Reviewing the practice in the economic management of hospitals in recent years, analyzing its changes and development tendencies, and identifying the strategies and methods for future development will be beneficial for hospitals to go with the social tide, seize development opportunities, expand advantages and improve management benefit. From a panoramic perspective, the changes and developments of the economic management of hospitals chiefly manifest as transformation in the concepts of operational management, enhancement of its strategic position, expansion of management functions, changes in organizational structures, increasing scientific nature of technical means and improvement of personnels quality. Now that the new century has set in, hospitals hoping to enhance their economic management must exert to constantly update concepts of operational management so as to be adapted to the market of health service under the reform policies. Only by expanding the functions of economic management and making constant innovations in line with hospital reforms can the benefit of economic management be brought into full play. It is necessary to regroup institutions of economic management before a cohesive force can be shaped. It is also a must to cultivate talented personnel in the field of operational management in order to improve management benefit and keep abreast of the development of hospitals in the new century.

Studies have shown that embryonic stem cells and neural stem ceils could be induced to differentiate into motor neurons by adding inducing factors to the culture to improve differentiation proportion of motor neurons. The development and maturation process of motor neurons in vitro culture is similar to that in embryo, involving participation of Pax6, Nk×6.1, Olig2 and HB9. Moreover, cells with HB9 as separation mark almost differentiate into motor neurons in vitro. In addition, other types of protein marks have been used for identification of motor neuronal induced differentiation, such as neuron mark MAP-2, β-tubulin-Ⅲ, cholinergic cell mark ChAT, VAChT, Schwann cell mitogen REG2, and voltage-dependent calcium ion channel 11.2 subunit. The identification of induced differentiation of motor neurons also involves detection of function of differentiated motor neurons by two methods. One method is to detect the function of in vitro cultured motor neurons, and the other is to detect the function of transplanted motor neurons in vivo. Although induced differentiation of stem cells into motor neurons becomes mature, many issues remain in clinical application, such as cell sources of induced differentiation of motor neuron, efficiency of induced differentiation, purification of differentiated motor neurons, and function of motor neurons.

Objective　To explore the correlative factors of ovulation-failure induced with clomiphene(CC) in polycystic ovary syndrome(PCOS).Methods　The relationship between ovulation rate(OR),ovulation cycle rate(OCR) among 178 cases,569 taking medicine cycles and clinical features,endocrine markers were retrospectively analysed.Results　OR was 73.0%,OCR was 45.3%,OCR of both the dysfunctional uterine bleeding(DUB) and anovulatory menstruation groups were markedly higher than those of secondary amenorrhea group(P＜0.005，0.05)，OCR of the DUB group was significantly higher than that in oligomenstruation group too(P＜0.05).OR of the nonpilosity and nonobesity groups were markedly higher than that of pilosity and obesity groups respectively(P＜0.01，0.005).OCR of the unmarried group was significantly higher than infertility and fertilized groups(P＜0.025，0.005).OCR of LH/FSH≥3 and serum testosterone(T) level-normal groups were respectively higher than LH/FSH＜3 and serum T level-elevated group(P＜0.05～0.025).There was markedly correlation among LH/FSH radio,T level,BMI and OR,OCR,PRL levels and OR(P＜0.05～0.01).Conclusions　The findings suggest that effect of the ovulation induced with CC in PCOS relate to types of menstruation,pilosity,status of marry-fertility,levels of LH/FSH,T,PRL.

Objective To study application of nursing intervention in perioperative care of breast cancer patients. Methods A retrospective analysis of 97 cases of breast cancer patients in our hospital was carried out. Psychological counseling and perfect preparation was executed preoperation, making patients emotional stability during the operation, and implemented effective measures after operation,besides relevant guidance was given upon discharge. Treatment effect was observed. Results 97 patients were cured and discharged, and only 1 case appeared ipsilateral upper extremity edema, other patients were in good conditions. Conclusions Perioperative nursing interventions can effectively relieve pressure of patients, improve effect of operations, prevent or reduce postoperative complications and promote early rehabilitation.

Objective:To investigate the expression of CXCL9 in oral submucous fibrosis (OSF) as well as oral lichen planus (OLP) and its role in the inflammatory and immunological reaction. Methods:The expression of CXCL9 was examined in 66 OSF specimens,17 OLP specimens and 10 normal buccal mucosa tissues by immunohistochemistry,and the protein level of CXCL9 were detected by Western blotting.Results:43 (65.2%) cases of OSF and 16 (94.1%) cases of OLP showed positive immunoreactivity for CXCL9 in the cytoplasm of inflammatory cells and endothelial cells. All normal buccal mucosa tissues stained negative for CXCL9. The expression levels of CXCL9 in OLP and OSF were significantly higher than that of normal controls,and the expression level in OLP was significantly higher than in OSF(?2= 4.20,P

Objective To explore the effect of insulin-like growth factor-1(IGF-1) on the growth of muscle-derived stem cells(MDSCs). Methods MDSCs were isolated from the hindlimb muscle of new born mice through serial preplates.DMEM with 2% fetal bovine serum was used to induce MDSCs to differentiate into skeletal muscle lineage.The expression of stem cell marker Sca-1 or skeletal muscle cell marker ?-sarcomeric actin was examined by immunocytochemistry SP method.MTT colorimetric microassay was conducted to determine the effect of IGF-1 on MDSCs proliferation.The relationship between functioning and culture time and the relationship between functioning and IGF-1 concentration were studied. Results MDSCs were successfully isolated from the hindlimb of neonatal mice.Over 90% of the MDSCs showed Sca-1 positive immunoreactivity.MDSCs could give rise to ?-sarcomeric actin positive myotubes in differentiation cultures.The proliferative effect of IGF-1 on MDSCs increased gradu ally with prolonged culture time.The elevation of IGF-1 concentration also increased the proliferative effect until it came to a saturation point.Conclusion IGF-1 can promote the proliferation of in vitro cultured MDSCs.

Objective To examine the feasibility by applying the CT density-difference postprocessing in early diagnosis of middle cerebral artery (MCA) occlusion. Methods The possible MCA occlusion patients were scanned with CT in early stage (within 3 hours) and divided into two groups randomly.Each group included 50 samples and one was processed technologically by CT density-difference and the other one was not.The two groups were scanned once more in 24 hours later and the data being processed by statistical method. Results The results showed that the accurate diagnosis rates were 14.3% within 60 minutes,93.3% from 60 to 119 minutes and 100% from 120 to 180 minutes .In comparing these two groups,there appeared no significant differences in the situation within 60 minutes and there was found significant difference in the other two situations. Conclusion Applying the technology of CT density-difference postprocessing,the middle cerebral artery (MCA) occlusion in early stage might be diagnosed.

Objective: To evaluate the efficacy and safety of lobaplatin combined with docetaxel in the treatment of the recurrent ovarian carcinoma. Methods: We collected 31 patients who diagnosed as recurrent ovarian carcinoma according to the histopathology or cytology from Tumor Hospital of Hunan Province from January 2011 to January 2013. The patients received cisplatin-based chemotherapy or carboplatin-based chemotherapy before the combined chemotherapy of docetaxel and lobaplatin. For patients with the recurrence, they received a treatment at a dose of 80 mg/m2 for docetaxel on day 1 and a dose of 30 mg/m2 for lobaplatin on day 2, which lasted for 21 days as a treatment cycle. hTe patients received 6-cycletreatment at most. Results: A total of 153 cycles of chemotherapy were given to 31 patients, with a median of 4 (2–6) cycles. Three patients showed complete response, 11 partial response, 2 stable disease, and 15 progressive disease. hTe objective response rate (ORR) was 45.16% (95% CI, 26.61%–63.72%), and the disease control rate (DCR) was 51.63% (95% CI, 32.98%–70.25%). The median progression-free survival was 7 months (95% CI, 4.27–9.73 months). For the toxic and side effects among the 31 patients, the numbers of the grade 3, grade 2, and grade 1 leukopenia were 1, 8, and 13, respectively. Meanwhile, 18 were grade 1–2 neutropenia, 17 grade 1–2 anemia, and 2 grade 1 thrombocytopenia. At the same time, alimentary canal reaction followed by nausea and vomiting was relatively modest and could be controlled. hTe serious damage of hepatic-renal function was not found. Conclusion: The chemotherapy of lobaplatin combined with docetaxel is effective on recurrent and refractory ovarian cancer with low side effects, which has advantages over the second-line chemotherapy protocols.

Objective To analyse the relationship between vascular dementia in senitity and diffuse white matter lucencice.Methods Observing 54 patients older than 60 of diffuse white matter lucencice for three years with clinical methods CT and/or MRI.Diagnosing vascular dementia by MMSE and Hachinski.Results The degree of diffuse white matter lucencice within the 54 patients were 34 cases as first degree,14 cases as second degree, 6 cases as third degree in the first year, 17 cases, 19 cases and 18 cases in the second year restectively, 8 cases, 17 cases and 29 cases in the third year.The 6 cases of group suffer from the vascular dementia in senitity in the first year,13 cases in the second year,33 cases in the third year.The average annual growth was 32%.Conclusion Diffuse white matter lucencice is a critical factor of vascular dementia in senitity.
