Insulin-like growth factors (IGF), regulated by their receptors and binding proteins, play a pivotal role in human cell proliferation, differentiation and apoptosis. Increasing evidence has revealed that IGF system is involved in the genesis and progress of various malignancies including lung cancer. Recent studies in regard to IGF axis expression in the lung cancer cell lines, pulmonary tissue samples and blood circulation of lung cancer patients have shown that the IGF axis may contribute to the transformation and progression of lung cancer. Several researches have shown that a number of drugs targeting the IGF receptor are being investigated in clinical trials and suggest a potential therapeutic efficacy. This article reviews the updates and progress in the research of IGF axis in lung cancer.
3.Three dimension finite element analysis of anisotropic mandible with dental implants effect on implant-bone interface

Objective To establish anisotropic mandible model with dental implants and to investigate the effect of anisotropy material on stress and strain distribution of implant-bone interface.Methods Three-dimensional finite element models of whole mandible with anisotropic and equivalent isotropic material were created by CT scanning and universal surgical integration system(USIS)software developed by the authors.Two ITI threaded implants were implanted in the posterior teeth area.The vslues of principal stress and principal strain on the bone around dental implants were calculated in two different finite element madels with buccolingual load.Results In the anisotropic madible model,nearly all values of the principal stress and principal strain on cortical and cancellous bone increased compared with the equivalent isotropy model,2.1%-174.1%for principal stress and 4.7%-57.3%for principal strain,but 10.3%-71.4%for principal stress and 19.5%-63.4%for principal strain on cancellous bone.Conclusions In the three-dimensional finite element analysis,anisotropic mandible model with dental implants has an apparent effect on the stress and strains of the implant-bone interface.Anisotropic mechanical properties of mandible should be emphasized in biomechanical study.
4.Evaluation of fluorodeoxylucose-position-emission tomography-computer tomography imaging on head and neck squamous cell cardnomas including primary tumors and lymph node metastases

Objective To evaluate ~(18)F-fluorodeoxylucose-position-emission tomography-computer tomography imaging(~(18)F-FDG-PET-CT)on head and neck squamous cell carcinoma(HNSCCA)and lymph node metastasis.Methods ~(18)F-FDG-PET-CT imaging of 20 patients with HNSCCA was evaluated retrospectively.Results A11 the primary tumors were correctly diagnosed by ~(18)F-PET-CT imaging and SUV_(avg) of the primary tumors was(6.22±2.20).All the sensitivity,specificity,positive predictive value and the negative predictive value were 100%.In detecting lymph node metastasis,the sensitivity was 51%,specificity 97.7%,false positive rate 2.3%,false negative rate 49%,positive predictive value 82%,and negative predictive value 91.2%. Conclusions ~(18)F-FDG-PET-CT imaging was valuable in detecting HNSCCA and lymph node metastasis.SUV was helpful for differential diagnosis between benign or malignant tumors but it needs further study to determine the cutoff SUV for differentiating lymph node etastasis.
5.Comparison of prognosis on patients with coronary critical lesion assessed by coronary angiography , FFR and FD-OCT

Objective :To explore influence on prognosis in patients with coronary critical lesion assessed by coronary angiography (CAG) ,CAG combined coronary fractional flow reserve test (FFR) and CAG combined frequency do-main optical coherence tomography (FD-OCT ).Methods : Patients with coronary critical lesion who don't need stenting were screened by three detection programs , they were divided into CAG group (n= 45 , only received CAG) ,CAG+ FFR group (n=45 ,received CAG+FFR) and CAG+ FD-OCT group (n=45 ,received CAG+ FD-OCT).All three groups received secondary prevention of stable coronary heart disease for half a year .Incidence rates of angina pectoris (aggravated ) ,myocardial infarction and target vessel revascularization rate were observed and compared among three groups .Results :Compared with CAG group ,there were significant reductions in inci-dence rates of angina pectoris (33.3% vs.4.4%,6.7%) ,myocardial infarction (20.0% vs.4.4%,2.2%) and target vessel revascularization rate (26.6% vs.6.7%,2.2%) in CAG+FFR group and CAG+FD-OCT group ,P<0.05 or <0.01. There were no significant difference between CAG + FFR group and CAG+ FD-OCT group , P>0.05 all.Conclusion : Compared with pure CAG ,CAG combined FFR or FD-OCT can more effectively assess cardi-ac blood supply ,and improvement prognosis in patients with critical lesion .
6.Differentiation of transplanted mouse c-Kit+lin- bone marrow cells into hepatocytes in vitro.

OBJECTIVETo ascertain whether mouse c-Kit(+)Lin- bone marrow cells have the potential of hepatic stem cells.
METHODSc-Kit(+)lin- bone marrow cells were isolated and purified by magnetic-activated cell sorting (MACS) from BALB/C male donor mice, and immediately transplanted into age-matched BALB/C syngeneic female mice with 35-Gy total liver irradiation. The recipients were sacrificed 1 month after the transplantation for pathological observation of the liver morphology. The presence of Y-chromosome was examined in the liver cells of the recipient by in situ hybridization (ISH), and alpha-fetoprotein (AFP) and albumin in the cells were detected by immunohistochemistry.
RESULTSThe hepatocytes positive for Sry gene on Y-chromosome were identified 1 month after transplantation, and immunohistochemistry for AFP and albumin confirmed that the donor mice-derived cells were hepatocytes.
CONCLUSIONc-Kit(+)lin- bone marrow cells have the potential of hepatic stem cells, which can reside and differentiate into hepatocytes in the liver after transplantation. c-Kit(+)lin- bone marrow cells can be used as the source cells of cell transplantation for liver disease.

7.Long-term results of endovascular therapy for proximal subclavian arterial obstructive lesions.

BACKGROUNDEndovascular therapy is a treatment option for localized occlusion of the subclavian artery. In this report the long-term experience with 59 patients is presented.
METHODSBetween June 1998 and September 2008, we used endovascular therapy to treat 61 subclavian arterial obstructive lesions in 59 patients (46 males and 13 females, 34 - 82 years of age with a mean age (61.9 + or - 11.0) years). Twenty patients (34%) had clinical symptoms due to vertebrobasilar insufficiency, 26 (44%) had disabling arm ischemia, and 13 (22%) had both symptoms. We performed all procedures under local anesthesia. The approaches were from the femoral artery (n = 47), brachial artery (n = 1, involving bilateral subclavian disease) or both (n = 11). Sixty stents were implanted. All patients were followed-up at 1, 3, 6, and 12 months post-procedure, and annually thereafter.
RESULTSWe achieved technical success in 58 (95.1%) arteries, all of which were stented. There were three technical failures; two were due to the inability to cross over an occlusion, necessitating the switch to an axillo-axillary bypass, and the third was due to shock after digital subtraction angiography and prior to stenting. Arterial stenosis pre- and post-stenting was (83.6 + or - 10.8)% and (2.5 + or - 12.5)% (P < 0.01). Clinical success was achieved in 55 of the 59 patients (93.4%). Of the four clinical failures, three were technical and the remaining patient had a stent thrombosis. Systolic blood pressure difference between the two brachial arteries was (44.7 + or - 18.5) vs. (2.2 + or - 3.9) mmHg (P < 0.01). Primary patency was 98% at 12 months, 93% at 24 months, and 82% at 5 years. Five patients were lost to follow-up by 12 months post-stenting. Significant recurrent obstruction developed in five patients with resumption of clinical symptoms. The overall survival rate was 98.2% at 12 months, 89.5% at 24 months, and 84.5% at 5 years.
CONCLUSIONSEndovascular therapy for proximal subclavian arterial obstructive lesions is effective and successful. This minimally invasive treatment may be the first choice of treatment for proximal subclavical arterial obstructive lesions.

8.Frequency of different subtypes of spinocerebellar ataxia in the Han nationality of Hunan province in China.

OBJECTIVE:
To determine the frequency of different subtypes of spinocerebellar ataxias (SCAs) in the Han nationality of Hunan province in China.
METHODS:
The mutations of SCA1, SCA2, SCA3, SCA6, SCA7, SCA17, and dentatorulral-pallidoluysian (DRPLA) were detected with the polymerase chain reaction (PCR), denaturing polyacrylamide gel and DNA sequencing techniques in 139 autosomal dominant SCA families and 61 sporadic SCA patients.
RESULTS:
Of the 139 families, 11 (7.9%) were positive for SCA1, 9(6.5%) were positive for SCA2, 71 (51.1%) were positive for SCA3, 4 (2.9%) were positive for SCA6, 2 (1.4%) were positive for SCA7, and none was positive for SCA17 and DRPLA. There was 1 SCA2 patient, 3 SCA3 patients, 1 SCA6 patient in the 61 sporadic SCA patients.
CONCLUSION
The frequency of SCA3 is substantially higher than that of SCA1 and SCA2 in the autosomal dominant SCA patients in the Han nationality of Hunan province. SCA6 and SCA7 are rare subtypes.

9.Infection of Epstein-Barr virus in Colorectal Cancer in Chinese

BACKGROUND & OBJECTIVE: Except for the tight correlation to nasopharyngeal carcinoma, accumulating evidences show that EpsteinBarr virus (EBV) is correlated to other carcinomas. This study was to investigate the correlation of EBV to colorectal carcinoma in Chinese.METHODS: EBV DNA was detected by polymerase chain reaction (PCR) in 90 specimens of primary colorectal carcinoma and 25 specimens of corresponding adjacent non-cancerous tissue, with the primers covering 2 different regions of EBV genome, BamH I W fragment and latent membrane protein 1 (LMP1) exon 3. The expression of EBV nuclear antigen 1 (EBNA1) and LMP1 were determined by immunohistochemistry, and the expression of EBV-encoded RNAs (EBERs) was detected by in situ hybridization.RESULTS: EBV LMP1 exon 3 and W fragment were detected in 27.7% and 32.2% of the 90 colorectal carcinoma specimens, which were significantly higher than the positive rate of EBV gene in the 25 adjacent non-cancerous tissues (4.0%, P<0.001). Of the 29 W fragment-positive tumors, 23 (79.3%) were EBNA1-positive, 1 (3.4%) was EBERs-positive; most EBNA1-positive cells were tumor cells with positive signals gathered in the nuclei. No expression of EBNA1 and EBERs were detected in the 8 W fragmentnegative tumors. No expression of LMP1 was detected in all tumor specimens. CONCLUSION: EBV infection might be associated with colorectal carcinoma in Chinese.
10.Early microsurgery for ruptured cerebral anterior circulating aneurysm

Objective To evaluate the clinical effect of early microsurgery for ruptured cerebral anterior circulating aneurysm. Methods Diagnosis and treatnent processes of 51 patients were retrospectively analyzed: these patients were presented with anterior circulating aneurysm induced acute spontaneous subarachnoid hemorrhage and they underwent early (within 3 d) microsurgical clipping in our hospital between January 2007 and June 2009. Glasgow outcome scale (GOS) was conducted to evaluate the outcomes. Results Of the 51 patients with anterior circulating aneurysm, 47 intracranial aneurysm lesions were clipped successfully, and 4 were wrapped. Good outcome was achieved in 39 patients, mild disability (being able to look after themselves in daily life) in 4, and severe disability in 3;persistent vegetative state appeared in 3 and 3 died after the surgery. Conclusion Early microsurgery for ruptured cerebral anterior circulating aneurysm is considered as the feasible opinion, by decreasing the recurrence of hemorrhage, disabled rate and mortality.
