1.Research progress on retinal protection of bone marrow mesenchymal stem cells
International Eye Science 2015;(5):799-802
?Bone marrow mesenchymal stem cells ( BMSCs ) is a kind of adult stem cells mainly enriched in bone marrow, which possesses multiple differentiation potential and can differentiate into trans-germinal layer. lt is easy for BMSC to be isolated and cultured, which has the ability of repairing various tissues with efficient proliferation and expression. BMSC could be used as seed cell for the transplantation therapy of retinal disease because of its properties of immunoregulation and neurotrophin secretion. This review focuses on research progress on retinal protection of BMSCs.

OBJECTIVE:To evaluate the economics of somatostatin, ulinastatin, octreotide and gabexate preventing hyperamy-lasemia and pancreatitis after endoscopic retrograde cholangiopancreatography (ERCP). METHODS:Medical records of 316 cho-ledocholithiasis patients underwent ERCP were selected from our hospital during Jul. 2008-Apr. 2016,and then divided into blank control group(58 cases),somatostatin group(64 cases),ulinastatin group(65 cases),octreotide group(68 cases)and gabexate group (61 cases) according to the use of protease inhibitor. Before ERCP,blank control group received routine treatment as fast-ing,parenteral nutrition support, acid suppression, etc. Based on it,other 4 drug groups received prophylactic drug use according to package inserts 6 h before ERCP. The incidence of hyperamylasemia and pancreatitis after ERCP,VAS scores 3,24,48 h after surgery and the occurrence of ADR were compared among 5 groups. The cost-effectiveness analysis was used to evaluate the eco-nomics of therapy plans in each group. RESULTS:Compared with blank control group, hospitalization time of somatostatin group, ulinastatin group, octreotide group and gabexate group were shortened significantly;the incidence of hyperamylasemia and pancre-atitis were significantly decreased;VAS score 3,24,48 h after surgery were significantly decreased, with statistical significance (P<0.05). Compared among 4 groups, above indexes had no significant difference, and the incidence of ADR was in low level and had no statistical significance(P>0.05). The cost-effectiveness ratio of somatostatin group was the lowest and has cost-effec-tiveness advantage. The results were supported by incremental cost-effectiveness and sensitivity analysis. CONCLUSIONS:Soma-tostatin,ulinastatin,octreotide and gabexate can significantly prevent the occurrence of hyperamylasemia and pancreatitis after ER-CP,and relieve pain with good safety. Somatostatin can achieve the best therapeutic efficacy at the lowest cost,so it is the best plan for hyperamylasemia and pancreatitis after ERCP.

7.Concept of Dynamic Stability in Old People for Fall Prevention(review)
Chinese Journal of Rehabilitation Theory and Practice 2017;23(11):1254-1257
Fall down is related to stability of persons, especially in the aged.Poor stability usually results in fall down.Compared with the concept of static stability, dynamic stability theory emphasizes the velocity-position of center of mass(CoM)to predict feasible move-ments rather than CoM position only. Dynamic stability can help to estimate the risk of fall, and guide the exercise about fall prevention from improvements of adaption to regain balance. The training program still needs more studies.

OBJECTIVETo compare the clinical effects of circumcision and the foreskin-deglove plus shaft-fix (FDSF) procedure in the treatment of phimosis or redundant prepuce in obese adult males (body mass index [BMI] ≥ 28 kg/m²).
METHODSForty-four obese adult men with phimosis or redundant prepuce underwent circumcision (n = 24) or FDSF (n = 20) according to their own wishes. The patients in the circumcision and FDSF groups were aged (26.38 ± 4.24) and (26.90 ± 3.14) years, with BMIs of (27.77 ± 0.77) and (28.07 ± 2.28) kg/m² and penis lengths of (3.51 ± 0.46) and (3.50 ± 0.59) cm, respectively. The operations were performed under local anesthesia with lidocaine plus ropivacaine mesylate.
RESULTSThe operation time of circumcision was (28.04 ± 2.65) min and that of FDSF was (45.45 ± 3.49) min. At 6 months after surgery, normal penile erection was found in all the patients, the penis length was significantly longer in the FDSF than in the circumcision group ([5.01 ± 0.73] vs [3.70 ± 0.47] cm) , and the rate of satisfaction with penile appearance was markedly higher in the former than in the latter group (3.25 ± 0.71 vs 2.83 ± 0.56).
CONCLUSIONThe foreskin-deglove plus shaft-fix procedure under local anesthesia with lidocaine and ropivacaine mesylate may achieve desirable penile erection and appearance in the treatment of phimosis or redundant prepuce in obese adult patients.
