Objective:To investigate the diagnostic value of spiral CT head and neck angiography on carotid artery stenosis in patients with acute cerebral infarction.Methods:A total of 60 patients with acute cerebral infarction(ACI),who were treated in Affiliated Hospital of Yan'an University from January 2014 to January 2016,were selected as observation group,and 60 patients with non-acute cerebral infarction,as control group.The two groups of patients underwent spiral CT head and neck angiography.The degrees of carotid artery stenosis and the distribution of vascular plaques were compared between the two groups.The diagnostic value of spiral CT head and neck angiography in the diagnosis of carotid artery stenosis of the patients with ACI was evaluated.Results:The detection rates of mild and moderate stenosis in the observation group were significantly higher than those in the control group (P<0.05),while the detection rates of severe stenosis and occlusion had no significant difference compared with the control group (P>0.05).The total detection rate of stenosis in the observation group was higher than that in the control group (P<0.05).There were plaques detected in the carotid bifurcation,common carotid artery,internal carotid artery and external carotid artery in the two groups.The total detection rate (69.4%) of the observation group was significantly higher than that (41.2%) of the control group (P<0.05).Conclusion:Application of 128-slice spiral CT can effectively assess the head and neck angiography on carotid artery stenosis in patients with acute cerebral infarction.This method has a higher detection rate from mild to moderate stenosis and bilateral carotid artery plaques in the patients with ACI,and it has application value in the clinical diagnosis and prognosis.
3.Effects of early rehabilitation on upper extremity function of patients with shoulder-hand syndrome after stroke

ObjectiveTo study the effects of early rehabilitation on symptom, range of motion (ROM) and motor function of upper extremity of patients with shoulder-hand syndrome after stroke.Methods47 patients were divided into two groups,early rehabilitation group and non-early rehabilitation group.Two groups were treated with comprehensive rehabilitation. The passive ROM, the pain and motor function of affected limb was assessed by Fugl-Meyer Assessment.The evalution was done in pre-treatment and post-treatment respectively. Results1.The passive ROM decreased and the pain of affected limb of the early rehabilitation group were less than the non-early rehabilitation gronp when the shoulder-hand syndrome occour.2.After comprehensive rehabilitation treatment,their clinical symptom ROM and motor function of upper extremity impoved significantly in patients with shoulder-hand syndrome after stroke( P<0.05).The early rehabilitation treatment has better effects. Conclusions The early rehabilitation treatment could decrease the degree of their passive ROM decrease and the pain of affected limb in patients with shoulder-hand syndrome after stroke,and the patients who accepted earlyrehabilitation seemed easly to improve by comprehensive treatment.
4.Distribution of HIV subtypes in HIV/AIDS patients in Guizhou from 2013 to 2017

Objective To investigate the molecular characteristics of human immunodeficiency virus/acquired immune deficiency syndrome (HIV/AIDS) patients receiving antiretroviral therapy (ART) in Guizhou Province. Methods Using a convenience sampling strategy, 8 583 samples were collected in Guizhou and an investigation was conducted including face-to-face questionnaire interview and HIV testing. Results 1 511 cases failed in HIV suppression (viral load, VL>1 000 copies/ml). 1 410 cases (93.31%) were successfully genotyped with HIV pol gene, among which 51.42% were genotyped as CRF01_AE, 26.67% as CRF07_BC and 16.1% as CRF08_BC. Conclusion The subtype changes caused by HIV gene mutation should precede the changes of main transmission routes of HIV through the analysis in recent years. Timely monitoring the changes of HIV subtypes can be one of the main bases for the prevention and control of AIDS.
5.The diagnostic and prognostic value of CT scans in patients with acute pancreatitis complications

Objective To analyze the diagnosis and prognosis value of abdominal CT scans in patients with acute pancreatitis complications.Methods 151 cases with acute pancreatitis were selected.The relationship between abdominal CT performances and the common complications and death in patients was analyzed.The prognosis of patients with different Balthazar CT severity index (CTSI) grade was compared.Results The complication rate of patients with CT signs of fatty liver, pleural effusion, liver gap effusion, adrenal gland involvement (AGI), penirenal space involvement (PSI) and gastric bare area involvement (GBAI) was significantly higher than that of patients with negative CT findings above (P<0.05).With CTSI grading increasing, the patient''s fasting time, heating time, hospital stay, recovery time of blood amylase were extended, and the incidence of pseudo cyst, transit surgery, organ failure and death was gradually increasing (P<0.05).Conclusion Severe fatty liver, AGI, GBAI, PSI and liver gap effusion are risk factors for acute complications and death in patients with pancreatitis.
7.Evaluation of therapy effect of 131Ⅰ treatment of nasopharyngeal carcinoma xenograft mediated by hNIS gene transfection in vivo using diffusion weighted MR

ObjectiveTo investigate the imaging characteristic of DW1,tumor cell apoptosis and proliferation of nasopharyngeal carcinoma (NPC) xenograft transfected hNIS gene after 131Ⅰ treatment.MethodsThe NPC xenograft models were established with CNE-2-hNIS cells(experimental group) and CNE-2 cells(control group) respectively.After i.p.injections 131Ⅰ in the experimental group and control group,the changes of xenograft tumor volume and ADC value were observed in 3,6,12,18,24 days by MR scan.The correlations of changes of ADC with pathological TUNEL,Caspase-3 immunohistochemistry of apoptosis,and Ki-67 proliferation detection were further investigated.Independent samples t-test were compared between the two groups and Pearson linear correlation analysis was used.ResultsThe tumor volume of the experimental group was significantly reduced compared with that of the control group after 131 Ⅰ treatment (t values:8.27-19.46,P <0.05 ).DW-MRI showed that in the 3th day after 131Ⅰ treatment,ADC values increased in the experimental group,and ADC values reached the peak in the 6th and 12th day after 131Ⅰ treatment,then ADC values were decreased. ADC values after treatment was positively correlated with apoptotic index ( r =0.72,P < 0.05 ) and caspase-3 positive rate ( r =0.65,P < 0.05) and there was a negative correlation with Ki-67 proliferation index ( r =- 0.71,P < 0.05 ).ConclusionADC values can reflect growth inhibition of NPC xenograft transfected hNIS gene after 131 I treatment.It is possible that DWMRI techniques can be used in early non-invasively monitor the therapy effect of 131Ⅰ treatment of NPC xenografts transfected hNIS gene.
8.Management of partial bone defect by a bone transport method using unilateral external fixator

Objective To evaluate the effect of a bone transport method using unilateral external fixator in treatment of partial bone defect.Methods Three patients with partial bone defect were reviewed,including 2 males and 1 female,and whose ages were 50,50,and 24 years,respectively.The defects were at medial part of the left proximal tibia in 2 cases.In the first case,the defect was 5 cm in length,1/3-2/3 of transverse diameter in width,with a 5 cm×3 cm skin loss.In the second case,the defect was 6 cm in length and 3 cm in width.For the remaining patient,the bone defect was located in lateral part of the right femur,which was 13 cm in length,1/3-2/3 of transverse diameter in width,with a 15 cm×7 cm scar on it.After debridement of the wound,the Orthofix's limb reconstruction external fixation system was mounted medially,initially with 2-3 HA coated screws in the middle clamp to anchor the near cortex of the segment to be transferred.A partial corticotomy was performed with multiple drill hole technique.The gradual segment transport was started 2 weeks after the operation at a rate of 1 mm/d,4 times/d.Results The follow-up time was 14,28 and 24 months,respectively.The external fixator was removed 8 and 6 months after the osteotomy in 2 patients,when radiographs demonstrated bony union of the docking site as well as mature consolidation of the generated callus.The range of motion of hip,knee,ankle on the injured side was similar to the uninjured side.The segment was unable to be transported successfully in the femur,because the compressiondistraction device was misused.Two months after the first operation,the osteotomy was performed at the same site for the second time.The frame was removed at 10 months after the second osteotomy when the new bone formed well and the fracture healed at the docking site.The patient was able to stand independently and walk with a stick at 17 months after the osteotomy.At the latest follow-up,there was no sign of osteomyelitis.Conclusion Bone transport method using unilateral external fixator is a practical option to treat partial bone defect.In addition to shorten the period with fixator,it can avoid malunion and donor injury.
9.Measurement and comparison of the digital anatomy of the tibia proximal part for the Han and Mongolian nationality

BACKGROUND:In the process of designing knee joint prosthesis and operating total knee arthroplasty for the Chinese people, we should not only take into account the proximal tibial anatomical characteristics and the difference of geometry, but also should pay attention to the differences between different races. OBJECTIVE:To compare the anatomical morphological differences between Mongolian and Han nationality through measuring the CT tomography scanning and three-dimensional reconstruction measurement of tibia proximal part of Han and Mongolian nationality, so as to provide the data references for prosthesis selection used for total knee arthroplasty. METHODS:Total y 60 patients who received the treatment at the Department of Joint Surgery were col ected, and divided into Han nationality and Mongolian nationality groups (n=30/group, 15 males and 15 females in each group). The age was (36.00+7.22) years old. 16-row helical CT scan (American GE Lightspeed 16) was used for spiral scanning, slice thickness 0.625 mm. Scanning images were exported in DICOM format and saved. Digital three-dimensional reconstruction measurement was conducted using Mimics 15.0 three-dimensional reconstruction software. The tibial plateau width, anteroposterior diameter of the medial tibial plateau and lateral anteroposterior diameter were measured respectively to observe whether there were any differences among sex, sides and nationality. RESULTS AND CONCLUSION:There were no significant differences in the above indicators between left and right sides (P>0.05). There were significant differences in tibial plateau width, anteroposterior diameter of the medial tibial plateau and lateral anteroposterior diameter between males and females, and Han and Mongolian nationality groups (P<0.05). Specific performed in:(1) There was significant difference in the morphological measurement parameters of proximal tibia between sex for Han and Mongolian nationality, and the mean value of male was larger compared with that of female. (2) There was certain significant difference in the part of the parameter indicators between Han and Mongolian nationality groups. These results suggest that the prosthesis should be chose and placed correctly according to the differences of morphological characteristics, gender, nationality, region of the Chinese people. (3) Digital three-dimensional reconstruction technology and individualized design can choose suitable prosthesis for different people, so as to ensure a good repair effect in patients after total knee replacement.

Objective To explore if continuous administration of adenosine by peripheral pathway can attenuate myocardial hypertrophy and pulmonary arterial hypertension(PAH)caused by chronic hypoxia and analyze the dose-effect relationships between them.Methods Forty-eight Sprague-Dawley(SD)rats were randomly divided into 8 groups:the hypoxia group(n=6),the hypoxia ade-nosine-treated groups [n=18,adenosine was administrated with different doses 50,100,150 ?g/(kg?min),the hypoxia adenosine-treated group A,B,C],the control group(n=6),the control adenosine-treated control groups [n=18,adenosine was administrated with different doses 50,100,150 ?g/(kg?min),the control adenosine-treated control group A,B,C].On the 21st day of the experiment,the Medlab-U/4CS was used to determined the right ventricular systolic pressure(RVSP)and the mean pulmonary arterial pressure(mPAP)of each rat.The ratio of the weight of right ventricle/left ventricle and septum[RV/(LV+S)] and the ratio of the weight of right ventricle/body weight(RV/BW)were also calculated.The morphological changes in myocardium cells and pulmonary vascular structure were observed.SAS 8.0 software was used to analyze the data.Results Twenty-one days after hypoxia,RVSP,mPAP,RV/(LV+S),RV/BW in hypoxia groups were higher significantly than those in control group and hypoxia adenosine-treated groups(Pa
