1.Application of Fast-track Surgery in Perioperative Period of Laparoscopic Cholecystectomy

Objective-To-discuss-the-value-of-fast-track-surgery-(-FTS-)-in-perioperative-period-of-laparoscopic-cholecystectomy-.-Methods-We-selected-200-cases-of-laparoscopic-cholecystectomy-in-our-hospital-from-January-2012-to-December-2012.The-cases-were-randomly-divided-into-either-traditional-group-(control-group)-or-fast-track-surgery-group-(FTS-group),-with-100-cases-in-each-group-.The-operation-time-,-intraoperative-blood-loss-,-time-to-get-out-of-bed-after-operation-,-time-to-intake-semi-liquid-diet,-time-to-flatus,-time-to-defecation,-and-length-of-hospital-stay-were-compared-.-Results-Compared-with-the-control-group-,-the-FTS-group-had-shorter-time-to-semi-liquid-diet-[(11.3-±2.0)-h-vs.(50.2-±8.7)-h,-t=-43.976,-P=0.000],-shorter-ambulation-time-[(6.2-±1.5)-h-vs.(14.3-±1.7)-h,-t=-35.728,-P=0.000],-shorter-flatus-time-[(12.0-±4.4)-h-vs.(24.9-±5.4)-h,-t=-18.519,-P=0.000],-shorter-defecation-time-[(15.8-±5.3)-h-vs.(25.2-±4.3)-h,-t=-13.773,-P=0.000],-and-shorter-postoperative-hospital-stay-[(3.2-±1.3)-d-vs.(4.2-±2.1)-d,-t-=-4.048,-P=0.000].-Conclusion-Fast-track-surgery-is-feasible-and-safe-,being-helpful-for-patient-’-s-postoperative-recovery-.
2.Experimental study of Renshen-heche Tablets on tonifying and warming Shen-Yang

Objective: To study the effect of Resen heche tablets (RSHC) on tonyfing and warming Shen Yang. Methods: The mice model of Shen Yang deficiency was established by injecting hydrocortisone. The body weight, autonomic activity and swimming time in ice water in model mice were observed. The level of superoxide dismutase (SOD) in red cell and lipid peroxide (LPO) in plasma of aged rats were analyzed. The effect on anti stress in mice was examined. Results: RSHC had the remarkable effect to improve body weight and autonomic activity and to prolong swimming time in ice water in model mice, and to increase SOD activity and reduce LPO in blood of aged rats, and to elevate the ability of anti fatigue and anoxic tolerance in mice. Conclusion: RSHC drug has the effect on tonifying and warming Shen Yang; can promote growth, enhance organism activity, elevate the adaptation to circumstances and protect injury from free radicals.

5."Effects of asphyxia as a ""Second Hit"" on renal function in small for gestational age infants"

ObjectiveTo investigate the effects of the asphyxia as a Second Hit on renal function during early stage after birth in small for gestational age (SGA) infants.MethodsThe infants who were hospitalized within 24 hours after birth in Peking University Third Hospital between January 2013 and March 2015 were retrospectively enrolled, and divided into different groups depending on gestational age and asphyxia history. There were 40 preterm non-asphyxia SGA infants and 80 controls who were preterm non-asphyxia appropriate for gestational age (AGA) infants; 11 preterm asphyxia SGA infants and an equal number of preterm asphyxia AGA infants as controls; 33 term non-asphyxia SGA infants and 33 term non-asphyxia AGA infants as controls; and four term asphyxia SGA infants and 13 term asphyxia AGA infants as controls. Blood urea nitrogen (BUN), serum creatinine (SCr), and estimate glomerular filtration rate (eGFR) were tested within 48 h after admission and the incidence of abnormal indexes was compared between groups byt-test and Fisher exact test.Results(1) Compared with preterm non-asphyxia AGA group, BUN level significantly decreased in preterm non-asphyxia SGA group [(3.99±1.69) vs (5.11±2.08) mmol/L,t=2.948, P=0.004]. Compared with term non-asphyxia AGA group, term non-asphyxia SGA group had higher SCr level [(72.03±10.29) vs (62.58±12.27)μmol/L,t=3.390,P=0.001] and lower eGFR level [(25.19±4.07) vs (33.99±8.75) ml/(min·1.73 m2),t=5.238,P=0.000]. (2) Compared with preterm non-asphyxia AGA infants, preterm asphyxia AGA infants had higher BUN [(6.96±3.09) vs (5.11±2.08) mmol/L,t=2.602,P=0.011] and SCr [(76.45±10.11) vs (66.70±13.18)μmol/L,t=2.357,P=0.021] and lower eGFR level [(15.86±2.31) vs (19.54±5.08) ml/(min·1.73 m2),t=2.361,P=0.020]. Compared with preterm non-asphyxia SGA group, there was a significant increase in BUN level [(6.70±3.37) vs (3.99±1.69) mmol/L,t=2.581,P=0.025] and decrease in eGFR level [(14.80±4.67) vs (18.66±5.03) ml/(min·1.73 m2),t=2.285,P=0.027] in preterm asphyxia SGA group. Changes in term infants were similar to preterm infants. (3) Compared with asphyxia AGA group, asphyxia SGA group showed a higher frequency of abnormal eGFR in term infants (4/4 vs 4/13, Fisher exact test,P=0.029). ConclusionsAsphyxia as a probable Second Hit can influence the renal function during early stage in both preterm and term infants, especially in SGA infants.
6.Fiber Choledochoscopic Plasma Shock Wave Lithotripsy with Inverse Flush for Difficult Bile Duct Residual Stones

[Summary] The article reported 107 cases of bile duct residual stones treated by plasma shock wave lithotripsy with inverse flush under fiber choledochoscope via T-tube fistula.Of the 107 patients, stones were completely removed in 98 patients, and residual stones remained in 9.Intraoperative biliary hemorrhage occurred in 5 cases, which were stopped after spraying the norepinephrine . There were 4 cases of arhythmia , 13 cases of subcostal or upper abdominal pain , and 11 cases of abdominal distention and diarrhea , all of which were cured by symptomatic treatment .Postoperative biliary infection occurred in 7 cases, which were cured by anti-infection and biliary drainage.Except for 1 patient of lost of follow-up, 106 patients were followed for 6 months.Ultrasound and CT examinations showed free of calculus in 101 cases, and intrahepatic bile duct residual stones in 5 cases.We deem that plasma shock wave lithotripsy combined with reverse flushing under fiber choledochoscope is a minimally invasive , safe, and effective treatment for difficult biliary residual stones .
7.Quality Control and Administration of Disinfectant in Manufacturing Laboratory

OBJECTIVE To prevent infection during the processes of dispensation and distribution of disinfectant. METHODS All the key processes were put under strictly control including purchase,preservation of disinfectant,quality of dispensing processes and administration of the disinfectant distribution. RESULTS Both effective ingredients and microorganism were coincided with the definite criterion. CONCLUSIONS Quality control and administration of disinfectant in manufacturing laboratory is important to guarantee the quality of disinfectant dispensation and to prevent from nosocomial infection as well.
8.Lymphocyte infusion with lymphoma and leukemia immunotherapy

Remarkable achievements have been made for lymphocyte infusion of lymphoma and leukemia, especially of lymphoproliferative disease within twenty-first century. The donor lymphocyte infusion or mobilized donor lymphocyte infusion (DLI/DSI) offer an opportunity of second remission for relapse patients post-transplantation. Cytokine-induced killer/DC cytokine-induced killer play an anti-tumor activity beyond non-MHC restricted. Cytotoxic lymphocyte infusion activated by synthetic tumor antigen produces targeted effects of anti-tumor. Transgenic CTL of anti-tumor TCR has brought the dawn in lymphoma and leukemia patients with defective lymphocytes. Maternal lymphocyte infusion play an anti-tumor/viral activity by avoiding the immune barrier of HLA mismatch.
9.Pathohistologic features and grading of biopsy mucosae and their correlation with disease severity of active ulcerative colitis

Objective To investigate the pathohistologic features and grading of biopsy mucosae and their correlation with disease severity of active ulcerative colitis(UC). Methods A prospective study was conducted in 133 patients with UC who were divided into three groups based on the degree of severity. Pathologic morphometry and grading with HE staining sections were analyzed. Results Pathologic features of active UC: there were neutrophilic leukocytes (100.0%), eosinophils (99.2%), plasmacytes(91.7%) and lymphocytes (75.2%) infiltration among mucosal epithelial cells, and lymphoid follicular formation(72.2%) and small vessels inflammation(63.9%) and focal hemorrhage(68.4%) in lamina propria. There were crypt abscesses(43.6%), glandular abnormalities (44.4%), goblet cell depletion (18.8%), epithelial cell regeneration (36.8%) , atypical hyperplasia (28.6%) and granulation tissue formation (42.9%) in mucosae. With the increase of severity of UC, there was a significant increasing incidence of small vessel inflammation, fiberoid necrosis of vessel wall, glandular abnormality, epithelial cell regeneration, atypical hyperplasia, goblet cell depletion, granulation tissue formation, fiber tissue hyperplasia, and crypt abscess. There was no significant difference of the incidence of lymphocyte hyperplasia, lymphoid follicular formation, eosinophil and plasmacyte infiltration between the groups. Mild UC was mainly characterized by the lesions of Grade Ⅰ and Ⅱ, moderate UC by those of Grade Ⅱ and Ⅲ and severe UC by those of Grade Ⅲ and Ⅳ. There were significant differences of grades among mild, moderate and severe UC. Conclusions There were some pathologic characters in active UC. The partial of markers and histological gradings can reflect the severity and activity of active UC.
10.Evaluation and Clinical Value of Contrast-Enhanced Fluid-Attenuated Inversion-Recovery MR Imaging in Diagnosis of Leptomeningeal Metastasis

Objective To explore the value of contrast-enhanced FLAIR imaging in diagnosis of leptomeningeal metastasis.Methods 23 cases of leptomeningeal metastasis with definite primitive-tumour were collected.All the cases were confirmed by clinic and laborotary examination.Of the 23 cases,7 cases underwent contrast-enhanced T1WI only,16 case underwent contrast-enhanced T1WI and FLAIR.Two groups were analyzed comparatively in diagnostic accuracy and displaying lesions between images of two sequence.Results(1)2 cases were missed,and 1 case was unsure diagnosed in T1WI contrast group;16 cases were all diagnosed accurately in both T1WI and FLAIR contrast-enhanced group.(2)11 cases with fine linear enhancement and 7 cases with small nodular enhancement were more obviously on contrast-enhanced FLAIR than that on contrast T1WI,but the thick linear enhancement were equal on two sequence.4 cases with more lesions were displayed on contrast-enhanced FLAIR images.The signal intensity on contrast FLAIR images were higher than that on contrast T1WI images at the same lesions.Conclusion Contrast-enhanced FLAIR has important value in diagnosing leptomeningeal metastasis.
