1.Discussion on imbalance of qi and blood as basic pathogenesis of cardio-cerebrovascular diseases

The physical functions of the heart and brain is based on the sufficiency,fluency and balance of qi and blood. Therefore,the basic pathogenesis of cardio-cerebrovascular diseases should be imbalance of qi and blood. Any causative agent is bound to influence the harmony of qi and blood once it attacks the heart and brain,its evolving laws are qi stagnancy and blood stasis at early stage,subsequent cementation of phlegm and blood stasis,and more qi deficiency and more blood stasis in the end. Regulating qi and activating blood circulation may harmonize qi and blood,balance yin and yang,which should be served as the basic therapeutic method for treating cardio-cerebrovascular diseases.
2.Study of Anatomical Structures of Middle Ear with Helical CT Virtural Endoscopy

Objective To evaluate the ability to show anatomical structures of the normal middle ears with helical CT virtual endoscopy(CTVE).Methods CTVE were performed to observe anatomical structures of bilateral middle ears in 100 healthy volunteers.Results Anatomical structures of middle ears were displayed stereoscopically by CTVE from multiple views.But superficial and ting structures were showed pooly.Conclusion CTVE is a new noninvasive imaging tool for observe stereoscopically auditory ossicudar chains.

H type hypertension is essential hypertension complicated elevated plasma homocysteine level ,which ac-counts for about 75% in Chinese adults with hypertension and is closely related to cerebral stroke and other cardio-vascular diseases .Lowering homocysteine level in patients with hypertension possesses important significance for preventing stroke .
4.Bioinformatic analysis on the Rap2B-like gene from Clonorchis sinnensis and its cloning and expression

To analyze the structural characteristics of the Rap2B gene from Clonorchis sinensis by bioinformatics and to clone and express this gene through genetic engineering methods in order to obtain the corresponding recombinant protein.The structural and functional characteristics of the Rap2B protein were analyzed by using the related bioinformatic softwares. Using the full-length cDNA plasmid clone (Noc009e03) as template , the coding region of Rap2B gene sequence was amplified with PCR and cloned into the prokaryotic expression vector pET-28a(+) and then expressed in E.coli BL21 through IPIG induction. The recombinant protein expressed was purified by Ni-IDA affinity chromatography and detected by SDS-PAGE and Western blotting. It was demonstrated that the size of this gene was 847 bp in length, encoding 141 amino acids and its theoretical molecular weight was 15851.1 daltons. As demonstrated by PCR, double enzyme digestion and DNA sequencing, the recombinant expression plasmid pET-28a(+)-Rap2B was successively constructed and the Rap2B-like gene was highly expressed in E.coli BL21. The recombinant protein was obtained through purification with His affinity chromatography and could react specifically with the serum of rats infected with C.sinensis. Through these investigations, the Rap2B protein may be predicted as a member of the Ras oncogene family protein and is potential in the carcinogenic process of C.sinensis.
5.Protective effects of hydroxyethylpuerarin on cultured bovine cerebral microvascular endothelial cells damaged by hydrogen peroxide

Aim To observe the damages induced by hydrogen peroxide in cultured bovine cerebral microvascular endothelial cells (BCMEC) and evaluate the protective effects of hydroxyethylpuerarin on hydrogen peroxide-injured BCMEC. Methods BCMEC were cultured and transferred into modified Eagle medium (MEM). The viability of cells was detected by MTT assay. Cell injury was determined by lactate dehydrogenase (LDH) activity in the extracellular medium. Flow cytometry was employed to observe the occurrence of apoptosis. Morphologic changes of cells were visualized under phase contrast and electron occurrence of apoptosis. Hydroxyethylpuerarin was shown to increase the survival rate and decrease the activity of LDH of BCMEC damaged by hydrogen peroxide. Hydroxyethylpuerarin was also found to protect BCMEC against apoptosis induced by hydrogen peroxide. Conclusion Hydrogen peroxide induces BCMEC injury either by apoptosis or through necrosis. Hydroxyethylpuerarin protects BCMEC against hydrogen peroxide-induced injury in a concentration-dependent manner. Its antioxidant effects might be involved as the mechanism protection.
6.Effect of compound EXP-2528 on angiotensin Ⅱ-induced E-selectin and VCAM-1 expression in rat brain microvascular endothelial cells in vitro

The aim of this study is to investigate the effect and mechanism of angiotensin (Ang) II on E-selectin and vascular cell adhesion molecule-1 (VCAM-1) expression in rat brain microvascular endothelial cells (BMEC) and evaluate the effect of compound EXP-2528, a novel Ang Ⅱ type 1 (AT1) receptor antagonist. The experiment was performed in cultured BMEC of rat. The mRNA and protein expression of E-selectin and VCAM-1 in BMEC was analyzed by RT-PCR and Western blotting, respectively. The results showed that the mRNA and protein expression of E-selectin and VCAM-1 in BMEC were significantly upregulated by 4 h or 18 h exposure to 1×10-7 mol·L-1 Ang Ⅱ. These effects were abolished by pretreatment with the selective AT1 receptor antagonists losartan and compound EXP-2528, but not with the AT2 selective antagonist PD123319. Combining losartan with PD123319 also significantly inhibited Ang Ⅱ-induced E-selectin and VCAM-1 expression in BMEC, but there was no significant difference compared with losartan group. These findings indicated that Ang Ⅱ upregulated E-selectin and VCAM-1 in BMEC by activating AT1 receptor and then involved in the development of cerebrovascular disease.

Inthis study, the DNA fragment encoding the regions Ⅰ to Ⅱ of CSP gene from Plasmodium falciparm isolate FCC1/HN was cloned into an. expression vector pcDNA3 contained cytomegalovius (CMV) and transformed into human Hela cell line. The expressed protein PfCSP (condidate vaccine) was used to immunize BALB/c mice by subcutaneous、 intravenous or intraperitoneal administration respectively. The splenocyte of BALB/c mice immumized with the condidate vaccine released significantly IL-2 and IFN-γ following stimulation with this vaccine. It is associated with increase of the splenic T lymphocyte proliferation stimulated by this vaccine and enhancement of NK cell killing activity in the former studies. These results suggested that the vaccine could stimulate T cell response and enhance the cell-mediated immunity.
8.Preventive effects of aminophylline on the pulmonary hypertension rebound reaction to exposure to NO in the hypoxic pigs

AIM: To evaluate the prophylactic effect of aminophylline on the pulmonary hypertension rebound reaction to exposure to NO in the hypoxic pigs. METHODS: The 10 pigs undergone Swan-Ganz catheter, the mPAP was measured with a Physio-recording instrument and PaO 2 was measured with a blood gas analyzer, when breathing NO for 30 minutes and suddenly stopping breathing NO, administing aminophylline 0.25 g, followed by 30 minutes with room air. The respiratory rate and heart rate were also monitorried with a Hewlett-Packard portable monitor. RESULTS: The mPAP of the acute hypoxic pig was induced significantly after breathing 10 -5 NO. When suddenly stopping breathing NO, the induced mPAP became more and more high, the level of the mPAP in 5 minutes was similar to the values before absorbing NO, the mPAP in 10 minutes was higher than values before absorbing NO, while the level of PaO 2 was lower than the values before absorbing NO; but suddenly stopping breathing NO, administing aminophylline, although the induced mPAP became high, the speed was slower than the controls, the level of the mPAP in 30 minutes still was lower than the values before absorbing NO. CONCLUSION: Aminophylline has preventive effects on the pulmonary hypertension rebound reaction to exposure to NO in the hypoxic pigs.
9.Haemodynamic changes in high altitude pulmonary edema and effects of oxygen breathing

AIM: To explore the pathogenic mechanism of high altitude pulmonary edema(HAPE). METHODS: Haemodynamic changes and effects of 100 percent oxygen breathing were measured by Swan-Ganz thermistor catheters, high altitude healthy volunteers were served as controls. RESULTS: The important features of haemodynamic changes in HAPE: (1)Pulmonary arterial pressure was raised; (2)Pulmonary arterial resistance and cardiac output were raised; (3)Pulmonary artery wedge pressures and right atrial pressure were normal; (4)Pulmonary arterial pressure and resistance were induced by oxygen breathing. CONCLUSIONS: The normal pulmonary artery wedge pressures with a high cardiac output indicated that HAPE was recognized as a form of noncardiogenic pulmonary edema. The pulmonary hypertension may play an important role in the development of HAPE.
10.Effect of interventional therapy guided by flow reserve fraction on prognosis of patients with acute coronary syndrome with multi-vessel disease

Objective To investigate the effect of percutaneous coronary intervention (PCI) guided by blood flow reserve score (FFR) on the prognosis of patients with acute coronary syndrome (ACS) with multiple vessel lesions.Methods From April 2015 to April 2017,three hundred and twenty patients with ACS complicated with multi-vessel disease in the Department of Cardiology,Shanghai Tenth People's Hospital were randomly divided into two groups,160 cases in each group.Flow reserve fraction (FFR) and coronary arteriography alone (CAG) were used to guide PCI treatment (CAG group).The basic data,the success rate of PCI and the number of stent implantation were compared between the two groups.The patients were followed up for 6 months and the incidence of major adverse cardiovascular events (MACE) was compared between the two groups.Results There was no significant difference in sex,age,type of lesion,risk factors,coexisting diseases,number of lesion vessels and preoperative left ventricular ejection fraction (LEVF) between the two groups (P>0.05).There were no significant differences in the success rate of PCI between FFR group and CAG group (97.7% (127/130) vs.(99.2% (129/130)),the time of PCI operation ((95.43±36.24) min vs.(101.36±28.16) min),the length of hospitalization ((6.12±1.74) d vs.(5.94± 1.55) d) ((x2 =1.02,t =1.47,t =1.01,P>0.05).Compared with CAG group,the number of stents in FFR group ((1.79±0.25) vs.(2.15±0.34)),the amount of contrast agent ((143.42±27.42) ml vs.(184.11± 31.05) ml) were significantly reduced (t =9.73,t =11.22,P < 0.05).Six months after operation,the incidence of target vessel revascularization and major adverse cardiovascular events in FFR group was 3.1% (4/130).The total incidence of major adverse cardiovascular events was 6.9% (9/130),significantly lower than that of CAG group (9.2% (12/130) vs.16.2% (21/130).The difference was statistically significant (x2 =4.26,x2 =5.43,P < 0.05).Conclusion FFR-guided PCI can reduce unnecessary stent implantation,reduce major adverse cardiovascular events and improve the prognosis of ACS patients with multi-vessel disease.
