Objective To study transcerebellomedullary fissure approach to the fourth ventricle and the lateral brainstem without splitting vermis and depict its adjacent structure, and provide clinicians with anatomical information.Methods Five cadaveric heads fixed with formaldehyde and perfused with color latex in their arteries and veins were dissected with microsurgical anatomical skills. The composition of cerebellomedullary fissure and the distribution of the tela choroida, the inferior medullary velum and the posterior inferior cerebellar artery, and study anatomic base of this approach.Results Cerebellomedullary fissure is a natural anatomical gap which is located between the cerebellar tonsil, biventral lobule and medulla oblongata. The tela choroida and inferior medullary velum do not contain nerve tissue, and thus can be dissected to expose the fourth ventricle. The tonsil- medulla segment and telovelotonsillar segment of the posterior inferior cerebellar artery are the most important blood vessels of the cerebellomedullary fissure approach.Conclusions The transcerebellomedullary fissure approach can reach the fourth ventricle and dorsal brain stem through normal anatomic spaces without splitting the vermis, thus reducing the post-operative complications.

Objective:To investigate the application value of multi-slice spiral CT angiography in the clinical diagnosis of aortic dissection.Methods:The clinical data of 78 patients with aortic dissection diagnosed by digital subtraction angiography and treated between January 2015 and December 2019 in Putuo District People's Hospital of Zhoushan and Zhoushan Hospital were retrospectively analyzed. All patients underwent multi-slice spiral CT angiography and digital subtraction angiography. The type of aortic dissection, the number of aortic lacunas, the distance between the lacuna and the left subclavian artery, the involved branches, the percentage of artery wall calcification, and arterial dissection thrombosis determined by multi-slice spiral CT angiography and digital subtraction angiography were compared.Results:All 78 patients were diagnosed with aortic dissection by digital subtraction angiography, and 75 (96.2%) were diagnosed with aortic dissection by multi-slice spiral CT angiography. The same type of aortic dissection was detected by multi-slice spiral CT angiography and digital subtraction angiography, but three patients were misdiagnosed with pericarditis by multi-slice spiral CT angiography. There were no significant differences in the number of aortic lacunas and the distance between the lacuna and the left subclavian artery between the two methods ( t = 1.02, 0.57, both P > 0.05). The involved branches [61.3% (46/78),] the percentage of artery wall calcification [26.7% (20/78)], and the percentage of arterial dissection thrombosis [78.7% (59/78)] determined by multi-slice spiral CT angiography were significantly higher than 43.6% (34/75), 12.8% (10/75), and 62.8% (49/75) respectively determined by digital subtraction angiography ( χ2 = 4.83, 4.65, 4.62, all P < 0.05). Conclusion:Multi-slice spiral CT angiography exhibits a similar capacity to identify aortic dissection to digital subtraction angiography. It can accurately determine the number and location of aortic lacunas, display the calcification and thrombosis of vascular wall, and is minimally invasive and simply operated. Therefore, multi-slice spiral CT angiography is of high application value in the clinical diagnosis and preoperative evaluation of aortic dissection.

Objective To investigate the correlation of dynamic contrast-enhanced (DCE) MRI features with microvessel density (MVD) and vascular endothelial growth factor (VEGF) in non-small cell lung cancer(NSCLC).Methods Conventional MR imaging and dynamic contrast-enhanced scan in thirty-three patients with NSCLC confirmed by pathologyn were performed. MVD and VEGF were stained with immuno-histochemical technique in all cases. Some parameters of DCE MRI, including maximum slope(Smax) and time to peak(TTP) were put more analysis. Objective To investigate the application of MSCT three-dimensional digital navigated biopsy in subcarinal lesions.Methods 82 patients were enrolled.Study subjects were randomly divided into control group and research group.Three-dimensional positioning and three-dimensional navigation needle biopsy were used in research groups, while CT cross-sectional image positioning with conventional puncture needle was used in control group.Puncture accuracy, one-time success rate of puncture, complications, diagnosis accuracy and operation time were compared between the two groups.Results Puncture success rate, definite diagnosis rate were 87.80%(36/41) and 97.56%(40/41) for the research group,and 60.97%(24/41) and 80.49% (33/41) for the control group, respectively,which on the research group were higher than that on the control group(χ2=8.945, 6.116;P<0.05).Complication rate and operating time were 14.63% (6/41) and (11.64±2.76) min for the research group, and 41.45% (17/41) and (22.22±6.31) min for the control group, respectively, which were lower on the research group than that on the control group (χ2=7.31,t=-11.70,P<0.05).Conclusion MSCT three-dimensional digital navigated biopsy technique could promote the efficiency of subcarinal space puncture biopsy significantly,which is a novel, convenient, precise and safe method.

It is known that extracorporeal shock wave (SW) may promote healing of fracture. A previous study reported that SW promoted human bone marrow stromal cells (hMSCs) towards osteoblasts in vitro. To study the osteogenesis ability of hMSCs treated by shock wave in porous hydroxyapatite (HA) in vivo, primary hMSCs of SW group and control group were cultured in the porous HA for 2 weeks and then implanted into subcutaneous sites of nude mouse. These implants were harvested and prepared for the biochemical analysis of alkaline phosphatase activity by AKP kit, histological analysis of decalcified and undecalcified sections and morphology by scan electric microscope (SEM), as well as osteocalcin mRNA expression by RT- PCR 4 weeks and 8 weeks after implantation. It showed that cells of SW and control group almost covered the rough surface of HA before implantation and the extracelluar matrix of SW group was abundant by SEM photomicrograph . The histological analysis and SEM photomicrograph showed active bone formation 4 weeks and 8 weeks after implantation, as well as tetracycline labeling under fluoroscopy analysis in SW group. Objective To investigate the distribution and antibiotic resistance of clinical isolates in People’s Hospital of Qingyang during 2014 to provide evidence for rational antimicrobial therapy.Methods Bacterial strains were isolated and identified by routine methods and tested by disk diffusion method and E-test for antimicrobial susceptibility. The data were analyzed according to CLSI breakpoints using WHONET 5.6 software.Results Of the 2 121 nonduplicate strains isolated from January through December 2014, 1 565 (73.8%) were gram-negative and 556 (26.2%) were gram-positive strains. The top ifve pathogens wereE. coli, K. pneumoniae, A. baumannii, S. aureusand P. aeruginosa. The prevalence of methicillin-resistantStaphylococcus aureus (MRSA) and methicillin-resistant coagulase negativeStaphylococcus (MRCNS) was 44.6% and 15.4%, respectively.E. coli was the most common gram-negative bacterial species, whileS. aureus was the most common gram-positive bacteria species. Gram-positive bacteria were still highly susceptible to vancomycin, and gram-negative bacteria (such as Enterobacteriaceae) were still highly susceptible to carbapenems.ConclusionsE. coli is the most prevalent pathogen isolated from inpatients in this hospital during 2014. The prevalence of ESBLs-producing strains is the highest inE. coli isolates. The pathogenic bacteria in this hospital showed increasing resistance to a variety of antibacterial agents. Surveillance of antibiotic resistance and effective control measures are necessary to control the growing antimicrobial resistance.
