1.Detecting, cloning and sequencing of Mycobacterium tuberculosis IS6110 in paraffin-embedded tissues

Objective To evaluate the sensitivity and specificity of a nested polymersase chain reaction (nested PCR) for detection of the Mycobacterium tuberculosis IS6110 in paraffin embedded tissues. Methods 31 samples from tuberculosis patients and 5 biopsy specimens from patients with hepatitis were subjected to detection of Mycobacterium tuberculosis IS6110 by nested PCR. PCR products of two randomly selected samples were cloned and sequenced. Results Mycobacterium tuberculosis IS6110 was positive in 28 of 31 samples of tuberculosis. The sensitivity and specificity of nested PCR for detection of IS6110 were 90.3% and 100%, respectively. The predictive value of nested PCR was 100%. The sequences of two samples were compared with known sequence of H37Rv isolate (reported by Thierry D) and with nucleotide homology of 97% and 95.3%, respectively. Conclusions Nested PCR is sensitive and specific in the diagnosis of Mycobacterium tuberculosis infection in tissues of routinely paraffin embedded. We propose the diagnostic application of nested PCR for the identification of Mycobacterium tuberculosis infection, especially in the cases which can not be distinguished with certainty from other diseases by histopathology and Ziehl Neelsen's staining.
2.Effects of osteoporosis on oral and maxillofacial bone

OBJECTIVE: As a part of the whole skeleton,oral maxilloficial bone can also be affected by the risk factors of systemic osteoporosis. This paper reviewed literature on the possible correlation of systemic osteoporosis with oral alveolar bone resorption,periodontal disease,healing of implanted denture and temporomandibular joint disease that result from oral maxillofacial osteoporosis.DATA SOURCES: Literature in English between January 1999 and December 2003 were searched with the computer in Medline Database by the following key words: osteoporosis,alveolar trophy,periodontitis,implanted denture and tempororomandibular joint with; meanwhile literature was also searched in Chinese Periodical Database and Wanfang Database of the same period by the same key words in Chinese.STUDY SELECTION: Literature involving treatment group and control group were screened in the first trial. Then those non-randomized clinical trials were excluded and the others were looked up for full text. Inclusion criteria: randomized controlled clinical trial(RTC) . Exclusion criteria: repeated study.DATA EXTRACTION: A total of 27 articles of randomized or non-randomized studies on the possible association of systemic osteoporosis with oral alveolar bone atrophy, implanted denture and temporomandibular joint disease were collected,and 18 trials met the inclusion criteria.DATA SYNTHESIS: All subjects in the 18 studies were confirmed of having osteoporosis according the diagnostic standards. Changes of oral maxillofacial bone in systemic osteoporosis were explored by observing bone density,residual teeth, the height of alveolar ridge so as to make comparative and correlation analysis.CONCLUSION: In systemic osteoporosis, mandible bone may also have rarefaction changes.
3.Medicines for maxillofacial osteoporosis

OBJECTIVE: Factors that result in systemic osteoporosis will also affect maxillofacial bones for the latter is a component of skeleton. Present studies are being focused on how is the effect of the drugs that are used to treat systemic osteoporosis on maxillofacial bones. In this article we reviewedclinical and experimental studies about the drugs that acts on maxillomandibular and alveolar bone loss in order to find treatment for maxillofacial bone loss.DATA SOURCES: Medline was searched from January 1999 to October 2001 for English literatures with the searching words of "osteoporosis,mandible, bisphosphonates, herb, estradiol." And Chinese journal database was also searched from January 1995 to December 2004 for Chinese literatures with the searching words of "osteoporosis, mandible, bisphosphonates,herb estradiol"DATA SELECTION: The literatures of therapeutic and controlled studies were chosen, from which the non-random trials and repeated ones were excluded. The full texts of the remaining literatures were found and those of random and controlled trials were included in this review.DATA EXTRACTION: Seventeen literatures that were about random trials on the treatment of maxillofacial bone loss were collected. Thirteen of these 17 were included in this review and 4 of repeated trials were excluded.DATA SYNTHESIS: The subjects in the 17 literatures were all patients with osteoporosis that were in compatible with diagnosis standard. Drugs that act by inhibiting bone resorption, increasing bone mass and ameliorating bone quality were used. The effects of the drugs were compared and analyzed in perspective of the density, mass and strength of bone.CONCLUSION: Medicines for systemic osteoporosis are effective in ameliorating maxillomandibular bone structure, delaying alveolar bone resorption and promoting new bone formation.
4.Optimized formulation of resveratrol solid lipid nanoparticles by uniform design in combination with central composite design/response surface methodology

AIM:To establish a method of optimizing the formulation of resveratrol solid lipid nanoparticles(SLN)quickly and accurately by uniform design in combination with central composite design and response surface.METHODS:Firstly,uniform design was used to optimize the main factors from the five influence factors such as entrapment efficiency,drug loading,average diameter,and PDI(Polydispersity Index).Then,on the ground of it,the main factors determined by unifrom design were optimized by central composite design with three evaluation parameters and response surfaces were nonlineated according to best-fit mathematic models,so optimized formulation was obtained.RESULTS:According to the optimal conditions,the entrapment efficiency,the drug loading,the average diameter and the PDI(Polydispersity Index)of the prepared Res-SLN were 56.77%,2.59%,167 nm and 0.176,respectively.CONCLUSION:The design of combination of uniform design with central composite design is a feasible and convenient method to optimize the prescription of Res-SLN.
5.Experiment on pharmadynamics and acute toxicity of Sanyuzhitong Pills

Objective: To study the pharmadynanics and acute-toxicity of Sanyuzhitongjiegu Pills Methods: Hot plate, body-twisted, in flammation caused by dimethyl benzene, cotton ball method, fracture-healed hare and hemorrheology were applied. Results: Sanyuzhitongjiegu Pills could markedly reduce the twisting times of mice and prolong the initiate of pain caused by heat irritation, inhibit mice's ear swelling caused by dimethyl benzene, diminish the weight of rat's myoidem, promote the healing of experimental fracture, reduce the sticky property of whole blood and improve hemorrheology. The maximum of dosage for mice is 10g?kg -1 , all of them survived for 7 day. Conclusion: Shayuzhitong Pills have effect on pain-killing, anti-inflammation, promotion of fracture healing, regulation of blood, remove of stagnant, The Pills have little toxicity.

7.Orthotopic spelenectomy and selective pericardial devascularization for portal hypertension

Objective To evaluate orthotopic splenectomy and pericardial devascularization for the treatment of portal hypertension.Methods The modified anterior approach splenectomy includes dissection of the peri-splenic vessels and ligments before division of short gastric vessels.During pericardial devascularization,the dessection panel was close to the esophagus and the stomach,leaving intact both the anterior and posterior vagus trunks.Results 63 patients underwent this modified operation.The free portal pressure decreased from (38 ±4) cmH2O to(28 ±4)cmH2O.The average blood loss was (530 ± 37)ml.There was no mortality,nor perioperative gastric paralysis and portal venous thrombosis.By 12-36 months follow-up,there was no pancreatic leakage,hepatic coma and recurrence of bleeding.Intrahepatic portal venous thromboses were detected in 4 cases at the sixth postoperative months.Conclusions This modified splenectomy plus selective pericardial devascularization carries less bleeding and is safe and effective for the treatment of portal hypertension.
8.Establishment and application of neutralization assay based on H5N1 avian influenza pseudotyped virus in vitro

Objective To establish neutralization assay based on H5N1 avian influenza pseudotyped virus in vitro and to evaluate neutralizing titer of convalescent serum from 2 patients with H5N1 avian influenza.Methods pHR-Luc,pCMV△8.2 and CMV/R-SH or CMV/R-TH were cotransfected into 293T cell by co-precipitation with calcium phosphate.Pseudotyped virus supernatant was harvested 72 h posttranofection and identified the expression of HA and P24 by Western blot,and then we analyzed infective activity of 200 μL supernatant of pseudotyped virus.293T cell integrated HA was prepared and anti-HA antibodies in convalescent serum were measured with FACS assay.Neutralizing titers of convalescent serums against Shenzhen and Thailand pseudotyped virus were determined based on calculating IC50 with neutralizing assay.Results Pseudotyped virus involved P24 and HA,and precursor protein HA0 could cleavage into HA1 and HA2 with biological activity.Pseudotyped virus possessed better infective activity,and RLA value was about 2 × 104 with 200 μL supernatant.Both convalescent serums contained anti-HA antibodies and had cross-reactivity against different virus clades with FACS assay.Both convalescent serums had neutralizingactivity and could cross-neutralize different virus clades.However,both serums'neutralizing titers against Shenzhen virus were higher than Thailand.Conclusion We successfully constructed infectious pseudotyped virus which integrated HA of Shenzhen or Thailand virus,and it could be used for evaluation of serum neutralizing activity fast,efficiently and safely with broadly application prospect.
9.Construction of pseudotype retrovirus which integrates hemagglutinin of H5N1 avian influenza virus isolated from human in Shenzhen

Objective To construct pseudotype retrovirus which integrates hemagglutinin(HA)of H5N1 avian influenza virus(AIV)isolated from human in Shenzhen.Methods AIV HA gene was amplified bv RT-PCR,then it was ligated with pGEM-T vector,and identified by restriction enzyme digestion and sequenced.HA gene was cloned into CMV/R vector at the site of Sal Ⅰ and BamH Ⅰ.pHR-Luc,pCMV&8.2 and CMV-HA were co-transfected into 293T cell by co-precipitation with calcium phosphate.The pseudotype virus supernatant was harvested 72 h post-transfection and ultracentrifugation,and the HA and P24 expression on the surface of pseudotype virus was analyzed by western blot.Meanwhile.the infection activity of HIV-HA pseudotype virus was identified in different kinds of cell lines,including MDCK,HeLa,CHO and 293T.Results A/Shenzhen/406H/06 belonged to subclade2.3 with open reading frame(ORF)of HA gene encoded 567 amino acides,whose accession number was EF137706 in GenBank.HA gene was cloned into CMV/R successfully.After co-transfection of above vectors,it revealed that HA protein could integrate pseudotype virus by western blot,and precursor protein HA0 could cleavage into HA1 and HA2 with biological activity.Finally.HIV-HA pseudotype virus could infect 4 kinds of cell lines,which indicated its property of infectivity and catholicity.Conclusion The pseudotype retrnvirns wassuccessfully constructed,which can integrate HA protein of A/Shenzhen/406H/06 and had property of infectivity.It call be used in the further research,including selection of neutralizing antibodies and epitope analysis.
10.Influence of CPAP treatment on maximal oxygen uptake etc.in patients with CHF complicated OS-AHS

Objective:To explore influence of nasal continuous positive airway pressure (nCPAP)on maximal oxygen uptake (V · O2 max )etc.in patients with chronic congestive heart failure (CHF)complicated moderate-to-severe ob-structive sleep apnea-hypopnea syndrome (OSAHS)and explore its significance.Methods:A total of 83 CHF com-plicated moderate-to-severe OSAHS patients were selected and randomly divided into routine treatment group (n=40)and nCPAP group (n = 43).Both groups were treated for six months.Left ventricular ejection fraction (LVEF),apnea hyponea index (AHI)and V · O2 max were measured and compared between two groups before and af-ter treatment.Results:Compared with routine treatment group after six-month treatment,there was significant re-duction in AHI [(27.5±6.2)vs.(6.8±1.2)],and significant rise in LVEF [(0.45±0.07)vs.(0.48±0.05)]and · V O2 max [(16.5±3.5)ml·kg-1 ·min-1 vs.(19.2±3.4)ml·kg-1 ·min-1 ]in nCPAP group,P <0.05 all.Con-· clusion:The nCPAP can improve ventilation function,heart function and V O2 max in patients with CHF complicated moderate-to-severe OSAHS,who have received basic medication.
