Cellulase system contains a series of complex components.There are still some problems remained unclear in cellulase and its mechanism of hydrolyzing lignocellulosic materials and its hydrolysis kinetics,so profound study is needed.The rapid development of many kinds of new interdiscipline such as biochemistry,molecular biology and gene engineering,has further clarified the structure and function of cellulase,and the relationshi Pof its gene expression and regulation,and furthermore resulted in derivative study methods about cellulase in more aspects.Cellulase system components according to synergistic catalytic mode and the sequence of the homology amino acids similarity,summarizes traditional detection methods of enzyme components,and emphasizes on research progress of various biosensors applied in detection of cellulase activity and gene expression was introduced.

Objective To evaluate the outcome of liver transplantation (LT) or combined liver-kidney transplantation (CLKT) for acute-on-chronic liver failure (ACLF) patients with renal dysfunction.Method From January 2001 to December 2009,133 patients underwent LT for ACLF at our center.Among them,30 had both ACLF and renal dysfunction.Of the 30 patients,12 underwent CLKT for end-stage renal disease (ESRD),and the other 18 with hepatorenal syndrome type 1 (HRS1) underwent LT alone.Their clinical data were reviewed and their survival outcomes were compared.Result The median model for end-stage liver disease scores (MELD) of the patients with ACLF were 28.133 patients received deceased donor liver grafts and 12 patients also received the same deceased donor kidney grafts,The hospital mortality rate was 21.8％ for all patients with ACLF.The 5-year survival rates were 72.8％ for patients without renal dysfunction and 70％ for patients with renal dysfunction.The curative effectiveness of the patients with ESRD who underwent CLKT was better than that of the patients without renal dysfunction or the patients with HRS1 who underwent LT alone.Conclusion LT alone improved renal function in most patients with HRS1.Simultaneous liver-kidney transplantation is an excellent strategy in patients with both ACLF and ESRD.It provides protection to kidney allograft in liver-based metabolic diseases affecting the kidney.

Objective To recognize CT signs of parapharyngeal space (PPS) changing in nasopharyngeal carcinoma (NPC) further.Methods 30 patients with proven NPC by biopsy were performed CT and MR scanning before radiotherapy. Results According to CT findings,four types of PPS display in 30 cases were found,each of which was observed and compared with MR findings and were as follows: (1)PPS ,which had no changing on CT, showed fat-filled triangular space continuously on MRI. (2) PPS, which became narrow and linear hypodense on CT,showed that fat intensity was pressed, but continuously on MRI; (3) PPS, which became narrow and linear hyperdense on CT,showed fat intensity,was pressed exceedingly but continuously either on MRI. (4)PPS,which was diminished on CT, showed the mass occupied PPS and lateral pterygoid muscles was pushed, medial fatty space of which was diminished, but intensity of medial or lateral pterygoid muscles was normal or abnormal on MRI. Conclusion (1)When PPS becomes narrow on CT,whether PPS shows hyperdense or hypodense,the mass makes a breakthrough PPS. (2) When PPS is diminished completely on CT,the mass steps PPS to the infratemporal fossa.

Objective To investigate the current status of lead(Pb) poisoning in children and correlations between Pb levels and calcium(Ca), iron(Fe), zinc(Zn) and environmental factors. Methods ①Questionnaire survey was conducted among 3160 parents in Qingdao area, regarding factors that might have effects on the levels of Pb. ②Levels of blood Pb, Ca, Fe and Zn were determined by graphite furnace atomic absorption (GFAA) spectrometry for 3160 children, who were divided into three groups: mildly increased Pb levels, moderately increased Pb levels and severely increased Pb levels, and the effects of Ca, Fe and Zn on children's blood Pb levels were analyzed by multiple linear regressions.③Children having mildly Pb increase were divided into four groups and received Ca, Fe or Zn preparations or placebo. Results ① Children living with smokers, in new houses, in houses or kindergartens located in industrial areas or main traffic roads, consuming preserved eggs or canned drinks regularly were more easily poisoned.②Percentages of children with increased levels who had insufficient blood Ca, Fe and Zn : 53.31% (306/574), 46.17% (265/574) and 68.47% (393/574), respectively, in the group of children with mildly increased Pb levels; 57.68% (263/456), 49.34% (225/456) and 77.41% (353/456), respectively, in the group of children with moderately increased Pb levels; 64.71% (11/17), 58.82% (10/17) and 82.35%(14/17), respectively, in the group of children with severely increased Pb levels. In contrast, the percentages of children in the control group who had low blood Ca, Fe and Zn were 19.78%(418/2113), 5.21% (110/2113) and 14.62%(309/2113), respectively. ③The levels of Ca, Fe and Zn had a negative correlations with blood levels of Pb(coefficient of regression were-0.870,-0.430,-0.565, respectively). ④When given Ca, Fe and Zn, the children had the blood Pb levels decreased by 78.32% (112/143), 66.43% (95/143) and 67.83% (97/143), respectively. Conclusions ①Children's increased levels of Pb have adverse effects on the absorption of Ca, Fe and Zn. ②Intaking Ca, Fe and Zn preparations can reduce blood Pb level.

Objective To investigate the causes of acute renal failure(ARF)after orthtopic liver transplantation(OLT)in patients of acute liver failure(ALF)and the effects of systemic therapy based on continuous renal replacement(CRRT).Methods Clinical data of 412 patients who underwent liver transplantations between January 2001 and June 2006 were analyzed retrospectively (all the cases were followed up to June 2007).According to UNOS grading scale,54 patients were of acute liver failure(UNOS 1 and 2A).Posttransplant ARF developing in 17 cases underwent a systemic therapy based on CRRT as well as anti-rejection,anti-infection and nutrition support.The perioperative courses,complications,causes of death and follow up results were analyzed.Results There were no severe complications during CRRT.Perioperative mortality was 5.4%and 58.8%in patients without ARF and those with ARF respectively.the rate of complications was 35.1%vs 100%.1 year survival rate Was 89.2% vs 41.2%.3 year survival rate was 81.1% vs 41.2%.Condusions The effect of surgery mainly depends on the function of liver and other vital organs.The ALF recipients suffered from a high perioperative mortality,especially those with posttransplant ARL.The systemic therapy based on CRRT benefits patients with postoperative ARF.

ObjectiveTo compare orthotopic liver transplantation (OLT)and combined liverkidney transplantation (CLKT) in the treatment of severe hepatitis B.MethodsIn this study 52 patients of severe hepatitis B were allocated to OLT (40 cases) or CLKT( 12 cases) at our department from Jan.2001 to Sep.2005.The perioperative complications and the result of follow-up were analyzed.ResultsThe preoperative renal functions in CLKT cases were severer than that in OLT cases.Postoperative severe infection was more common in CLKT cases than that in OLT cases.In OLT group 28 patients (70％)suffered from early posttransplant renal dysfunction,among them 11 patients needed dialysis,whilst there were 2 (16.7％ ) patients who needed dialysis in CLKT group (P ＜0.01 ).The posttransplant mortality in OLT group was 40％ ( n =16),significantly higher than that in CLKT ( 16.7％,n =2) ( P ＜ 0.01 ).In OLT group,9 cases developed severe renal failure and died.No one died of renal failure in CLKT group.ConclusionsThe prognosis is more favorable to perform CLKT in patients who suffered from severe hepatitis B with chronic renal dysfunction before transplantation.

Objective To evaluate the endovascular treatment of diffuse aortoiliac occlusive diseases. Methods Thirty-two patients underwent endovascular treatment in which rest pain was found in 84.38%, foot local gangrene in 15.62%. Mean age was 69.7 years (range, 52 years to 81 years) and 71.9% was male. Trans Atlantic Inter-Society Consensus C and D disease was respectively in 40.6% and 59.4% patients. Mean length of vasculopathy was (14.6 ± 1.2) cm (range, 4.5 cm to 19.5 cm) All patients had prohibitive risk for open revascularization. With the approach from femoral artery or brachial artery, combined techniques, such as recanahzation, balloon dilation, stent placement and concomitant common femoral endarterectomy were used. Results Technical success was achieved in twenty-nine patients(90.63%). The complication rate was 3.45%. Clinical status was markedly improved in eight cases (27.59%) and moderately improved in twenty-one cases(72.41%). Mean postoperative ABI was 0.73 ± 0.12, mean preoperative ABI was 0.32 ± 0.09. Significant differences were seen between postoperative ABI and preoperative ABI(P<0.05). Mean time of follow-up was (13.9±6.2) months. At 6 months, primary patency was 81.82% and secondary patency was 89.09%. At 12 months, primary patency was 63.64% and secondary patency was 80.18%. Conclusion Combined multiple endovascular technique is a safe and effective method in the treatment of poor risk diffuse aortoiliac occlusive diseases.

Objective To study the state of erythrocyte immune function in neonates with hyperbilirubinemia,and analyze the influence of various clinical status on erythrocyte immune function.Methods Fifty-two neonates with hyperbilirubinemia were enrolled and 104 healthy neonates as the control group.The adherence rate of complement 3b-receptor on the surface of red blood cell(RBC-C3bRR) and the immune complex adherence rate of red blood cell(RBC-ICR) were detected with erythrocyte saccha-romycete rosettet test.Results 1.The level of RBC-C3bRR in neonates with hyperbilirubinemia was lower than that in control group,and the level of RBC-ICR in neonates with hyperbilirubinemia was higher than that of control group(Pa0.05).3.Comparing the neonates with unconjugated bilirubin of different concentrations,there were significant difference in RBC-ICR(Pa0.05).4.There were positive correlation between RBC-ICR and bilirubin,unconjugated bilirubin in the neonates(Pa0.05).Conclusion Erythrocyte immune function in neonates with hyperbilirubinemia is obviously lower than that of control group and it is influenced by the concentratron of bilirubin and the time of phototherapy.

Objective To monitor the quality changes of iodized salt and analyze its impact factor in Chongqing between 2001 and 2009. Methods Salt samples were collected according to the east, west, south,north and center locations in iodized salt production, wholesale and household sectors. Two units in iodized salt production and wholesale segment were sampled from north, south, east and west places and only 1 unit was sampled from the central place. Nine samples were collected every month in each place. If the place had less than 9 units, and then taken all the units. About resident household, 2 townships were sampled from north, south, east and west places, and 1 township was sampled from the central place, then 20 samples were collected from each township. Iodine content was detected by oxidation-reduction assay. The index of mean iodine, qualified rate from factories and wholesale, coverage rate and taking rate of qualified iodized salt in residents were calculated.Significance was analyzed by trend test, analysis of variance and X2 test. Results The qualified rate of iodized salt from the manufacturers was 92.9%(13/14) in 2001 and the rate was 100.0% each year from 2002 to 2009. The qualified rates of iodized salt from the wholesale were 88.7%(282/318) - 99.8%(431/432). The rates of 2001 and 2002 were lower than that of other years(X2 = 4.98 - 45.69, all P< 0.05 or < 0.01). The coverage rate and taking rate of qualified iodized salt in residents were 94.2% (11 154/11 841 ) - 98.9% ( 14 061/14 217), 83.5% (9 887/11 841 ) -95.8% (13 449/14 039), respectively. The rates showed an increasing tendency (F = 9.27, 26.39, all P < 0.05).The districts(counties) with qualified iodized salt consumption rate > 90% kept increasing. The mean iodine from the manufacturers and wholesale were 29.71 - 36.25, and 31.26 - 36.13 mg/kg, respectively. The iodine level showed a descending trend(F = 35.45, 140.59, all P < 0.01 ). The mean iodine level from the inhabitants were 28.84 - 30.98 mg/kg which remained stable (F = 3.05, P > 0.05 ). The iodine level from manufacturers, wholesale to inhabitants showed an descending trend(F = 38.46 - 671.23, all P < 0.01 ). Conclusions The surveillance results of iodized salt shows an increasing tendency in quality of iodized salt, eoverage rate and taking rate of qualified iodized salt. Factors that affect the quality of iodized salt is that the enterprise does not add iodine to salt strictly by the standard.

Objective To evaluate the preliminary outcome of postoperative three-dimensional conformal radiation therapy(3DCRT)as compared with conventional radiation therapy(CRT)in stageⅢA non-small cell lung cancer(NSCLC).Methods From December 1994 to February 2002,114 such patients received postoperative radiotherapy in M.D.Anderson Cancer Center,with 63 patients in CRT group and 51 in 3DCRT group.The median radiation dose was 53.5 Gy/27f/5-6w and 52.8 Gy/26f/5-6w in CRT and 3DCRT group, respectively.Results The median follow-up time was 28 and 13 months in CRT and 3DCRT group.The 1-,2- year overaLl survival were 80%,62% and 93%,70%.The 1-,2-year disease free survival was 62%,44% and 58%,32% in CRT group and in 3DCRT group,respectively.There was no significant difference between CRT group and 3DCRT group in terms of overall survival,disease-free survival,local-regional control,distant metastasis-free survival or treatment-related complication.There was significant difference in the death cause between two group,with 4 patients having died of cardiac complication in the CRT group and none in the 3DCRT group.Conclusions Postoperative three-dimensional conformal radiotherapy can reach the same immediate results of conventional radiotherapy in stageⅢA NSCLC,with a lower incidence of cardiac complication. However,the long-term results need further follow-up.
