1.Application of ECToolbox software in evaluation of left ventricular ejection fraction

Objective To evaluate the applicability of formula R0, R1, R2 of ECToolbox software in measurement of left ventricular ejection fraction (LVEF) with gated myocardial imaging. Methods Rest gated myocardial SPECT with ~(99m)Tc-MIBI and rest gated blood-pool SPECT with ~(99m)Tc-RBC were performed on 64 patients (44 with CHD, 20 with hypertension). LVEFs were separately calculated with formula R0, R1, R2 of ECToolbox software, and the results were recorded as R0 LVEF, R1 LVEF and R2 LVEF, respectively, and compared with LVEFs from gated blood-pool imaging (GBPI LVEF). Results The LVEFs from formula R0, R1 and R2 were all significantly correlated with GBPI LVEF (r=0.905, 0.905 and 0.903, P<0.05, respectively). The accuracy within ±15% deviation from the GBPI LVEF was 54.30%, 71.40% and 22.90%, and within ±30% was 81.40%, 88.60%,74.30%, respectively. The differences among R0 LVEF, R2 LVEF and GBPI LVEF were statistically significant (P<0.05), while between R1 LVEF and GBPI LVEF was not (P>0.05). Conclusion The formula R1 of ECToolbox software is most applicable in measuring LVEF with gated myocardial imaging.

Glioma is the most common malignant brain tumor with high malignancy and lethality.The specific potential radiolabeled nanoparticles have been applied in the glioma research for non-invasive,dynamic,real-time and quantitative evaluation.Furthermore,radiolabeled nanoparticles have shown great potential in targeted therapy of glioma.The up-to-date application of radiolabeled nanoparticles in SPECT imaging,PET imaging,multimodality imaging and theranostics in glioma are reviewed in this article.
3.The endoscopic diagnosis, treatment and prognosis analysis of 72 cases of gastrointestinal carcinoid tumors

Objective To explore endoscopic diagnosis,treatment,pathological features and prognosis of gastrointestinal carcinoid tumors.Methods Retrospective analysis of 72 cases of gastrointestinal carcinoid tumors which were endoscopically diagnosed,treated,pathologically confirmed and followed up from July 2004 to July 2010 in Shanxi Provincial People' s Hospital.Common endoscopic diagnosis,endoscopic ultrasound(EUS)diagnosis,pathological features,treatment and prognosis were summarized.Results Twenty six cases of upper gastrointestinal carcinoid tumors and 46 cases of lower gastrointestinal carcinoid tumors were diagnosed and followed up.A total of 65 cases were single carcinoid and 7 were multiple carciniods.Tumors were mostly yellowish hard nodules with limited range of movement.EUS showed uneven hypoechoic nodules in the submucosal layer.Fifty-one cases of single carcinoid tumor were successfully treated with endoscopic mucosal resection(EMR)or endoscopic submucosal dissection(ESD)and the cure rate was 96.2%(51/53).Three cases of multiple carciniods were cured with EMR and the cure rate was 3/7.There was no tumor recurrence after treatment during follow-up.Conclusions Endoscopy examination is an important way to detect early carcinoid tumors.Endoscopic ultrasound probe can further clarify source and level of the lesion,and provide the basis for the endoscopic therapy.EMR and ESD are the preferred radical treatment for early carcinoid tumors.
4.Perioperative anxiety and depression of patients undergoing prosthetic augmentation mammoplasty

Objective To investigate perioperative anxiety and depression of patients undergoing prosthetic augmentation mammoplasty.Methods A total of 123 patients undergoing prosthetic augmentation mammoplasty were involved in the retrospective analysis.Hamilton anxiety scale (14 Edition) and Hamilton depression scale (version 24) were used to evaluate anxiety and depression perspectively before and after surgery.The data were subjected to statistical analysis and correlation factor analysis.Results We found that anxiety scale scores had significant difference before and after surgery (P<0.05),and ression scale scores had no significant difference (P>0.05).We also found that the anxiety after surgery were positively correlated with anxiety and depression before surgery (r=0.377,0.313,P=0.001),and the depression after surgery was also positively correlated with anxiety and depression before surgery (r=0.359,0.298,P=0.001).It suggested the patients with symptoms of anxiety and depression before surgery might appear the same symptoms within a week after surgery.Conclusions Anxiety symptoms before surgery in patients with prosthetic augmentation mammoplasty are significantly higher than that after surgery,but the symptoms decrease when finished surgery.The patients with symptoms of anxiety and depression before surgery probably present with symptoms of anxiety and depression in a week after surgery.
5.Research progress of the central mechanism of acupuncture-moxibustion for simple obesity

Obesity often results when regular caloric intake exceeds the energy expenditure. Hypothalamus is the center that regulates feeding and energy balance inside the body, especially the lateral hypothalamic area (LHA) and ventromedial nucleus (VMN). Acupuncture-moxibustion has achieved significant progress in treating simple obesity, and the relevant studies have revealed the central action mechanism underlying the treatment. By reviewing the literatures associated with the mechanism studies on acupuncture-moxibustion for simple obesity, this article is going to provide clinical and scientific guidance for the treatment of simple obesity with acupuncture-moxibustion.
6.Effect Evaluation of Prophylactic Application of Antibiotics in Cardiothoracic Surgery before and after Clini-cal Pharmacist Intervention

OBJECTIVE:To investigate the effect of clinical pharmacist intervention on prophylactic application of antibiotics in cardiothoracic surgery. METHODS:Medical records of patients underwent cardiothoracic surgery were collected from our hospi-tal during Mar. to Apr. in 2014 (before intervention) and during Jun. to Jul. in 2014 (after intervention). Those were divided into pre-intervention group(n=115)and post-intervention group(n=119). The prophylactic application effect of antibiotics was com-pared before and after intervention. RESULTS:After intervention,the rates of prophylactic application were decreased significantly from 96.5% to 72.3%;the rationality rate of antibiotics selection was improved significantly from 27.9% to 94.2%;The course of prophylactic medication decreased significantly from(5.4±2.8)days to(2.3±1.8)days;the difference had statistical significance before and after intervention(P<0.01). The postoperative infection rate was decreased from 13.0% to 5.9%,the difference had no statistical significance(P=0.074). The average hospitalization time,average drug costs,and average hospitalization expenses were decreased significantly,the difference had statistical significance(P<0.05 or P<0.01). CONCLUSIONS:Clinical pharmacist inter-vention to prophylactic application of antibiotics in cardiothoracic surgery can control the infection effective and guarantee reason-able and safe use of drugs during perioperative period.
7.Effect of malignant tumor on neuromuscular block of cisatracurium

Objective To investigate the effect of malignant tumor on neuromuscular block of cisatracurium. Methods Sixty ASA Ⅰ or Ⅱ patients with head and neck neoplasms (15 cases with benign tumor, 45 with malignant tumor), aged 18-64 yr, were randomly divided into 4 groups ( n = 15 each): Ⅰ benign tumor group (group B,3 × ED95 ); Ⅱ -Ⅳ different dose cisatracurium group (group C1 (2 × ED95 ), C2 (3 × ED95 ) and C3 (4 ×ED95)). Neuromuscular block was assessed with accelerograph F (TOF-watch SX). Single stimulation of ulnar nerve was used. Anesthesia was induced with TCI of propofol (target plasma concentration 3 μg/ml) and remifentanil (target effect-site concentration 3 ng/ml). Tracheal intubation was facilitated with cisatracurium 0.15 mg/kg in group B, and with cisatracurium 0.10, 0.15 and 0.20 mg/kg in group C1, C2 and C3 respectively. The onset time, clinical duration, time for recovery of T/Tc to 75 % and recovery index were recorded. Results The clinical duration, time for recovery of T/Tc to 75 % and recovery index were significantly longer in group C2 than in group B (P < 0.05). The onset time was significantly shorter, while the clinical duration and time for recovery of T/Tc to 75% were significantly longer in group C2 and C3 than in group C1 , and in group C3 than in group C2 ( P <0.05) .Conclusion The duration of action and recovery times of cisatracurium were prolonged in patients with malignant tumor.
8.Modern research progress regarding effect mechanism of acupuncture on post-traumatic stress disorder.

From the TCM theoretical basis and the evidences of acupoint selection and acupuncture and moxibustion methods regarding acupuncture for post-traumatic stress disorder (PTSD), a theoretical discussion is made in this article, also a review regarding the key mechanism of experiment researches on acupuncture for PTSD is made from aspects of neurobiology and brain functional imaging, etc., which could further clarify the effect mechanism of acupuncture on PTSD. It is found that PTSD is a kind of stress syndrome accompanied with a variety of abnormal mental symptom , and acupuncture has superior effect on PTSD. Based on these, the research progress and deficiency on current mechanism study of PTSD are reviewed, hoping to offer ideas for upcoming research and to serve the clinical practice better.

9.The value of diagnosing kidney neoplasms with 18F-FDG PET/CT

Objective To explore the clinical diagnostic value of 18 F-FDG PET/CT in patients with kidney neoplasms.Methods Seventy-nine patients (52 males,27 females,average age (57.3 ± 14.1) years),who had definitive diagnosis of kidney neoplasms by 18F-FDG PET/CT and pathological or clinical comprehensive diagnosis in recent five years,were analyzed retrospectively.The diagnosis by PET/CT was made according to the changes of kidney shape,tumor density and FDG uptake (SUVmax).The diagnostic efficacy was calculated.Results Among the 79 patients,70 cases were finally diagnosed as malignant tumors (including 40 cases of renal cell carcinoma,5 cases of renal pelvis carcinoma,8 cases of lymphoma,16 cases of metastatic tumor,and 1 case of renal fascia capsule liposarcoma) and 9 cases of benign tumors (including 7 cases of angiomyolipomas,1 case of renal acidophilic cell adenoma,1 case of metanephric adenoma; the benign tumors did not contain small lipid sample hamartoma cases).The detective rate of kidney neoplasms with 18F-FDG PET/CT was 97.5% (77/79).For the identification of benign and malignant lesions,the sensitivity of 18F-FDG PET/CT was 92.9% (65/70),specificity was 7/9,accuracy was 91.1% (72/79),positive predictive value was 97.0% (65/67),and negative predictive value was 58.3 % (7/12).Conclusions 18 F-FDG PET/CT can detect and identify most of kidney tumors.Whole-body checking and comprehensive evaluation on kidney cancer patients are still the main advantages of PET/CT.
10.Application of small surgical incision bilateral spinous process in posterior single segment lumbar interbody fusion

ObjectiveTo compare the clinical results of small surgical incision of bilateral spinous process and traditional open surgical incision in posterior single level lumbar interbody fusion,and assess the application value of the small surgical incision of bilateral spinous process in posterior single level lumbar interbody fusion.MethodsFrom December 2006 to June 2008,70 patients with lower lumbar vertebral diseases underwent single segment posterior lumbar interbody fusion.Patients were randomly divided into small surgical incision of bilateral spinous process group(Hereinafter referred to as small incision group) of 36 cases and 34 cases of conventional open group.Small incision group included 20 males and 16 females with an average age of 52.0 years.Traditional open group included 16 males and 18 females with an average age of 53.2 years.Two groups of operative time,blood loss,postoperative drainage,X-ray projection operation frequency,hospital stay,complication rate,creatine phosphokinase (CPK) level,multifidus cross-sectional area,postoperative low back pain visual analogue scale(VAS),Oswestry disability index(ODI),interbody fusion rate were compared,respectively.ResultsAll of 70 cases were followed up for 12-24 months (average,16).There were no statistically differences in the operation time,the number of X-ray projection,complication rate,and fusion rate between the two groups (P>0.05),but there were significant differences in blood loss,postoperative drainage,the first day and the third day of postoperative the level of CPK,postoperative multifidus muscle cross-sectional area,postoperative low back pain VAS,hospital stay,and postoperative ODI between the two groups(P<0.05).ConclusionSmall surgical incision of bilateral spinous process and traditional open surgical posterior lumbar interbody fusion were satisfied with the efficacy,but small surgical incision of bilateral spinous process with less trauma,shorter hospital stay,and rapid postoperative recovery.
