BACKGROUND:The long-term contraction of skeletal muscle can lead to the decrease of resting potential, affect its contraction, and result in the occurrence of fatigue. Whether acupuncture and injury-free magnetic lifting needle are effective on the recovery of resting potential? OBJECTIVE: To observe and compare the influence on the recovery of resting potential of skeletal muscle by taking acupuncture and magnetic needle as the recovery means.DESIGN: A controlled study.SETTINGS: College of Physical Education, Shanxi Normal University;Binzhou College of Shandong Province.MATERIALS: Thirty experimental toads, which were provided by Beijing University of Physical Education, were randomly divided into acupuncture group (n=10), magnetic needle group (n=10), magnetic needle+pressing group (n=10). Left and right semitendinosus muscles were taken as controls.METHODS: The in vitro toad's semitendinosus muscles (experimental side and control side) were treated with electrostimulation for 40 minutes,and then the resting potential of skeletal muscular cells were measured with the intracellular glass microelectrode technique. After the electrostimulation, filiform needle acupuncture, magnetic needle, magnetic needle plus pressing were also given.MAIN OUTCOME MEASURES: The resting potentials were observed before exercise, after electrostimulation and other treatments in the groups.RESULTS: ① The resting potentials of skeletal muscular cells after electrostimulation were decreased as compared with those before electrostimulation [experimental side: (59.48±7.37), (84.54±5.72) mV; control side:(59.43±7.14), (83.62±5.86) mV]. ② After the treatments of acupuncture,magnetic lifting needle, magnetic lifting needle plus pressing, the decreased resting potentials after electrostimulation were obviously recovered (P ＜ 0.01). ③ The recovered percentage of resting potential in the magnetic needle group was obviously smaller than those in the acupuncture group and magnetic needle+pressing group [(42.50±12.41)%, (53.71±10.93)%,(49.10±8.39)%, P ＜ 0. 01].CONCLUSION: Acupuncture, magnetic lifting needle, magnetic lifting needle plus pressing plays a role in promoting the recovery of resting potential of skeletal muscular cells after long-term electrostimulation, and the effect of simple magnetic needle is smaller than that of acupuncture and magnetic needle plus pressing.

During 14-17th days, the duodenum of the rat fetus was composed of stratified epithelium and peripheral mesenchyme. On 18th day, the mesenchyme began to protrude toward the basal aspect of the epithelium, resulted in the formation of primary villi. At the sametime, goblet cells and the primordia of intestinal glands appeared. The number of goblet cells,which located at the surface of the villi, increased gradually with the fetus age, whereas after birth the goblet cells decreased with the development and maturation of intestinal glands. From 19th day till 3-4 weeks after birth, the shape of villi changed continuously, and attained to the adult shape in 4th week.

Objective: To explore the correlation between the parameters of eTRACKING detection of femoral artery and the syndrome types of traditional Chinese medicine (TCM) in type 2 diabetic patients so as to provide clinical evidence for early prevention and treatment of diabetic lower extremity arterial disease. Methods: A total of 147 cases of type 2 diabetic patients were included. Basic data and TCM clinical information were collected, and eTRACKING detection of common femoral arteries was performed. Differentiation of symptoms and signs for classification of TCM syndromes was performed in all patients. The correlations between TCM syndrome and pressure-strain elastic modulus (Ep), stiffness parameter beta, arterial compliance (AC), pulse wave velocity beta (PWVbeta), and augmentation index (AI) in common femoral arteries were observed. Results: In the patients with deficiency of both yin and yang, the Ep value was higher than that in the patients with deficiency of both qi and yin, the stiffness parameter beta was higher than that in the other three syndrome types (deficiency of both qi and yin, excessive heat due to yin deficiency, accumulation of damp-beat in spleen), the AC value was lower than that in the patients with excessive heat due to yin deficiency, the PWVbeta value was higher than that in the patients with excessive heat due to yin deficiency and deficiency of both qi and yin, and the AI value was higher than that in the patients with excessive heat due to yin deficiency. The stiffness parameter beta in the patients with deficiency of both qi and yin was higher than that in the patients with accumulation of damp-heat in spleen. In the patients with blood stasis, the Ep value was higher and the AC value was lower than that in the patients without blood stasis. Conclusion: The decrease of elasticity in lower extremities can be detected by eTRACKING. This study reveals that type 2 diabetic patients with deficiency of both yin and yang, accumulation of damp-heat in spleen and blood stasis have more severe lower extremity arteriosclerosis. In eTRACKING parameters, stiffness parameter beta, AC and PWVbeta may become the objective indexes in evaluating early diabetic lower extremity arteriosclerosis.

BACKGROUND: Adequate preparation of donors and recipients prior to living-related donor renal transplantation, short warm and cold ischemia time for donor kidney, good histocompatibility of human leukocyte antigen match, and low postoperative rejection incidence provide feasibility for use of low-dose immunosuppressive agents after living-related donor renal transplantation. OBJECTIVE: To investigate the safety and effectiveness of low-dose calcineurin inhibitors (CNI), an immunosuppressive agent, in living-related donor renal transplantation. METHODS: A total of 38 recipients who underwent living-related donor renal transplantation at the Center of Renal Transplantation of the First Affiliated Hospital of Nanjing Medical University from January 2006 to June 2008 were randomized for treatment with mycophenolate mofetil (750 mg twice a day), prednisone, and either standard-dose CNI (n=18) or low-dose CNI (n=20) during 12 months post-transplantation. Ciclosporin A was given orally (starting dose, 6 and 4 mg/kg per day, respectively) in two divided doses to achieve the 12-hour whole blood concentration as measured by fluorescence polarization immunoassay. The starting dose of tacrolimus was 0.12 and 0.08 mg/kg per day respectively, and its whole blood concentration was measured by enzyme-multiplied immunoassay technique. After transplantation, patients were followed up. Renal function, pulmonary infection, liver dysfunction, and CNI nephrotoxicity at different time periods were compared between different regimens. RESULTS AND CONCLUSION: During 12 months post-transplantation, patient death occurred in one of 18 patients (5.6%) in the CNI standard-dose group and none of 20 patients (0%) in the CNI low-dose group. There was no significant difference in renal function and acute rejection between CNI standard-dose and CNI low-dose groups (P > 0.05). The incidence of liver dysfunction and CNI nephrotoxicity was significantly lower in the CNI low-dose group than in the CNI standard-dose group (P < 0.05). In addition, a low-dose CNI regimen helped recipients to lessen the economic burdens. These findings indicate that it is effective, safe and economical to use a low-dose CNI regimen in living-related donor renal transplantation.

Objective To establish rSAG1-IgM-ELISA with purified rSAG1 fusion protein for immunodiagnosis of toxoplasmosis. Methods The rSAG1 fusion protein was purified by Ni 2+ column. The ELISA plate was coated with different concentrations of rSAG1, reacted with pooled positive and negtive human sera. Goat anti-human IgM conjugated to horseradish peroxidase was used as the second antibody. The appropriate detecting condition of the rSAG1-IgM-ELISA assay was determined by orthogonal experiment. The reproducibility, sensitivity and specificity of the assay were assessed. Thirty-five IgM-positive and 57 IgM-negative human sera detected by the imported IgM-ELISA kit were detected with the rSAG1-IgM-ELISA. Results The purity of rSAG1 was above 90%. The appropriate detecting condition was that the coated rSAG1 was 2 5 ?g/ml, the human serum was in 1∶100 dilution, and the second antibody was in 1∶4000 dilution. The coefficient of variation (CV) value of IgM-positive and IgM-negative pooled sera were 13 8% and 7 7% respectively. The inhibition rate of the assay was 62 0% The positive correspondence rate and negative correspondence rate were 82 9% (29/35) and 91 2% (52/57) respectively,the total correspondence rate was 88 0%, compared with the imported IgM-ELISA kit. Conclusions The rSAG1-IgM-ELISA has high sensitivity and specificity, and good correspondence rate with the imported IgM-ELISA kit. It indicates that rSAG1-IgM-ELISA has potential value for early diagnosis of toxoplasmosis.

Regarding the needling manipulation, Chifeng Yingyuan, there are different descriptions in ancient medical literature and the current teaching materials or clinical reports, especially for the understanding recorded in Jinzhenfu Poem of Golden Needle). In the current teaching materials and clinical reports, it is described that the needle is inserted deeply at first, and then shallowly; afterward, the needle tip is pushed to the middle layer and manipulated with lifting, thrusting and rotating technique, combined with twisting and flying method. After analyzing the original description in the book and the feasibility of function and operation, the authors introduced their recognition on its manipulation and techniques in this article. This manipulation should be: the needle is inserted to the middle layer, afterward, the needle body is pulled and tilted toward each of the four directions, on this basis, then twisting and flying method is applied.

OBJECTIVETo investigate the regulation of Cha Gan Beng Ga on the activity of biomarker PGC-1α in vivo and in vitro, and lay the foundation for studying the efficacy result of Cha Gan Beng Ga on xenograft tumor model and extracting active constituents.

METHOD(1) The coarse powder of Cha Gan Beng Ga was extracted with 70% ethanol solution through heating and refluxing, and finally was used to freeze dry powder. (2) 50 mg x kg(-1) of freeze-dried power was orally administrated to KM and C57BL/6J mice once daily, lasting for 5 consecutive days; different concentrations of extracted materials was given to non-small cell lung cells A549. (3) The expression level of PGC-1α mRNA was quantitatively determined in lung tissue of mice and non-small cell lung cells A549.

RESULTThe expression levels of PGC-1α in lung tissue of different mice strains had an increasing tendency. Furthermore, the expression levels of PGC-1α in non-small cell lung cells A549 also had an increasing tendency, showing dose and time-dependent relationships.

CONCLUSIONMongolian Medicine Cha Gan Beng Ga could induce the over-expression of PGC-1α mRNA in lung tissue of mice and in non-small cell lung cells A549. The present results will lay foundation for studying the efficacy result of antitumor and active constitutes in future.

Objective To detect the expression of COX-2 and p53 in breast carcinoma tissue and investigate their associations with clinical prognosis.Methods The expression of COX-2 and p53 was carried out in 16 cases of normal epithelial tissue and 152 cases of breast carcinoma tissue using immunohistochemistry SP method.The correlation of their expression with clinical characteristics was analyzed using SPSS software 16.0.Survival analysis was used to investigate their effects on tumor prognosis.Results No positive COX-2and p53 expressions were observed in normal epithelial tissue.Among 152 patients,89 (58.6 ％) were positive staining rete for COX-2 and 93 (61.18 ％)were shown p53 expression, with a statistically significant associations between expressions of COX-2 and p53 and breast cancer (r =0.426,P ＜ 0.01).The COX-2 and p53 expressions were significantly correlated with pathological grade, clinical stage, lymph node or distant organ metastasis.There was a statistically significant correlation between COX-2 and p53 expression and pathological grade Ⅰ / Ⅱ.The 5-year progress-free survival (PFS) rate in patients with COX-2 over-expression was 61.3 ％, which was remarkably lower than that in those with low COX-2 expression.There was no statistically significant difference of 5-year PFS between positive and negative p53 expression.A shorter 5-year PFS was seen in patients with co-expression of COX-2 and p53 than in those with either COX-2 or p53positive expression alone and also than in those with both COX-2 and p53 negative expression.Conclusion Detection of the expressions of COX-2 and p53 can be used to predict the prognosis of breast cancer.

Objective To observe the effect of liver disease special-purpose enteral nutrition preparation on protein metabolism and liver function in children with liver injury.Methods Sixty cases of severe ill with liver injury in hospital,with mean age of (7.8?6.3) years old.All patients were randomly divided into experimental group (n=30) and control group(n=30).The experimental group was treated by adding the liver disease special-purpose enteral nutrition preparation homogenized diet and control group was treated by adding entire protein entire nutrition type enteral nutrition preparation.All patients in both 2 groups were nasally fed with intestinal nutrition,which contained 418-628 kJ/(kg?d).One day before nutritional support and 14 days after nutritional support,the liver function,total serum protein,albumin,hemoglobin were recorded.SPSS 11.5 software was used to analyze the data.Results The baseline indicators were similar before nutritional supports.Fourteen days after nutritional support,alanine aminotransferase(ALT) and aspartate aminotransferase(AST) were all significantly lower in experimental group than in control group(Pa

Objective To explore the clinical efficacy of laparoscopic transabdominal preperitoneal (TAPP) hernia repair and risk factors affecting postoperative complications.Methods The retrospective casecontrol study was conducted.The clinical data of 595 patients who received laparoscopic TAPP hernia repair in the First Affiliated Hospital of Soochow University from February 2008 to August 2016 was collected.Operations were performed by the same doctors' team.Observation indicators:(1) surgical situations;(2) postoperative situations;(3) follow-up situations;(4) risk factors affecting complications after laparoscopic TAPP hernia repair.Follow-up using outpatient examination and telephone interview was performed to detect the recovery time of non-restricted activity,postoperative complications and hernia recurrence up to February 2017.Measurement data with normal distribution were represented as (x)±s.The univariate analysis and multivariate analysis were done using the chi-square test and Logistic regression model.Results (1) Surgical situations:595 patients underwent laparoscopic TAPP hernia repair using the heavy meshes.Overall operation time and overall volume of blood loss were (55±25) minutes and (7±5)mL,including operation time of (50±20)minutes in 502 unilateral hernias and operation time of (81 ± 29)minutes in 93 bilateral hernias.Of 595 patients,34 had incarcerated hernia,the contents of hernia:greater omentum,small intestine and sigmoid colon were detected in 21,11 and 2 patients,respectively,with an incarcerated time of 2-21 hours;4 with incarcerated hernia induced small intestinal necrosis received laparoscopy-assisted small intestinal resection ± anastomosis,1 with sigmoid colon necrosis received necrotic sigmoid canal resection ± sigmoidostomy and 29 received repair after the contents restoration of hernia.Operation time and volume of intraoperative blood loss in 34 patients with incarcerated hernia were (84 ± 39)minutes and (12±6) mL.Thirteen of 595 patients (10 with indirect hernia and 3 with direct hernia) had recurrent hernia,and operation time and volume of intraoperative blood loss were (75±-26)minutes and (10± 5)mL.(2) Postoperative situations:time to initial exsufflation of 595 patients was (19± 12)hours.Of 595 patients,590 took fluid diet at 6 hours postoperatively and 5 undergoing enterectomy took fluid diet at 24 hours postoperatively.The pain score at 1 day postoperatively and duration of hospital stay were respectively 2.5± 1.4 and (2.1± 1.9)days.(3) Follow-up situations:of 595 patients,593 recovered non-restricted activity at 2 weeks postoperatively and 2 didn't recover non-restricted activity at 2 weeks postoperatively.Of 595 patients,542 were followed up for 6-60 months,with a median time of 31 months.Fifty-seven,25,13 and 1 patients were respectively complicated with seroma,surgical pain,urinary retention and enteroparalysis,they were improved by symptomatic treatment,and the same patient can have multiple complications.There were no severe complications which needed surgical intervention,such as vascular injury,damnify of intestinal canal and poke hole hernia.Of 2 patients with recurrence of hernia,1 with right indirect hernia had recurrence of direct hernia and then received Lichtenstein tension-free hernia repair,and 1 received treatment in other hospital.(4) Risk factors affecting complications after laparoscopic TAPP hernia repair:results of univariate analysis showed that age,diameter of hernia sac,incarcerated hernia,recurrent hernia,operation time and volume of intraoperative blood loss were related factors affecting complications after laparoscopic TAPP hernia repair (x2 =6.657,55.296,44.305,5.253,117.461,100.722,P＜0.05).Results of multivariate analysis showed that diameter of hernia sac ≥ 4 cm,incarcerated hernia,operation time ≥ 100 minutes and volume of intraoperative blood loss ≥ 10 mL were independent risk factors affecting complications after laparoscopic TAPP hernia repair (OR =3.610,11.315,12.401,7.346,95％ confidence interval:2.009-6.486,3.579-35.772,5.408-28.437,3.739-14.434,P＜ 0.05).Conclusion Laparoscopic TAPP approach for inguinal hernia is safe and effective,and diameter of hernia sac ≥4 cm,incarcerated hernia,operation time ≥ 100 minutes and volume of intraoperative blood loss ≥ 10 mL are independent risk factors affecting complications after laparoscopic TAPP hernia repair.
