Objective To evaluate the sensitivity and specificity of a nested polymersase chain reaction (nested PCR) for detection of the Mycobacterium tuberculosis IS6110 in paraffin embedded tissues. Methods 31 samples from tuberculosis patients and 5 biopsy specimens from patients with hepatitis were subjected to detection of Mycobacterium tuberculosis IS6110 by nested PCR. PCR products of two randomly selected samples were cloned and sequenced. Results Mycobacterium tuberculosis IS6110 was positive in 28 of 31 samples of tuberculosis. The sensitivity and specificity of nested PCR for detection of IS6110 were 90.3% and 100%, respectively. The predictive value of nested PCR was 100%. The sequences of two samples were compared with known sequence of H37Rv isolate (reported by Thierry D) and with nucleotide homology of 97% and 95.3%, respectively. Conclusions Nested PCR is sensitive and specific in the diagnosis of Mycobacterium tuberculosis infection in tissues of routinely paraffin embedded. We propose the diagnostic application of nested PCR for the identification of Mycobacterium tuberculosis infection, especially in the cases which can not be distinguished with certainty from other diseases by histopathology and Ziehl Neelsen's staining.

AIM:The main purpose of this study is to investigate the regulatory role of C/EBP? in mouse vesicular glutamate transporter 2(mVGLUT2) gene expression. METHODS:The promoter region of mVGLUT2 was cloned to PGL3-basic vector. Site-direction mutation was used to identify the CCAAT-enhancer-binding protein ?(C/EBP?) binding site. The promoter activity was observed by luciferase system. The binding between C/EBP? protein and mVGLTU2 promoter region was determined by EMSA. The C/EBP? gene expression was inhibited by its specific siRNA. RESULTS:mVGLUT2 promoter activity decreased about 50% after mutation of C/EBP? binding site. EMSA showed that C/EBP? protein bound onto mVGLUT2 promoter region. Meanwhile,when C/EBP? gene expression was inhibited by its specific siRNA,mVGLUT2 promoter activity,mRNA level and protein level were decreased about 60%,40% and 45%,respectively. CONCLUSION:C/EBP? is involved in the regulation of mVGLUT2 gene expression.

Objective The purpose of this study was to examine the effect of preoperative glucose infusion on glucose transport-4 mRNA ( Glut-4 mRNA) expression in skeletal muscle before and after thoracic operation. Methods Twelve male ASA Ⅰ - Ⅱ patients undergoing elective radical surgery for esophagus cancer were randomly divided into 2 groups : (Ⅰ ) control group; (Ⅱ) glucose infusion group. Patients with hypertension, hyperlipidemia or diabetes mellitus were excluded. The patients in both groups were premedicated with phenobarbitol and atropine. Anesthesia was induced with midazolam 0.1 mg ? kg-1 , propofol 1.5 mg ?kg-1 , fentanyl 4?g?kg-1 and vecuronium 0.15 mg?kg-1 . The patients were mechanically ventilated after trachea! intubatioa Anesthesia was maintained with continuous iv infusion of propofol (80?g ? kg-1? min-1 ) and vecuronium (1.0 ?g?kg-1?min-1) supplemented with droperidol 0.1 mg?kg-1 and fentanyl 6 ?g ?kg-1 given in several boluses. In group Ⅱ 5 % glucose (0.25 g?kg-1) was infused 2 h before operation. Blood samples and 1 g of skeletal muscle were obtained while chest was being opened and closed for determination of plasma glucose and insulin levels and Glut-4 mRNA expression in muscle. The total RNA of the muscle cells was extracted by trizol one-step template. The Glut-4 mRNA amplification products were determined using RT-PCR with ?-action mRNA as an internal control. Renal cell carcinoma (RCC) accounts for approximately 2% of all malignancies. The incidence of RCC has increased by 2% per year for the past 65 years. Over the past two decades, with the widespread use of imaging methods, the frequency of incidental detection of RCC has increased. Surgery is an important therapeutic method for renal cell carcinoma. Originally, nephron-sparing surgery was indicated only for those treated with radical nephrectomy which would render the patients functionally anephric and requiring dialysis. These settings include RCC in a solitary kidney, RCC in one kidney with contralateral inadequate renal fuction, and bilateral synchronous RCC. However, nephron-sparing surgery has been used increasingly in patients with T_(1a) and T_(1b) renal tumors (i.e. up to 7 cm in the greatest dimension) and a normal contralateral kidney, with equivalent outcomes to radical nephrectomy. Compared with radical nephrectomy, nephron-sparing surgery can increase patient survival and reduce the risk of cardiovascular events. However, nephron-sparing surgery has not been widely used yet because most surgeons are confused of its safety, indication, resection range and complications.

Benign symmetric lipomatosis is a rare disease and may appear as a huge tumor in the neck. Four benign symmetric lipomatosis associated with gigantic painless mass or neck motion limitation were reported. Operative technique of one-time radical resection or stage resection was used to remove these tumors. One patient had a postoperative complication of incision effusion and infection, and three patients had no significant complication. At more than one-year follow-up, the motion and appearance of a patient's neck returned to normal, and no recurrence was observed. The etiology, clinical manifestations, diagnosis, and treatment of the disease were discussed.

Objective To explore the effect of time tracking on speed of tissue-type plasminogen activator treatment in patients with acute ischemic stroke,and the correlation between door to needle time (DNT) and onset to door time (ODT).Methods Time tracking tables had been prospectively collected since October 2012.The data of intravenous thrombolytic candidates with acute ischemic stroke were retrospectively reviewed from June 2009 to September 2013.Baseline characteristics and the correlation between ODT and DNT were assessed respectively before and after the implementation of time tracking.Results Three hundred and forty-two cases were finally included.Before the implementation of time tracking,ODT was negatively correlated with DNT (r =-0.169,P =0.015) ; Patients with transient ischemic attack (TIA)/stoke history (β =-0.168,P =0.020) and ODT (β =-0.246,P =0.001)accounted for the length of DNT independently.Since the implementation of time tracking,the elderly accounted for more (19.4％ (25/129) vs 10.3％ (22/213) ; x2 =5.552,P =0.018),the baseline nervous impairment was NIHSS scores (milder 11.0 ± 6.3 vs 12.5 ± 6.7 ; t =2.065,P =0.040),the proportions of patients taking multi-modality imaging were larger (63.6％ (82/129) vs 51.6％ (110/213) ; x2 =4.638,P =0.031) and the DNT decreased significantly ((87.6 ± 33.2) min vs (108.4 ± 52.4) min;t =4.274,P =0.000),which was especially seen in patients arrived within 1 hour after onset ((90.3 ±21.0) min vs (132.5 ± 46.0) min; t =5.048,P =0.000),and the previous inversely correlated DNT and ODT (r=-0.169,P=0.015) became irrelevant (r=-0.013,P=0.885).Conclusion Implementation of time tracking reduces DNT,and clears up the effect of ODT on DNT.

Objective To evaluate the value of diffusion-weighted MRI ,time of flight MR angiography (TOF-MRA) and 3D dynamic contrast enhanced MR angiography (3D DCE-MRA) in the diagnosis of transient ischemic attacks(TIA). Methods 176 consecutive patients with TIA were undergone MRI, including FLAIR,DWI,TOF-MRA and 3D CE-MRA sequences. Results Of 176 cases,48.9% of patients were found having hyperacute ischemic lesions by DWI sequence and 18.2% of lesions could be detected by T2WI.TOF-MRA showed arterial stenosis and occlusion in 76 cases(43.2%),false positive rate and excessive diagnosis were happened in 20 cases.3D CE-MRA of brain showed arterial stenosis and occlusion in 69 cases(39.2%).3 cases with aneurysm and 2 cases with arteriovenous malformation were detected by two methods.3D CE-MRA of neck showed arterial stenosis and occlusion in 38 cases(21.6%).Conclusion DWI and MRA are helpful in detecting the hyperacute ischemic lesion and evaluating the arterial lesions in the patients with TIA.

OBJECTIVE:To establish the quality control method of angelica capsule.METHODS:The chief components of the preparation,radix angelica sinensis,radix salvia miltiorrhizae,rhizoma corydalis,radix astragali,which were identified by TLC;the content of ferulic acid in angelica and tanshinoneⅡ A in Salvia were determined by HPLC method.RESULTS:The TLC(thin layer chromatogram)spots were clear;the linearity of ferulic acid was within the range of0.00255?g～0.0816?g(r=0.9997)and the linearity of tanshinoneⅡ A was within the range of0.01?g～0.06?g(r=0.9995).CONCLUSION:Th_ is method is simple and highly reproductive,which can be used for the quality control of preparation.

Objective:To introduce a new style of platysma myocutaneous flap and to evaluate its application value in the restoration of oral-maxiofacial defects.Methods:Platysma myocutaneous flap with apron incision was used for the restoration of oral-maxiofacial defects after oral lesion ablation in 15 cases from January,2014 to September,2015.The platysma muocutaneous flap was harvested with a U shaped apron incision.The skin above the flap pedicle was preserved.The platysma pedicle was widened to ensure the flap survival.All the patients were followed up form 4 to 33 months.Results:Of the 15 platysma myocutaneous flaps,12 survived completely and 3 had partial flap necrosis.The total survival rate of the flaps at the recipient site of buccal mucosa,tongue and alveolar was 8/9,1/3 and 3/3 respectively.Additionally,there were 2 cases of disturbed wound healing in the neck.Except for 1 case of carcinoma of gingiva which had apparent cervical scar and 1 case of tongue carcinoma which had limited tongue mobility,the other cases showed a satisfactory recovery of oral-maxillofacial contour and fucnction.Conclusion:Compared with the traditional platysma myocutaneous flap,the platysma myocutaneous flap with apron incision can provide a larger skin paddle,and is suitable for the restoration of small and medium sized buccal mucosa and alveloar defects,but not for tongue defect.
