Objective To investigate the cause and countermeasures of frequent shocks in patients with implantable cardioverter defibrillators (ICD). Methods Eighty ICD patients with heart failure and malignant ventricular arrhythmias were followed up, including sixty-two male and eighteen female patients. There were 35 patients with single-chamber ICD, 23 with dual-chamber ICD and 22 with three-chamber ICD. Patients in this study were followed up for 1-6 years to analyze the reasons for ICD discharge. According to the specific circumstances, patients were treated. Results Twenty-three pa-tients in 80 patients suffered from shock treatment. Ten patients (12.5%) experienced frequent shocks. The causes of fre-quent shock included repeated episodes of ventricular tachycardia, invalid shock due to increased defibrillation threshold (DF) and false identification of the frequent episodes of paroxysmal ventricular tachycardia or arrhythmias. The management included the identification process adjustment of ventricular tachycardia and supraventricular tachycardia, increased num-bers of beats of ventricular tachycardia judgment and increase the basic pacing rate. The anti-arrhythmic drugs should be combinedly used, especially metoprolol and amiodarone. The ICD shock was significantly reduced after parameter optimiza-tion and anti-arrhythmic therapy. Conclusion The ICD shocks were effectively reduced with rational use of anti-arrhyth-mic drugs and valid ICD programming.

Objective To study the effect of various methods determining lung volume and fraction dose on the lung dose-volume parameters for lung cancer patients. Methods Twenty patients with lung cancer were rantdomly enrolled into our study and the plan of three dimensional eonformal radiation therapy(3DCRT)was designed by Varian Eclipse TPS.The lung volumes and the dose-volume parameters were measured under CT value of-300- -980, -400- -980 and -500- -980.Under CT value of -400- -980,total lung volumes were confirmed.The dose-volume parameters of V30,V20,V10 and mean lung dose(MLD)were reevaluated after GTV,CTV and PTV were subtracted from the total lung volumes and when the fraction dose was elevated from 2.0 Gy to 10.0 Gy. Results When the CT value ranged from-300--980 to-500--980,the median reduction of the total lung volumes(-9.10%)was significantly higher than that of V30,V20,V10 and MLD(-3.18%,-1.13%,0.82%and-0.79%,respectively).When the total lung volume was fixed at CT value of-400--980,the alterations of V30,V20,V10 and MLD became more apparent as the increase of the subtracted lung volume,among which the alteration of V30 was most significant while V10 the least.Among five cases with a fixed total dose of 60 Gy and PTV less than 140 cm3,the V30,V20,V10 and MLD were increased to a similar extent(about 40%)when the fraction dose was increased from 2.0 Gy to 10.0 Gy.MLD was increased(36%)when the fraction dose was above 6.0 Gy. Conclusions When CT value ranges from-300- -980 to-500- -980,the total lung volume is influenced most.The alteration of V30,being statistically significant,might have some significance but is not enough to determine the plan of radiotherapy clinically.The alteration of V20、V10 and MLD is not statistically significant.When the overlapped target volume is subtracted from the total lung volumes,the alteration of V30 is the most sign:tifhcant while V10 the least.The fraction dose,being the most consuming factor(＞10%)when comparing with the CT valHe and targeted volume,can significantly influence the dose-volume parameter.

Seeds of Bupleurum chinense cultivar, Zhongchai No. 1, were sowed in plastic pots which used the arable layer soil as the nursery bed and putted in the artificial climate incubator at various temperatures (15, 20, 25, 15-25 degrees C) and light (8,12 h) to germinate, respectively. The lower constant temperature (15 degrees ) and the higher constant temperature (25 "C) were not conducive to the sprouting characteristics of B. chinese. While they were able to enhance root activity to some extent; The seeding growth of B. chinese was significantly better in the variable temperature than correspondence in the constant temperature, significantly. The emergence speed, emergence index, vigor index and root activity of Bupleurum were improved under the 12 h of light-time, but the germination rate was not improved. The sprouting of Bupleurum's seeds could be improved to some extent by soaking with hormone, such as gibberellin, cytokinin, salicylic acid. Gibberellin promoted seeds' sprouting and seedings's root activity of Bupleurum, while salicylic acid increased the root activity of seeding. There is a significant influence of light, temperatures and hormone treatment on the germination of Zhongchai No. 1 seeds, and all three are remarkably interacted; It is beneficial to promote seed germination by the temperature (20 + 5) degrees C, lighting (8 h) and gibberellin concentration (10 x 10(-6)).

Objective To learn the temporal-spatial distribution and clustering of hand-foot-mouth disease (HFMD)in Xiaoshan in 2014,and to provide reference and basis for prevention and control.Methods The HFMD data of Xiaoshan in 2014 was derived from the China Information System for Diseases Control and Prevention.The vector map was created by Map Info 10.0 on the background of 1∶65 000 zoning map ,extracted village and community geographical position information from Baidu map.At the village level,the spatial autocorrelation analysis and spatial scanning analysis were made using software ArcGIS10.2 and SaTScan9.2.Results In Xiaoshan,the epidemic curve of HFMD in 2014 showed two peaks,during April to July and during September to October.The global spatial autocorrelation Moran's I index was 0.442 7(P<0.001),and the global Getis-Ord G value was 0.003 3,(E(G)=0.002 1,Z(G)=11.82,P<0.001). Local autocorrelation analysis showed that the cluster state was high-high.Ningwei Street had the most hot spots.Five statistically significant HFMD clusters were identified by space-time scan statistics,the most likely cluster was located in Heshang Town,from January 6 to February 4 (RR=23.00,LLR=17.45,P<0.05).Conclusion In Xiaoshan,the major epidemic peak of HFMD in 2014 was from April to July.A positive spatial correlations was found,and the disease showed a distinct regional distribution feature and temporal-spatial clustering.The clusters were observed including the villages and communities of rural-urban continuum and in vicinity of industrial development zone in countryside.

Objective To evaluate the effect of calcipotriol on the proliferation of and cytokine secretion by melanocytes and perilesional CD8+ cytotoxic T lymphocytes (CTLs) from patients with vitiligo.Methods Melanocytes isolated from abdominal skin and CD8+ CTLs from perilesional skin of patients with vitiligo were subjected to successive culture in vitro.After several passages,the melanocytes and CD8+ CTLs were cultured alone or in combination with or without the presence of various concentrations of calcipotriol for 24 to 48 hours.MTS (3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfopheny)-2H-tetrazolium,inner salt) method was used to evaluate the proliferative activity of cells,enzyme-linked immunosorbent assay to determine the levels of interleukin (IL)-6,tumor necrosis factor (TNF)-α and interferon (IFN)-γ in the culture supematant of cells,flow cytometry to detect cell apoptosis.Some co-cultured melanocytes and CTLs were treated with calcipotriol of 10-8 mol/L and anti-IL-6 antibody of various concentrations (0,1,2,2.5,5,10 mg/L) for two days followed by enumeration of cells.The concentrations of 108 and 10-9 mol/L (calcipotriol) were chosen for relevant tests.Results There was a marked apoptosis in MCs after coculture with CD8+ CTLs.The 24-hour treatment with calcipotriol of 104 and 10-9 mol/L had no obvious effect on the proliferation of melanocytes cultured alone (both P ＞ 0.05),but accelerated the proliferation of melanocytes cocultured with CTLs (both P ＜0.05) as well as that of CD8+ CTLs cultured alone or in combination with melanocytes (all P ＜0.05).A statistical decrease was observed in IL-6,TNF-α and IFN-γlevels in the supernatant of cocultured melanocytes and CTLs compared with those in the supernatant of melanocytes and CTLs cultured alone,and calcipotriol of 10-9 mol/L intensified the decrease in supernatant IL-6 level (t =2.89,P ＜0.05),but no statistical changes were noted for the level of TNF-α or IFN-γin the supernatant of the coculture system after treatment with calcipotriol of 104 or 104 mol/L compared with that before treatment (both P ＞ 0.05).In the coculture system pretreated with calcipotriol of 10-8 mol/L,the number of CD8+ CTLs significantly decreased (t =3.15,P ＜0.05),whereas that of melanocytes significantly increased (t =3.53,P ＜0.05) after the treatment with anti-IL-6 antibody of 5 mg/L.Conclusions Perilesional CD8+ CTLs have a specific cytotoxic effect on melanocytes,and calcipotriol may inhibit the cytotoxic effect of CD8+ CTLs by suppressing the secretion of IL-6,which may partly explain the therapeutic mechanism of calcipotriol for vitiligo.

AIM: To investigate the influence of serum apolipoprotein (apo) CII concentrations on the distribution of serum high-density lipoprotein (HDL) subclasses. METHODS: Serum HDL subclasses in 247 subjects were determined by two dimensional gel electrophoresis-immunodetection. RESULTS: With the increase in serum apolipoprotein CII levels, age, BMI, the contents of TG, TC, apoB100, apoCII, apoCIII, apoE, preβ_1-HDL, preβ_2-HDL, HDL_3b and HDL_3a increased significantly, but the contents of HDL-C, HDL_(2a) and HDL_(2b) decreased remarkably. The contents of preβ_1-HDL increased with the rise in apoCII and apoA I levels, whereas the content of HDL_(2b) increased with the rise in serum apoA I level in the same apoC II group, but decreased with the increase in serum apolipoprotein CII level in the same apoA I group. With the increase in the ratio of apoCII/ apoCIII, the content of preβ_1-HDL elevated, but the content of HDL_(2b) decreased. The correlation analysis illustrated that the apoCII level was positively correlated with preβ_1-HDL (r=0.186, P<0.01), but inversely correlated with HDL_(2b) (r=-0.149, P<0.05). The apoA I level was positively associated with all HDL subclasses (r in the range of 0.349-0.587, P<0.01). In addition, the apoCIII level was positively correlated with preβ_1-HDL (r=0.184, P<0.01) and preβ_2-HDL (r=0.178, P<0.01), while the apoE level was positively correlated with HDL_(3a) (r=0.040, P<0.05). The apoB100 level was inversely correlated with HDL_(2a) (r=-0.102, P<0.05). CONCLUSION: The particles of HDL show a general shift towards smaller size with the increase in apoCII levels, indicating that the maturation of HDL is abnormal. Whereas the contents of apoA I level correct the effect of apoCII on the distribution profile of HDL subclasses. The ratio of apoCII/apoCIII might also been taken as one of the indexes reflecting the distribution profile of serum HDL subclasses.

Objective:To detect the expression and methylation of TIA1 in breast cancer and to study its correlation with multi-slice spiral CT signs.Methods:50 patients with breast cancer were collected from Feb.2019 to Mar. 2020. The expression levels of TIA1 in breast cancer tissues and in peritumoral tissues were estimated by quantitative real-time polymerase chain reaction. Bioinformatics software MethPrimer was used for predicting TIA1 promotor and confirmed the existence of cPG island. Methylation-specific PCR (MSP) was performed to detect TIA1 DNA promoter methylation. All patients were examined by multi-slice CT. CT images were analyzed through observing the tumor size, shape, calcification area, lymph node metastasis and margin. The correlation between CT signs and TIA1 methylation status was further analyzed.Results:The expression levels of TIA1 in breast cancer tissues were lower than in peritumoral tissues (0.50±0.12, 0.95±0.10, P=0.00) , while TIA1 DNA promoter methylation rate was higher than in peritumoral tissues (64%, 42%, χ2=4.86, P<0.05) .There were no significant differences in TIA1 DNA promoter methylation rate among patients with different tumor shape and micro calcifications. TIA1 DNA promoter methylation rate in patients with mass diameter≥2 cm were significantly higher than those in patients with mass diameter<2 cm (78.57%, 45.45%, P<0.05) , and TIA1 DNA promoter methylation rate in patients with lymph node metastasis was higher than those without lymph node metastasis (79.17%, 50%, P<0.05) . TIA1 DNA promoter methylation rate in patients with burr at edge of mass was higher than those without burr at edge of mass (80.77%, 45.83%, P<0.05) . Conclusion:There is a correlation between CT imaging signs and TIA1 DNA promoter methylation rate in patients with breast cancer, which can provide more reference for the judgment of malignant degree and prognosis of patients with breast cancer.

OBJECTIVE: To explore an approach to distinguish avulsion fracture of articular portion from anatomic separated epiphysis. METHODS: 21 cases with suspicioned avulsion fracture of articular portion of tubular bone were reviewed. The cortical continuity of separate small bone and contiguous bone portion was investigated. The swell of soft tissue around these small bones was observed simultaneously. Then the configuration of the fracture was researched by injury mechanics. RESULTS: Continued bone cortex between separate small bone and corresponding bone was seen in 15 cases, so they were considered as a separate epiphysis. Sharp linear fracture between the separate small bone and corresponding bone were seen in 6 cases. The swell of soft tissue around the separate small bone was observed in all cases, so they were considered as a avulsion fracture. CONCLUSION The meticulous investigation of separated small bone at the articular portion by image observation is of important value for distinguishing avulsion fracture from anatomic separate epiphysis.
