1.Clinical trial of Shengmai capsules in the treatment of patients with chronic congestive heart failure

Objective To observe the clinical efficacy and safety of Shengmai capsules in the treatment of chronic congestive heart failure.Methods Eighty-four patients with chronic congestive heart failure were randomly divided into control group and treatment group with 42 cases per group.Control group was given placebo 0.3 mg each time,tid,orally.Treatment group was given Shengmai capsule 0.3 mg each time,tid,orally.Two groups were treated for 2 weeks.The clinical efficacy,heart rate,left ventricular ejection fraction (LVEF),maximal velocity of bicuspid blood flow in the early (E),E/ maximal velocity of bicuspid blood flow in the advanced (E/A),pulmonary artery pressure (PAS),heart function echocardiographic multi-parameter scores (HF-EMPS) and adverse drug reactions were compared between two groups.Results After treatment,the total effective rates of treatment and control groups were 90.47% (38 cases/42 cases) and 73.81% (31 cases/42 cases) with significant difference (P < 0.05).After treatment,the main indexes in treatment and control groups were compared:heart rate were (66.54 ±7.02) and (72.12 ± 8.14) beat · min-1,LVEF were (46.03 ±5.56)% and (41.26 ±5.12)%,E were(92.48 ±14.32)and(75.41 ± 13.62)cm · s-1,E/A were 1.65±0.34 and 1.35 ± 0.31,PAS were (24.85 ± 4.21) and (30.12 ± 4.22) mmHg,HF-EMPS were (7.03 ±1.12) and (7.68 ± 1.32)points with significant difference (all P < 0.05).There were no adverse drug reactions occurred during the treatment in two groups.Conelusion Shengmai capsules have a definitive clinical efficacy and safety in the treatment of chronic congestive heart failure.
2.Study on Mechanism of Loganin Against AGEs Induced GMCs Injury via AGEs/RAGE/SphK1 Pathway

OBJECTIVE To study the mechanism of loganin,the effective componet of Cornus officinalis,on protecting the glomerulus mesangial cell (GMCs) injured by advanced glycation end products (AGEs).METHODS GMCs were divided into control,model (AGEs: 200mg/L),loganin (0.1,1,10μmol/L) groups.The cell proliferation,FN and COL-Ⅳ secretion,microstructure lesions,RAGE,SphK1,S1P and TGF-β protein expressions were measured.RESULTS AGEs induced cell proliferation,FN and COL-Ⅳ secretions,microstructure lesions,RAGE,SphK1,S1P and TGF-β protein expressions were suppressed by loganin.CONCLUSION Loganin can improve the AGEs induced GMCs injury by regulating the AGEs/RAGE/SphK1 signal pathway,and then ameliorate the diabetic nephropathy.
3.Value of transesophageal echocardiography in case selection of transthoracic minimally invasive device closure of ventricular septal defect

Objective To evaluate the value of transesophageal echocardiography (TEE) in transthoracic minimally invasive device closure of ventricular septal defect(VSD).Methods A total of 164 cases of VSD were recruited as candidates to receive transthoracic minimally invasive device closure between January 2007 and October 2010,including 138 perimembranous VSDs,3 muscular VSDs and 23 supracristal VSDs.Among these groups,85 male patients were included.Four-champer view,five-champer view,left ventricular long-axis view,short-axis view and right ventricular inflow view were detected to evaluate the availability of device closure.Results A total of 152 cases(92.7%)were successfully closed with a device.All the patients were followed up more than 3 months arranged with a standard protocol.No complete atrioventricular block (CAVB) or associated valvular complications were observed.Three of the five cases with traced residual shunt after device closure closed spontaneously.Conclusion TEE plays an important role in transthoracic minimally invasive device closure of ventricular septal defect,which has been proved by the good follow-up results without CAVB and associated valvular complications.
4.The relationship between BMI and striatal dopamine transporter with 99Tcm-TRODAT-1 brain SPECT

Objective To assess the relationship between the BMI and the brain DAT, and the influence of BMI on the brain SPECT imaging with 99Tcm-TRODAT-1. Methods MRI and 99Tcm-TRODAT-1SPECT imaging were performed in 31 healthy volunteers(16 males and 15 females), and then the three-dimensional reconstruction of SPECT images were completed. Based on the MRI images, right striatum (RST) and the left striatum (LST) were drawn as ROI on the 4 most clearly consecutive transverse slices.The cerebellum (CB) was taken as the background reference area and the corresponding uptake ratios of ST/CB, LST/CB and RST/CB were calculated. The Pearson correlation tests for radio-uptake ratios (ST/CB, LST/CB, RST/CB), BMI and age were performed, Then multiple linear regression analysis using ST/CB as dependent variable and BMI and age as independent variables was performed. SPSS 15.0 was used in data analysis. Results The ST imaging was symmetrical. The radioactivity was higher in the ST front area than that of the back area. The average uptake ratios of ST/CB, LST/CB, RST/CB were 1.71±0.16,1.70 ± 0. 16 and 1.72±0.17 respectively, in which the three ratios of the female were 1.74 ± 0. 18, 1.71±0. 19 and 1.76 ± 0. 19 respectively and those of the male were 1.68 t 0. 14, 1.68 ± 0. 13 and 1.69± 0.15respectively. ST/CB, LST/CB and RST/CB were negatively correlated with patients'BMI (r = -0. 53,-0.57,-0.47, all P<0.05). The ST/CB was negatively correlated with patients' age(r=-0.39, P=0. 03). The multiple linear regression analysis showed that the BMI was significant independent variable (β=-0.53, t= -3.36, P=0. 002). Conclusions TheSTDAT,evel may decrease as patients' BMI and age increase. Females' DAT level is slightly higher than males'. For ST DAT imaging, age, gender and BMI should be all taken into consideration.
5.Relationship between CT classification of traumatic posterior occipital epidural hematomas and both its curative effect and prognosis

Objective To analyze the therapeutic effect and prognosis according to a new CT classification of traumatic posterior occipital epidural hematomas (POEH). Methods We classified the CT presentation of 104 patients with POEH by sinus transverses: type Ⅰ hematomas were defined as up-sinus transverses; type Ⅱ hematomas were defined as sub-sinus transverses and type Ⅲ hematomas were defined as straddling sinus transverses. The above types were divided into unilateral or bilateral subtypes. Bone flap craniotomy through a median posterior approach was performed in patients with unilateral hematoma adjacent to the midline. Bone flap craniotomy through a median suboccipital approach was adopted for patients with bilateral middle occipital hematoma. Results Type Ⅰ enjoyed mild symptoms, good curative effect and low mortality rate (7%). Type Ⅱ and Ⅲ had serious symptoms and high mortality rate (13.3%, 16.6%, respectively). Conclusion The classification of the CT presentation of POEH is helpful in confirming the diagnosis, drawing up the scheme of treatment and judging the prognosis.
6.Effects of tetramethylprazine on left ventricular hypertrophy and nicotinamide-adenine dinucleotide oxidases in renovascular hypertension of rats

Objective To investigate the effects of tetramethylprazine (TMP) on left ventricular hypertrophy and the regulation of nicotinamide-adenine dinucleotide(NADPH) oxidases in renovascular hypertension of rat.Methods Hypertensive rats were obtained by using two-kidney-two-clip (2K2C) method.Rats were classified into sham-operated group,model group,low,high dose experimental groups (TMP,30,60 mg · kg-1 · d-1) and control group (candesartan,10 mg · kg-1 · d-1).The drugs were continuously administered for 2 weeks.Left ventricular (LV) hemodynamic parameters,serum superoxide ismutase (SOD),dmalondialdehyde (MDA) and hydrogen peroxide(H2O2)levels were measured.The ratio of LV weight to body weight (LVW/BW) was calculated.Hematoxylin-eosin(HE) staining was used for morphological analysis.RT-polymerase chain reaction and Western-blot were performed to investigate the protein expressions of brain natriuretic peptide (BNP),β-myosin heavy chain(β-MHC) and NADPH oxidases(Nox2,Nox4 and P47phox).Results Compared with model group,aortic systolic pressure (AoSP),aortic diastolic pressure (AoDP),LV end-systolic pressure (LVESP),LV end-diastolic pressure(LVEDP) significantly decreased in low,high dose experimental groups with siginificanly (all P < 0.05) while (+ dp/dtmax) and (-dp/dtmax) increased in low,high dose experimental groups with siginificanly (P < 0.05).Compared with model group on LVW/BW (2.8 ± 0.3) mg · g-1,LVW/BW of (1.9 ± 0.2),(1.7 ± 0.4) mg · g-1 in low,high dose experimental groups significantly decreased (P < 0.05).Compared with model group SOD significantly increased while MDA and H2O2 decreased in low,high dose experimental groups with significandy (all P < 0.05).The protein expressions of Nox2 in low,high experimental groups and model group were 0.82 ± 0.08,0.57 ± 0.05,0.30 ± 0.13;the protein expressions of Nox4 in above three groups were 1.00±0.10,0.79 ± 0.07,1.32 ± 0.15 and the protein expressions of P47phox in above three groups were 0.88 ± 0.08,0.65 ± 0.06,1.23 ± 0.15,compared with model group,the protein expression significantly decreased in low,high dose experimental groups with significantly (all P < 0.05).Conclusion TMP attenuated cardiac hypertrophy possibly through decreasing NADPH oxidases (Nox2,Nox4,P47phox) expressions and oxidative stress in renovascular hypertensive rats.
7.Magnum resistance strength of the bone-bonding screw orthodontic anchorage

8.Effects of losartan and simvastatin on collagen content, myocardial expression of MMP-2 mRNA, MMP-9 mRNA and TIMP-1 mRNA, TIMP-2 mRNA in pressure overload rat hearts

Objective To investigate the effects of simvastatin(Sim) and losartan(Los) on cardiac fibrosis and myocardial expression of MMP-2 mRNA, MMP-9 mRNA and TIMP-1 mRNA, TIMP-2 mRNA in pressure overloaded rat hearts. Methods The pressure overload model was induced by descending aortic constriction (DAC) in rats. SD rats were randomized into 6 groups (n=20 each) : normol control group, control sham group, DAC group, Los group (DAC+Los, 5 mg/kg), Sire group ( DAC + Sire, 2 mg/kg), Los + Sire group (DAC+Los+Sire, Los 5 mg/kg, Sire 2 mg/kg). Water, Los or Sire drug was administrated by garage daily beginning from day 5 after operation for 30 days. Collagen was measured on Masson stained myocardial sections, and the level of MMP-2 mRNA, MMP-9 mRNA and TIMP-1 mRNA, TIMP-2 mRNA in left ventricle were detected by RT-PCR. Results Collagen volume fraction (CVF) in DAC group was significantly higher than the normal control and sham groups (P<0.01) which could be significantly reduced by Los and Sire (P<0.05), especially in DAC + Los + Sire group (P<0.01). The levels of myocardial MMP-2 mRNA, MMP-9 mRNA and TIMP-1 mRNA, TIMP-2 mRNA were also significantly higher in DAC group than in normal control and sham groups (P<0.01). Treatment Sim and Los alone and especially in combination significantly decreased the TIMP-1 mRNA, TIMP-2 mRNA expressions (P<0.01) while MMP-2 mRNA, MMP-9 mRNA levels remained unchanged (P>0.05). Conclusion Upregulation of myocardial MMP-2 mRNA, MMP-9 mRNA and TIMP-1 mRNA, TIMP-2 mRNA expressions might contribute to myocardial fibrosis in this model, Sire and Los significantly inhibited myocardial fibrosis possibly by downregulating myocardial TIMP-1 mRNA, TIMP-2 mRNA expressions in this model.
9.Consistency and clinicopathologic significance of BRAFV600E protein expression and genic mutation in papillary thyroid carcinoma

Purpose To investigate the consistency and clinicopathologic correlation of BRAFV600E protein expression and gene mutation in papillary thyroid carcinoma. Methods BRAFV600E protein expression and genn mutation was detected respectively by immunohistochemistry of SP and real time-PCR, then the consistency between the both methods was analyzed by Kappa-test, the correlation between BRAFV600E and clinicopatho-logic parameters was analyzed by Chi-square test in papillary thyroid carcinoma. Results The gene mutation and protein expression rates of BRAFV600E were 89.3% and 88.3%, respec-tively, the differences were not significant, the concordance rate of the both methods was 97.0%, Kappa value was 0.847, the consistence was higher, meanwhile the mutation rates between age <45 and ≥45 were respectively 96.8% and 85.9%, there were significant differences, the positive rates of the both detec-tion methods were higher in thyroid capsule invaded group than non-invaded group, the differences were significant. Conclusion The both methods have higher consistency, the immunohisto-chemistry can be used as an initial screening tool for detecting gene mutation, the gene mutation of BRAFV600E is significantly associated with age and capsule invasion, the relationship is not found between BRAFV600E mutation and the other clinicopatholog-ic parameters.
10.Stents coated with sirolimous and anti-CD34 antibody can optimize the performance of sirolimouseluting stents

Objective To investigate the impact of the stents coated with sirolimous and anti-CD34 antibody on the short-term re-endothelialization and the long-term restenosis in Chinese Minipigs.Methods Three different types of stents [ bare-metal stent ( BMS ),sirolimous-eluting stent ( SES ) and anti-CD34 antibody and sirolimous-coated stent (ASES) ] were randomly implanted in the coronary arteries of 22Chinese Minipigs.At two weeks after stenting,coronary angiography and optical coherence tomography (OCT) were performed in 10 experimental animals.At three months after stenting,coronary angiography and OCT were performed in the remaining 12 experimental animals.Histopathologic examination was performed on the coronary artery segments containing stent after the animals were executed.Results (1) No in-stent thrombosis and parietal thrombus were found by coronary angiography,OCT and histopathologic examination at two weeks post stenting.OCT analysis showed that the covered ratio of stent struts by neointima in ASES group was higher than in SES group [ ( 55.56 + 35.27 ) % vs.(41.82 +23.28 ) %,P < 0.05 ].The mean thickness of neointima in ASES group was significantly higher than in SES group [(89.0+5.0)1.μn vs.(32.0 +4.9) μm,P<O.O1] and BMS group [(89.0 +5.0) μm vs.(44.0 ± 7.2) μm,P < 0.01].Histopathologic and scanning electron microscopy examinations demonstrated that the covering level and quality of stent struts by neointima in BMS and ASES group were both better than in SES group.(2) At three months follow-up,quantitative coronary angiography analysis found that late instent lumen loss in ASES group was significantly lower than in BMS group [ (0.18 ±0.06)mm vs.(0.35 ±0.06)mm,P < 0.05].OCT analysis showed that the percent neointimal hyperplasia in ASES and SES group was significantly lower than in BMS group [ ( 34.75 + 2.64) % and ( 35.63 + 2.07 ) % vs.(48.28 ±3.25) %,both P < 0.01 ].Histopathologic analysis demonstrated that the percent areal restenosis of ASES and SES group were both significantly lower than that of BMS group [ (28.65 + 5.64)% and (29.33 ±6.07)% vs.(46.18 +8.25)%,both P <0.05].Conclusion The stents coated with anti-CD34 antibody and sirolimous can attenuate the inhibitory effect of sirolimous on the re-endothelialization at two weeks after stenting and the anti-hyperplasia effect of sirolimous at three months after stenting.
