2.Advances in biological predictors of radioactive esophagitis

Radioactive esophagitis is one of the most common complications in thoracic tumor radiotherapy,biological factors such as single nucleotide polymorphisms (SNPs),miRNAs,and HIV infection may play key roles in the occurrence and development of radioactive esophagitis,and they have become an active field in protection areas of radiotherapy.We can identify the patients who may cause radioactive esophageal in high dose radiotherapy as early as possible,and modify the treatment plan to protect the esophagus.Therefore the biological factors of radioactive esophagitis are of important clinical significance.
3.Diagnostic and therapeutic value of intradiscal steroid injection in patients with discogenic low back pain combined with radial fissure of annulus fibrosis

Objective To retrospectively investigate diagnostic and therapeutic value of intradiscal steroid injection in patients with discogenic low back pain combined with radial fissure of annulus fibrosis.Methods Totally 120 patients who had undergone CT guided intervertebral discography and steroid injections,including 72 males and 48 females,aged from 25 to 60 years (average,48 years),were recruited to this study.According to Dallas discography description system,7 cases were rated as grade 0,36 grade 1,48grade 2,and 29 grade 3.The Visual Analogue Scale (VAS) and Roland Morris Disability Questionnaire (RMDQ) were used to evaluate clinical effect at 2 weeks,2 months,6 months,12 months and 24 months postoperatively.Results Most of patients who underwent steroid injection in single highly suspect diseased disc claimed significant symptom improvement.There were statistically significant differences between pre- and post-operative VAS and RMDQ.The clinical effect in grade 3 group was better than those in grade 1 and grade 2 groups,and the differences were significant.Patients with severe disc disruption (grade 3) had a lower recurrence rate of low back pain compared with other patients.Conclusion Intradiseal steroid injection not only can significantly relieve discogenic low back pain,but also can benefit diagnosis of discogenic low back pain,especially in patients who fail to reproduce concordant pain on discography.For patients with severe rupture of the annulus fibrosus,the relief of discogenic low back pain is more significant.
4.Distribution and antimicrobial resistance of pathogens causing bloodstream infection in a hospital in Sanya city

Objective To understand the distribution and antimicrobial resistance of pathogens isolated from blood culture in a hospital in Sanya city.Methods Blood culture specimens and antimicrobial susceptibility testing results in this hospital from January 2013 to December 2015 were analyzed retrospectively.Results A total of 356 isolates of pathogenic bacteria were isolated from 3 195 blood culture specimens,the positive rate was 11.14%,including 215(60.39%)gram-negative bacterial strains,122(34.27%) gram positive bacterial strains,and 19(5.34%) fungi strains.The top 3 gram negative bacteria were Escherichia coli (n =90,25.28%),Klebsiella pneumoniae (n =60,16.85%),and Burkholderia pseudomallei (n =24,6.74%);the most common gram positive bacteria were Staphylococcus aureus (n =42,11.80 %),coagulase-negative staphylococcus (n =38,10.67 %),and Streptococcus spp.(n =33,9.27 %).Resistance rates of Escherichia coli and Klebsiella pneumoniae to cefoperazone/sulbactam,amikacin,and carbapenems were all lower than 10.00%;resistance rates of Burkholderia pseudomallei to most antimicrobial agents were lower than 10.00%.There were no strains of main gram-positive bacteria that were found to be resistant to linezolid and vancomycin.Conclusion Gram-negative bacteria are the main pathogens causing bloodstream infection in this hospital in recent years,especially the isolation rate of Burkholderia pseudomallei is higher,which should arouse more attention in clinic.

6.Application of the Hopkins Endoscope on Removal of Esophageal Foreign Bodies

Objective To evaluate the clinical application value of Hopkins endoscope on removal of esophageal foreign bodies. Methods The clinical data of 96 patients with esophageal foreign body were analyzed retrospectively. Then the differences of the operation time between the visual surgery guided by Hopkins endoscope and conventional surgery of removal of esophageal foreign bodies were compared. Results Improved surgical operation time is significantly shorter than the traditional operation group. In addition,the operation time of improved surgery in the denture group,≥50 years old age group,≥24 h course group and the first stenosis of esophagus group is significantly shorter than the traditional ones. Conclusion Compared with the traditional surgery,the visual surgery of removal of esophageal foreign bodies guided by Hopkins endoscope with rigid esophago?scope has more advantages,especially for complex esophageal foreign body,which can significantly short the operation time and improve the opera?tion efficiency.
7.Effect of Human Chorionic Gonadotrophin on mRNA Expression of Human PBMC MIF

Objective: To investigate the effect of human chorionic gonadotrophin (hCG) on the gene expression of migration inhibitory factor (MIF) in human peripheral blood mononuclear cell (PBMC). Methods: The healthy human PBMC was cultured with hCG at 37 ℃, 5%CO_2 for 2 hours. The mRNA of harvested cells was isolated. The MIF mRNA was detected by real-time RT-PCR. Results: In a certain range of doses, the mRNA expression of MIF significantly increased following the increase of hCG in a dose depandent manner, and it reached to a peak 1-2 hours after culture, then returned to the minimum level after 8 hours. Conclusion: In a certain range of doses, hCG can increase the mRNA expression of MIF. This effect is correlated with reacting time. It is suggested that hCG may involve in immune response by up-regulating the production of cytokines by PBMC.
8.Damage control surgery for acute superior mesenteric occlusion

Objective To investigate the outcome of damage control sugery in patients with acute superior mesenteric occlusion (ASMO).Methods Seventeen patients with acute superior mesenteric occlusion from June 2008 to May 2012 were reviewed retrospectively.Diagnosis was confirmed by CT and/or DSA.Patients were divided into two groups:seven patients in damage control group (shortening surgical time,bowel exteriorization,temporary abdominal closure) ; ten patients in conventional surgical treatment group.The treatment outcomes of the two groups were compared.The survival of all patients was followed up for 6 months to 5 years at outpatient clinic.Results The overall mortality rate was (5/17) and surgical complications rate was (8/17).Operation time was (97 ±42) min and (236 ± 137) min(t =-4.72,P <0.01,P < 0.05) respectively in damage control group and conventional surgical treatment group.Intestine resection length was (114 ± 94) cm and (229 ± 93) cm (t =-2.49,P =0.03,P < 0.05) respectively.Complications rate was (1/7) and (7/10) (Fisher's dxact test P =0.05,P ≤0.05) respectively; mortality was (1/7) and (4/10) (Fisher's dxact test P =0.34,P > 0.05) respectively.Conclusions Although damage control does not significantly reduce mortality,but the rational use of damage control can effectively reduce operation time,shorten intestine resection length and decrease surgical complications.

10.Prospective randomized comparison of single pacemaker implantation using the subclavian vein puncture versus the cephalic vein cutdown

Objective To compare the value of pacemaker implantation using the subclavian vein (SCV) puncture and thecephalic vein(CV) cutdown approach.Methods One hundred and forty-six patients were randomized into SCV group (98 patients) and CV group (48 patients).We observed the success rate,surgery duration,fluoroscopy time and complications of the two groups.Results The SCV group had significantly higher success rate,shorter duration of surgery and fluoroscopy than the CV group (success rate:100% (98/98) vs.89% (43/48),x2 =10.5,P < 0.05 ; duration of surgery:(118 ± 35) min vs.(256 ± 75)min,t =12.12,P <0.01 ; duration of fluoroscopy:(15 ±5) min vs.(35 ± 10) min,t =13.08,P <0.01).Both groups had one case who developed compilations which were pocket hematoma and wire electrode dislocation respectively.The rate of complications was not significantly different between the two groups (P > 0.05).Conclusion The SCV approach should be considered a preferable access in pacemaker implantation.
