1.Endoscopic ultrasonography applied in the management of suspected obstructive jaundice in acute biliary pancreatitis

Objective To explore the application value of endoscopic ultrasonography (EUS) in the diagnosis of suspected obstructive jaundice in acute biliary pancreatitis and its effect on treatment outcomes. Methods Clinical data were retrospectively collected in 96 patients with acute biliary pancreatitis (ABP) who were suspected obstructive jaundice. Patients were divided into early EUS scanning group (EES) and delay EUS scanning group (DES). Clinical treatment results and test results were compared between the two groups. Results The diagnosis sensitivity, specificity, accuracy, positive predictive value and negative predictive value of EUS were 96.43 %, 82.5 %, 90.63 %, 88.52 % and 96.29 %. Early EUS scan with 72 h could identify the etiology of ABP and subsequent treatment based on the EUS can easily decrease the white blood cell count, percentage of neutrophils, total bilirubin and serum amylase (P < 0.05). The pain relief rate in EEI group after 1 week treatment was significantly higher than that in the DES group (87.50 % vs 66.67 %, P = 0.027) and average length of hospitalization was shorter [(12.70 ±2.10) d vs (14.10 ± 3.00) d, P = 0.006]. Delay EUS scan were likely to have higher white blood cell count and total bilirubin, meanwhile seemed to increase the organ failure rate and necrotic infection of the pancreas. Conclusion ABP with suspected obstructive jaundice should be considered to have endoscopy intervention. Results of EUS are the basis for treatment decision, which can avoid the unnecessary endoscopy treatment.
2.Early total care pattern for intertrochanteric fracture of femur in the elderly.

OBJECTIVETo evaluate clinical results of early total care (ETC) treatment for elderly patients with intertrochanteric femur fractures.
METHODSClinical data of 106 elderly patients with intertrochanteric fracture treated from January 2012 and February 2015 were retrospectively studied. According to whether receiving the early total care mode, the patients were divided into 2 groups, 34 cases were diagnosed and treated with early total care pattern (ETC group), including 14 males and 20 females with an average age of (74.88 ± 4.38) years old ranging from 70 to 86. According to Evans types, 4 cases were type I, 5 cases were type II, 13 cases were type III, 11 cases were type IV, 1 case was type V. Seventy-two patients were treated with conventional trauma method (conventional group), including 35 males and 37 females with an average age of (74.46 ± 3.63) years old ranging from 70 to 85. According to Evans type, 8 cases were type I ,13 cases were type II, 25 cases were type III, 25 cases were type IV, and 1 case was type V. All fractures were treated with proximal femoral nails anti-rotation (PFNA). Operative time, hospital stays, leaving bed time, complications, cases of death at 1 year after operation, postoperative Harris score at 12 months were observed and compared.
RESULTSAll patients were followed up, the time of ETC group ranged from 9 to 18 months with an average of 13.29 ± 1.51, and the time in conventional group ranged from 12 to 16 months with an average 12.93 ± 1.15, while there was no significant difference between two groups in time of following-up (t = 1.368, P = 0.174). There was no significant meaning in cases of death between ETC group (2 cases) and conventional group (8 cases). Three cases occurred complications in ETC group, and 20 cases in conventional group,there was obvious meaning between two groups (χ² = 0.739, P = 0.318). Operative time,hospital stays,leaving bed time in ETC group respectively was (2.03 ± 0.67) d, (15.41 ± 2.87) d and (3.62 ± 0.74) d; while in conventional group respectively was (4.17 ± 1.59) d, (20.11 ± 4.24) d and (5.35 ± 1.22) d; there were significant differences between two groups in operative time, hospital stays, leaving bed time. Postoperative Harris scores at 12 months in ETC group was (82.32 ± 4.56), and (79.24 ± 5.52) in conventional group, there was obvious meaning between two groups (t = 2.833, P = 0.006).
CONCLUSIONETC pattern is a novel method for diagnosis and treatment of intertrochanteric femur fractures in elderly, it could shorten operative time, hospital stays, leaving bed time, decrease complications and promote recovery of function.

3.Synergistic effect of atorvastatin and probucol on severity of coronary lesion in patients with borderline coronary lesion

Objective To study the changes of plasma cystatin C level (PcyC),and evaluate the effects of the joint use of atorvastatin and probucol on PcyC and severity of coronary lesion in patients with borderline lesion of coronary artery.Methods One hundred and thirty consecutive patients with borderline coronary lesion assessed by quantitative coronary angiography were enrolled into borderline coronary lesion group (BCL),and another 136 subjects without coronary lesion were enrolled as controls (CTR).And in the meantime,the subjects in BCL group were randomized (closed envelope method) into routine treatment subgroup ( RTT,n =60),and combined treatment subgroup in which patients were treated with atorvastatin 20 mg plus probucol 1.0 g daily in addition to routine medication ( CBT,n =70) for 6 months.There were no statistical differences in basic clinical features between two subgroups.PcyC,high-sensitive C-reactive protein (hs-CRP),total cholesterol (TC),low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol ( HDL-C ) and triglycerides (TG) were determined.Of them,104 patients in BCL group rechecked by coronary angiography.Comparison of biomarkers carried out between two groups by using a number of independent-sample t-test and analysis of variance.For enumeration data,chi-square test was used to compare mean values of biomarkers between groups. P < 0.05 was considered statistically significant.Results PcyC levels were significantly higher in BCL group than those in CTR group ( P < 0.05 ).Compared with RTT subgroup,levels of PcyC,TC,LDL-C,TG and hs-CRP were more significantly decreased in CBT subgroup (P < 0.05,P < 0.01 ).Moreover,there was a trend of slight decrease in the mean percent of stenosis (MPS) of coronary artery with borderline lesion in RTT subgroup treated for 6 months,whereas more marked decrease in the MPS of coronary artery with borderline coronary lesion in CBT subgroup treated for 6 months ( P > 0.05 ; P < 0.05 ).Conclusions Cystatin C plays an important role in the pathogenesis of coronary artery,and PcyC is associated with severity of coronary lesion,the combination of atorvastatin and probucol decreases the PcyC level,and it may be the treatment of choice for borderline lesion of coronary artery.
4.Advances in the research of HIV-1 envelope glycoprotein gp120 inhibitors

From the process of human immunodeficiency virus-1 (HIV-1) invading cells, the combination of gp120 and CD4 is the first step for HIV-1 to invade cells. Interfering with this process can prevent HIV from recognizing target cells and inhibit virus replication. Therefore, HIV-1 gp120 is an important part of the HIV-1 life cycle. Fostesavir, a phosphatate prodrug derived from the gp120 inhibitor BMS-626529 modified by the prodrug strategy, was approved for the treatment of adult patients with multidrug resistant HIV-1 infection by the US FDA and the European Medicines Agency in 2020 and 2021, respectively. In this review, we focus on the research progress of small molecule inhibitors targeting the interaction of gp120-CD4 from the perspective of medicinal chemistry, in order to provide reference for the subsequent research of gp120 inhibitors.
5. Relationship among perceived stress,sleeping and fatigue status in soldiers acutely exposed to high altitude for the first time

OBJECTIVE: To investigate the relationship among perceived stress,sleep and fatigue status; and to analyze the effect of sleep status in mediating and regulating perceived stress and fatigue status in soldiers that were acutely exposed to high altitude for the first time. METHODS: A total of 700 soldiers that were acutely exposed to high altitude for the first time were investigated with convenient sampling. They were evaluated using Perceived Stress Scale,Sleep Status Self-evaluation Test,and Fatigue Scale-14. The mediating effect was analyzed by structural equation model,and the moderating effect was analyzed by hierarchical regression analysis. RESULTS: The total score of fatigue among soldiers that often took part in recreational activities was significant lower than that who did not( P < 0. 01). The total score of fatigue among soldiers that had training injury was significant higher than that who had not( P < 0. 01). The total score of fatigue among soldiers that had high training expectations was significant higher than that that didn't have high training expectations( P < 0. 01). Sleep status,sense of overload and training expectations had remarkable predicting effect on fatigue status among soldiers that were acutely exposed to high altitude for the first time( P < 0. 01). Sleeping status has a complete mediating effect on the sense of prediction,sense of overload and fatigue status( P < 0. 01),and has a moderating effect on the sense of control,sense of overload and fatigue status( P < 0. 01). CONCLUSION:s Perceived stress and sleeping status had close relationship with fatigue status among soldiers that were acute exposed to high altitude for the first time. Sleeping status has intermediary and regulating effect between senses of overload and fatigue state.
6.Ten significantly differentially expressed genes in prostate cancer: Screening and verification.

OBJECTIVETo screen and verify differentially expressed genes in prostate cancer.
METHODSUsing DNA microarray, we screened differentially expressed genes in prostate cancer tissue and its adjacent tissue followed by verification by PCR.
RESULTSA total of 1 444 genes were found to be differentially expressed (differentiation ≥ 1.5-fold; P≤ 0.05) in the prostate cancer tissue, of which 769 (53%) were up-regulated and 675 (47%) down-regulated. Fifty percent of the differentially expressed genes showed a 1.5- to 2-fold differentiation, including 396 up-regulated and 182 down-regulated ones. Additionally, 308 up-regulated and 334 down-regulated genes exhibited a >2- to 5-fold, 46 up-regulated and 78 down-regulated genes a > 5- to 10-fold, and 19 up-regulated and 81 down-regulated genes a > 10-fold differentiation. Verification by subjecting 15 most significantly up-regulated and another 15 most markedly down-regulated genes to quantitative real-time PCR (qRT-PCR) showed that most of the genes had a transcriptional profile similar to that in the microarray data, with a Pearson correction coefficient of 0.83 between the microarray data and qRT-PCR results. Totally, 10 significantly differentially expressed genes were identified.
CONCLUSIONDNA microarray analysis provides reliable information on differentially expressed genes in prostate cancer and benign tissues. The 10 significantly differentially expressed genes verified by qRT-PCR could possibly become new bio-markers and specific molecules for tumor identification.

7.Methylation status of zo-1 gene promoter in acute leukemia.

This study was purposed to investigate the difference of zo-1 gene promoter methylation between healthy individuals and acute leukemia patients. BS-PCR method was used to detect the status of zo-1 gene methylation in healthy individuals, acute leukemia patients and leukemic cell line NB4 cells. The results showed that zo-1 gene was hypomethylated in bone marrow samples from healthy individuals (1.9%). In newly diagnosed AL and relapsed patients, the rate of zo-1 gene methylation was 93.2% and 66.9% respectively, while it was 16.4% in AL patients in complete remission, which was much higher than that in healthy individuals. There was significant difference between them. It is concluded that as compared with healthy individuals, zo-1 gene in acute leukemia patients is hypermethylated and with different degrees in various phases of leukemia. Analysis of zo-1 gene methylation status may be useful to monitor the development of acute leukemia.

8.Poliovirus receptor on surface of acute B lymphoid leukemia cells modulated by epigenetic mechanism.

The aim of this study was to identify if the expression of poliovirus receptor (PVR) on the surface of acute B lymphoid leukemia (B-ALL) cells RS4:11 and SUP-B15 is modulated by epigenetic mechanism. B-ALL cell lines RS4:11 and SUP-B15 were treated with demethylation agent. Bisulfite PCR was performed to detect percentage change of the methylated CpG islands in the promoter region of PVR. In the meantime, the expression levels of PVR at the translation and transcription levels were detected by flow cytometry and RT-PCR respectively. The B-ALL cell lines were also treated with histone deacetylase (HDAC) inhibitor. The expression level of the gene mRNA and protein was detected too. The results indicated that after treated with 5-azacytidine, the hypermethylated status of PVR promoter region was partly reversed, and the expression of PVR at both mRNA and protein levels was restored in the meanwhile. HDAC inhibitor suberoylanilide hydroxamic acid could also increase the PVR expression. But there was no synergistic function between hypermethylation and HDAC as for repressing PVR transcription in B-ALL cell lines. It is concluded that the expression of PVR in B-ALL cells is modulated by epigenetic mechanisms. Treatment with corresponding inhibitors can partly restore the gene's expression in both mRNA and protein levels.

9.Observation on the therapeutic effect of painless festering moxibustion on knee osteoarthritis.

OBJECTIVETo observe the therapeutic effect of painless festering moxibustion on knee osteoarthritis and to probe into a new therapy.
METHODSThirty-four cases of knee osteoarthritis were treated with painless festering moxibustion at the best moxibustion part close to the knee joint. The internationally acknowledged knee joint function rating scale (percentage method) including such indexes as pain, walking, joint flexion mobility, instability, engorgement, stair activity and twist-locking before and after treatment was used for assessment of the therapeutic effect.
RESULTSTen cases were cured, 14 cases were markedly effective, 9 cases were effective and 1 cases was ineffective, with a total effective rate of 97.060%.
CONCLUSIONPainless festering moxibustion has an obvious therapeutic effect on knee osteoarthritis.

10.Moxibustion on heat-sensitive acupoints for treatment of myofascial pain syndrome: a multi-central randomized controlled trial.

OBJECTIVETo observe the clinical therapeutic effect of moxibustion on heat-sensitive acupoints on myofascial pain syndrome (MPS).
METHODSThree-centre, single blind, randomized controlled trial method was adopted. One hundred and seven cases were randomly divided into an observation group (n = 57), and a control group (n = 50). The observation group were treated by moxibustion on the heat-sensitive acupoints and the control group by acupuncture, cupping and TDP. The therapeutic effects were assessed according to changes of the score of the short-form of McGill pain questionnaire composing of pain rating index (PRI), visual analogue scale (VAS) and present pain intensity (PPI) before and after treatment.
RESULTSThe cured rate of 86.0% in the observation group was better than 24.0% in the control group. After treatment, the score of PRI, VAS, PPI indexes significantly improved in the observation group (P < 0.001), and were significantly better than those in the control group (P < 0.001).
CONCLUSIONMoxibustion on the heat-sensitive acupoints has a high therapeutic effect on MPS.
