1.Role of ultrasonic three -dimensional speckle tracking imaging in assessment left ventricular systolic function in patients with uremia before and after hemodialysis

Objective To assess the left ventricular global systolic function changes using ultrasonic three -dimensional speckle tracking imaging (3 D -STI )in patients with uremia undergoing high -flux hemodialysis (HFHD)treatment.Methods 86 patients with uremia and 59 healthy volunteers were selected.Participants of control group underwent plasma brain natrinuretic peptide(BNP)examinations before 3D -STI examinations.All the uremia patients received HFHD therapy,and underwent BNP and 3D -STI examinations before and after 4 weeks HFHD therapy.Parameters of left ventricular three dimensional global area strain(GAS),global circumferential strain (GCS),global longitudinal strain(GLS)and global radial strain(GRS)were measured in each group.The correlation between each 3D -STI parameters and BNP level were analyzed.Results Compared with the control group,GAS, GCS,GLS and GRS of left ventricular were significantly lower(t =7.564,6.866,7.946,5.477,all P <0.05)and BNP level in the control group were significantly higher in the uremia group[(60.75 ±17.22)ng/L vs.(792.68 ± 184.56)ng/L,t =30.876,P <0.05].After HFHD therapy 4 weeks,GAS,GCS,GLS and GRS of left ventricular were significantly higher(t =4.233,5.933,6.546,2.959,all P <0.05)and BNP level were lower(t =27.533,P <0.05) than before treatment in the uremia group.GAS,GCS,GLS and GRS of left ventricular were negatively correlated with BNP level(r =-0.51,0.64,-0.46,-0.43,all P <0.05).Conclusion 3D -STI can accurately evaluate the changes of left ventricular global systolic function in patients with uremia before and after hemodialysis,and provide reliable basis for clinical treatment and efficacy assessment.
2.Determination of Residual Organic Solvents in Bromfenac Sodium by Headspace Gas Chromatography

Objective:To establish a headspace capillary GC method for the determination of residual solvents in bromfenac sodi-um. Methods:A headspace GC was used to separate the residual solvents on an HP-5 capillary column with an FID detector. The car-rier gas was nitrogen at the flow rate of 0. 8 ml·min-1 . The temperature of the injector was 200 ℃ and that of the FID was 250 ℃. The programmed column temperature was set as follows:maintained at 40℃ for 10 min, and then raised to 150℃ at the rate of 30℃·min-1 and maintained for 5 min. The containers of headspace injector were in equilibrium at 100 ℃ for 20 min. Dimethyl sulfoxide was used as the solvent. The amount of the residual solvents, such as methylbenzene, isopropanol, dichloromethane, methanol and iso-propyl ether was calculated by an external standard method. Results:All the solvents could be completely separated with good linear relationship. The average recovery of the five solvents was 98. 9%( RSD =2. 01%),99. 2% ( RSD =1. 95%),99. 6% ( RSD =1. 65%),100. 5%(RSD=1. 38%)and 100. 8%(RSD=1. 36%)(n=9),respectively. Conclusion:The method is simple and accu-rate in the determination of the five residual solvents in bromfenac sodium.
3.The Expression of EphA4 in Inferior Colliculus Involves in the Pathology of Salicylate Induced Tinnitus in Rat

Objective To investigate the influences of sodium salicylate on the auditory brainstem response (ABR)and expression of EphA4mRNA in rat inferior colliculus and its effects on salicylate ototoxicity.Methods A total of 30 healthy SPF rats were randomly divided into five groups:the control group (without any treatment),S7 (i.m.injection of sodium salicylate,175mg/kg,twice daily for 7 days),S14(the same method as S7,twice daily for 14 days),S14+R14(the same method as S14,twice daily for 14 days and recovered for 14 days),and S14+R28(the same method as S14,twice daily for 14 days and recovered for 28 days).After the ABR assessment,rats were sacri-ficed after deep anesthesia and the inferior colliculus tissues were dissected.Real-time PCR was used to detect the expression of EphA4mRNA.Results Compared with the control group,ABR thresholds in the S7 group and S14 group were increased significantly (P<0.05),while there was no significant difference between the control group and the S14+R28 group (P>0.05).The inferior colliculus EphA4mRNA expression level of S7 group was signifi-cantly decreased than the control group (P<0.05).The EphA4mRNA expression level of S7 group was significant-ly decreased than the control group (P<0.05),while there was no significantly difference between the control group and the S14+R28 group (P>0.05).Conclusion Long term injection of sodium salicylate can cause changes in the inferior colliculus of EphA4mRNA which are related closely with synaptic plasticity.It may lead the alteration of the inferior colliculus synaptic plasticity,which is associated with the changes of the hearing failure and the tinni-tus behavior.This indicates that EphA4 which is considered as a related protein in the inferior colliculus may play an important role in the pathology of tinnitus.
4.Research on cellular receptors associated with HBV preS 1 adhesion

Objective To seek the specific receptors associated with hepatitis B virus (HBV) adhesion by separating the binding protein of the HBV preS1 region in HepG2 and performing the mass spectrometry .Methods The immunomagnetic bead method was adopted to separate HepG2 membrane protein combined with preS1 peptide fragment and the binding protein was separated by the sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) ,then the destination strips was analyzed by LC-MS/MS mass spectrometry and retrieved by the database .Results 16 bands were separated from HepG2 membrane proteins combined with preS1 by SDS-PAGE ;14 kinds of proteins were identified from 6 bands with better repeatability separated from HepG 2 membrane proteins combined with preS1 .Conclusion Protein analyzed by the mass spectrometry is mainly related with the material transport , cellular signal transduction ,antigen presentation ,immune regulation and energy metabolism .
5.Pharmacokinetics study of albumin-interferon ?-2b fusion protein (HSA-IFN?-2b) in Rhesus Monkey

Aim To investigate the pharmacokinetics of albumin-interferon ?-2b fusion protein(HSA-IFN?-2b) in Rhesus monkeys.Methods Three groups of monkeys were administrated by single injection of HSA-IFN?-2b at 50 ?g?kg-1 iv,50 ?g?kg-1 sc and 300 ?g?kg-1 sc,respectively.Blood samples were taken at 14 different time points from each animal.ELISA was employed to determine the drug concentrations in plasma for all samples.Results Drug was detectable in all of the treated monkeys up to 336 hours after administration.One-compartment model analysis showed that distribution volume was 71 ml?kg-1 in 50 ?g?kg-1 iv group,67 ml?kg-1 in 50 ?g?kg-1 sc group,and 63 ml?kg-1 in 300 ?g?kg-1sc group,respectively.The absorption of HSA-IFN?-2b was relatively slow when given by subcutaneous injection.After 50 ?g?kg-1 sc,the absorption half-life was 19h and elimination half-life was 53h,the bioavailability(F) was approximately 73%,and the apparent clearance(clearance divided by bioavailability,CL/F) was 0.92 ml?h-1?kg-1.Conclusion The result showed that HSA-IFN?-2b may offer the benefits of less frequent dosing and a potentially improved efficacy profile compared with non-albumin fused IFN-alpha.
6.Management of valgus or varus knee deformity with fixator assist nailing technique and distal femur osteotomy

Objective:To discuss the result of treating valgus or varus knee deformity with fixator assist nailing (FAN)technique and distal femur osteotomy(DFO).Methods:In this retrospective study,14 patients with 17 limbs,12 valgus and 5 varus knee deformity were treated with DFO and FAN.The aver-age age of the patients was 23 (17-44)years .The unilateral external fixator was mounted on the lateral side of femur,the minimal invasive distal femoral osteotomy was performed.After the deformity was cor-rected,the intramedully nail was inserted to fix the femur.None of the patients need bone autograft.The preoperation and postoperation lateral distal femur angle (LDFA ),mechanical axis deviation (MAD ) and range of motion (ROM)were measured and analyzed.Results:All the 14 patients were followed-up for 12 -72 months (average 34 months).The osteotomy site united in 3 -5 months (average 3.5 months)post-operatively.According to Paley’s functional scores,12 patients were excellent,2 patients were good.We achieved desired postoperative MAD (from medial 15 mm to lateral 10 mm)in 15 limbs of 12 patients.The LDFA in 1 1 limbs was corrected to normal (84°-90°).The ROM was not signifi-cantly changed before and after operation in the both groups.No infection or neuro-vascular injury oc-curred.Conclusion:The FAN technique is an effective method to treat valgus knee or varus knee de-formity in young patients with DFO.
7.Risk factors for positive surgical margin after laparoscopic radical prostatectomy in high risk prostate cancer patients with and without neoadjuvant hormornal therapy

Objective:To explore risk factors for positive surgical margin (PSM) after laparoscopic radical prostatectomy (LRP) in high risk prostate cancer (PCa) patients with and without neoadjuvant hormornal therapy (NHT).Methods:The clinicopathological data of 202 high risk patients who underwent LRP from January 2012 to July 2020 was retrospectively analyzed. There were 111 cases performed in Beijing Chaoyang Hospital and 91 cases in National Cancer Center. Mean age was(67.7±6.5)years, mean BMI was (25.65±3.21)kg/m 2. Median highest preoperative PSA was 20.97(11.00, 34.40)ng/ml, median preoperative prostate volume was 32.88(23.33, 46.20)ml. Among all 202 high risk PCa patients, 97 did not receive NHT(non-NHT group) and 105 received NHT(NHT group). There were significant statistical difference in term of highest PSA, preoperative prostate volume between NHT and non-NHT groups ( P<0.05), while there was no significant difference in term of age or BMI between the two groups. Among NHT patients, 80 cases accepted complete androgen blockade therapy with median course of 3 months; 3 cases accepted simple castration therapy with median course of 3 months; 22 cases accepted simple anti-androgen therapy with median course of 1 month. Risk factors for PSM after LRP in NHT and non-NHT groups were respectively explored, including age, BMI, hypertension, diabetes, history of pelvic surgery, highest PSA before puncture, ISUP before puncture, preoperative prostate volume, ISUP after LRP, postoperative pathological stage T, pathological lymph node involvement, vessel carcinoma embolus, etc. Results:PSM rate was 50.5%(49/97) and 24.8% (26/105) in non-NHT and NHT, respectively. The apex was the most common location of PSM in non-NHT group(35.1%, 34/97), while the fundus was the most common location of PSM in NHT group(14.3%, 15/105). Multiple logistic regression revealed that postoperative pathological stage T was the only independent factors affecting the PSM for high risk patients without NHT ( OR=3.814, 95% CI 1.302-11.173, P=0.015), while postoperative pathological stage T, pathological lymph node involvement, and vessel carcinoma embolus were independent risk factors affecting PSM for high risk patients with NHT ( OR=18.434, 95% CI 4.976-68.297, P<0.001; OR=7.181, 95% CI 2.089-24.689, P =0.002; OR=3.545, 95% CI 1.109-11.327, P=0.033). Conclusions:Postoperative pathological stage T was independent risk factors affecting PSM for all high risk PCa patients no matter with or without NHT, while pathological lymph node involvement, and vessel carcinoma embolus were also independent risk factors affecting PSM for high risk PCa patients with NHT.
8.Effects of transcranial magnetic stimulation on motor function after acute cerebral infarction

ObjectiveTo investigate the effects of transcranial magnetic stimulation(TMS)on motor function after acute cerebral infarction.Methods60 patients who suffered acute cerebral infarction were assigned to experiment group and control group(30 cases each group). Both groups received rehabilitation theropy. Meanwhile,experiment group received TMS. Fugl-Meyer Assessment(FMA), National Institute of Health Scale of Stroke(NIHSS), Barthel Index(BI) were used to measure the outcome.ResultsThe scores of FMA and BI increased significantly(P<0.05) while that of NIHSS decreased significantly(P<0.05) in both groups after treatment. However, these scores in experiment group improved more compared with those of control group(P<0.05).ConclusionTMS is helpful to improve the motor function, ability of daily living and neurological function.
9.Comprehensive yield and quality comparison of Bupleurum chinense' s seeds of different harvesting time from Qingchuan County with Topsis approach.

In order to investigate the comprehensive quality differences of the seeds harvested in different growing time, and offer theoretical guide for the optimization of seeds' production technology, we analyzed the apparent size, 1 000-grain weight, water absorbency, germination indexes, postharvest embryo rate change, relatively contents of coumarins and the yield of single plant of its seeds of different harvesting time, and compared their comprehensive quality by Topsis analysis. The results showed that there existed obvious differences in yield and quality between seeds of 3 harvesting times. While the harvesting time postponed, the yield of single plant raised, and the shrunken seeds rate decreased, the quality of seedlings increased, while the contents of coumarins showed a steady increase, and the germination rate decreased. The comprehensive quality of the seeds harvested in the black ripe time rank the first place, followed by the brown ripe time and the yellow ripe time. As the harvesting time delays, the seeds' comprehensive quality increases, therefore, we could put off the seeds' harvesting time properly for the high efficient seed production.

10.Analysis on clinical results of hemiarthroplasty for femoral neck fractures in the elderly

Objective To summarize effect of hemiarthroplasty in treatment of femoral neck fractures in the elderly.Methods From January 1998 to January 2008,hemiarthroplaaties were performed in 84 patients with femoral neck fractures,including 24 males and 60 females,at age range of 75-97 years (mean 80 years).Of all,40 patients were treated with unipolar hemiarthroplasty and 44 with bipolar hemiarthroplasty.In the meantime,29 patients were treated with cement fixation and 15 with biological fixation with old femoral stems,and 40 with biological fixation with contemporary femoral stem.Results There were 50 patients(24 in unipolar and 26 in bipolar group)were followed up for average five years (1-10 years).X-ray showed total acetabular erosion rate of 50%(25/50),with 83%(20/24)in unipolar group and 19%(5/26)in bipolar group.Total definite loosening rate was 40%(20/50),which was distributed in six patients treated with cement fixation and 14 with biological fixation.Total revision rate was 24%(12/50),with 37.5%(9/24)in unipolar group and 12%(3/26)in bipolar group.Statistical analysis showed that acetabular erosion rate and revision rate in bipolar group were lower than those in unipolar group(P<0.01).Definite loosening rate in cement fixation group was significantly lower than biological fixation group,with no statistical difference compared with biological fixation of contemporary stem(P>0.05).Conclusions Bipolar replacement is significantly better than unipolar replacement for femoral neck fracture in the elderly.Cement fixation with old femoral stem is significantly better than biological fixation with old femoral stem,which is similar to biological fixation with contemporary femoral stem.
