1.Study on risk factors for coronary artery plaque by 64-slice multi-detector computed tomography coronary angiography

Objective To explore the value of 64-slice multi-detector computed tomography coronary angiography (64SCTA) in detecting the coronary artery plaque and to analyze the risk factors for unstable plaque. Methods A total of 112 inpatients who had been diagnosed as coronary artery disease by 64SCTA received catheter coronary angiography (CAG). The levels of serum endothelin-1 (ET-1), matrix metalloproteinase-9 (MMP-9), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and high-sensitivity C-reactive protein (hs-CRP) were measured. The effect of 64SCTA in detecting the coronary artery plaque was evaluated as compared with CAG. The patients were divided into the soft plaque group (n=51) and non-soft plaque group (n=61) according to the CT value of correctly detected plaque. The differences in the above detection indexes between two groups and the risk factors for soft plaque forming were analyzed. Results The 64SCTA had 87.4% sensitivity and 87.1% specificity in detecting coronary artery plaque, the positive predictive value was 82.2% and the negative predictive value was 91.0%. There were significant differences between soft plaque group and non-soft plaque group in the levels of MMP-9, IL-6, hs-CRP, the number of coronary lesions and the composition ratios of gender, diagnosis and diabetes. Logistic regression analysis showed that MMP-9>5.231 ng/L (P=0.0215, OR=2.33, 95%CI 1.13-4.79), hs-CRP>3.583 mg/L (P=0.0008, OR=4.32, 95%CI 1.84-10.15) and unstable angina pectoris (P=0. 0339, OR=4.33, 95% CI 1.12-16.77) were the risk factors for soft plaque formation. Conclusions 64SCTA has highervalue in detecting the coronary artery plaque, and is one of most reliable means in non-invasive methods. MMP-9, hs-CRP and unstable angina pectoris are independent risk factors of plaque instability.
2.The function of Z-axis tube-current modulation technique with desired noise level to decrease radiation dose in MSCT chest scanning

Objective Retrospectively evaluate the effect of Z-axis tube-current modulation technique with desired noise level to improve image quality (image noise level) and decrease radiation doses of MSCT (16-slice CT) in chest scanning. Methods Consecutive two hundred patients whose CT scan projection radiographs showed no significant abnormal were randomly divided into two groups by the examination order: Z-axis tube-current modulation (ZTCM) group (odd number, test group) and constant tube-current (CTC) group (even number, contrast group). The desired noise level of ZTCM group was 10HU and the machine automatically set the dynamic tube-current in scanning according to attenuated information of chest acquired in scan projection radiographs, the tubo-current of CTC group was set at 200mA, while the other scan parameters remained totally the same. The maximum tube-current value,CTDIvol, DLP and the tube-current of the slice at the maximum breast level of female patients were recorded respectively. The noise of image at upper lung, aorta arch, left atrium and bottom lung level were measured and compared. The qualities of Images were classified in three levels (excellent, good, poor) with double blind method. Results The mean value of maximum mA, CTDIvol, DLP and mA of the slice at the maximum breast level of ZTCM group were (178.5±125.6) mA, (10.5±3.8) mGy, (231.6±24.3)mGy/cm and (116.0±22.5) mA, those of CTC were 200.0 mA, 12.8 mGy, (274.7±18.4)mGy/cm and 200.0 mA, ZTCM group decreased by 10.8%, 19.9%, 15.7% and 42.0%,respectively, as compared with CTC group. The image quality at upper lung and bottom lung level in ZTCM group was improved significantly (P < 0.05) and the cases of excellent images in ZTCM group was significantly higher than that of CTC group (P < 0.05). Conclusion ZTCM technique not only contributes to more rational distribution of radiation doses but also realizes individuation, decreases the total radiation doses and improves image quality in chest CT scanning. It is valuable and promising in chest CT scan.
3.Preparation and stability of β-carotene loaded using mesoporous silica nanoparticles as carriers system.

1,3,5-Trimethylbenzene (1,3,5-TMB) was used as the pore-enlarging modifier to expand the pore size of MCM-41 (mobil company of matter) mesoporous silica nanoparticles. The solvent impregnation method was adopted to assemble non-water-soluble β-carotene into the pore channel of MCM-41. The MCM-41 and drug assemblies were characterized by TEM, FT-IR, elemental analysis and N2 adsorption-desorption. The results showed that MCM-41 has good sphericity and regular pore structure. The research also investigated the optimal loading time, the drug loading and the vitro stability of the β-carotene. As a drug carrier, the modified MCM-41 showing a shorter drug loading time, the drug loading as high as 85.58% and the stability of β-carotene in drug assemblies has improved. The study of this new formulation provides a new way for β-carotene application.

5.Effects of mesenchymal stem cell transplantatiOn on cardiac function and structure and eIectrophysiology in rabbits with dilated cardiomyopathy

BACKGROUND: The study of cell transplantation to repair injured cardiac muscle and improve cardiac function of dilated cardiomyopathy (DCM) has become a hotspot in recent years. However, the effect of cardiac electrophysiology following transplantation is still unknown.OBJECTIVE: To explore the influence of ailogenic implanted mesenchymal stem cells (MSCs) on cardiac function, structure and electrophysiology of rabbits with DCM.DESIGN, TIME AND SETTING: Randomized controlled animal trial was performed at Electrophysiology Laboratory of Institute of Pediatrics, Chongqing Medical University between January 2004 and May 2006.MATERIALS: Forty-three New Zealand whiterabbits, weighing 3.0-3.5 kg, irrespective of gender, were selected. Adriamycin was produced by Haizheng Medical Products Company of Zhejiang Province, China, No. 050307. The Ultrasonograph SSD-5000 came from Aloka, Japan, and RM6240 multiplying channel electrophysiolograph of Chengdu Instrument Company was applied.METHODS: All animals were randomized into normal group (n=12), cell transplantation group (n=13), and DCM model group (n=13). Except the normal group, adriamycin was applied to create rabbit DCM model, I mg/kg, twice a week for successive 8 weeks. Bone marrow solution was harvested from the remaining 5 rabbits, and MSCs were isolated, amplified and purified using attachment method. Three weeks after modeling, the MSCs were transplanted into left ventricle anterior wall at four sites in cell transplantation group, model group was injected with matching DMEM/FI2 medium, while the normal group was not given any treatment.MAIN OUTCOME MEASURES: At four weeks postoperatively, left ventricular end-diastolic dimension, end systolic dimension,left ventricular ejection fraction, and shortening fraction were monitored by ultrasonograph; the value for monophasic action potential amplitude (MAPA) and the maximum velocity in 0 phase (V,,~), the value for 50% monophasic action potential durations (MAPDs0) and 90% monophasic action potential durations (MAPDg0) were detected by electrophysiolograph. In addition, the cardiac tissues harvested from implanted region were observed by light microscope and electron microscope, and also the expression of cardiac troponin T (cTnT) and connexin 43 (Cx43) was detected through immunohistochemistry.RESULTS: Of 43 rabbits, 9 rabbits each of the transplantation group died of the acute or chronic toxic effects of Adriamycin, finally, 25 rabbits were included in final analysis. Compared with model group, the end systolic dimension was diminished, and the left ventricular ejection fraction and shortening fraction in cell transplantation group were significantly increased (P < 0.05). The MAPDs0 and MAPDgo in cell transplantation group prolonged significantly compared with model group (P < 0.05). In model group,cardiac myocytes appeared mitochondria swelling and hyperplasia, and their sarcomere had different lengths, arranged unevenly,accompanied by abnormal contraction bands. In addition, myolysis was found in parts of myocardium under electron microscope.However, the structural abnormalities in the cell implantation group were less than the DCM model group, and the implanted MSCs could express cTnT and Cx43.CONCLUSION: Allogenic MSCs transplantation can improve cardiac function, lessen structural abnormalities and may inhibit the progression of electrophysiology derangement.
6.Effect of Volatile Oil of Radix Angelicae Sinensis on experimental asthma in rats.

OBJECTIVETo study the effects of Volatile Oil of Radix Angelicae Sinensis (VOA) on experimental asthma in rat model based on abnormal immune functions of Treg cells.
METHODSAfter grouping, the asthmatic rats were developed through injecting OVA and AI(OH)3 for sensitization and then administering OVA aerosol for challenge, and the respiratory functions, asthmatic behaviors, IL-10 levels in bronchoalveolar lavage fluid (BALF) (ELISA) and Foxp3 expression (immunohistochemistry) in lung of asthmatic rats were observed.
RESULTSVOA at the doses of 40-160 mg/kg could improve the respiratory functions and the asthmatic behaviors, and upgrade IL-10 levels in BALF and Foxp3 expression in lung of asthmatic rats.
CONCLUSIONVOA has some effects of anti-asthma and one of the mechanisms is to improving the lower immune functions of Treg cells.

8.Experimental study of autologous skin grafting on retained denatured dermis for the treatment of partial thickness burn wound.

OBJECTIVETo provide experimental evidence for the feasibility of repair of deep partial thickness burn wounds with autologous skin grafting on retained denatured dermis.
METHODSDeep partial thickness burn wounds 3.5 cm x 3.5 cm in size were produced on the back of SD rats. Superficial tangential excision was performed on the burn wounds on 2 - 5 postburn days with the preservation of denatured dermis. Split thickness autoskin was grafted on the wounds immediately. Tissue samples of whole layer of the skin were harvested from the grafted sites at different time points after the skin grafting, with 8 rats in each group. The morphology and the changes in the collagen fibres in the above tissue samples were observed by light microscopy (LM) and the changes in the biodynamics were determined. The normal skin from the same rat receiving skin grafting was employed as the control.
RESULTS(1) Vitreous degeneration was observed in the preserved denatured dermis. (2) The grafted skin was fused to the burn wound on the 7th day after autoskin grafting in rats. The dermal papillae and reticular layer could be discernible under LM. The thickness, structure and morphology of the skin on the grafted area were similar to normal tissue on the 21st day after grafting with atrophic hair follicles. The density of collagen fiber cord increased gradually, and almost fused at last. (3) The tensile strength, and maximum strain value of the rat skin after operation increased gradually, approaching normal on 60 post-operational day.
CONCLUSIONThe denatured dermis can recover gradually with normal structure and morphology after the application of autoskin on it for the repair of deep partial thickness burn wounds.

9.Constricted ear therapy with free auricular composite grafts.

OBJECTIVEA simple and effective therapy for single side constricted ear.
METHODSTransplanting normal side free composite auricular grafts to constricted ear (15 patients and 15 sides), then lengthening the helix, exposing the scapha, correcting deformity.
RESULTSThe 15 patients composite grafts all survived. The helix has been lengthened, the scapha exposed, the normal ear reduced, the constricted ear augmented and two sides ear have become symmetry.
CONCLUSIONThis method is simple and results are satisfied.

10.Clinical analysis of childhood and adolescent Hodgkin's lymphoma: a report of 88 cases.

OBJECTIVEThe aim of this study was to investigate the clinicopathological characteristics, effective treatment and prognosis in childhood and adolescent Hodgkin's lymphoma.
METHODSA total of 88 patients with childhood and adolescent Hodgkin's lymphoma were treated in the Cancer Hospital of CAMS from 1998 to 2005. The clinicopathological and follow-up data of the patients were retrospectively reviewed. The survival rate was calculated by Kaplan-Meier method and compared by log-rank test. COX multivariate prognosis analysis was performed.
RESULTSThe 2-year event-free survival rate of the 88 patients was 86.4%, the 5-year event-free survival rate was 61.4%, and the 5-year overall survival rate was 95.5%. Univariate analysis showed that the stage of disease (P = 0.033), "B" symptoms (P = 0.028), bulky disease (P = 0.007), splenomegaly (P = 0.050), LDH elevation (P = 0.020), chemotherapy regimen (P = 0.003) were prognostic factors in the 5-year event-free survival rate. Splenomegaly (P = 0.039), LDH elevation (P = 0.033), chemotherapy regimen (P = 0.008) were prognostic factors of 5-year overall survival rate. Multivariate analysis showed that chemotherapy regimen (P = 0.033), stage of disease (P = 0.023), LDH elevation (P = 0.008), "B" symptoms (P = 0.044), bulky disease (P = 0.009) were independent prognostic factors of 5-year event-free survival rate. The chemotherapy regimen (P = 0.012) and LDH elevation (P = 0.046) were independent prognostic factors of 5-year overall survival rate.
CONCLUSIONSThe non-ABVD chemotherapy regimen, stage IV disease, LDH elevation, associated with "B" symptoms and bulky disease are independent prognostic factors of 5-year event-free survival rate. LDH elevation and non-ABVD chemotherapy regimen are independent prognostic factors of 5-year overall survival rate.
