1.Role of ultrasonic three -dimensional speckle tracking imaging in assessment left ventricular systolic function in patients with uremia before and after hemodialysis

Objective To assess the left ventricular global systolic function changes using ultrasonic three -dimensional speckle tracking imaging (3 D -STI )in patients with uremia undergoing high -flux hemodialysis (HFHD)treatment.Methods 86 patients with uremia and 59 healthy volunteers were selected.Participants of control group underwent plasma brain natrinuretic peptide(BNP)examinations before 3D -STI examinations.All the uremia patients received HFHD therapy,and underwent BNP and 3D -STI examinations before and after 4 weeks HFHD therapy.Parameters of left ventricular three dimensional global area strain(GAS),global circumferential strain (GCS),global longitudinal strain(GLS)and global radial strain(GRS)were measured in each group.The correlation between each 3D -STI parameters and BNP level were analyzed.Results Compared with the control group,GAS, GCS,GLS and GRS of left ventricular were significantly lower(t =7.564,6.866,7.946,5.477,all P <0.05)and BNP level in the control group were significantly higher in the uremia group[(60.75 ±17.22)ng/L vs.(792.68 ± 184.56)ng/L,t =30.876,P <0.05].After HFHD therapy 4 weeks,GAS,GCS,GLS and GRS of left ventricular were significantly higher(t =4.233,5.933,6.546,2.959,all P <0.05)and BNP level were lower(t =27.533,P <0.05) than before treatment in the uremia group.GAS,GCS,GLS and GRS of left ventricular were negatively correlated with BNP level(r =-0.51,0.64,-0.46,-0.43,all P <0.05).Conclusion 3D -STI can accurately evaluate the changes of left ventricular global systolic function in patients with uremia before and after hemodialysis,and provide reliable basis for clinical treatment and efficacy assessment.
2.Effect of blood pressure on perihematomal edema following intracerebral hemorrhage in spontaneously hypertensive rats

Objective To investigate the changes of hematoma and perihematomal edema in spontaneous hypertensive rats (SHRs),as well as the correlation between blood pressures and perihematomal edemaMethods A total of 24 6-month-old male SHRs were included.They were randomly divided into intracerebral hemorrhage 1-,3-,5-,and 7-day groups (n =6 in each group).Blood pressure was determined by tail-cuff sphygmomanometry.Collagenase Ⅳ was injected into caudate nucleus in order to induce a model of intracerebral hemorrhage.Magnetic resonance T2 weighted imaging was used to observe hematoma and perihematomal edema.Results On day 1 after modeling,the blood pressure was decreased significantly compared to that before modeling,and it was elevated gradually on day 3.Parallel profile test showed that the overall profile was parallel between the change rate of blood pressure and the change rate of perihematomal edema volume (F =2.820,P =0.063).The coincident profile test showed that both the overall profiles did not coincide (F =10.961,P =0.000).The change rate of systolic blood pressure was significantly positively correlated with the change rate of perihematomal edema volume (r =0.527,P =0.024) and the change rate of perihematomal edema volume (r =0.755,P =0.000) showed a significant positive correlation.Conclusions Brain edema appeared and expanded rapidly on the day of intracerebral hemorrhage in SHRs.It reached the peak on day 3,and then dissipated gradually.The blood pressure decreased significantly on day 1 after intracerebral hemorrhage,and it began to increase from day 3 till day 7.The changing trends of the blood pressure and perihematomal edema volume were parallel to each other,and the change rate of systolic blood pressure and the change rate of perihematomal edema volume showed a positive correlation.
3.Management of postt aumatic flexion contracture in knee joint using Ilizarov technique

ObjectiveTo evaluate the effectiveness of correction of posttraumatic flexion contracture of knee(FCK) using Ilizarov technique.MethodsSix male patients with posttraumatic FCK,aging from 9 to 43 years(mean,24.5 years),treated by using Ilizarov technique from January 2006 to December 2010,were retrospectively analyzed.The preoperative flexion deformity of knee ranged from 35° to 85° (mean,47.6°),and the range of motion ranged from 0° to 70° (mean,15.8°).Among them,five patients had old fracture of knee joint combined with equinus foot deformity (deformity ranging from 25° to 37°,with an average of 31.8°); one patient had femoral supracondylar fracture.We used circular external fixator to correct the FCK and equinus foot deformity gradually,the knee arthrodesis operation was performed in 4 patients because of the damaged hone structure and poor soft tissue condition.ResultsAll patients were followed up for 12-22 months(mean,18 months).The flexion of knee joint was recoverd from preoperative 47.67°±18.63° to postoperative 9.33°±3.50°.Plantar flexion of ankle joint in five equinus foot deformity was recovered from preoperative 31.80°±4.65° to postoperative 3.00°±4.47°.The kuee joint was arthrodesised successfully in 4 patients and the other 2 patients got 30° and 75° ROM.All the patients can walk with a cane,and satisfied with the result of treatment.Pin-site infection was found in 4 patients and was controlled within two weeks.ConclusionIlizarov technique is an effective treatment method for posttraumatic FCK.Knee arthrodesis should be performed when patients had damaged bone structure and poor soft tissue condition.
4.Feasibility and safety of fetal intravascular transfusion via the intrahepatic vein in the treatment of fetal anemia

Objective:To investigate the feasibility and safety of fetal intravascular transfusion via the intrahepatic vein in the treatment of fetal anemia.Methods:This was a retrospective analysis of all fetuses requiring intrauterine transfusion (IUT) in the Shanghai First Maternity and Infant Hospital between January 2010 and December 2019. According to the different ways of IUT, they were divided into intrahepatic venous transfusion group and umbilical venous transfusion group, fetal outcomes and the incidence of procedure-related complications between the two groups were compared.Results:A total of 97 IUTs were performed on 48 fetuses. Among them, 16 cases were performed in the intrahepatic vein (31 transfusions), 32 cases were performed in the cord of the umbilical vein (66 transfusions).There were no significant differences between the two groups in age, labor history and the proportion of fetal hydrops before the first transfusion. In the intrahepatic venous transfusion group, the posterior placenta was 14/16, which was significantly higher than 78% (25/32) in the umbilical venous transfusion group ( P<0.01). The live-birth rates of the two groups were 13/16 and 75% (24/32). There was no significant difference between the two groups ( P>0.05). Before intrahepatic venous transfusion, the proportion of fetal hydrops was significantly higher than that of umbilical venous transfusion [55% (17/31) vs 24% (16/66), P<0.05]. Puncture success rate of intrahepatic venous transfusion and umbilical venous transfusion were both 100%. In the umbilical venous transfasion group, the incidence of needle slippage (5%, 3/66) and the abnormality of fetal heart rate (11%, 7/66) were higher than those in the intrahepatic venous transfasion group [0 and 3% (1/31)], but there were no significant differences between the two groups (all P>0.05). There were no cases of fetal loss within 24 hours, premature rupture of membranes, infection within 7 days and emergency cesarean section after IUT in both groups. Conclusions:Fetal intravascular transfusion via the intrahepatic vein is safe and feasible in the treatment of fetal anemia. But the requirements of puncture technique are relatively high, so it is recommended to be carried out in experienced fetal treatment center.
5.Correlation between serum MMP-2 level and severity of coronary artery disease in aged ACS patients with hyperhomocysteinemia

Objective:To explore the correlation between serum level of matrix metalloproteinase 2 (MMP-2)and se-verity of coronary artery disease in aged patients with acute coronary syndrome (ACS)complicated hyperhomocys-teinemia (HHCY).Methods:According to serum level of homocysteine (Hcy),a total of 105 aged ACS patients were selected and divided into ACS+HHCY group (n=56)and pure ACS patients (n=49),another 65 healthy in-dividuals were enrolled as healthy control group.The severity of coronary artery disease was showed by Gensini score ;enzyme linked immunosorbent assay was used to measure serum MMP-2 level in each group.Results:Com-pared with healthy control group,there were significant rise in MMP-2 level [(140.8±50.1)ng/ml vs.(442.5± 98.2)ng/ml,(297.9±86.3)ng/ml]in ACS+HHCY group and pure ACS group,P <0.01. Gensini score in ACS+HHCY group was significantly higher than that of pure ACS group [(1.9±0.2)scores vs.(1.1±0.3)scores,P<0.01].Pearson correlation analysis indicated that Gensini score was positively correlated with MMP-2 level in ACS+HHCY group (r =0.424,P <0.05).Conclusion:The serum MMP-2 level is significantly positively correla-ted with severity of coronary artery disease in aged patients with ACS complicated HHCY.

7.The Analgesia of Oxymatrine Affecting Calcium Channel and GABA Release.

OBJECTIVETo explore the analgesia of oxymatrine (OMT) affecting high voltage-dependent calcium channels (HVDCCs) and GABA release under neuropathic pain condition.
METHODSTotally 66 C57BL/6 mice were randomly divided into the sham-operation group, the model group, and the OMT group, 22 in each group. Neuropathic pain models were established by partial sciatic nerve ligation (PSNL). Hind paw plantar mechanical response threshold (MWT) was measured by up-and-down method with Von-Frey filament. mRNA expression of HVDCCs in brains and spinal cords was detected with Real-time PCR and concentration of GABA was determined using ELISA kit.
RESULTSCompared with day 0, the left hind paw MWTwas decreased on day 7, 10, and 14 in the model group (P < 0.05). Compared with the sham-operation group, the left hind paw MWT was significantly reduced in the model group on day 7 (P < 0.05). The MWT of PSNL ipsilateral hind paw was decreased on day 7 before OMT administration, when compared with day 0 (P < 0.05), and increased after OMT administration (P < 0.05). Compared with the sham-operation group, mRNA levels of Cav1.2, Cav1.3, Cav2.1, and Cav2.3 in brain tissues were increased and those of Cav2.2 were decreased significantly in the model group (P < 0.05). In spinal cord tissues, mRNA levels of Cav1.2 and Cav1.3 were increased, but those of Cav2.1, Cav2.2, and Cav2. 3 were decreased significantly in the model group, when compared with those of the sham-operation group (P < 0.05). Compared with the model group, mRNA levels of Cavl.2, Cavl.3, Cav2.1, and Cav2. 3 in brain tissues were decreased, and those of Cav2.2 were increased significantly in the OMT group (P < 0.05). In spinal cord tissues of the OMT group, mRNA levels of Cav1.3 decreased and those of Cav2.1, Cav2.2, and Cav2.3 increased significantly with statistical difference, when compared with those of the model group (P < 0.05). Compared with the sham-operation group, GABA levels in brain tissues decreased in the model group (P < 0.05). Compared with the model group, GABA levels in brain tissues increased in the OMT group (P < 0.05). There was no statistical difference in GABA levels of spinal cord tissues among these groups (P > 0.05).
CONCLUSIONSOMT had analgesic effect on neuropathic pain, which might be probably related to HVDDCs. Cav2.2 might directly affect GABA release.

8.Effects of Ca2+ on photosynthetic parameters of Pinellia ternata and accumulations of active components in heat stress.

OBJECTIVETo study the effect of exogenous Ca2+ on photosynthetic parameters of Pinellia ternate and accumulations of active components under high temperature stress.
METHODThe pigment contents of P. ternata leaves, photosynthesis parameters and chlorophyll fluorescence parameters of P. ternata leaves, the contents of guanosine, adenosine and polysaccharide in P. ternata tubers were measured based on different concentrations of exogenous Ca2+ in heat stress when the plant height of P. ternata was around 10 cm.
RESULTThe contents of total chlorophyll and ratio of chlorophyll a/b were relatively higher by spaying Ca2+. Compared with the control, spaying 6 mmol x L(-1) Ca2+ significantly enhanced the net photosynthetic rate (Pn), transpiration (Tr) and stomatal limitation (L8), but reduced intercellular CO2 concentration (C) in P. ternata leaves. With the increase of Ca2+ concentration, maximal PS II efficiency (Fv/Fm), actual photosynthetic efficiency (Yield) and photochemical quenching coefficient (qP) initially increased and then decreased, however, minimal fluorescence (Fo) and non-photochemical quenching coefficient (NPQ) went down first and then went up. The contents of guanosine and polysaccharide and dry weight of P. ternata tubers showed a tendency of increase after decrease, and the content of adenosine increased with the increase of Ca2+ concentration. The content of guanosine and polysaccharide in P. ternata tubers and its dry weight reached maximum when spaying 6 mmol x L(-1) Ca2+.
CONCLUSIONWith the treatment of calcium ion, the inhibition of photosynthesis and the damage of PS II system were relieved in heat stress, which increased the production of P. ternata tubers.

9.Liver histological changes in chronic hepatitis B patients with alanine aminotransferase lower than two times the upper limit of normal

Objective To assess the liver histopathological characteristics of chronic hepatitis B (CHB) patients with alanine aminotransferase(ALT)lower than 2 times the upper limit of normal (ULN) through liver biopsy, and try to provide subjective evidence for clinical anti-viral treatment.Methods From October 2005 to August 2010, patients accepted liver biopsy in department of infectious disease, Sichuan provincial people's hospital were enrolled. The criteria for liver biopsy was as follow, (1) HBsAg-positive for more than 6 months, (2) HBeAg-positive patients with HBV DNA ≥103 copies/ml or HBeAg-negative patients with HBV DNA≥ 104copies/ml, (3) ALT was lower than 2 times ULN for more than 6 months,and without any hepatic protectants, (4) never accepted any antiviral treatment before, including IFN or nucleoside analogues, (5) willing to accept liver biopsy. Before liver biopsy, routine blood test, prothrombin time, liver function test, hepatitis B antigen and antibody test, HBV DNA quantification were examined. The biopsy position was located under routine ultrasound, liver biopsy were performed to assess the grading of inflammation and necrosis and the degree of fibrosis. The correlation between all the factors and liver inflammation and fibrosis were analyzed. Results Totally 383 cases (240 males and 143 females) met the diagnostic criteria, aged from 16 to 59 years old and the mean age was 28.0 years old. Cases of liver inflammation in G0, G1, G2, G3andG4 grade was 2 cases (0.5%), 165 cases (43.1%), 191 cases (49.9%), 25 cases (6.5 % ) and 0 cases (0 % ) respectively, cases≥G2 grade accounted 56.4 % of total. Meanwhile,stage of fibrosis in S0, S1, S2, S3 and S4 was 103 cases (26.9%), 265 cases (69. 2%), 13 cases (3.4%), 2 cases (0.5%) and 0 cases (0%) respectively, percentage of liver fibrosis in S2stage and over was only 3.9%. The occurrence of serious liver inflammation was associated with age, ALT levels, HBV DNA levels and HBeAg status (P<0.05). There was no obvious association between HBV DNA level and liver fibrosis (P>0.05). Conclusions There were obvious liver inflammation and different degree of liver fibrosis in CHB patients with alanine aminotransferase(ALT)lower than 2 times ULN. The degree of liver injury assessed by liver biopsys is recommended as an evaluation for the necessary of anti-viral therapy.

Aim The effect of tetrandrine on TGF-?1 mRNA expression in glomerulosclerosis rat was observed. Methods The rats were randomly divided into four groups, such as the normal control group (sham operative rat), glomerulosclerosis model group,tetrandrine group and amlodipine group. The expression of TGF-?1 mRNA was analyzed by Northern blot hybridization. Results The expressions of TGF-?1 mRNA in two treating groups were much lower than untreated model group. There were no difference between these two treating groups. Conclusion Tetrandrine can decrease the expression of TGF-?1 mRNA in glomerulosclerosis rat induced by unilateral renctomy plus adriamycin.
