1.Cleft lip and cleft palate in Jangxi province: A case-control study on familial occurrence of 1 967 cases

Objective:To assess whether familial occurrence has an influence on the morphological characteristics of patients with nonsyndromic cleft lip and/or palate (NsCL/P).Methods:A case-control analysis was performed on the morphological characteristics of familial group and sporadic group,using medical records of 1967 patients with NsCL/P treated in the Affiliated Stomatological Hospital of Nanchang University from 2002 to 2014.Results:164 (8.34%) cases presented a positive history of cleft in their families.The cleft types,the positive familial rate of cleft lip only (CLO),cleft lip and alveolar ridge(CLA),cleft lip and palate (CLP) and cleft palate only (CPO) were 8.11%,8.54%,6.19% and 9.65% respectively.A positive family history of NsCL/P was associated with 0.66 times risk of CPO (P =0.036,OR =0.66,95% CI 0.44-0.98) compared to those of CLO,CLA and CLP.In familial group of CLP,the lateral incidence of male patients was different from that of female patients (P < 0.001).There was no significant difference between familial group and sporadic group on birth weights,parental child-bearing age and clinical manifestations of patients.Conclusion:Familial occurrence might have an influence on cleft type,laterality and gender of the patients with NSCL/P.
2.Simple bone cyst of the jaw: a retrospective study of 11 cases.

OBJECTIVEThis study described the clinical, surgical, and radiographic findings of simple bone cysts.
METHODSA retrospective study was conducted for patients diagnosed with simple bone cysts in the Department of Oral and Maxillofacial Surgery of the Affiliated Stomatological Hospital of Nanchang University from March 2005 to March 2015. Clinical, radio-graphic, surgical, and follow-up data were gathered. Results were statistically analyzed by central tendency and dispersionusing SPSS 20.0 software.
RESULTSEleven cases of simple bone cysts were collected, including three male and eight female patients. Ten cases (90.9%) were asymptomatic and one case developed symptoms of swelling. All of the cases had no history of trauma in the affected area, and all were solitary; ten cases (90.9%) were unilocular, and one (9.1%) was multilocular.The shape of each lesion could be assigned to four categories: cone (3 cases), round (2 cases), oval (4 cases), and irregular (2 cases). The treatment in 10 cases consisted of surgery to explore the cavity and curettage of the bone walls. During surgery,the bone cavity in seven cases (70%) was vacant, whereas serous fluid was found in two cases (20%) and serous-bloody fluid in one case (10%). Of the ten cases, three cases exhibited complete bone healing and seven cases showed new bone formation.
CONCLUSIONSimple bone cysts of the jaws are usually asymptomatic and appear incidentally on routine radiographies. The prevalence is higher in the mandible and young people. The patient usually has no history of trauma, and the bone cavity of lesion is mostly vacant. Curettage of the bone walls of the lesion is suggested for simple bone cysts. Systemic clinical and radiologic follow-up are necessary to ensure successful treatment.

3.Clinical applications of serum infliximab trough levels and antibodies to infliximab monitoring in ankylosing spondylitis patients treated with infliximab

Objective:To investigate the relationship between serum concentration of infliximab (IFX) and its impact on clinical efficacy to ankylosing spondylitis (AS) and explore the situation of antibodies to IFX(ATI).Methods:A total of 38 AS patients treated with IFX from Jan 2017 to Dec 2018 at Hospital were enrolled. Serum IFX-trough levels (TLs) and ATI were detected before the 8th IFX treatment. Ankylosing spondylitis disease activity score (ASDAS) were measured at the first, 6th, 8th IFX treatment. The results were analyzed in relation to the clinical response to treatment. One-way analysis of variance, Kruskal-Wallis test, χ2-test, Logistic regression were used for statistical analysis. Results:① ATI against IFX were detected in6 patients (16%). ② The increase of ASDAS in the 8th IFX treatment compared with the 6th IFX treatment was grouped. Based on the receiver operating characteristic curve (ROC), it was calculated that when IFX-TLs was maintained above 0.635 μg/ml, the patient's condition could be kept stable. ③ Logistic regression analysis showed that IFX-TLs was related to BMI [ OR(95% CI)=1.536(1.023, 2.308), P=0.039] and drug combination [ OR(95% CI)=0.218(0.06, 0.797), P=0.021]. Conclusion:The ATI and IFX-TLs is related to the loss of response to IFX treatment. We suggest that the simultaneous measurement of ATI and IFX-TLs may further help to optimizethe therapy in critical situations.
4.Clinical and laboratory diagnosis of indolent leukemic mantle cell lymphoma: report of one case and review of literature

Objective:To improve the understanding of indolent mantle cell lymphoma (MCL).Methods:The data of a patient with indolent leukemic MCL in the Second Affiliated Hospital of Nanjing Medical University in May 2013 were collected. The cell morphology was analyzed by using cell smear, the flow cytometry was used to make immunophenotype analysis, the karyotype analysis was performed by usig cytogenetic technique, and polymerase chain reaction (PCR) was used to make the immunoglobulin gene analysis. At the same time, lymph node pathology and immunohistochemistry were also analyzed. The related articles published were reviewed to sum up the characteristics and the treatment of indolent MCL.Results:The male patient aged 60 years was obviously asymptomatic accompanied with slow disease progression, leukemic manifestation and without lymphadenopathy. He received pathological biopsy because of located lymphadenopathy in 2008. Small cell morphology, Kappa light chain immunophenotype, t(11;14) translocation showed after the cytogenetic examination, clonal immune globulin gene rearrangement and low Ki-67 positive index were identified. In situ MCL was diagnosed by retrospective pathology.Conclusions:Indolent MCL is extremely rare. It is typically asymptomatic with none or minimal nodal involvement, indolent disease course, leukemic phase with mild lymphocytosis, Kappa light chain expression, simple karyotype, classical or small cell morphology of tumor cells and the positive index of Ki-67 <10%. In situ MCL can be seen in pathology examination. IgVH gene mutation positive and SOX11 negative expression are notable in indolent MCL. International prognostic index of MCL is probably not appropriate in the prognostic analysis of leukemic indolent MCL. It is emphasized that initial observation and having therapies only after the disease progression can be suited for indolent MCL.
5.Effects of Quercetin on the Expression of Ang Ⅱ-induced Myocardial Contractile Protein of Rats through ACE 2- Ang-(1-7)-Mas Axis

OBJECTIVE:To in vestigate the effects of quercetin (Que)on the expressio n of angiotensin Ⅱ(AngⅡ)-induced myocardial contractile proteins of primary rats through angiotensin-converting enzyme 2-angiotensin-(1-7)-Mas (ACE2-Ang- (1-7)-Mas)axis. METHODS :Cardiac tissue of rats aged 1-2 d were collected ,and primary cardiomyocytes were isolated and cultured. The gene silencing model of cardiomyocytes ACE2 was constructed. Experiments were divided into 12 groups. Among them,AngⅡ group,AngⅡ+ small interference RNA (siRNA)group,and Ang Ⅱ+ A 779 group were the model groups ;AngⅡ+ losartan group was positive control group ;AngⅡ+Que40 group,AngⅡ+Que80 group,AngⅡ+siRNA+Que40 group,AngⅡ+ siRNA+Que80 group,AngⅡ+A779+Que40 group and Ang Ⅱ+A779+Que80 group were the experimental groups ;blank group and siRNA group were set up. Ang Ⅱ concentration was 1×10-6 mol/L;siRNA final concentration was 50 nmol/L;Que concentration was 40 and 80 μmol/L;A779(Mas receptor inhibitor )concentration was 1 μmol/L;losartan concentration was 1×10-4 mol/L. mRNA and protein expression of ACE 2,Ang-(1-7) and Mas in primary cardiomyocytes were detected ;the expressions of myocardial contractile proteins were also determined ,such as Na +/Ca2+ exchange channel (NCX),calcium pump (SERCA2a), phosphoprotein (PLB). RESULTS :Compared with Ang Ⅱ group,mRNA expression of Mas was increased significantly in Ang Ⅱ + Que 80 group (P<0.05);mRNA expression of ACE2 and Mas were increased significantly in Ang Ⅱ + CZ0210-01) losartan group (P<0.05). Compared with Ang Ⅱ group,the 851136165@qq.com protein expression of ACE 2 and Ang- (1-7) were increased significantly in Ang Ⅱ+ Que 40 group(P<0.05);compared with Ang Ⅱ + siRNA group ,the protein expression of Ang-(1-7)were increased significantly in Ang Ⅱ+ siRNA+Que 40 group(P<0.05);compared with Ang Ⅱ+A779 group,the protein expression of Ang- (1-7)were increased significantly in Ang Ⅱ+A779+ Que 40 group(P<0.05). Compared with Ang Ⅱ group,the protein expression of NCX was decreased in Ang Ⅱ+Que40 group(P<0.05),protein expression of NCX was reduced in Ang Ⅱ+ losartan group (P<0.05);compared with Ang Ⅱ+A779 group,the protein expression of NCX was decreased in Ang Ⅱ+A779+ Que80 group (P<0.05). CONCLUSIONS :Que improves the expression of Ang Ⅱ -induced ACE 2-Ang-(1-7)-Mas axis in cardiomyocyte model to some extent ,so as to regulate myocardial contractile protein.
6. Expression of Oct4 and Sox2 and their clinical significance in tongue squamous cell carcinoma

Objective:
To investigate Oct4 and Sox2 protein expression in tongue squamous cell carcinoma (TSCC) and the relationships between the expressions of Oct4 and Sox2 and clinical pathological characteristics and survival of patients.
Methods:
The paraffin imbedded tissue specimens of 51 patients with histologically confirmed TSCC were included. Immunohistochemistry was employed to detect the protein expression of Oct4 and Sox2 in 51 TSCC tissue samples. The protein expression levels of Oct4 and Sox2 and their relationships with both clinicopathological features and survival of patients with TSCC were evaluated.
Results:
In 51 TSCC cases,positive expressions of Oct4 and Sox2 were mainly located in the nucleus of tumor cells. The expression of Oct4 was strongly positive in 27 cases (53%), weakly positive in 16 (31%) and negative in 8 (16%), whereas that of Sox2 was strongly positive in 25 cases (49%), weakly positive in 22 (43%) and negative in 4 (8%). 7.Effects of Tingli dazao xiefei decoction on ventricular remodeling in model rats with heart failure after myocardial infarction through ACE2-Ang(-1-7)-Mas axis

OBJECTIVE To explore the mechanism of Tingli dazao xiefei decoction on ventricular remodeling in model rats with heart failure after myocardial infarction. METHODS The rat model of heart failure after myocardial infarction was established by ligation of anterior descending branch of left coronary artery, which was divided into 8 groups: sham operation group, model group, A779 group (1 mg/kg), A779 (1 mg/kg)+Tingli dazao xiefei decoction equivalent-dose group (0.8 g/kg), A779 (1 mg/kg) +Tingli dazao xiefei decoction high-dose group (1.6 g/kg), Tingli dazao xiefei decoction equivalent-dose group (0.8 g/kg), Tingli dazao xiefei decoction high-dose group (1.6 g/kg) and losartan potassium group (10 mg/kg). Each group was given equal volume of distilled water or corresponding drugs intragastrically for 4 weeks. Masson staining was used to determine the distribution of collagen fibers in rat myocardium. The content of hydroxyproline (Hyp) in myocardium was determined by alkaline hydrolyzation. The expressions of type Ⅰ and Ⅲ collagen (COLⅠ, COLⅢ)in myocardium were detected by immunohisto-chemistry. Myocardial fibrosis-related indexes such as matrix metalloproteinase-2 (MMP-2), MMP-9, tissue inhibitor of metalloproteinase-1 (TIMP-1) and soluble suppression of tumorigenicity-2 (sST-2) were detected by ELISA. The protein expressions of angiotensin converting enzyme 2-angiotensin-(1-7)-Mas [ACE2-Ang-(1-7)-Mas] axis were detected by Western blot. RESULTS Compared with sham operation group, myocardial cells in model group and A779 group were disordered, collagen fiber deposition was significantly increased and myocardial fibrosis was obvious; the Hyp content and MMP-2, MMP-9, sST-2 levels were increased, and COL Ⅰ and COL Ⅲ positive expressions were significantly enhanced; TIMP-1 level, protein expressions of ACE2, Ang-(1-7) and Mas were significantly decreased (P<0.05). Compared with model group, above indexes of Tingli dazao xiefei decoction equivalent-dose and high-dose groups were improved to different extents. Compared with A779 group, A779+Tingli dazao xiefei decoction equivalent-dose and A779+high-dose groups could improve myocardial arrangement and collagen distribution, reduce the Hyp content and MMP-2, MMP-9 levels, reduce positive expressions of COL Ⅰ and COL Ⅲ (P<0.05), but couldn’t improve Ang-(1-7) and Mas protein expression. CONCLUSIONS Tingli dazao xiefei decoction can improve ventricular remodeling in myocardial failure model rats after myocardial infarction by improving the expression of ACE2- Ang(- 1-7)-Mas axis proteins.
