2.Surgical therapy of primary leiomyosarcoma of the inferior vena cava

Objective To discuss the surgical therapy of primary leiomyosarcoma of the inferior vena cava(PIVCLS).Methods Retrospective analysis of was made 5 patients of PIVCLS from Oct 2009 to May 2011 hospitalized in Department of Vascular Surgery,Provincial Hospital Affiliated to Shandong University.All patients underwent surgical resection,combined with reconstruction of bilateral renal vein and distal inferior vena cava using artificial vascular graft.Results Surgical resection was performed successfully in all patients.The mean operation time was 166.6 min,with mean blood loss 1 560 mL.Leiomyosarcoma intruding the inferior vena cava and right renal vein were observed in all patients during operation.The mean size was 12 cm × 10 cm× 8 cm.The diagnosis of PIVCLS in 5 patients was confirmed by postoperative pathologic examination.All patients did not present lower extremity swelling after surgery and discharged from hospital with normal blood (BUN) and (CREA).All patients were administrated with oral warfarin therapy after discharge.No clinical relapse and pulmonary embolism was observed during the follow-up (range 3 months to 12 months).The ultrasound revealed the patency of artificial vascular grafts in all patients.Conclusions Surgical resection combined with reconstruction of bilateral renal vein and distal inferior vena cava using artificial vascular graft is an effective and feasible treatment of PIVCLS.Leiomyosarcoma is completely eliminated and important abdominal viscera are protected well during the procedure because of minimal impact on hemodynamics.The incidence of postoperative pulmonary embolism also decreases obviously.
3.Surgical treatment for carotid body tumors

Objective To discuss the diagnosis and surgical treatment for carotid body tumors (CBT). Methods Retrospective analysis was made on 16 cases of carotid body tumors hospitalized in Shandong Provincal Hospital from January 2003 to October 2010. All patients were diagnosed by digital subtraction angiography, including 3 case of Shamblin type Ⅰ,11 cases of Shamblin type Ⅱ and 2 cases of Shamblin type Ⅲ. Three cases of type Ⅰ and 3 cases of type Ⅱ underwent carotid body tumor resection. Three cases of type Ⅱ underwent carotid body tumor plus external carotid artery resection, 3 cases underwent carotid body tumor plus external carotid artery resection plus carotid artery repairment, 2 cases did carotid body tumor plus external carotid artery resection plus internal carotid artery reconstruction. One of type Ⅲ underwent carotid body tumor plus external carotid artery resection plus carotid artery repairment, and the other one underwent carotid body tumor plus external carotid artery resection plus internal carotid artery reconstruction. Results Diagnosis of CBT was confirmed by pathology in all cases. There was no postoperative death、hemiplegia and blindness. The cranial nerve injury was caused in 7 cases, accounting for 43. 75%. 13 cases ( 81. 25% ) were followed up for 2 to 76 months ( mean 42 months), no tumor recurrence and metastasis was found. Conclusions Digital subtraction angiography (DSA) is important in the diagnosis and therapy of carotid body tumor. Surgical treatment is the choice of therapy for carotid body tumors.
4.Effects of exogenous melatonin on plasma corticosterone and adrenal glucocorticoid receptor mRNA in neonatal rats

Objective To study the effects of exogenous melatonin on plasma corticosterone(CS)and adrenal glucocorticoid receptor(GR)mRNA in neonatal rats after hypoxic-ischemic brain damage(HIBD). Methods One hundred and forteen 7-day-old Sprague-Dawley rats were randomly divided into four groups:model group(K group),sham operation group(H group),melatonin-treated group(T group),and normal control group(S group). Blood was obtained in different time points after HIBD. Plasma CS levels were measured by radioimmunoassay(RIA). The expression of GR mRNA was detected by semi-quantitative reverse transcriptase polymerase chain reaction(RT-PCR). Results Plasma level of CS increased and the expression of GR mRNA decreased significantly after HIBD,however,these changes were markedly reversed by exogenous melatonin. Conclusions Plasma CS and adrenal gland GR were involved in the pathological process of HIBD. Exogenous melatonin could up-regulate the expression of GR by decreasing the level of endogenous CS,so it may alleviate the excess stress injury after HIBD.
5.Effects of compound danshen dripping pill on the structure and functions of sternohyoid muscle in metabolic syndrome rats.

OBJECTIVETo study the effects of Compound Danshen Dripping Pill (CDDP) on the structure and functions of sternohyoid muscle in metabolic syndrome (MS) rats, and to study whether it has therapeutic effects on obstructive sleep apnea-hypopnea syndrome (OSAHS).
METHODSTwenty-one healthy male SD rats were randomly divided into three groups, i.e., the normal control group (n = 6), the MS group (n = 8), and the CDDP group (n = 7). Rats in the normal control group were routinely fed. High lipid forage was given to rats in the rest two groups. Nine weeks later, CDDP (at the dose of 375 mg/kg) was additionally given to rats in the CDDP group by gastrogavage, and then rats in the CDDP group and the MS group were fed with the same high lipid forage for 12 successive weeks. The content of malondialdehyde (MDA) and the activities of superoxide dismutase (SOD) in the sternohyoid muscle were detected in the three groups. The capillary density, capillary-to-fiber ratio (C/F), the section area of type I muscle fiber were detected using myosin-ATPase histochemical assay. The contractile changes of isometric stemohyoid muscles were determined under electric stimulation by different frequencies.
RESULTSThe contents of MDA were obviously lower in the CDDP group than in the MS group, while the activities of SOD, the capillary density, C/F, the section area of type I muscle fiber, the tension of stemohyoid muscle at 10 -60 Hz, and the 1-5 min tension percentages of the stemohyoid muscle were higher in the CDDP group than in the MS group (P < 0.05, P < 0.01).
CONCLUSIONCDDP could improve oxidative stress induced intramuscularly microcirculation disturbance and changes of muscular fiber structures of the upper airway muscles, and elevate their contractile functions, thus possibly contributing to favorable effects on OSAHS.

6.Advanced gastric cancer of preoperative inter ventional therapy for dinical application

Objective To investigate the curative effect of the preoperative intervention transcatheter arterial chemoembolization and preoperative intravenous systemic simple chemotherapy for gastric cancer.Methods Retrospective analysis of 55 patients diag-nosed with gastric cancer.These patients were divided into two groups:treatment group and control group (simple chemotherapy group),with treatment group of 35 cases and control group of 20 cases,and cases of two groups all received surgical radical surgery for gastric carcinoma treatment.Preoperative and postoperative clinical symptoms were observed.Data of tumor sizes and tumor vessel changes were analyzed and compared.The difference of curative effect between two groups was observed.Results Compared with control group,degeneration and necrosis of tumor cells in gastric cancer tissue of treatment group were obvious,and tumor ves-sels significantly reduced,with nonuniformed,and became thinner.Number of tumor vessels of treatment group was obviously low-er than that of control group (P <0.05).Conclusion The interventional therapy of gastric cancer has obvious short-term therapeutic effects,and can reduce tumor vessels effectively and improve surgical resection rate.
7.Establishment and application of CODEHOP PCR assay for detection of Pasteurella spp.in laboratory animals

Objective We established a rapid detection method of Pasteurella spp.and provided a reference for microbiological quality control of laboratory animal .Methods According to the β subunit of bacterial RNA polymerase ( rpoB) protein multiple alignments of 13 different Pasteurella spp.published in NCBI .The degenerate primers were designed by CODEHOP designer online .CODEHOP PCR method was applied to detecting Pasteurella spp.after the specificity and sensitivity of the method had been evaluated by 21 reference strains .Results Standard strain amplified fragment were about 200 bp by degenerate primers PastF6/PastR5.The primers are able to distinguish between Pasteurella spp.and the other pathognic organisms of laboratory animal respiratory tracts .Sensitivity of this method were 0.2 pg/μL~2 pg/μL to different Pasteurella.The Pasteurella positive rate was 19.1% in 609 animal ' s respiratory samples .The accuracy of positive results was 100%through verifying by sequenced and blast .Conclusions The established method has good specificity and sensitivity .It can be used to detect Pasteurella spp.in animal samples .
8.Replantation of fingertip amputation in lack of availability of intravenous anastomosis.

OBJECTIVETo discuss the replantation of fingertip amputation in lack of availability of intravenous anastomosis.
METHODSFrom November 2009 to November 2010, 86 patients (104 fingers) with fingertip amputation were treated with replantatioin, including 64 males and 22 females, with an average age of 26 years ranging from 2 to 64 years. The time from injury to therapy was from 30 min to 12 h, time of broken finger ischemia was from 2.5 to 12 h. Preoperative examination showed no obvious abnormalities. Four different replantation methods were selectively applied to these 104 amputated fingertips of 86 cases: (1) replantation with anastomosis of single or bilateral proper digital artery in 37 fingers; (2) replantation with arteriovenous bypass in 27 fingers; (3) replantation with exclusive anastomosis of digital artery in 24 fingers; (4) replantation with removing the palmar pocket method in 16 fingers.
RESULTSOne hundred and two of 104 amputated fingertips were survived. Among these survived fingers,75 cases (92 fingers) were followed-up for 6 to 24 months. According to the assessment standard of Chinese Medical Association of Hand Surgery, the results were excellent in 52 cases, good in 19, poor in 4.
CONCLUSIONIt benefits to expand the indications and improve the survival rate of replantation of fingertip amputation with the correct choice of different replantation methods according to the injury situation of the broken fingertip artery after debridement under the microscope.

9.Interaction of the polymorphism of vitamin D receptor gene start codon with physical activity on bone mass accrual in Chinese adolescent girls.

OBJECTIVETo investigate the association between the polymorphism of vitamin D receptor (VDR) gene start codon (Fok I) and bone mass accrual, and assessing if such an association could be modified by physical activity in Chinese adolescent girls.
METHODSA total of 228 premenrche Chinese girls (9-11.5-years-old) were recruited for 2-year study. Bone mineral densities (BMD) at the total body, total left hip (including femoral neck, trochanter, intertrochanteric and Ward's triangle area) and lumbar spine (L1-L4) were measured by dual energy X-ray absorptiometry. The Fok I polymorphism of VDR gene was detected with PCR-RFLP.
RESULTSThere remained 176 available subjects in our cohort when 2-year study was completed. No significant association was observed between Fok I polymorphism of VDR gene and percentage change in BMD at all sites. Girls with FF genotype had lower percentage change in total left hip BMD (THBMD) and femoral neck BMD (FNBMD) than girls with Ff + ff genotype only in low physical activity(< 1197 kJ/d), and physical activity was associated with percentage change in THBMD and FNBMD only in FF genotype group.
CONCLUSIONThe Fok I polymorphism of VDR gene should have significant interaction effect with physical activity on bone mass accrual in Chinese adolescent girls. Girls with FF genotype in low physical activity would be the potential risk population for low bone mass accrual, and high physical activity would be of benefit to gain higher bone mass accrual for girls with FF genotype.

10.Every-other-day fasting improves motor functional recovery of rats with clip-compression injury of the spinal cord

BACKGROUND: Dietary intervention may be the most available, safe, economic and efficient approach to treat spinal cord injury. OBJECTIVE: To observe the effects of dietary restriction in the form of every-other-day fasting (EODF) on the pathological changes and functional recovery of rats with clip-compression injury of the spinal cord. METHODS: The models of spinal cord injury were established by a medical aneurysm clip in 36 Sprague-Dawley rats, and model rats were then randomized into four groups: group A with spinal cord injury and EODF, group B with spinal cord injury, group C with sham operation and EODF, and group D with sham operation. Food intake and body mass were observed. Motor functional recovery in rats was assessed by Basso, Beattie, Bresnahan scores at preoperative 1 day and postoperative 1 day, 1, 2, 4, 6, 8, 10, and 12 weeks. At 12 weeks after operation, the morphological changes of the spinal cord were observed through hematoxylin-eosin staining. RESULTS AND CONCLUSION: The Basso, Beattie, Bresnahan score in the group B was decreased to the lowest at postoperative 1 day, and gradually increased with time. At 8-12 weeks after operation, the scores in the group A were significantly superior to those in the group B. At 12 weeks after operation, hematoxylin-eosin staining showed less lesion of the spinal cord in the group A than the group B. These results indicate that EODF can improve the motor functional of rats with spinal cord injury, and exerts effective protection on the injured spinal cord.
