1. Research progress in surgical procedures for osteochondral lesions of talus

Objective: To summarize the research progress of surgical procedures in osteochondral lesions of the talus (OLT). Methods: By consulting the related literature of OLT in recent years, the advantages and disadvantages of various surgical treatment schemes were analyzed and summarized. Results: There are many surgical treatments for OLT, including bone marrow stimulation, osteochondral transplantation, autologous chondrocyte transplantation, and biologically assisted therapy. Various schemes have different indications and limitations. With the continuous development of various technologies, the effectiveness of OLT treatment will gradually improve. Conclusion: There are still many difficulties and controversies in the treatment of OLT, and there is no unified treatment plan. It is suggested that individualized operation plan should be formulated according to the specific conditions of patients.
2.Effects of Shenfukang Capsule Combined with Sodium Ferulate on Inflammatory Factors and Immune Function of Patients with Chronic Glomerulonephritis

Objective:To study the effect of shenfukang capsule combined with sodium ferulate on the inflammatory factors and immune function of patients with chronic glomemlonephritis.Methods:78 patients with chronic glomerulonephritis who admitted in our hospital from February 2015 to January 2016 were divided into the observation group and the control group according to the random number method.The control group was treated with sodium ferulate,and the treatment group was treated with Shenfukang capsule combined with sodium ferulate.The clinical curative effect,serum inflammatory factors levels and immune function before and after treatment were compared between two groups.Results:After treatment,the total clinical efficacy of observation group was significantly higher than that of the control group (P <0.05).The serum serum levels ofIL-6 observation group,TNF-α and hs-CRP,CD8 in the two groups were significantly higher than those before treatment,which were significantly lower in the observation group than those of the control group (P <0.05).After treatment,the levels of CD3,CD4 and CD4 / CD8 in the two groups were significantly higher than those before treatment (P <0.05),which were significantly higher in the observation group than those of the control group(P<0.05).Conclusion:Shenfukang capsule combined with sodium ferulate could effectively relieve the inflammatory response and improve the immune function with good clinical curative effect in the treatment of chronic glomerulonephritis.
3.Antimicrobial Resistance of Streptococcus pneumoniae and Mechanisms of Resistance to Macrolides

OBJECTIVE To investigate the antimicrobial susceptibility of Streptococcus pneumoniae isolated in Yantai and their mechanisms of resistance to macrolides.METHODS Antimicrobial susceptibility of S.pneumoniae was determined by agar dilution method.Phenotypes of macrolide-resistant S.pneumoniae were determined using double disk test with erythromycin and clindamycin disks.ermB And mefE genes were amplified by PCR.RESULTS Among 42 strains of S.pneumoniae,65.0% were intermediate to and no strain was resistant to penicillin.The resistance rates to erythromycin and clindamycin were 93.0%,respectively.Of 41 erythromycin resistantstrains,93.0% were constitutive resistant.ermB Was detected in 40 strains and mefE in 1 strain,both ermB and mefE genes were found in 9 strains.CONCLUSIONS The resistance rate of S.pneumoniae to penicillin is high in Yantai area,the resistance rates to erythromycin and clindamycin are very high.Target modification by ermB methylase is the predominant mechanism in macrolide-resistant S.pneumoniae in Yantai.
4.Protective effect of extract of Apocynum venetum on kidneys of streptozotocin-induced diabetic rats.

The aim of this study is to investigate the effects and mechanism of extract of Apocynum venetum (AV) on kidneys of streptozotocin-induced diabetic rats. Diabetes mellitus (DM) was induced in rats by intraperitoneal injection of streptozotocin (STZ). The indexes of the blood glucose, renal function and oxidative stress were observed. The DM rats were administrated with the AV for 8 weeks, the above-mentioned indexes were detected. The blood glucose level, BUN, 24 h urine protein excretion, urine volume, renal index, renal cortex's MDA level in model groups all increased significantly. Renal cortex's SOD and GSH activities decreased significantly compared with the normal control group (P < 0.05). The above-mentioned indexes were significantly improved by the AV treatment (P < 0.05). AV have protective effects on renal function of kidneys of streptozotocin-induced diabetic rats, and maybe via inhibition of the renal oxidative stress.
5.Adenovirus mediated MDA-7/IL-24 gene transfer selectively kills hepatocellular carcinoma lines HepG2

Objective To investigate the selective killing effect of MDA/IL-24 on human hepatocellular carcinoma line HepG2 in vitro and provide a theoretical basis for gene therapy of hepatocellular carcinoma.Methods The MDA-7/IL-24 gene was transfected into human hepatocullular carcinoma cell line HepG2 and normal liver cell line L02 with a replication-incompetent adenovirus vector.The mRNA and protein expression of MDA7/IL-24 in HepG2 and L02 cells was examined by RT-PCR and ELISA assay respectively.MTT assay and flow cytometry were used to study tumor cell proliferation and cell cycle in vitro.Hoechst and Annexin-V and PI staining were studied to indicate the apoptosis.Results RT-PCR confirmed that the exogenous MDA-7/IL-24 gene was expressed in HepG2 and L02 cells.The protein product was confirmed by assay of the supernatant with ELISA.MTT and apoptosis test indicated MDA-7/IL-24 induced growth suppression and cell apoptosis of the HepG2 cell in vitro but not in cell line L02,and cell cycle test revealed MDA-7/IL-24 could block HepG2 cell in G2/M but not in L02.Conclusions MDA-7/IL-24 selectively induces growth suppression and apoptosis in lines HepG2 in vitro but not in L02 cell,which indicates that adenovirus mediated MDA-7/IL-24 can be an excellent tool for gene therapy in hepatocellular carcinoma.
6.Mechanism of apoptosis of HCC HepG2 cells induced with replication-defective virus carrying mda-7 in combination with ardriamycin

Objective To explore the mechanism of melanoma differentiation associated gene-7(mda-7) in combination with adriamycin(ADM) killing the HCC HepG2 cells and reversing their multidrug resistance (MDR). Methods The experiment was conducted in three groups including the combined group, ADM group and mda-7 group. MTT assay and FCM were used to determine the differences among the 3 groups and clarify the reversing effect of combined treatment on multidrug resistance of the tumor cells. Expression levels of MDR-1, STAT-3, BCL-2, BAXmRNA were determined with real-time PCR. Western blotting was performed to observe the changes of proteins gp-l70, stat3,P-stat3, PKB, bcl-2,bax in all 3 groups. Result After transfection with 100VP/cell Ad. mda-7,the growth suppression rate of HepG2 treated by ADM (1.5 mg/L) rose from 17.46% to 79. 5%.According to the changes, killed HepG2 cells were increased by a factor of 4.55. times. MDR-1 mRNA was decreased from (16.49 ± 0. 11) to (5.48±0.05) and STAT-3 mRNA increased from (13.17±0. 08) to (21. 57±0. 11)(P<0.05). Western blotting also showed that P-170 and PKB was decreased and the phosphorylation-stat-3 increased after the combined treatment. Conclusion Ad.mda-7 can reverse the multidrug resistance HepG2 cells. It inhibits the expression of MDR-1 mRNA,then arrests PKB protein and the signaling pathway of active stat-3 to induce apoptosis of HCC cells.
7.Structural equation model analysis of substance addiction, T lymphocyte subsets and depression in methamphetamine withdrawal patients

Objective:To investigate the relationship between substance addiction, T lymphocytes and depression in methamphetamine (MA) withdrawal patients.Methods:A total of 105 men who met the inclusion criteria were selected from compulsory isolation drug rehabilitation center of Baini lake, Hunan Province.All participants were suveyed by desires for drug questionnaire(DDQ) and self-rating depression scale (SDS), and the function of T lymphocyte subsets were detected by flow cytometry.All data were managed and analyzed using the SPSS 21.0 and AMOS 21.0 statistical software packages.Results:(1) The respondents were mainly junior high school or below (81.9%), unmarried and divorced (67.6%), unemployed (55.2%), hot suction (47.6%), and careless friends (32.4%), and 67.7% of MA abstinence had depressive symptoms. (2) SDS was positively correlated with desire and intention(DI), negative reinforcement (NR) and CD8+ %( r=0.408-0.897, all P<0.01), while negatively correlated with medication control, CD3+ %, CD4+ %, and CD4+ /CD8+ ( r=-0.792--0.263, all P<0.01). DI had significant positive correlation with CD8 + %( r=0.216, P<0.05), and negative correlation with CD4+ /CD8+ ( r=-0.217, P<0.05). NR had significant positive correlation with CD8 + %( r=0.259, P<0.05), and had significant negative correlations with CD3+ %, CD4+ %, and CD4+ /CD8+ ( r=-0.275-0.200, all P<0.05). Medication cotrol had a significant negative correlation with CD8+ %( r=-0.363, P<0.05), and significant positive correlations with CD4+ % and CD4+ /CD8+ ( r=0.288, 0.261, both P<0.05). (3)The model fitting index showed that DI, NR and medication control had significant direct effects on T lymphocyte subsets (all P<0.05). DI, medication control and T lymphocyte subsets had significant direct effects on self-evaluation of depressive symptoms (all P<0.05). NR, DI and medication control indirectly affected depressive symptoms through T lymphocyte subsets (all P<0.05). There were three path relationships: ① DI indirectly affected SDS via T lymphocyte subsets; ② Medication control indirectly affected SDS via T lymphocyte subsets; ③ NR indirectly affected SDS via T lymphocyte subsets. Conclusion:The structural equation model suggests that the degree of addiction in MA abstinence affecting depressive mood may be related to hypofunctional T lymphocyte subsets and provide methods for the prevention and treatment of drug addiction.
8.Qualitative research on experience of caregivers of palliative care for minors: a Meta synthesis

Objective:To systematically review the qualitative research on the care experience of caregivers of palliative care for minors, in order to provide a reference for formulating a care plan that is more in line with the needs of palliative care for minors and their families.Methods:The Cochrane Library, Australia Joanna Briggs Institute Evidence-based Health Care Center Library, Embase, PubMed and Web of Science, CINAHL, China National Knowledge Infrastructure, Wanfang Database, VIP Database and China Biology Medicine Disc were searched by computer for literature on qualitative research on care experience of underage hospice care caregivers from the establishment of the database to December 30, 2023.Results:A total of 11 articles were included and 70 research results were extracted, which were summarized into 7 new categories and formed 2 integrated results: professional caregivers and relative caregivers had multiple care burden, and need multiple care support; professional caregivers and relative caregivers can shoulder the care responsibility of children together, actively respond to the difficulties in care, gain personal care experience and growth.Conclusions:Hospice care for minors has its own characteristics, and caregivers have various care burdens and needs. Special attention should be paid to the emotional experience and care demands of caregivers, and targeted support and protection should be provided to promote the construction of hospice care team for minors in China and improve the level of hospice care for minors in China.
9.Replication-incompetent Adenovirus Vector-mediated MDA-7/IL-24 Selectively Induces Growth Suppression and Apoptosis of Hepatoma Cell Line SMMC-7721

In order to investigate the effect of replication-incompetent adenovirus vector expressing MDA-7/IL-24 on tumor growth and apoptosis of human hepatocellular carcinoma (HCC) cell line SMMC-7721 and normal liver cell line L02, the recombinant replication-incompetent Ad.mda-7 virus vector was constructed and infected into the HCC cell line SMMC-7721 and normal liver cell line L02. RT-PCR was performed to examine the expression of MDA-7 mRNA. The concentrations of MDA-7/IL-4 in culture supernatants were determined by using ELISA. MTT and Hoechst staining assay were applied to observe the inhibitory and killing effects of MDA-7 on the HCC cells. By using flow cytometry, the apoptosis, cell cycle and proliferation of SMMC-7721 and L02 cells were meas- ured. The results showed recombinant replication-incompetent virus expressing MDA-7/IL-24 was constructed successfully, and RT-PCR revealed that it could mediate the high expression of the ex- ogenous gene MDA-7/IL-24 in SMMC-7721 and L02 cells. The expression of MDA-7/IL24 proteins in the culture supernatant was detectable by ELISA. Ad.mda-7 infection induced apoptosis and growth suppression in SMMC-7721 cells and an increased percentage of HCC cells in the G2/M phase of the cell cycle, but not in L02 cells. It was concluded that mda-7/IL-24 gene, mediated with replication-incompetent adenovirus vector, could selectively induce growth suppression and apoptosis in HCC cell line SMMC-7721 but without any toxic side-effect on normal liver line L02.
10.Cigarette smoking in different manners induces acute lung injury in rats

Objective: To investigate the effects of cigarette smoking in different manners on acute lung injury in rats .Methods: The commercially available cigarettes with tar of 1,5, 11 mg were smoked in Canada depth smoking ( health canada method , HCM) manner , and those with tar of 11 mg were also smoked in international standard ( ISO) smoking manner .Rats were fixed and exposed to mainstream in a manner of nose-mouth exposure .After 28 days, the bronchoalveolar lavage fluids from left lung were collected for counting and classification of inflammatory cells and determination of pro -inflammatory cytokines IL-1βand TNF-α.The right lungs were subjected to histological examination and determination of myeloperoxidase ( MPO ) and superoxide dismutase (SOD) activities and glutathione , reactive oxygen species (ROS) and malondialdehyde (MDA) levels.Results:In both HCM and ISO manners , the degree of lung injury was closely related to the tar content of cigarettes , and significant decrease in the body weight of rats was observed after smoking for one week .In a HCM manner , smoking with cigarette of 11 mg tar resulted in robust infiltration of macrophages , lymphocytes and neutrophils into lungs , significant increase in IL-1βand TNF-αlevels and MPO activities , and significant decrease in GSH levels and SOD activities and increase in ROS and MDA levels ( all P <0.05 ) .Smoking with cigarette of 5 mg tar led to moderate increase in IL-1βand TNF-αlevels, and MPO activities (all P<0.05), and moderate decrease in GSH levels and SOD activities and increase of ROS and MDA levels ( all P<0.05).However, smoking with cigarette of 1 mg tar affected neither inflammatory cell infiltration nor IL-1βand TNF-αlevels.Conclusion: Cigarette smoking in nose-mouth exposure manner can induce acute lung injury in rats; and the degree of lung injury is closely related to the content of tar and other hazards in cigarettes .
