additionally, a penalty term for angle recognition error was integrated into the loss function, and modifications were made to the non-maximum suppression (NMS) strategy to address cases where multiple detection results were associated with the same target. In order to verify the effectiveness of different improvement points on the optimization of the algorithm model, ablation experiments were carried out on all the improvement points, and the effectiveness of the improvement points was proved by comparing the prediction results before and after the addition of a certain improvement point on the basis of the original model or the model with the addition of an improvement point that has been verified to be effective, in order to evaluate the improvement of the indexes. ResultThe number of parameters required for the improved slicing angle recognition algorithm of FTB was about 55.4% of the original algorithm, and the amount of computation was about 59.4% of the original algorithm. The mAP@0.5[mean average precision at an intersection over union(IoU) of 0.5] increased by 12.2%, the mean absolute error(MAE) of the recognized angle was 5.02°, representing a reduction of 4.58° compared to the original algorithm. In the experimental environment of this paper, the average recognition time per image was as low as 8.7 ms, significantly faster than the average human reaction time. ConclusionThis study, by utilizing the improved YOLOv7-tiny algorithm, achieves effective slicing angle recognition of FTB with high accuracy and more lightweight, which provides a novel approach for stable and precise automated slicing of FTB, thereby providing valuable insights into the automation of processing other traditional Chinese medicines.

Objective:To explore the therapeutic characteristics of population with gout achieving treat-to-target (T2T) indicators through real-world research and evaluate their safety.Methods:A total of 3 287 patients diagnosed with gout by rheumatologists in 21 first-class tertiary hospitals in 10 provinces, municipalities, and autonomous regions in China from January 2015 to December 2021 were included in this polycentric cross-sectional study. The database included patients′ general information, disease characteristics, and clinical application of traditional Chinese and Western medicine treatment measures. SPSS and Excel software were used for data analysis. Frequency analysis, cluster analysis, and factor analysis were used to summarize the characteristics and rules of treatment measures for patients with gout who achieved the target after treatment. The occurrence of adverse events (AE) was recorded during treatment.Results:After treatment, 691 visits (7%) achieved the serum urate (SUA) target, and the most frequent use of urate-lowering therapy (ULT) was febuxostat, followed by benzbromarone. The most common treatment options were following: GroupⅠ: traditional Chinese medicine (TCM) decoction-TCM external treatment-physical exercise-proprietary Chinese medicine; GroupⅡ: ferulic acid-nonsteroidal anti-inflammatory drugs (NSAIDs); Group Ⅲ: allopurinol-sodium bicarbonate-benzbromarone; Group Ⅳ: glucocorticoid-colchicine; Group Ⅴ: febuxostat. A total of 5 898 visits (60%) chieved manifestations of joint pain VAS scores target, and the most frequently used drug to control joint symptoms was NSAIDs. The frequency of use of drugs to control joint symptoms were 2 118 times (usage rate reached 35.9%), while the frequency of ULT were 2 504 times (usage rate reached 42.5%), which was higher than the joint symptom control drug. The most common treatment options were following: Group Ⅰ: proprietary Chinese medicine-TCM decoction-TCM external treatment-physical exercise; Group Ⅱ: NSAIDs-colchicine hormones; Group Ⅲ: allopurinol, Group Ⅳ: benzbromarone; Group Ⅴ: febuxostat. A total of 59 adverse events occurred during treatment.Conclusion:The proportions of gout patients who reach target serum urate level & good control of joint symptoms are both very low, and ULT and anti-inflammatory prescription patterns are very different from international guidelines, so it is necessary to strengthen the standardized management of gout patients. At the same time, life intervention measures account for a certain proportion of the treatment plans for the T2T population, and further exploration is needed.
