1. Treatment of multiple vertebral pathological fractures in multiple myeloma patients by percutaneous vertebroplasty: Analysis of 44 cases

Objective To investigate the clinical efficacy percutaneous vertebroplasty (PVP) for multiple vertebral pathological fracture (MVPF) in multiple myeloma (MM) patients. Methods PVP was performed for MVPF in 44 consecutive patients with MM from 2010 to 2014 in our hospital. All the patients had complete medical records. The clinical data were analyzed retrospectively. Preoperative and postoperative visual analog scale (VAS) scores for pain and ECOG scores were measured and analyzed statistically in all the patients. Then the changes of back pain, locomotor activity and life quality after surgery were compared. Results Technical success was achieved in all the 139 diseased vertebrae. The mean injected amount of PMMA was 3.0ml. The mean value of VAS was 7.8 ± 2.9 preoperatively, and significantly decreased to 4.1 ± 1.6(P<0.0l) 1 day after vertebroplasty, to 2.2 ± 1.6(P<0.0l) 1 week after vertebroplasty, and to 2.6 ± 1.9(P<0.0l) 1 month after vertebroplasty. The mean preoperative ECOG scores was 2.9 ± 0.7. One week after the operation, the mean ECOG scores decreased to 2.2 ± 0.3 (P<0.01). There were significant differences between preoperative and postoperative visual analog scale (VAS) scores and ECOG scores. Conclusion PVP is an effective and safe method for treating symptomatic MM with (MVPF).
2.Semen quality and its influencing factors in preconception males in Nanjing area.

OBJECTIVETo investigate the semen quality and its influencing factors in preconception males in Nanjing area so as to provide some evidence for working out effective intervention measures.
METHODSTotally 687 men receiving preconceptional physical examination were enrolled in this study. A questionnaire survey was conducted among the subjects along with an analysis of their semen quality.
RESULTSThe median of sperm concentration was 63.3 x 10(6)/ml (95% CI [19.88-119] x 10(6)/ml). The median of grade a sperm was 33.03% (95% CI [19.38-55.05]%), that of grade a + b sperm was 52.08% (95% CI [39.53-69.37]%), and that of teratosperm was 91.75% (95% CI [69-100]%). The median concentration of seminal plasma PMN-elastase was 195.55 ng/ml (95% CI [76.16-3330.38] ng/ml) and that of seminal plasma zinc was 7.62 μmol/L (95% CI [1.5-23, 45] μmol/L). The positive rates of Ureaplasma urealyticum (UU), Chlamydia trachomatis (CT), and Gardnerella vaginalis (GV) were 42.4%, 0.3%, and 2.4%, respectively. The median of sperm DNA fragmentation index (DFI) of those whose wives had a history of adverse pregnancy was 20.25% (95% CI [2.15-68.25]%). Multivariate logistic regression analysis suggested that mental stress (OR 1.567, 95% CI [1.081-2.27]) and sedentariness (OR 1.772, 95% CI [1.211-2.592]) were independent risk factors for asthenospermia.
CONCLUSIONThe sperm quality of preconception males in Nanjing area is not encouraging, and it can be improved by changing undesirable lifestyle and reducing mental stress.

3.Evaluation of incidence of coronary myocardial bridge in the Uyghur and Han peoples with 64-slice CT

Objective To analyze the incidence of coronary myocardial bridge (MB) of the Uyghur and Han peoples with 64-slice CT. Methods A total of 1350 patients with suspected of coronary disease or other diseases underwent coronary angiography with 64-slice CT, among them 341 patients were diagnosed as MB, including 260 Han people, 70 Uyghur people, 7 Muslim people and 4 people of Kazak ethnic minority group. The incidence of MB of the Uyghur and Han people was calculated respectively; incidence of MB in proximal, middle and distal segment of left anterior descending (LAD), the first diagonal branch (D1), the second diagonal branch (D2), obtuse marginal (OM), left circumflex (LCX) and right coronary artery (RCA) between two races were compared with Chi-Square test. Results Totally 341 patients had 439 MB, 332 in Han people, 92 in Uyghur people, 9 in Muslim people and 6 in Kazak ethnic minority group with 64-slice CT. There was no statistical significance of MB incidence between the Uyghur and Han people in coronary artery (P>0.05), including the proximal, middle and distal segment of LAD. Conclusion The 64-slice CT can effectively detect MB with a high special resolution. There is no statistical significance of MB incidence between the Uyghur and the Han people in each coronary artery.
4.Epidemiological study of the genes conferring aminoglycoside resistance in 36 strains of multidrug-resistant Acinetobacter baumannii

Objective To investigate the prevalence of genes conferring aminoglycoside resistance in multidrug-resistant strains of Acinetobacter baumannii (MDR-ABA).Methods Multidrug-resistant A.baumannii strains were isolated during the period from August to November 2012 from patients in the affiliated hospital of Jiangsu University and the First Hospital of Zhen-jiang.Kirby-Bauer diffusion method was used to determine the susceptibility of these strains to antimicrobial agents.PCR was performed to detect the aminoglycoside resistance genes.Results The 36 MDR-ABA strains showed high resistance rates to most antimicrobial agents except cefoperazone-sulbactam.The prevalence of the genes conferring aminoglycoside resistance, aac (3)-I,aac (6’)-Ib,aph (3’)-I and armA,was 72.2% (26/36),72.2% (26/36),80.6% (29/36)and 80.6% (29/36), respectively.Conclusions The MDR-ABA strains in this study are highly resistant to antimicrobial agents,which is closely as-sociated with the genes conferring aminoglycoside resistance.
5.Experimental study on NK cells promoting donor marrow engraftment and hematopoietic reconstitution after MHC haploidentical BMT in mice.

OBJECTIVETo explore the effect of donor-derived NK cells added to pretreatment conditioning regimen on hematopoietic reconstitution after MHC haplotype-mismatched BMT in mice.
METHODSMurine model of MHC haplotype-mismatched BMT was established by using BALB/c(H-2d) x C57BL/6(H-2b) (CB6F(1)(H-2d/b)) mouse as recipient, and C57BL/6(H-2b) mouse as donor. Fifty recipient mice were divided into 5 groups. The mice in the first three groups were each infused 1 x 10(6), 5 x 10(5), 2 x 10(5)/mouse donor-derived NK cells, respectively before TBI ((60)Co, 9.0 Gy) and then conditioned with TBI, followed by infusion of C57BL/6(H-2b) mice bone marrow cells four hours later. The mice in the fourth group received TBI only, and in the fifth group, TBI and BMT at the some doses as the first three groups. Hematopoietic reconstitution, survival time, body weight, histopathology of the recipients were followed up.
RESULTS(1) Survival time was (5.15 +/- 0.66) days for the fourth group, and > 30 days for the other 4 groups. (2) Leukocyte and platelet counts at day 10 after BMT were (0.99 +/- 0.22) x 10(9)/L and (61.0 +/- 7.27) x 10(9)/L respectively for the fifth group and (2.01 +/- 0.21) x 10(9)/L, (101.50 +/- 16.34) x 10(9)/L; (1.98 +/- 0.29) x 10(9)/L, (99.50 +/- 16.41) x 10(9)/L and (1.97 +/- 0.21) x 10(9)/L, (98.0 +/- 16.19) x 10(9)/L for the first three groups, respectively. Histopathology displayed no GVHD in all the groups.
CONCLUSIONDonor-derived NK cells could promote hematopoietic reconstitution after MHC haplotype-mismatched BMT in mice.

6.Treated for newly diagnosed Hodgkin 's lymphoma with brentuximab vedotin: one case report and literature review

Objective To deepen the understanding of curative effect and adverse drug reaction of brentuximab vedotin in the treatment of Hodgkin's lymphoma.Methods One case of newly diagnosed Hodgkin's lymphoma who treated with brentuximab vedotin was collected.The clinical features,laboratory examination,treatment procedure,prognosis were analyzed,and the relevant literature was reviewed.Results The patient was twenty years old,female,found the right neck masses for five days,diagnosed with Hodgkin's lymphoma,mixed cell type,stage Ⅳ group B.Chose the chemotherapy regimen BV + GABVD for 4 courses,the PET-CT indicated CR,then used GABVD 4 courses,now in the stage of clinical observation and followed up.Conclusion BV is a kind of antibody-drug conjugate which targeted on CD30 protein.The FDA approved for the treatment of Hodgkin's lymphoma and systemic anaplastic large cell lymphoma.The common adverse reactions are granulocyte,peripheral neuropathy,fatigue,nausea and vomiting.BV can significantly improve the prognosis of CD30 positive Hodgkin's lymphoma,but need to pay attention to the prevention of adverse reactions.
7.Clinicopathological significance of altered metallothionein 2A expression in gastric cancer according to Lauren's classification.

BACKGROUNDDysregulated metallothionein 2A (MT2A) has been implicated in carcinogenesis. The purpose of this study was to investigate the expression of MT2A in gastric cancer (GC) and its correlation with prognosis.
METHODSReverse transcription-polymerase chain reaction and real-time polymerase chain reaction were used to detect the mRNA expression of MT2A in 12 GC cell lines, normal gastric epithelial GES-1 cells, and 36 GC and adjacent normal tissues. MT2A protein expression was determined in 258 GC tissues and 171 adjacent normal tissues by immunohistochemistry.
RESULTSMT2A mRNA expression was lower in GC cells and primary tumors than in GES-1 cells and adjacent normal tissues, respectively. High protein expression of MT2A was present in 130 of 171 normal tissues (76.0%) and in 56 of 258 GC tissues (21.7%; P < 0.001). MT2A protein expression was higher in well/moderately differentiated GC (22/54; 40.7%) than in poorly differentiated GC (34/204; 16.7%; P < 0.001). Moreover, the protein expression of MT2A was lower in diffuse-type GC (6/82; 7.3%) than in intestinal-type GC (50/176; 28.4%; P = 0.0001). Importantly, MT2A expression was an independent prognostic factor for GC, and decreased MT2A expression was associated with poor clinical outcome (P < 0.001). The expression status of MT2A could predict prognosis in intestinal and diffuse-type GCs.
CONCLUSIONExpression status of MT2A might be a useful prognostic biomarker for GC, especially when used in combination with Lauren's classification.

8.Study on correlation between time within glucose target range and left ventricular mass index in patients with type 2 diabetes mellitus complicating hypertension

Objective To investigate the correlation between the glucose time in range(TIR)and left ventricular mass index(LVMI)in the patients with type 2 diabetes mellitus(T2DM)complicating hyperten-sion.Methods A total of 102 inpatients with T2DM in the Mingguang Municipal People's Hospital from January to December 2022 were selected as the study subjects and conducted the continuous dynamic glucose monitoring.They were divided into the group A(TIR≤40%,n=31),group B(TIR>40%-70%,n=41)and group C(TIR>70%,n=30)according to the TIR level.The clinical data,laboratory indicators and echo-cardiographic parameters were collected and compared among the three groups.Results Compared with the group A,the age,disease course,systolic blood pressure and the levels of HbA1c,FPG,SCr and TG in the group C were significantly decreased,the HDL-C level was increased,and the differences were statistically sig-nificant(P<0.05).Compared with the group B,the age and the levels of SCr,TG,HDL-C,HbA1c and FPG in the group C were decreased,and the differences were statistically significant(P<0.05).Compared with the group A and group B,LAD,LVPW,IVS and LVMI in the group C were reduced with statistical differences(P<0.05).The multiple linear stepwise regression analysis results showed that TIR was a protective factor for LVMI(OR=0.562,95%CI 0.413-0.673,P<0.05),and the disease course and systolic blood pressure were the risk factors(OR=1.511,95%CI:1.067-2.947;OR=2.325,95%CI:1.086-5.574,P<0.05).Conclusion TIR in the patients with T2DM complicating hypertension is correlated with LVMI,and the TIR level may be an influencing factor for early cardiovascular events.
9.Construction of a training course system for online appointment nurses based on job competence

Objective:To construct a training course system for online appointment nurses based on post competence,and to provide references for the training and development of online appointment nurses in China.Methods:From January 2021 to May 2022, based on the theory of post competence, literature review, policy research, questionnaire survey and and expert consultation were used to establish the training course system of online appointment nurses.Results:The effective recovery rates of the two rounds of expert consultation questionnaires were 18/18 and 17/18, respectively; the expert authority coefficient was 0.928 and 0.938, respectively; and the Kendall harmony coefficient was 0.185 and 0.284, respectively, the differences were statistically significant (all P<0.001). The final content of the training course system for online appointment nurses included 4 first-level indicators,which were professional knowledge, professional skills, professional abilities and traits, 19 second-level indicators and 58 third-level indicators. Conclusions:The training course system of online appointment nurses based on post competency is scientific, reasonable and prominent,which can provide reference for training of online appointment nurses in China.
10.Treatment of varicocele by microsurgical varicocelectomy: report of 94 cases.

OBJECTIVETo investigate the clinical effect of microsurgical varicocelectomy in the treatment of varicocele.
METHODSWe retrospectively analyzed the clinical data of 94 varicocele patients treated by microsurgical varicocelectomy, of whom 36 complained of testicular pain or dragging and distending discomfort, and 58 infertility or oligoasthenospermia. Microsurgical varicocelectomy was performed through inguinal or subinguinal approach. Complications were observed and semen parameters detected through follow-up visits at 1, 3 and 6 months after the operation.
RESULTSTesticular pain or discomfort disappeared in 21 (65.6%) of the 32 followed-up patients who had complained of such symptoms, was relieved in 6 (18.8%), and remained unimproved in the other 5 (15.6%). The 56 followed-up patients with infertility or oligoasthenospermia all showed significantly improved sperm concentration and motility (grade a + b sperm), (15.47 +/- 3.21) x 10(6)/ml and (13.34 +/- 5.16)% at 3 months, and (18.39 +/- 4.05) x 10(6)/ml and (17.23 +/- 4.69)% at 6 months after operation, as compared with (8.26 +/- 1.68) x 10(6)/ml and (5.25 +/- 1.09)% preoperatively (P < 0.01).
CONCLUSIONMicrosurgical varicocelectomy can effectively improve symptoms and semen parameters in varicocele patients.
